However, describing and understanding functioning in its complexity with all its determinants is one of the biggest challenges faced by clinicians and researchers. Design: This paper focuses on the development of a cohort study on functioning, outlining the relevant steps and related methods, and illustrating these with reference to the Swiss Spinal Cord Injury Cohort Study (SwiSCI). Methods and results: In setting up a cohort study the initial step is to specify which variables are to be included, i.e. what to assess. The International Classification of Functioning, Disability and Health (ICF) is valuable in this process. The second step is to identify how to assess the specified ICF categories. Existing instruments and assessments can then be linked to the ICF. Conclusion: The methods outlined here enable the development of a cohort study to be based on a comprehensive perspective of health, operationalized through functioning as conceptualized and classified in the ICF, yet to remain efficient and feasible to administer. Abschlussbericht N2 - Die Altbausanierung nimmt im gesamten Baubereich eine immer bedeutendere Stellung ein. Falls die geplanten Sanierungsmaßnahmen nicht unter den Bestandsschutz von Baudenkmälern fallen, sind die Anforderungen der zum Zeitpunkt der Sanierung baurechtlich eingeführten Normen zu berücksichtigen. Das Bauteil, das bei der Sanierung eine besonders sorgfältige Planung erfordert, ist die Wohnungstrenndecke, die in Altbauten häufig als Holzbalkendecke ausgeführt wurde. Die vorhandenen Planungsgrundlagen für den Schallschutznachweis von Holzbalkendecken sind sowohl in der Normung als auch in der Literatur sehr lückenhaft. Um bessere Planungsdaten zu erhalten, wurde deshalb am ift Rosenheim zunächst ein Forschungsvorhaben durchgeführt, in dem die Luft‐ und Trittschalldämmung typischer Altbaudecken und deren Verbesserungen durch unterschiedliche Sanierungsmaßnahmen unter Laborverhältnissen ohne Flankenübertragung untersucht wurden [1], [2]. In einem weiteren Forschungsvorhaben [3] wurde in Kooperation mit der Hochschule Rosenheim die Flankenübertragung bei unterschiedlichen Mauerwerkstypen und Deckeneinbindungen ermittelt. Der vorliegende Beitrag beschreibt den Einfluss der Deckenkonstruktion auf die Schalldämmung und die Ermittlung der Planungswerte für die direkte Übertragung über die Decke. In einem noch folgenden zweiten Beitrag werden Planungsdaten für die Flankenübertragung bei Altbaukonstruktionen behandelt. JF - Disability and Rehabilitation N2 - Background: Existing instruments measuring participation may vary with respect to various aspects. This study aimed to examine the comparability of existing instruments measuring participation based on the International Classification of Functioning, Disability and Health (ICF) by considering aspects of content, the perspective adopted and the categorization of response options. Methods: A systematic literature review was conducted to identify instruments that have been commonly used to measure participation. Concepts of identified instruments were then linked to the ICF following the refined ICF Linking Rules. Aspects of content, perspective adopted and categorization of response options were documented. Results: Out of 315 instruments identified in the full-text screening, 41 instruments were included. Concepts of six instruments were linked entirely to the ICF component Activities and Participation; of 10 instruments still 80% of their concepts. A descriptive perspective was adopted in most items across instruments (75%), mostly in combination with an intensity rating. An appraisal perspective was found in 18% and questions from a need or dependency perspective were least frequent (7%). Conclusion: Accounting for aspects of content, perspective and categorization of responses in the linking of instruments to the ICF provides detailed information for the comparison of instruments and guidance on narrowing down the choices of suitable instruments from a content point of view. Implications for Rehabilitation For clinicians and researchers who need to identify a specific instrument for a given purpose, the findings of this review can serve as a screening tool for instruments measuring participation in terms of the following: • Their content covered based on the ICF. • The perspective adopted in the instrument (e.g., descriptive, need/dependency or appraisal). • The categorization of their response options (e.g., intensity or frequency). Several studies have examined the independent effects of single diseases; however, little research has focused on interaction between diseases. The purpose of this study was to assess the impact of six self-reported major conditions and their combinations on HRQL measured by the EQ-5D. Methods The EQ-5D was administered in the population-based KORA-Age study of 4,565 Germans aged 65 years or older. A generalised additive regression model was used to assess the effects of chronic conditions on HRQL and to account for the nonlinear associations with age and body mass index (BMI). Disease interactions were identified by a forward variable selection method. Results The conditions with the greatest negative impact on the EQ-5D index were the history of a stroke (regression coefficient -11.3, p < 0.0001) and chronic bronchitis (regression coefficient -8.1, p < 0.0001). Patients with both diabetes and coronary disorders showed more impaired HRQL than could be expected from their separate effects (coefficient of interaction term -8.1, p < 0.0001). A synergistic effect on HRQL was also found for the combination of coronary disorders and stroke. The effect of BMI on the mean EQ-5D index was inverse U-shaped with a maximum at around 24.8 kg/m2. Conclusions There are important interactions between coronary problems, diabetes mellitus, and the history of a stroke that negatively affect HRQL in the older German population. Not only high but also low BMI is associated with impairments in health status. Dafür wurden eine Rohholzpotentialanalyse sowie eine Prognose auf Basis der Daten der dritten Bundeswaldinventur erarbeitet. Weiter analysierte der Verbund Kenngrößen entlang der Logistikkette zur Rohstoffbereitstellungslogistik und schuf damit eine Berechnungsgrundlage zur Abbildung funktioneller Abhängigkeiten sowie Zusammenhänge von Erntemethode, genutztem Transportweg und Bearbeitungsverfahren. Den Herausforderungen des dreijährigen Forschungsprojekts stellte sich der Verbund aus den Unternehmen Knauf Deutsche Gipswerke KG, Georg Fehrensen GmbH, Holzindustrie Templin, DB Schenker Nieten GmbH, Bruno Reimann GmbH & Co. KG, Eickelmann Transport und Logistik GmbH & Co. KG und den Wissenschaftlern der Hochschule Rosenheim, der Otto-von-Guericke-Universität Magdeburg und der Nordwestdeutschen Forstlichen Versuchsanstalt sowie den Niedersächsischen Landesforsten, dem Landesbetrieb Hessen-Forst und den Landesforsten Thüringen. Der Nutzen für den Spitzencluster liegt in der besseren Planungsmöglichkeit und dem Ausschöpfen von Optimierungspotentialen in Abhängigkeit des verfügbaren Rohholzpotentials. ED - Elsevier, T1 - Men benefit more from midlife leisure-time physical activity than women regarding the development of late-life disability--results of the KORA-Age study JF - science direct N2 - Objective Encouraging physical activity is an important public health measure to reduce disability prevalence in the aged. The aims of this study were to determine the association between midlife physical activity and late-life disability and to investigate gender-specific differences. Method This data originates from the KORA-Age cohort, a follow-up in 2008 of the MONICA (Multinational Monitoring of Trends and Determinants in Cardiovascular Diseases)/KORA (Cooperative Health Research in the Region of Augsburg) S1–S4 surveys (1984–2001) situated in Augsburg, a city in Southern Germany. We applied a multivariable hurdle model to investigate the association of physical activity and disability. Results We analysed 3333 persons with a mean follow-up of 18 ± 5.5 years. Using hurdle models, moderate activity and high activity had a protective effect on the occurrence of disability (OR (odds ratio) = 0.80 and 0.73), but not on severity (i.e. number of limitations). We observed a strong gender-specific difference in this association, with men benefitting more from exercise. Conclusion Elevated physical activity reduces the risk of becoming disabled and postpones the onset of disability by several years, but we could not show an effect on the severity of disability.
