Active Assisted Living – Szenarien und Löösungsansäätze füür ein selbstbestimmtes Leben

In dem vorliegenden Open-Access-Buch identifizieren Studierende und Lehrende interprofessionell Entwicklungen und Problemfelder der Assistiven Technologien. Diese halten zunehmend Einzug ins Leben versorgungsbedürftiger, häufig älterer Menschen. Nach einigen Jahren der Entwicklung kann konstatiert werden, dass der Transfer von Entwicklung zur Anwendung im Alltag infolge unterschiedlicher Perspektiven und Zielsetzungen nur unzureichend stattfindet. Die Beitragsautoren bieten kreative Lösungsansätze, die zu einer nachhaltigen Umsetzung führen sollen
Die Pflege von Patienten, die sich einer Carotis-Endarteriektomie (CEA) unterziehen müssen, ist herausfordernd für die betreuenden Pflegekräfte. Aspekte wie das zunehmende Alter und die Multimorbidität der Patientengruppe erhöhen die Anforderungen an eine sichere und gute Versorgungspraxis. Als anwendungsorientierte Instrumente für die pflegerische Praxis verbinden Standardized Care Plans (SCP) aktuelle wissenschaftliche Erkenntnisse, Leitlinien und die Expertise von erfahrenen Pflegefachkräften. Mithilfe des SCPs kann der Transfer von wissenschaftlichen Erkenntnissen in die Praxis gelingen und die Versorgungsqualität dieser Patientengruppe verbessert werden. Der erstellte SCP für Patienten mit CEA gliedert sich in die vier Teilbereiche präoperative Pflege, postoperative Pflege, neurologische Überwachung und Entlassung und Tertiärprävention und zeigt alle Aspekte der perioperativen Pflege auf
Diagnostic skills and decision-making processes are a substantial part of nursing and midwifery education. In a real-life clinical setting, the situation is complex and time-critical with a major impact on patient safety. The goal of simulation-based training allows the reduction of the complexity and offers the focus on new skills, diagnostic steps, engaging evidence-based-practice and receiving real-time
feedback. According to Work-integrated-learning (WIL) it is an ideal method fostering authentic work-focused experience, students learning through doing and students discipline of professional development (Zegwaard et al., 2023). Simulation in nursing and midwifery education as a pedagogical tool has been used widely in the clinical health-care of students (WHO Regional office for Europe, 2018).
Within complex scenario-based-simulation students may be engaged and immersed by working with the “patient or women” as autonomous clinicians, making their own diagnoses, clinical-decisions and evaluating their interventions (Olaussen, Heggdal & Tvedt, 2019). For the evaluating process a two-fold process that focuses on the assessment of students performance and the simulation process per se is
recommended (World Health Organization, 2018). However, the research findings in competence development and learning outcomes during complex scenario-based simulations are limited in nursing (Koukourikos et al., 2021) and especially in the midwifery profession (Changuiti et al., 2021). The aim of this study was to evaluate the competence progress in midwifery-students through complex simulation scenarios. Thus to define, if the scenarios are an improved teaching and learning method.
Die Zufriedenheit mit der stationären Versorgung, Pflege und Therapie stellt für onkologische Patienten einen Schlüsselindikator für die erlebte Lebensqualität dar. Zahlreiche Studien untersuchten die Einflussfaktoren auf die Zufriedenheit bei onkologischen Patienten, Aspekte wie das Fatigue-Syndrom spielen dabei eine zentrale Rolle (Lis et al., 2009). Ferner ist die Zufriedenheit ein entscheidender Aspekt für die Beurteilung der pflegerischen Versorgungsqualität (Engel, Brinkmann-Stoppelenburg, Nieboer & van der Heide, 2018). Die Patientenzufriedenheit beschreibt das Ausmaß, inwiefern die Erfahrungen in der Gesundheitsversorgung mit den Erwartungen an die Qualität übereinstimmen (Brédart et al., 2005). Sie wird als multidimensionales Konstrukt beschrieben und beinhaltet technische, interpersonelle, soziale und moralische Aspekte der Pflegeversorgung (Al-Abri & Al-Balushi, 2014). Pflegekräfte spielen im multidisziplinären Team eine elementare Rolle in der Gesundheitsversorgung, der -edukation, der Unterstützung im Selbstmanagement und damit letztendlich auch der Förderung der Lebensqualität (Campbell et al., 2017; Jang, Kim & Kyungee, 2016; Kullberg et al., 2015; Galway et al., 2012)Zur Messung der Lebensqualität und Zufriedenheit über die Versorgung solcher Patienten steht eine Reihe von Instrumenten zur Verfügung. Die überwiegende Anzahl ist jedoch generisch angelegt und bildet die komplexen Bedarfe während einer stationären Versorgung nur bedingt ab (Saathoff et al., 2016).
Purpose Video-based monitoring systems in long-term inpatient care represent an opportunity to increase patient safety. Technologies using infrared technology, as well as video and sound transmission are used to recognize movement to prevent falls and enable the continuous recording of vital parameters. Individual assessment of the necessity and limits of application from a nursing care perspective is essential due to the restriction on the patient’s privacy and autonomy. As an example, the Oxevision monitoring system is analysed in more detail in this project. Care professionals as well as the public were asked about the potentials and risk factors of video-based monitoring systems using semi-structured interviews and an online survey. This paper intends to provide a basis for discussion for the use of video-based monitoring systems in long-term inpatient care facilities. The aim is to conduct qualitative and quantitative fundamental research as a basis for generating data and a critical analysis of the topic. In addition to displaying the opinion of the general public, the limitations and potential benefits of these technologies are illustrated and ethical aspects are discussed. Method The research was conducted in a mixed-method design by a student research group. All surveys are based on prior literature research however, the analysis is based on defined criteria. Interviews were conducted with nursing care professionals (n=4) and people from the public (n=244) were surveyed on the topic of video-based monitoring systems via an online survey. The interviews were analysed using inductive, structured content analysis methods as Mayring describes to be a suitable analysis method. The online survey was analysed using statistical methods to draw credible quantitative results. Results and Discussion The results clearly show that video-based monitoring systems have both advantages and disadvantages as well as ethical aspects and factors that influence the acceptance of video-based monitoring which must be weighed up individually. From the total responses of n=244, 47,1% (n=115) of survey participants are in favour of the use of monitoring systems in nursing, while 40,1% are undecided. The most frequently mentioned advantages from the interviews included continual vital sign monitoring, faster reactions in emergencies, a reduction in the workload of nursing staff and an increase in patient safety, for example through improved fall prevention for patients. In contrast, 185 people (75,8%) of the survey participants are of the opinion that video monitoring systems should not be used across the board and that specific groups of people should be considered differentially. Frequently cited reasons for this are data protection, restriction of privacy and self-determination, as well as the fear of data abuse. The hereby collected data suggests that a general interest in the implementation of video-based monitoring systems is apparent among the population. In order to increase acceptance, of aspects such as data protection, privacy and possible abuse of data, further research is needed to establish a reliable conclusion to this matter.
Am Gesundheitscampus Ulm-Wiblingen der DHBW Heidenheim wurde zum Wintersemester 2019 ein Simulationslabor (SimLab) für die Studiengänge „Angewandte Gesundheits- und Pflegewissenschaft“ sowie „Angewandte Hebammenwissenschaft“ eröffnet. Im Rahmen der Implementierung wurden Simulationsszenarien entwickelt, die in einer Begleitforschung evaluiert werden. Initial wurdeein interprofessionelles Peer-Teaching mit Studierenden des fünften Semestersentwickelt. Die Studierenden sind bereits examinierte Gesundheits- und Krankenpfleger*innen, Kinderkrankenpfleger*innen und Hebammen, die fachspezifische Themenbereiche im interprofessionellen Kontext vermittelten, da in derPraxis komplexe Fälle interprofessionell von unterschiedlichsten Fachrichtungenbetreut werden, was Kommunikation, eigene und übergreifende Fachkompetenzbenötigt. In einer qualitativen Analyse wurden Fokusgruppeninterviews mit 24 Studierenden geführt und mit der Methodik der qualitativen Inhaltsanalyse nach Mayring ausgewertet. Die Ergebnisse spiegeln ein positives Feedback der Studierenden zur abwechslungsreichen Lehrmethode wider. Hervorzuheben sind ein gelungener Theorie-Praxis-Transfer, die Sensibilisierung auf die professionelle Identität, ein effektives Kommunikationstraining sowie der Erwerb einer über-greifenden Handlungskompetenz.
