Instructor Presence Stabilizes Performance Distributions in Digitally Scaffolded CPR Training

Background: Digital feedback systems are increasingly integrated into school-based cardiopulmonary resuscitation (CPR) training. Prior research has largely emphasized mean performance outcomes, whereas assessment theory suggests that distributional characteristics—particularly lower-tail reliability—may be equally relevant in safety-critical competence contexts.
Purpose: This study examined whether different instructional configurations under digitally scaffolded CPR training—peer-led and adult-led instruction—differ in distributional properties of performance, and how these formats compare to a minimal-scaffolding digital-only configuration.
Methods: A quasi-experimental comparative design was conducted across three conditions: peer led instruction (n = 342), adult-led instruction (n = 69; individual-level subsample), and digital-only training without instructor guidance during assessment (n = 23). Mechanical CPR performance was assessed using QCPR-enabled manikins generating composite percentage scores. Non-parametric tests were used to compare distributions and frequencies of catastrophic low-performance cases (<50%), operationalized as lower-tail competence failures.
Results: Mean performance was comparable across groups (Peer: 84.45%; Adult: 83.54%; Digital only: 82.52%). However, distributional differences emerged. Catastrophic lower-tail failures occurred exclusively in the digital-only condition (3/23; 13.0%) and were absent in instructor present formats. Overall group differences were statistically significant (H = 11.38, p = .003), with pairwise contrasts indicating differences between instructor-present and digital-only conditions, but not between peer-led and adult-led instruction.
Conclusions: Although mean performance did not differ, instructor-present formats were associated with reduced lower-tail vulnerability. From an assessment perspective, distributional integrity may therefore complement mean-based interpretations when evaluating instructional effects in safety critical skills training.
Einfluss der Änderung der AMNOG-Leitplanken durch das GKV-FinStG auf die Nutzenbewertungen des G-BA

Zielsetzung
Das zentrale Ziel der Studie ist die Analyse möglicher Veränderungen bei der Zusatznutzenbewertung durch den Gemeinsamen Bundesausschuss (G-BA) in Folge des GKV-Finanzstabilisierungsgesetzes (GKV-FinStG). Die Untersuchung prüft, ob die Verschärfung der AMNOG-Leitplanken zu einer restriktiveren Bewertungspraxis geführt hat. Operationalisiert wird dies durch eine vergleichende Analyse deutscher und französischer Nutzenbewertungen vor und nach dem GKV-FinStG. Die Studie verfolgt zwei zentrale Forschungsfragen: (1) Lassen sich statistisch signifikante Veränderungen in den Zusatznutzenbewertungen des G-BA nachweisen? (2) Können beobachtete Veränderungen kausal auf die Verschärfung der AMNOG-Leitplanken zurückgeführt werden?
Methodik
Die Untersuchung verwendet ein quasi-experimentelles Difference-in-Difference (DiD) Design zur Analyse von Veränderungen der Zusatznutzenbewertung. Deutschland fungiert als Interventionsgruppe, Frankreich als Kontrollgruppe. Die Erfassung der deutschen Nutzenbewertungen erfolgte über den AMNOG-Monitor für den Zeitraum 2020–2024, französische Bewertungen über die Haute Autorité de Santé (HAS). Separate Analysen wurden nach Patentschutzstatus der zweckmäßigen Vergleichstherapie (zVT) durchgeführt.
Ergebnisse
Die Studie liefert erstmals empirische Evidenz für einen möglichen Einfluss der verschärften AMNOG-Leitplanken auf das Bewertungsverhalten des G-BA. Der statistisch signifikante Difference-in-Difference-Effekt von −9,74% (p=0,028) zeigt, dass der Anteil mindestens beträchtlicher Zusatznutzenbewertungen in Deutschland nach der Reform um nahezu 10 Prozentpunkte stärker zurückging als in Frankreich. Bei Verfahren mit patentgeschützter zVT zeigt sich mit −14,56% der stärkste Effekt, während bei generischer zVT erwartungskonform kein relevanter Effekt (0,88%) auftritt.
Schlussfolgerungen
Die Verschärfung der AMNOG-Leitplanken hat zu einer systematischen Reduktion des Anteils mindestens beträchtlicher Zusatznutzenbewertungen geführt. Die beobachtete Veränderung könnte auf unbewusste kognitive Anpassungen oder bewusste strategische Überlegungen zur Begrenzung der GKV-Ausgaben zurückzuführen sein. Die Befunde legen nahe, dass Einsparungen nicht nur durch direkte Preiseffekte, sondern auch durch restriktivere Bewertungspraxis entstehen.
Background: Technology yields the potential to support independence in later life and facilitate ageing in place. Barriers such as limited usability, varying levels of digital literacy, and persistent age-related stereotypes are documented to hinder technology adoption among older adults. User-centered design approaches, such as the persona method, offer strategies to address these challenges.
Objective: Personas were created to inform the development of technologies supporting ageing in place by enhancing stakeholders’ understanding of needs and technology experience among older adults.
Methods: The personas were developed through an iterative process, with this paper detailing the last stage, which employed the a PATHY technique as conceptual framework for content analysis. Eleven focus group discussions were conducted in southern Germany, of which nine were included in the analysis. These involved older adults in need of support, with orthopedic and neurological conditions, and in informal caregiving roles.
Results: Five hybrid personas were created to represent the target groups’ experiences related to ageing in place, each reflecting needs, problems, existing solutions, habits, sociodemographic characteristics, and dimensions of technology experience. Key factors influencing technology experience and uptake among the personas were identified, including the social context and availability of support, the perceived trade-off between safety and digital privacy, and the persistence of the digital divide.
Conclusion: The findings underscore the necessity of adaptable, context-sensitive technology solutions, displayed by the personas’ needs, pains, routines and technology experiences. Thereby, the hybrid personas reinforce the importance of a more relational and context-aware approach in user-centered design.
