The concept of Personal Knowledge and Information Management (PKIM) is, among others, based on two heories: Personal Information Management (PIM) and Personal Knowledge Management (PKM) which were, hitherto, the subject of separate studies. Moreover, the PKIM concept is relating to the so-called Information Literacy (IL), which is a concept of information skills and competences of individuals. According to the literature, in practice the person who manages knowledge has to be information literate.
Some of the empirical studies results in the PKIM scope, started in Poland (Świgoń, 2012) and recently conducted in Germany, are presented in this article.
The aim of the PKIM empirical studies was to get to know the individuals’ opinions about management of knowledge and information in the context of learning and studying. This part of the studies in Germany asked the following research questions (analogical to studies in Poland): 1. What are the main definitions, dimensions and manifestations of management of knowledge and information through the eyes of individuals (students)?; 2. What is the difference, if any, between knowledge management and information management in the opinion of students?; 3. What is the role of knowledge and information management in studying and other spheres of life?; 4.Which factors influence management of knowledge and information in students’ opinion? The qualitative method was used with an unstructured questionnaire. Respondents were asked to share their own opinions according to the abovementioned open questions in a written form.
Questionnaires were distributed during classes at the Brandenburg University of Technology, Cottbus (BTU) in January 2013. Finally 62 opinions were analysed.
In the light of these empirical studies results, as well as the literature review Świgoń, 2012), three concepts: PIM, PKM and IL are compatible and merged with each other.
Individuals perceive knowledge and information as well as knowledge management (KM) and information management (IM) as integrated areas of interests. Although they do see differences between them (knowledge vs. information, KM vs. IM), the issue of interconnections and relations seem more important. KIM is the process of following steps: gathering, selecting, organising, disseminating and using. In the context of personal knowledge and information, this process involves personal needs, objectives and benefits. Individuals understand that this process, or set of activities is strongly connected with their and skills. These are not only the so-called information skills, but knowledge skills as well, because they tion of new information and knowledge. Sometimes KIM is perceived as a way of coping with information ovabilities involve creative thinking, reflection and creaerload. Thiscompetence is not easy and depends on personal abilities, especially in selecting of information.
Moreover, it depends on appropriate education and training, sometimes on other people, colleagues and experts, and sometimes on technology. All factors and variables influencing on KIM can be negative or positive in their nature. Furthermore, they are related to three categories: personal characteristics, environment (macro and micro environment), and to knowledge and information sources.

Ästhetik reflektiert Kunst, kaum aber deren konkrete Verwendungsformen. Um zu verstehen, was Kunst ist, muss sie jedoch als spezifische Situationen konstituierende besondere Praxis untersucht werden. Kunst ist kein Erkennen oder Erfahren, sondern strategisches Tun: Als (feierliches) ästhetisches Sorgen kultiviert sie in öffentlichen wie privaten Kontexten eine Praxis existenziellen kultischen Sorgens. Christian Zürner hinterfragt ästhetische Topoi wie Autonomie und (Zweck-)Freiheit angesichts solch immanenter Notwendigkeit ebenso wie normative philosophische Zuschreibungen. Kunst bedeutet keinen souveränen Überschuss, sondern lebt vom Drang, menschliche Existenz in einer unheimlichen Welt überhaupt erst einzurichten.
Background:
Chronic obstructive pulmonary disease (COPD) has a functional definition. However, differences in clinical characteristics and systemic manifestations make COPD a heterogeneous disease and some manifestations have been associated with different risks of acute exacerbations, hospitalizations, and death.
Objective:
Therefore, the objective of the study was to evaluate possible clinical clusters in COPD at two study centers in Brazil and identify the associated exacerbation and mortality rate during 1 year of follow-up.
Methods:
We included patients with COPD and all underwent an evaluation composed of the Charlson Index, body mass index (BMI), current pharmacological treatment, smoking history (packs-year), history of exacerbations/hospitalizations in the last year, spirometry, six-minute walking test (6MWT), quality of life questionnaires, dyspnea, and hospital anxiety and depression scale. Blood samples were also collected for measurements of C-reactive protein (CRP), blood gases, laboratory analysis, and blood count. For the construction of the clusters, 13 continuous variables of clinical importance were considered: hematocrit, CRP, triglycerides, low density lipoprotein, absolute number of peripheral eosinophils, age, pulse oximetry, BMI, forced expiratory volume in the first second, dyspnea, 6MWD, total score of the Saint George Respiratory Questionnaire and packs-year of smoking. We used the Ward and K-means methods and determined the best silhouette value to identify similarities of individuals within the cluster (cohesion) in relation to the other clusters (separation). The number of clusters was determined by the heterogeneity values of the cluster, which in this case was determined as four clusters.
Results:
We evaluated 301 COPD patients and identified four different groups of COPD patients. The first cluster (203 patients) was characterized by fewer symptoms and lower functional severity of the disease, the second cluster by higher values of peripheral eosinophils, the third cluster by more systemic inflammation and the fourth cluster by greater obstructive severity and worse gas exchange. Cluster 2 had an average of 959 3 peripheral eosinophils, cluster 3 had a higher prevalence of nutritional depletion (46.1%), and cluster 4 had a higher BODE index. Regarding the associated comorbidities, we found that only obstructive sleep apnea syndrome and pulmonary thromboembolism were more prevalent in cluster 4. Almost 50% of all patients presented an exacerbation during 1 year of follow-up. However, it was higher in cluster 4, with 65% of all patients having at least one exacerbation. The mortality rate was statistically higher in cluster 4, with 26.9%, vs 9.6% in cluster 1.
Conclusion:
We could identify four clinical different clusters in these COPD populations, that were related to different clinical manifestations, comorbidities, exacerbation, and mortality rate. We also identified a specific cluster with higher values of peripheral eosinophils.
Meniscal tears in the avascular zone have a poor self-healing potential, however partial meniscectomy predisposes the knee for early osteoarthritis. Tissue engineering with mesenchymal stem cells and a hyaluronan collagen based scaffold is a promising approach to repair meniscal tears in the avascular zone. 4 mm longitudinal meniscal tears in the avascular zone of lateral menisci of New Zealand White Rabbits were performed. The defect was left empty, sutured with a 5-0 suture or filled with a hyaluronan/collagen composite matrix without cells, with platelet rich plasma or with autologous mesenchymal stem cells. Matrices with stem cells were in part precultured in chondrogenic medium for 14 days prior to the implantation. Menisci were harvested at 6 and 12 weeks. The developed repair tissue was analyzed macroscopically, histologically and biomechanically. Untreated defects, defects treated with suture alone, with cell-free or with platelet rich plasma seeded implants showed a muted fibrous healing response. The implantation of stem cell-matrix constructs initiated fibrocartilage-like repair tissue, with better integration and biomechanical properties in the precultured stem cell-matrix group. A hyaluronan-collagen based composite scaffold seeded with mesenchymal stem cells is more effective in the repair avascular meniscal tear with stable meniscus-like tissue and to restore the native meniscus.
Einleitung Die Endoskopische Retrograde Cholangiopankreatikographie (ERCP) ist der Goldstandard in der Diagnostik und Therapie von Erkrankungen des pankreatobiliären Trakts. Jedoch ist sie technisch sehr anspruchsvoll und weist eine vergleichsweise hohe Komplikationsrate auf.
Ziele
In der vorliegenden Machbarkeitsstudie soll geprüft werden, ob mithilfe eines Deep-learning-Algorithmus die Papille und das Ostium zuverlässig detektiert werden können und somit für Endoskopiker mit geringer Erfahrung ein geeignetes Hilfsmittel, insbesondere für die Ausbildungssituation, darstellen könnten.
Methodik
Wir betrachteten insgesamt 606 Bilddatensätze von 65 Patienten. In diesen wurde sowohl die Papilla duodeni major als auch das Ostium segmentiert. Anschließend wurde eine neuronales Netz mittels eines Deep-learning-Algorithmus trainiert. Außerdem erfolgte eine 5-fache Kreuzvaldierung.
Ergebnisse
Bei einer 5-fachen Kreuzvaldierung auf den 606 gelabelten Daten konnte für die Klasse Papille eine F1-Wert von 0,7908, eine Sensitivität von 0,7943 und eine Spezifität von 0,9785 erreicht werden, für die Klasse Ostium eine F1-Wert von 0,5538, eine Sensitivität von 0,5094 und eine Spezifität von 0,9970 (vgl. [Tab. 1]). Unabhängig von der Klasse zeigte sich gemittelt (Klasse Papille und Klasse Ostium) ein F1-Wert von 0,6673, eine Sensitivität von 0,6519 und eine Spezifität von 0,9877 (vgl. [Tab. 2]).
Schlussfolgerung
In vorliegende Machbarkeitsstudie konnte das neuronale Netz die Papilla duodeni major mit einer hohen Sensitivität und sehr hohen Spezifität identifizieren. Bei der Detektion des Ostiums war die Sensitivität deutlich geringer. Zukünftig soll das das neuronale Netz mit mehr Daten trainiert werden. Außerdem ist geplant, den Algorithmus auch auf Videos anzuwenden. Somit könnte langfristig ein geeignetes Hilfsmittel für die ERCP etabliert werden.
GraphMIC is a cross-platform image processing application utilizing the libraries ITK and OpenCV. The abstract structure of image processing pipelines is visually represented by user interface components based on modern QtQuick technology and allows users to focus on arrangement and parameterization of operations rather than implementing the equivalent functionality natively in C++. The application's central goal is to improve and simplify the typical workflow by providing various high level features and functions like multi threading, image sequence processing and advanced error handling. A built-in python interpreter allows the creation of custom nodes, where user defined algorithms can be integrated to extend basic functionality. An embedded 2d/3d visual-izer gives feedback of the resulting image of an operation or the whole pipeline. User inputs like seed points, contours or regions are forwarded to the processing pipeline as parameters to offer semi-automatic image computing. We report the main concept of the application and introduce several features and their implementation. Finally, the current state of development as well as future perspectives of GraphMIC are discussed

This White Paper outlines how the ethical discourse on cybersecurity has developed in the scientific literature, which ethical issues gained interest, which value conflicts are discussed, and where the “blind spots” in the current ethical discourse on cybersecurity are located. The White Paper is based on an extensive literature with a focus on three reference domains with unique types of value conflicts: health, business/finance and national security. For each domain, a systematic literature search has been performed and the identified papers have been analysed using qualitative and quantitative methods. An important observation is that the ethics of cybersecurity not an established subject. In all domains, cybersecurity is recognized as being an instrumental value, not an end in itself, which opens up the possibility of trade-offs with different values in different spheres. The most prominent common theme is the existence of trade-offs and even conflicts between reasonable goals, for example between usability and security, accessibility and security, privacy and convenience. Other prominent common themes are the importance of cybersecurity to sustain trust (in institutions), and the harmful effect of any loss of control over data.
Dieser Artikel soll durch die wissenschaftlich fundierte Darstellung der (Brustkrebs)-Selbsthilfe mögliche Handlungsunsicherheiten im Umgang mit Selbsthilfegruppen (SHG) in der täglichen Praxis verringern.
Die Gesundheitsversorgung kommt ohne die Eigenverantwortung, Selbsthilfe und aktive Mitarbeit der Patienten(inn)en nicht aus. Welche Mechanismen in SHG wirken, wurde bisher nicht im Einzelnen wissenschaftlich untersucht. Im Zusammenhang mit dem gesicherten Wissen über die salutogenen und tertiärpräventiven Wirkungsweisen von Kommunikation und sozialen Bindungen, kann aus dem Stand der Selbsthilfeforschung jedoch ein allgemeines heuristisches Modell der Wirkung von SHG abgeleitet werden, welches kurz erläutert wird. Die am häufigsten auftretende Krebserkrankung in Deutschland ist Brustkrebs. Davon sind zu 99 % Frauen betroffen. Frauen mit Brustkrebs werden im professionellen Versorgungssystem und insbesondere durch die vielfältigen Angebote der Selbsthilfe unterstützt.
Insbesondere SHG tragen zur gegenseitigen sozialen Unterstützung und zur Informationsaneignung bei den Betroffenen und in deren sozialen Umfeld bei. Sie entlasten die primären Netzwerke der Betroffenen und unterstützen eine gezielte Inanspruchnahme professioneller Dienste. Sind die Ergebnisse und Schlussfolgerungen der Studien zur Wirksamkeit von Selbsthilfe bei Brustkrebs aufgrund methodischer Schwierigkeiten der Wirksamkeitsforschung auch limitiert, heben doch die meisten Forscher hervor, dass die Unterstützung durch SHG allein und in Kombination mit einer professionellen Versorgung psychische Störungen reduzieren und subjektiv Gesundheit und Lebensqualität erhöhen.
Selbsthilfeaktivitäten werden zunehmend durch das Sozialversicherungssystem gefördert und in das Versorgungssystem integriert. Die Kooperation zwischen Selbsthilfe und professionellem System kann durch das Kennenlernen der regionalen Selbsthilfelandschaft mit ihren Strukturen, Unterstützungsleistungen und Besonderheiten verbessert werden.

Von der Weltgesundheitsorganisation werden rheumatische Erkrankungen als mehr oder weniger klar definierte Erkrankungen der Gelenke, gelenkumgebenden Strukturen und Muskel- oder Bindegewebe beschrieben, die zu schmerzhaften, häufig chronischen Einschränkungen des Bewegungsapparates führen können. Neben der fachärztlichen Versorgung besteht durch die Deutsche Rheuma-Liga (DRL) ein flächendeckendes SH-Angebot für Betroffene. Insbesondere SHG tragen zur gegenseitigen sozialen Unterstützung und zur Informationsaneignung bei den Betroffenen und in deren sozialen Umfeld bei. Sind die Ergebnisse und Schlussfolgerungen der Studien zu den Wirkungen von SH bei rheumatischen Erkrankungen aufgrund methodischer Schwierigkeiten der Wirkungsforschung auch limitiert, heben doch die meisten Forscher hervor, dass die Unterstützung durch SHG allein und in Kombination mit einer professionellen Versorgung neben vielen Einzelaspekten auch die Lebensqualität erhöht.

Backround
Scaphoidectomy and midcarpal fusion can be performed using traditional fixation methods like K-wires, staples, screws or different dorsal (non)locking arthrodesis systems. The aim of this study is to test the Aptus four corner locking plate and to compare the clinical findings to the data revealed by CT scans and semi-automated segmentation.
Methods:
This is a retrospective review of eleven patients suffering from scapholunate advanced collapse (SLAC) or scaphoid non-union advanced collapse (SNAC) wrist, who received a four corner fusion between August 2011 and July 2014. The clinical evaluation consisted of measuring the range of motion (ROM), strength and pain on a visual analogue scale (VAS). Additionally, the Disabilities of the Arm, Shoulder and Hand (QuickDASH) and the Mayo Wrist Score were assessed. A computerized tomography (CT) of the wrist was obtained six weeks postoperatively. After semi-automated segmentation of the CT scans, the models were post processed and surveyed.
Results
During the six-month follow-up mean range of motion (ROM) of the operated wrist was 60°, consisting of 30° extension and 30° flexion. While pain levels decreased significantly, 54% of grip strength and 89% of pinch strength were preserved compared to the contralateral healthy wrist. Union could be detected in all CT scans of the wrist. While X-ray pictures obtained postoperatively revealed no pathology, two user related technical complications were found through the 3D analysis, which correlated to the clinical outcome.
Conclusion
Due to semi-automated segmentation and 3D analysis it has been proved that the plate design can keep up to the manufacturers’ promises. Over all, this case series confirmed that the plate can compete with the coexisting techniques concerning clinical outcome, union and complication rate.
The Impact of Semi-Automated Segmentation and 3D Analysis on Testing New Osteosynthesis Material

A new protocol for testing osteosynthesis material postoperatively combining semi-automated segmentation and 3D analysis of surface meshes is proposed. By various steps of transformation and measuring, objective data can be collected. In this study the specifications of a locking plate used for mediocarpal arthrodesis of the wrist were examined. The results show, that union of the lunate, triquetrum, hamate and capitate was achieved and that the plate is comparable to coexisting arthrodesis systems. Additionally, it was shown, that the complications detected correlate to the clinical outcome. In synopsis, this protocol is considered beneficial and should be taken into account in further studies.
Introduction: Previous studies of bone stresses in the human lumbar spine have relied on simplified models when modeling the spinal musculature, even though muscle forces are likely major contributors to the stresses in the vertebral bones. Detailed musculoskeletal spine models have recently become available and show good correlation with experimental findings. A combined inverse dynamics and finite element analysis study was conducted in the lumbar spine to investigate the effects of muscle forces on a detailed musculoskeletal finite element model of the 4th lumbar vertebral body. Materials and Methodology: The muscle forces were computed with a detailed and validated inverse dynamics musculoskeletal spine model in a lifting situation, and were then applied to an orthotropic finite element model of the 4th lumbar vertebra. The results were compared with those from a simplified load case without muscles. Results: In general the von Mises stress was larger by 30%, and even higher when looking at the von Mises stress distribution in the superio-anterior and central part of the vertebral body and in the pedicles. Conclusion: The application of spine muscles to a finite element model showed markedly larger von Mises stress responses in the central and anterior part of the vertebral body, which can be tolerated in the young and healthy spine, but it would increase the risk of compression fractures in the elderly, osteoporotic spine.

FrauenspezifischeAngebote erreichen vermehrt Frauen,die in herkömmlichen Einrichtungenseltener anzutreffen sind. Die erzielten Ergebnisse sind mindestens so gut wie bei traditionellen Angeboten. Als besonders günstig haben sich möglichst vielfältige Hilfsangebote erwiesen, die sich an den Problemlagen der Frauen orientieren. Wenig erfolgversprechend dagegen erscheint, lediglich Frauen gemeinsam mit anderen Frauen zu behandeln.
Schlussfolgerungen:
Die Evaluation und Dokumentation frauenspezifischer Angebote in Deutschland muss vorangebracht werden.
Behandlungsangebote für substanzabhängige mütter kleiner kinder: Bindungstheoretische überlegungen

Substanzabhängigkeit der Mütter stellt für ihre Kinder sowohl wegen des Substanzkonsums, als auch wegen zahlreicher psychosozialer Belastungen, denen diese Mutter-Kind-Dyaden zusätzlich ausgesetzt sind, ein erhebliches Entwicklungsrisiko dar. Die Entwicklung einer positiven Mutter-Kind-Beziehung, die als wichtiger Schutzfaktor für die Entwicklung des Kindes gilt, ist gefährdet. Basierend auf den Annahmen der Bindungstheorie werden Möglichkeiten zur Gestaltung eziehungsorientierter Behandlungsangebote, die vor allem die Steigerung der mütterlichen Feinfühligkeit zum Ziel haben, dargestellt. Ergebnisse: Bindungsorientierte Angebote für substanzabhängige Mütter und ihre Kinder können Angebote der stationären und ambulanten Sucht- und Drogenhilfe sinnvoll ergänzen.
Alkoholabhängige Frauen nehmen im Bereich der Alkoholismusbehandlung in mehrfacher Hinsicht den Status einer Minderheit ein. Zum einen sind sie in Einrichtungen, in denen stationäre Behandlung angeboten wird, in Relation zu ihrem Anteil an der Gesamtzahl der alkoholabhängigen seltener anzutreffen als Manner; desweiteren sind sie in den Kliniken, in denen sie gemeinsam mit Männern behandelt werden, deutlich in der Minderheit und schließlich werden Themen, die sie betreffen im Rahmen der Behandlung seltener berücksichtigt.
Es werden mögliche Erklärungen dafür gesucht, weshalb Frauen Fachkliniken seltener in Anspruch nehmen als Männer. Au ßerdem werden Folgen der Minderheitssituation der Frauen für die Kliniken als Institutionen und die Interaktion zwischen Patientinnen und Patienten aufgezeigt.
Die Notwendigkeit geschlechtsspezifische Aspekte auch bei der Gestaltung der stationären Behandlungsangebote im Suchtbereich zu berücksichtigen, gewinnt zunehmend an Akzeptanz. Bisher fehlen jedoch Daten dazu, inwieweit dieser Anspruch im Rahmen der stationären Behandlungsangebote für Frauen bereits umgesetzt wird. Die vorliegende Untersuchung hatte daher zum Ziel aufzuzeigen, wie sehr sich die Behandlungsangebote in Alkoholfachkliniken an den Bedürfnissen alkoholabhängiger Frauen orientieren. Dazu wurde, basierend auf den in der Literatur formulierten Forderungen, die an die Gestaltung der Behandlungsangebote für alkoholabhängige Frauen gestellt wurden, ein Index gebildet, der das Ausmaß der Frauenorientierung der Behandlungsangebote in den Kliniken beschreibt. Die Verwendung des Index erlaubt den direkten Vergleich der verschiedenen Kliniken. In die Untersuchung gingen Daten aus 27 zufällig ausgewählten Fachkliniken ein. Die Kliniken unterscheiden sich deutlich hinsichtlich des Ausmaßes der Frauenorientierung der Behandlungs-
angebote.
This paper investigates DevOps adoption, emphasizing its cultural dimensions in the context of the information systems domain. Utilizing a robust systematic literature review methodology, we discerned nine salient success factors and delineated three stage models instrumental for a smooth transition into DevOps-centric projects. Despite the increasing breadth of research in the DevOps area, the absence of a consolidated definition remains a challenge, underscoring the importance of a unified interpretative framework. This research notably extends the current body of knowledge by focusing on the cultural dynamics of DevOps adoption. As a result, it offers a refined lens through which both practitioners involved in the adoption process and academic researchers can comprehend the nuances and implications of cultural shifts in the evolving landscape of DevOps within information systems.
Der Weg in eine Professur an einer Hochschule für angewandte Wissenschaften (HAW) setzt eine „Dreifachqualifikation“ in Lehre, Forschung und Berufserfahrung voraus. Dieser hohe Anforderungsstandard limitiert aber nicht nur den Pool geeigneter Bewerber:innen (In der Smitten et al., 2017). Diese Personen sind auch bei Wirtschaftsbetrieben, Universitäten und anderen Organisationen begehrt, so dass sich der „Kampf“ um Talente mit Potenzial für eine HAW-Professur in Zukunft eher verschärft. Wissen um die Karrierewege, förderliche sowie hinderliche Aspekte in HAW-Professuren, können Hochschulen dabei helfen, geeignete Personen auf das Berufsbild aufmerksam zu machen und das Arbeitgebermarketing gegenüber potenziellen Bewerber:innen zu verbessern.
Der Vortrag präsentiert erste Ergebnisse zu den Karrierewegen in eine HAW-Professur und fasst hierfür Erkenntnisse aus einer qualitativen Studie sowie Überlegungen zu deren quantitativen Überprüfung zusammen. Semi-strukturierte Interviews mit Professor:innen, welche einen Ruf an einer HAW in Bayern angenommen hatten, konnten u. a. verschiedene Karrierewege in eine Professur aufzeigen, ebenso wie die Motive, sich für dieses Berufsbild zu entscheiden. Des Weiteren wurden mögliche unterstützenden Faktoren und Hindernisse, welche den Weg in eine HAW-Professur erleichtern oder erschweren können, identifiziert. Zum Abschluss wird ein Ausblick auf eine geplante quantitativen Online-Befragung von HAW-Professor:innen gegeben, um die qualitativen Erkenntnisse zu validieren.
Die Ergebnisse werden unter Bezugnahme auf die relevante Hochschulforschung (z. B. Armutat, 2022; Braun & Wilson, 2023; In der Smitten et al., 2017) ebenso wie relevante Karrieretheorien (z. B. Lent et al., 1994) diskutiert. Die abschließenden Handlungsempfehlungen für die Hochschulentwicklung sowie das Hochschulmarketing sind von praktischer Relevanz.

Customized additively manufactured (laser powder bed fused (L-PBF)) stents could improve the treatment of complex lesions by enhancing stent-artery conformity. However, geometric irregularities inherent for L-PBF stents are expected to influence not only their mechanical behavior but also their interaction with the artery. In this study, the influence of geometrical irregularities on stent-artery interaction is evaluated within a numerical framework. Thus, computed arterial stresses induced by a reconstructed L-PBF stent model are compared to those induced by the intended stent model (also representing a stent geometry obtained from conventional manufacturing processes) and a modified CAD stent model that accounts for the increased strut thickness inherent for L-PBF stents. It was found that, similar to conventionally manufactured stents, arterial stresses are initially related to the basic stent design/topology, with the highest stresses occurring at the indentations of the stent struts. Compared to the stent CAD model, the L-PBF stent induces distinctly higher and more maximum volume stresses within the plaque and the arterial wall. In return, the modified CAD model overestimates the arterial stresses induced by the L-PBF stent due to its homogeneously increased strut thickness and thus its homogeneously increased geometric stiffness compared with the L-PBF stent. Therefore, the L-PBF-induced geometric irregularities must be explicitly considered when evaluating the L-PBF stent-induced stresses because the intended stent CAD model underestimates the arterial stresses, whereas the modified CAD model overestimates them. The arterial stresses induced by the L-PBF stent were still within the range of values reported for conventional stents in literature, suggesting that the use of L-PBF stents is conceivable in principle. However, because geometric irregularities, such as protruding features from the stent surface, could potentially damage the artery or lead to premature stent failure, further improvement of L-PBF stents is essential.
Advances in additive manufacturing enable the production of tailored lattice structures and thus, in principle, coronary stents. This study investigates the effects of process-related irregularities, heat and surface treatment on the morphology, mechanical response, and expansion behavior of 316L stainless steel stents produced by laser powder bed fusion and provides a methodological approach for their numerical evaluation. A combined experimental and computational framework is used, based on both actual and computationally reconstructed laser powder bed fused stents. Process-related morphological deviations between the as-designed and actual laser powder bed fused stents were observed, resulting in a diameter increase by a factor of 2-2.6 for the stents without surface treatment and 1.3-2 for the electropolished stent compared to the as-designed stent. Thus, due to the increased geometrically induced stiffness, the laser powder bed fused stents in the as-built (7.11 ± 0.63 N) or the heat treated condition (5.87 ± 0.49 N) showed increased radial forces when compressed between two plates. After electropolishing, the heat treated stents exhibited radial forces (2.38 ± 0.23 N) comparable to conventional metallic stents. The laser powder bed fused stents were further affected by the size effect, resulting in a reduced yield strength by 41% in the as-built and by 59% in the heat treated condition compared to the bulk material obtained from tensile tests. The presented numerical approach was successful in predicting the macroscopic mechanical response of the stents under compression. During deformation, increased stiffness and local stress concentration were observed within the laser powder bed fused stents. Subsequent numerical expansion analysis of the derived stent models within a previously verified numerical model of stent expansion showed that electropolished and heat treated laser powder bed fused stents can exhibit comparable expansion behavior to conventional stents. The findings from this work motivate future experimental/numerical studies to quantify threshold values of critical geometric irregularities, which could be used to establish design guidelines for laser powder bed fused stents/lattice structures.
In-stent restenosis remains a major problem of arteriosclerosis treatment by stenting. Expansion-optimized stents could reduce this problem. With numerical simulations, stent designs/ expansion behaviours can be effectively analyzed. For reasons of efficiency, simplified models of balloon-expandable stents are often used, but their accuracy must be challenged due to insufficient experimental validation. In this work, a realistic stent life-cycle simulation has been performed including balloon folding, stent crimping and free expansion of the balloon-stent-system. The successful simulation and validation of two stent designs with homogenous and heterogeneous stent stiffness and an asymmetrically positioned stent on the balloon catheter confirm the universal applicability of the simulation approach. Dogboning ratio, as well as the final dimensions of the folded balloon, the crimped and expanded stent, correspond well to the experimental dimensions with only slight deviations. In contrast to the detailed stent life-cycle simulation, a displacement-controlled simulation can not predict the transient stent expansion, but is suitable to reproduce the final expanded stent shape and the associated stress states. The detailed stent life-cycle simulation is thus essential for stent expansion analysis/optimization, whereas for reasons of computational efficiency, the displacement-controlled approach can be considered in the context of pure stress analysis.
Simulation of Fluid-Structure Interaction between injection medium and balloon catheter using ICFD

Arteriosclerosis is a major health issue worldwide. While it is commonly treated by the implantation of an balloon-expandable stent, micro injuries may occur during stent deployment, and induce in-stent restenosis, whose consequence can be fatal. Studying this undesirable phenomenon is usually limited as experimental data is hard to obtain on ethical ground. Numerical simulation are performed to better understand this problem. To construct a more realistic simulation of a balloon-expandable stent, a partitioned strongly-coupled FSI simulation of the balloon deployment was set up using the ICFD solver of LS-DYNA, - a quite innovative approach. The complex balloon configuration as well as the interaction of the injection medium and the balloon structure was considered. The balloon structure consisting of shell elements was obtained from preliminary balloon folding and pleating simulations. The balloon consists of a flexible thin walled polyamide. The injection fluid is implemented using volume elements. Balloon deployment was initiated by a pressure boundary condition inducing a volume flow into the balloon. The initial feasibility analysis showed promising result including a continuous balloon deployment and a reasonable development of the fluid pressure and velocity field. However, applying this FSI approach to a more complex balloon structure led to a non convergent solution. The non-convergence could be mainly reduced to mechanical factors including the low wall thickness of the balloon (< 0.05 mm) and the flexibility of the polyamide. Further, the ICFD solver shows less accuracy concerning the FSI conditions when dealing with thin flexible structures as well as enclosed volumes. A shell thickness of 0.06 mm is believed to result in a convergent solution.
Aim of the study was to develop a standardized model system to investigate endodontic irrigation techniques and assess the efficiency of different activation methods on the removal of hard tissue debris in complex root canal systems. Mesial roots of mandibular molars were firstly scanned by micro-computed tomography (µCT) and allocated to three groups of irrigant activation: sonic activation (EDDY, VDW, Munich, Germany), laser activation (AutoSWEEPS, FOTONA, Ljubljana, Slovenia) and conventional needle irrigation (control). Roots were fixed in individual 3D-printed holders to facilitate root canal enlargement under constant irrigation with NaOCl (5%). To enable standardized quantification of remaining debris, BaSO4-enriched dentine powder was compacted into the canals, followed by another µCT-scan. The final irrigation was performed using 17% ethylenediaminetetraacetic acid (EDTA) and 5% sodium hypochlorite (NaOCl) with the respective activation method, and the volume of remaining artificial debris was quantified after a final µCT-scan. The newly developed model system allowed for reliable, reproducible and standardized assessment of irrigation methods. Activation of the irrigant proved to be significantly more effective than conventional needle irrigation regarding the removal of debris, which persisted particularly in the apical third of the root canal in the control group. The efficiency of irrigation was significantly enhanced with laser- and sonic-based activation, especially in the apical third.
The University of Shanghai for Science and Technology (USST), the Coburg University of Applied Sciences and Arts (CUASA) and the OTH Regensburg, University of Applied Sciences (OTHR) established an English taught international cooperative bachelor program in the area of Engineering Physics/Optoelectronics. Students from China study their first four semesters at USST. They continue their studies in Germany for the last three semesters, including an internship and a bachelor thesis, graduating with a Chinese and a German bachelor degree. Students from Germany study their third and fourth semester at USST to gain international experience. While the first cohort of Chinese students is currently in Germany, the second cohort of German students is in Shanghai. Up to now the feedback regarding this study program is completely positive, thus it is planned to develop it further.
We conceptualize the new phenomenon of the Fractional Chief InformationOfficer (CIO) as a part-time executive who usually works for more than one pri-marily small- to medium-sized enterprise (SME) and develop promising avenuesfor future research on Fractional CIOs. We conduct an empirical study by drawingon semi-structured interviews with 40 individuals from 10 different countries whooccupy a Fractional CIO role. We derive a definition for the Fractional CIO, dis-tinguish it from other forms of employment, and compare it with existing researchon CIO roles. Further, we find four salient engagement types of Fractional CIOsoffering value for SMEs in various situations: Strategic IT management, Restruc-turing, Rapid scaling, and Hands-on support. The results reveal similarities withexisting CIO roles as well as novel insights concerning the different engagementtypes. Lastly, we propose a research agenda for the Fractional CIO field, based onfour research themes derived from existing CIO research and insights from theinterviews.
The paper at hand describes how a regional university of applied sciences tried to internationalize its learning and teaching environment. It describes the challenges encountered, the managerial approach taken, illustrates the implemented initiatives, and how effective they turned out. The results might be relevant to faculty staff in managerial positions at regional universities all over the world that face the challenge to internationalize their teaching and learning environment for the benefit of their domestic students.
Der Freistaat Bayern finanziert seit einigen Jahren ein Hochschulprogramm zur Förderung von internationalen Gastprofessorinnen und Gastprofessoren an seinen Hochschulen. Mit diesen Finanzmitteln fördert die OTH Regensburg unter anderem internationale Gastvorlesungen. In der Fakultät Informatik und Mathematik wurde seit 2015 eine Zusammenstellung internationaler Gastvorlesungen entwickelt, die als Wahlpflichtfächer systematisch in ein bestehendes Curriculum integriert sind. So werden den heimischen Studierenden internationale Erfahrungen ermöglicht ("internationalization at home"), die fachliche Breite des Curriculums wird erhöht und es können auf Basis persönlicher Kontakte internationale Hochschulpartnerschaften neu geknüpft und weiter entwickelt werden. Jede Vorlesung in diesem Programm besteht aus mehreren Teilen. Zum Vorlesungsbeginn werden die geplanten Inhalte und Anforderung den Studierenden in einer Videokonferenz erläutert. Die Studierenden erhalten Unterlagen, um sich in Selbstlerneinheiten auf die Präsenzvorlesungen des Gastes vorzubereiten. Im Präsenzteil verbringt die/der Gastdozierende ca. 2 bis 3 Wochen in Regensburg und hält an der Hochschule Blockvorlesungen. So ist es möglich, Veranstaltungen mit 5 ECTS Credits in einem "blended learning" Lehr-/Lernformat durchzuführen. Studierende schätzen das Programm sehr und äußern sich in Evaluationen äußerst positiv über die fachliche und didaktische Erweiterung des Fächerspektrums. Dieses Programm wird als "good practice" innerhalb der Hochschule angesehen und soll in anderen technischen Fakultäten verbreitet werden, u.a. wurde ein "Prozesshandbuch" mit konkreten Beispielen und Handreichungen erstellt. Der Vortrag geht auf diese und weitere Umsetzungsaspekte sowie zugehörige Herausforderungen ein.
Purpose
In 1969, Germany founded a new type of tertiary education institution, the so-called “Universities of Applied Sciences” (UAS). In contrast to traditional universities, UAS are supposed to educate their students based on scientific methods but with a high degree of application orientation (Wissenschaftsrat, 2010). The goal was to generate highly employable graduates. Since then the number of UAS in Germany has risen to 216 in 2018/2019 (Statistisches Bundesamt, 2019). ~45% of all German students start their tertiary education at a UAS (Autorengruppe Bildungsberichterstattung, 2018). UAS graduates are well perceived by domestic employers, enjoy high recruiting rates and attractive salary levels.
The educational model has proven so successful that its approach and underlying philosophy has been replicated under terms such as “cooperative education” or “applied learning” in a vast number of different countries and educational systems around the globe (Altbach, Reisberg, & Rumbley, 2011).
This talk critically describes the key aspects that constitute German UAS’ approaches to industry-oriented applied learning. The findings are relevant for academics, policy makers, and industry representatives to generate ideas on how to improve study curricula, policies, or collaboration between academia and industry.
Method
The approach taken consists of a descriptive and structured account of the key aspects that constitute the study curricula of a UAS from an applied learning perspective. By building on their own experiences as teaching professors and part of a German UAS’ managerial team, the research method taken resembles an action research-oriented approach.
Key Findings
German UAS have numerous application-oriented learning elements in their curricula. These elements are performed inside the university like lab courses but many of these learning elements are carried out in cooperation with industry.
Bachelor programs span over seven semesters. Fundamentals are taught in the first two semesters and from the third semester on, more and more application-oriented lectures and labs become part of the curricula. In the fifth semester, a mandatory internship is integrated in the study program. This internship is carried out either at companies in Germany or abroad. It is the goal of the internship that the students gain first practical experience in a real work environment. The work tasks in internship should be similar to the later professional demands and should contain a project. The students themselves are responsible for finding an appropriate job and organize the administrative aspects around the internship, thus their self-organization skills are advanced. The UAS teaching staff supervises the internships from an academic perspective. This mandatory internship has numerous advantages: students experience real industry environment and they build contacts to later employers thus forming a potential pathway to future employment.
Bachelor and master courses each finish with thesis work. The majority of theses is carried out in companies under university supervision. This is a second chance for the students to accomplish a company project, to gain practical experience and to get in touch with potential employers.
In addition to undergraduate and postgraduate education, German UAS do a lot of applied research together with companies. This applied research can also be the basis for the work of Ph.D students.
Industry representatives are also involved when designing new study programs as consultants and give advice for accreditation and reaccreditation of programs.
All these activities of German universities of applied sciences lead to a high practical relevance of education programs and to a high degree of employability of UAS graduates.
Objective
The risk of in-stent restenosis can be considerably reduced by stents eluting cytostatic compounds. We created a novel drug-eluting stent system that includes several new features in the rapidly evolving field of stent-based drug delivery.
Methods and Results
The aim of the present study was the preclinical evaluation of a stent-coating system permitting individual, on-site coating of stents with a unique microporous surface allowing for individualizable, dose-adjustable, and multiple coatings with identical or various compounds, designated ISAR (individualizable drug-eluting stent system to abrogate restenosis). Stents were coated with 0.75% rapamycin solution, and high-performance liquid chromatography (HPLC)-based determination of drug release profile indicated drug release for >21 days. Rapamycin-eluting microporous (REMP) stents implanted in porcine coronary arteries were safe. To determine the efficacy of REMP stents, this novel drug-eluting stent platform was compared with the standard sirolimus-eluting stent. At 30 days, in-stent neointima formation in porcine coronary arteries was similar in both groups, yielding a significant decrease of neointimal area and injury-dependent neointimal thickness compared with bare-metal stents.
Conclusion
The ISAR drug-eluting stent platform as a novel concept for stent coating allows for a safe, effective, on-site stent coating process, thus justifying further clinical evaluation to decrease in-stent restenosis in humans.
In-stent neointima formation can be successfully attenuated by drug-eluting stents. We introduce a novel conceptual approach for stent-coating that allows for dose-adjustable, on-site stent coating process if desired with multiple compounds. Microporous stents coated with rapamycin proved safe and effective for the limitation of neointima formation in a porcine coronary stent model.
This paper discusses modeling and nonlinear control of a joint antagonistically actuated by two pneumatic, artificial muscles. A single model of the whole system is obtained by a combined physical and phenomenological modeling approach. The combined model for the joint, the muscles and the proportional valves results in a nonlinear, affine-in-control system description. The model is used to derive control laws for an input/output linearization approach to linearize the plant. Modeling and parametrization errors are covered via an outer control loop consisting of a state-feedback which is extended by an additional feedback of error integral. Extensive experimental results show the quality of the model and the performance of the respective control laws.
Optimum experimental design for extended Gaussian disorder modeled organic semiconductor devices

We apply optimum experimental design (OED) to organic semiconductors modeled by the extended Gaussian disorder model (EGDM) which was developed by Pasveer et al. [Phys. Rev. Lett. 94, 206601 (2005)]. We present an extended Gummel method to decouple the corresponding system of equations and use automatic differentiation to get derivatives with the required accuracy for OED. We show in two examples, whose parameters are taken from Pasveer et al. [Phys. Rev. Lett. 94, 206601 (2005)] and Mensfoort and Coehoorn [Phys. Rev. B 78, 085207 (2008)] that the linearized confidence regions of the parameters can be reduced significantly by applying OED resulting in new experiments with a different setup.
In this paper, we address the problem of retrospective color shading correction. An extension of the established gray-level shading correction algorithm based on signal envelope (SE) estimation to color images is developed using principal color components. Compared to the probably most general shading correction algorithm based on entropy minimization, SE estimation does not need any computationally expensive optimization and thus can be implemented more effciently. We tested our new shading correction scheme on artificial as well as real endoscopic images and observed promising results. Additionally, an indepth analysis of the stop criterion used in the SE estimation algorithm is provided leading to the conclusion that a fixed, user-defined threshold is generally not feasible. Thus, we present new ideas how to develop a non-parametric version of the SE estimation algorithm using entropy.
We present the Regensburg Breast Shape Model (RBSM)—a 3D statistical shape model of the female breast built from 110 breast scans acquired in a standing position, and the first publicly available. Together with the model, a fully automated, pairwise surface registration pipeline used to establish dense correspondence among 3D breast scans is introduced. Our method is computationally efficient and requires only four landmarks to guide the registration process. A major challenge when modeling female breasts from surface-only 3D breast scans is the non-separability of breast and thorax. In order to weaken the strong coupling between breast and surrounding areas, we propose to minimize the variance outside the breast region as much as possible. To achieve this goal, a novel concept called breast probability masks (BPMs) is introduced. A BPM assigns probabilities to each point of a 3D breast scan, telling how likely it is that a particular point belongs to the breast area. During registration, we use BPMs to align the template to the target as accurately as possible inside the breast region and only roughly outside. This simple yet effective strategy significantly reduces the unwanted variance outside the breast region, leading to better statistical shape models in which breast shapes are quite well decoupled from the thorax. The RBSM is thus able to produce a variety of different breast shapes as independently as possible from the shape of the thorax. Our systematic experimental evaluation reveals a generalization ability of 0.17 mm and a specificity of 2.8 mm. To underline the expressiveness of the proposed model, we finally demonstrate in two showcase applications how the RBSM can be used for surgical outcome simulation and the prediction of a missing breast from the remaining one. In order to weaken the strong coupling between breast and thorax, we propose to minimize the variance outside the breast region as much as possible. To achieve this goal, a novel concept called breast probability masks (BPMs) is introduced. A BPM assigns probabilities to each point of a 3D breast scan, telling how likely it is that a particular point belongs to the breast area. During registration, we use BPMs to align the template to the target as accurately as possible inside the breast region and only roughly outside. This simple yet effective strategy significantly reduces the unwanted variance outside the breast region, leading to better statistical shape models in which breast shapes are quite well decoupled from the thorax. The RBSM is thus able to produce a variety of different breast shapes as independently as possible from the shape of the thorax. Our systematic experimental evaluation reveals a generalization ability of 0.17 mm and a specificity of 2.8 mm for the RBSM. Ultimately, our model is seen as a first step towards combining physically motivated deformable models of the breast and statistical approaches in order to enable more realistic surgical outcome simulation.
Whole-body PET/CT imaging

Aim
Combined whole-body (WB) PET/CT imaging provides better overall co-registration compared to separate CT and PET. However, in clinical routine local PET-CT mis-registration cannot be avoided. Thus, the reconstructed PET tracer distribution may be biased when using the misaligned CT transmission data for CT-based attenuation correction (CT-AC). We investigate the feasibility of retrospective co-registration techniques to align CT and PET images prior to CT-AC, thus improving potentially the quality of combined PET/CT imaging in clinical routine.
Methods
First, using a commercial software registration package CT images were aligned to the uncorrected PET data by rigid and non-rigid registration methods. Co-registration accuracy of both alignment approaches was assessed by reviewing the PET tracer uptake patterns (visual, linked cursor display) following attenuation correction based on the original and co-registered CT. Second, we investigated non-rigid registration based on a prototype ITK implementation of the B-spline algorithm on a similar targeted MR-CT registration task, there showing promising results.
Results
Manual rigid, landmark-based co-registration introduced unacceptable misalignment, in particular in peripheral areas of the whole-body images. Manual, non-rigid landmark-based co-registration prior to CT-AC was successful with minor loco-regional distortions. Nevertheless, neither rigid nor non-rigid automatic co-registration based on the Mutual Information image to image metric succeeded in co-registering the CT and noAC-PET images. In contrast to widely available commercial software registration our implementation of an alternative automated, non-rigid B-spline co-registration technique yielded promising results in this setting with MR-CT data.
Conclusion
In clinical PET/CT imaging, retrospective registration of CT and uncorrected PET images may improve the quality of the AC-PET images. As of today no validated and clinically viable commercial registration software is in routine use. This has triggered our efforts in pursuing new approaches to a validated, non-rigid co-registration algorithm applicable to whole-body PET/CT imaging of which first results are presented here. This approach appears suitable for applications in retrospective WB-PET/CT alignment.

Kombinierte PET/CT-Bildgebung ermöglicht verbesserte Koregistrierung von PET- und CT-Daten gegenüber separat akquirierten Bildern. Trotzdem entstehen in der klinischen Anwendung lokale Fehlregistrierungen, die zu Fehlern in der rekonstruierten PET- Tracerverteilung führen können, falls die unregistrierten CT-Daten zur Schwächungskorrektur (AC) der Emissionsdaten verwendet werden. Wir untersuchen daher die Anwendung von Bildregistrierungsalgorithmen vor der CT-basierten AC zur Verbesserung der PET-Aufnahmen.
Methoden
Mittels einer kommerziellen Registrierungssoftware wurden die CT-Daten eines PET/CT- Tomographen durch landmarken- und intensitätsbasierte rigide (starre) und nicht-rigide Registrierungsverfahren räumlich an die unkorrigierten PET-Emissionsdaten angepasst und zur AC verwendet. Zur Bewertung wurden die Tracerverteilungen in den PET-Bildern (vor AC, CT-AC, CT-AC nach Koregistrierung) visuell und mit Hilfe korrelierter Fadenkreuze verglichen. Zusätzlich untersuchten wir die ITK-Implementierung der bekannten B-spline basierten, nicht-rigiden Registrierungsansätze im Hinblick auf ihre Verwendbarkeit für die multimodale PET/CT-Ganzkörperregistrierung.
Ergebnisse
Mittels landmarkenbasierter, nicht-rigider Registrierung konnte die Tracerverteilung in den PET-Daten lokal verbessert werden. Landmarkenbasierte rigide Registrierung führte zu starker Fehlregistrierung in entfernten Körperregionen. Automatische rigide und nicht-rigide Registrierung unter Verwendung der Mutual-Information-Ähnlichkeitsmetrik versagte auf allen verwendeten Datensätzen. Die automatische Registrierung mit B-spline-Funktionen zeigte vielversprechende Resultate in der Anwendung auf einem ähnlich gelagerten CT–MR-Registrierungsproblem.
Fazit
Retrospektive, nicht-rigide Registrierung unkorrigierter PET- und CT-Aufnahmen aus kombinierten Aufnahmensystemen vor der AC kann die Qualität von PET-Aufnahmen im klinischen Einsatz verbessern. Trotzdem steht bis heute im klinischen Alltag keine validierte, automatische Registrierungssoftware zur Verfügung. Wir verfolgen dazu Ansätze für validierte, nicht-rigide Bildregistrierung für den klinischen Einsatz und präsentieren erste Ergebnisse.
Das Coronavirus (SARS-CoV-2) hat die Hochschulen vor ungeahnte Herausforderungen gestellt. Neben der Lehre mussten auch studentische Projekte ad hoc virtuell durchgeführt werden. Das vermeintliche Problem erwies sich aber als „Türöffner“ für eine intensivere und unkompliziertere Zusammenarbeit zwischen Hochschule und externen Auftraggebern. Die spezifischen Erfahrungen der Universität Würzburg und der OTH Regensburg werden vorgestellt. Die neuen Möglichkeiten bereichern zukünftige Lehrveranstaltungen und geben wertvolle Impulse, auch für Projektarbeit außerhalb der Hochschulen. Die beiden Autoren bieten interessierten Unternehmen und Organisationen eine Zusammenarbeit über studentische Projektarbeiten an.

Influence of minimally invasive total hip replacement on hip reaction forces and their orientations

Minimally invasive surgery (MIS) is becoming increasingly popular. Supporters claim that the main advantages of MIS total hip replacement (THR) are less pain and a faster rehabilitation and recovery. Critics claim that safety and efficacy of MIS are yet to be determined. We focused on a biomechanical comparison between surgical standard and MIS approaches for THR during the early recovery of patients. A validated, parameterized musculoskeletal model was set to perform a squat of a 50th percentile healthy European male. A bilateral motion was chosen to investigate effects on the contralateral side. Surgical approaches were simulated by excluding the incised muscles from the computations. Resulting hip reaction forces and their symmetry and orientation were analyzed. MIS THR seemed less influential on the symmetry index of hip reaction forces between the operated and nonoperated leg when compared to the standard lateral approach. Hip reaction forces at peak loads of the standard transgluteal approach were 24% higher on the contralateral side when compared to MIS approaches. Our results suggest that MIS THR contributes to a greater symmetry of hip reaction forces in absolute value as well as force-orientation following THR.
We have developed a novel, computer-assisted operation method for minimal-invasive total hip replacement (THR) following the concept of “femur first/combined anteversion,” which incorporates various aspects of performing a functional optimization of the prosthetic stem and cup position (CAS FF). The purpose of this study is to assess whether the hip joint reaction forces and patient’s gait parameters are being improved by CAS FF in relation to conventional THR (CON). We enrolled 60 patients (28 CAS FF/32 CON) and invited them for gait analysis at three time points (preoperatively, postop six months, and postop 12 months). Data retrieved from gait analysis was processed using patient-specific musculoskeletal models. The target parameters were hip reaction force magnitude (hrf), symmetries, and orientation with respect to the cup. Hrf in the CAS FF group were closer to a young healthy normal. Phase-shift symmetry showed an increase in the CAS FF group. Hrf orientation in the CAS FF group was closer to optimum, though no edge or rim-loading occurred in the CON group as well. The CAS FF group showed an improved hrf orientation in an early stage and a trend to an improved long-term outcome.
Measuring functional outcome after total hip replacement with subject-specific hip joint loading

Total hip replacement is an often-performed orthopedic surgical procedure; the amount of procedures undertaken will increase since our life expectancy is growing. In order to optimize function, hip biomechanics should be restored to as near normal as possible. The goal of this pilot study was to determine whether or not it is feasible to compute the vectorial hip reaction force pathways on the head of the prosthesis and the force angles relative to the cup of the prosthesis that occur during gait in total hip replacement patients, serving as an objective measurement of the functional outcome following hip replacement. A three-dimensional gait analysis, measuring ground reaction forces and kinematics, was performed. The data retrieved from the gait analysis was used as the input for the musculoskeletal model to compute vectorial joint reaction forces for data processing. To evaluate the position and orientation of the joint reaction forces, the force path, as well as the force angles for the operated and non-operated joint, has been calculated during the stance phase of the specific leg. The force path for subject 2 on the non-operated side is only located in the posterior-lateral quarter, as is the force path for subject 1. In contrast to this subject, the force path for subject 2 at the operated hip joint can be found only within the anterior quarter of the head of the implant, where it is nearly equally distributed in the medio-lateral half of the prosthesis head. The force-inclination angles on the cup of subject 1, with respect to the plane of the socket face, indicates that the force vector is mainly positioned in the same quadrant when compared with subject 2 (in a cup-fixed coordinate system). The force-anteversion angle behaves similarly to the force-inclination angle, even when the effects are not as pronounced. The proposed methods in this article are aiming to define two functional outcomes of total hip replacement that are related to wear and rim loading. It is accepted that wear is not only a function of time, but a function of use. Owing to the methods listed in this article, we are able to determine a) the applied force and b) the sliding distance (force pathway) in a subject-specific manner. The computed hip-reaction force angles and the distance to the rim cup are a measurement for cup or rim loading, and occurs in the so-called safe-zones. This method may well give us insight into the biomechanical situation during gait, after receiving total hip replacement, that we need to fully understand the mechanisms acting on a hip joint and to prove a possible increase of functional outcome after receiving total hip replacement.
A prospective randomized controlled trial is presented that is used to compare gait performance between the computer assisted Femur First (CAS FF) operation method and conventional THR (CON). 60 patients underwent a 3D gait analysis of the lower extremity at pre-operative, 6 months post-operative and twelve months post-operative. Detailed verification experiments were facilitated to ensure the quality of data as well as to avoid over-interpreting of the data. The results confirm a similar data-quality as reported in the literature. Walking speed, range of motion and symmetry thereof improved over the follow-up period, without significant differences between the groups. While all parameters do significantly increase over the follow-up period for both groups, there were no significant differences between them at any given time-point. Patients undergoing CAS FF showed a trend to improved hip flexion angle indicating a possible long-term benefit.
Vascular malformations (VMs) are a rare condition. They can be categorized into high-flow and low-flow VMs, which is a challenging task for radiologists. In this work, a very heterogeneous set of MRI images with only rough annotations are used for classification with a convolutional neural network. The main focus is to describe the challenging data set and strategies to deal with such data in terms of preprocessing, annotation usage and choice of the network architecture. We achieved a classification result of 89.47 % F1-score with a 3D ResNet 18.
Different target areas within the concept of combined cup and stem anteversion have been published for total hip arthroplasty (THA). We asked whether component positioning according to eight standard combined anteversion rules is associated with (1) more physiological gait patterns, (2) higher improvement of gait variables and (3) better clinical outcome after THA.In a prospective clinical study, 60 patients received cementless THA through an anterolateral MIS approach in a lateral decubitus position. Six weeks postoperatively, implant position was analysed using 3D-CT by an independent external institute. Preoperatively, six and 12 months postoperatively range of motion, normalized walking speed and hip flexion symmetry index were measured using 3D motion-capture gait analysis. Patient-related outcome measures (HHS, HOOS, EQ-5D) were obtained by an observer blinded to 3D-CT results. Eight combined anteversion definitions and Lewinnek's "safe zone" were evaluated regarding their impact on gait patterns and clinical outcome.Combined cup and stem anteversion according to standard combined anteversion definitions as well as cup placement within Lewinnek's "safe zone" did not influence range of motion, normalized walking speed and/or hip flexion symmetry index six and 12 months after THA. Similarly, increase of gait parameters within the first year after THA was comparable between all eight combined anteversion rules. Clinical outcome measures like HHS, HOOS and EQ-5D did not show any benefit for either of the combined anteversion definitions.Standard combined cup and stem anteversion rules do not improve postoperative outcome as measured by gait analysis and clinical scores within one year after THA.
Static pelvic tilt impacts functional cup position in total hip arthroplasty (THA). In the current study we investigated the effect of kinematic pelvic changes on cup position. In the course of a prospective controlled trial postoperative 3D-computed tomography (CT) and gait analysis before and 6 and 12 months after THA were obtained in 60 patients. Kinematic pelvic motion during gait was measured using Anybody Modeling System. By fusion with 3D-CT, the impact of kinematic pelvic tilt alterations on cup anteversion and inclination was calculated. Furthermore, risk factors correlating with high pelvic mobility were evaluated. During gait a high pelvic range of motion up to 15.6° exceeding 5° in 61.7% (37/60) of patients before THA was found. After surgery, the pelvis tilted posteriorly by a mean of 4.0 ± 6.6° (p < .001). The pelvic anteflexion led to a mean decrease of −1.9 ± 2.2° (p < .001) for cup inclination and −15.1 ± 6.1° (p < .001) for anteversion in relation to the anterior pelvic plane (APP). Kinematic pelvic changes resulted in a further change up to 2.3° for inclination and up to 12.3° for anteversion. In relation to the preoperative situation differences in postoperative cup position ranged from −4.4 to 4.6° for inclination and from −7.8 to 17.9° for anteversion, respectively. Female sex (p < .001) and normal body weight (p < .001) correlated with high alterations in pelvic tilt. Kinematic pelvic changes highly impact cup anteversion in THA. Surgeons using the APP as reference should aim for a higher anteversion of about 15° due to the functional anteflexion of the pelvis during gait.
Offset restoration in total hip arthroplasty (THA) is associated with postoperative range of motion (ROM) and gait kinematics. We aimed to research into the impact of high offset (HO) and standard stems on postoperative ROM. 121 patients received cementless THA through a minimally-invasive anterolateral approach. A 360° hip ROM analysis software calculated impingement-free hip movement based on postoperative 3D-CTs compared to ROM values necessary for activities of daily living (ADL). The same model was then run a second time after changing the stem geometry between standard and HO configuration with the implants in the same position. HO stems showed higher ROM for all directions between 4.6 and 8.9° (p < 0.001) compared with standard stems but with high interindividual variability. In the subgroup with HO stems for intraoperative offset restoration, the increase in ROM was even higher for all ROM directions with values between 6.1 and 14.4° (p < 0.001) compared to offset underrestoration with standard stems. Avoiding offset underrestoration resulted in a higher amount of patients of over 20% for each ROM direction that fulfilled the criteria for ADL (p < 0.001). In contrast, in patients with standard stems for offset restoration ROM did increase but not clinically relevant by offset overcorrection for all directions between 3.1 and 6.1° (p < 0.001). Offset overcorrection by replacing standard with HO stems improved ROM for ADL in a low number of patients below 10% (p > 0.03). Patient-individual restoration of offset is crucial for free ROM in THA. Both over and underrestoration of offset should be avoided.

Was zunächst nur als verheißungsvolle Utopie am Zukunftshorizont der Gentechnik aufschien, ist längst dem bioethischen Diskurs entwachsen und konfrontiert uns mit grundlegenden Fragen unseres menschlichen Selbstverständnisses: die Verbesserung und zunehmende Technisierung des Menschen. In diesem Band wird das Thema Enhancement aus sozialphilosophischer und technikphilosophischer Perspektive behandelt. Thomas Zoglauer zeichnet die wesentlichen ethischen Argumentationslinien dieser Debatte nach und setzt sich kritisch mit dem Enhancement unter den Aspekten der Freiheit, sozialen Gerechtigkeit, Chancengleichheit und menschlichen Wohlergehens auseinander. Karsten Weber zeigt wie das Konzept des Cyborgs von der Science-Fiction-Literatur geprägt wurde und als utopisches Leitbild die Medizin- und Neurotechnik beeinflusst hat. Als Folge dieser Entwicklung beginnen sich die Unterschiede zwischen Mensch und Maschine und die damit verbundenen anthropologischen Dichotomien aufzulösen, was auch Konsequenzen für unser Menschenbild und die normative Bewertung des Enhancements hat.

Die Autoren kommen zu dem Schluss, dass es in absehbarer Zukunft zwar keine „moralische Maschinen“ geben mag, es aber Aufgabe der Maschinenethik sein sollte, Maschinen so zu gestalten, dass sie als quasi-moralische Akteure akzeptiert werden können. Dabei dürfen jedoch jene menschlichen Akteure, die entsprechende Maschinen gestalten, entwickeln, bauen und nutzen, nicht aus ihrer Verantwortung entlassen werden.

Noch immer vollzieht sich Technikentwicklung in erster Linie produktorientiert, Partizipation und Akzeptanz werden wenig berücksichtigt. Dabei hängt der Produkterfolg maßgeblich von der Akzeptanz der Nutzer ab. Grundannahme des folgenden Texts ist, dass bei Einbindung möglichst vieler Stakeholder in die Entwicklung von Technik diese noch vor Markteinführung verbessert werden kann und somit ein größerer Erfolg sowie eine nachhaltige Implementierung zu erwarten ist. Es werden Akzeptanzmodelle vorgestellt, die sich durch eine immer stärkere Einbeziehung der Nutzer auszeichnen. Danach wird ein ethisches Evaluierungsmodell skizziert, das im Kontext altersgerechter Assistenzsysteme bereits umfänglich genutzt wird. Zuletzt wird der Versuch unternommen Akzeptanzmodelle und ethische Evaluierungswerkzeuge zu integrieren, um einen Beitrag zur Entwicklung von besser an die Erwartungen der Stakeholder angepassten altersgerechten Assistenzsystemen zu leisten und dadurch deren Akzeptanz zu erhöhen.
Partizipative Technikgestaltung altersgerechter Assistenzsysteme als Verfahren der angewandten Ethik

Ambient-Assisted-Living- (AAL) bzw. altersgerechte Assistenzsysteme sollen in der Pflege alter und hochbetagter Menschen, aber auch anderer physisch oder psychisch gehandicapter Personen, eingesetzt werden. Damit dieser Einsatz nicht zu sozialen und normativen Konflikten führt, muss die Technik so gestaltet werden, dass die in der Gesellschaft verankerten normativen Prinzipien ebenso wie die subjektiven Erwartungen der prospektiven Anwender adressiert werden. Diese Aufgabe gehört in den Bereich der Technikbewertung und partizipativen Technikgestaltung. Dazu gehören Methoden der ethischen Evaluierung der zu entwickelnden Technik, um Konfliktfelder identifizieren und mögliche Lösungsvorschläge entwickeln zu können, die dann in die Gestaltung der Technik einfließen sollten. Nachdem kurz auf die Herkunft altersgerechter Systeme und auf allgemeine normative Fragen des Einsatzes solcher Systeme eingegangen wurde, werden im Folgenden vier Verfahren zur Technikbewertung und partizipativen Technikgestaltung präsentiert, Bezüge zur Akzeptanzforschung hergestellt sowie kurz die Reichweite und die Grenzen ethischer Evaluierung behandelt.
Ambient Assisted Living- (AAL) bzw. altersgerechte Assistenzsysteme sollen in der Pflege alter und hochbetagter Menschen, aber auch anderer physisch oder psychisch gehandicapter Personen, eingesetzt werden. Dafür müssen diese Systeme hochverlässlich sein, aber gleichzeitig auch Erwartungen erfüllen, die weniger im technischen und stärker im sozialen Bereich angesiedelt sind. Nach einer kurzen Einführung der Geschichte dieser Systeme und der Beschreibung des Stands der Technik soll auf verschiedene Evaluationswerkzeuge eingegangen werden, die dazu genutzt werden können, eine partizipative Technikgestaltung von AAL-Systemen zu unterstützen
Der Einsatz von künstlicher Intelligenz im Gesundheitsbereich verspricht besonders großen Nutzen durch eine bessere Versorgung sowie effizientere Abläufe und bietet damit letztlich auch ökonomische Vorteile. Dem stehen unter anderem Befürchtungen entgegen, dass sich durch den Einsatz von künstlicher Intelligenz das Arzt-Patienten-Verhältnis verändern könnte, Arbeitsplätze gefährdet seien oder die Ökonomisierung des Gesundheitswesens einen weiteren Schub erfahren könnte. Zuweilen wird die Debatte um diese Technologie, zumal in der Öffentlichkeit, emotional und fern sachlicher Argumente geführt. Die Autorinnen und Autoren untersuchen die Geschichte des KI-Einsatzes in der Medizin, deren öffentliche Wahrnehmung, Governance der KI, die Möglichkeiten und Grenzen der Technik sowie Einsatzgebiete, die bisher noch nicht oder nur wenig im Fokus der Aufmerksamkeit waren. Dabei erweist sich die KI als leistungsfähiges Werkzeug, das zahlreiche ethische und soziale Fragen aufwirft, die bei der Einführung anderer Technologien bereits gestellt wurden; allerdings gibt es auch neue Herausforderungen, denen sich Professionen, Politik und Gesellschaft stellen müssen.
Wie Untersuchungen zeigen, beeinträchtigen nicht medizinisch indizierte Verlegungen die Lebens- und Versorgungsqualität Sterbender stark. Das Projekt Avenue-Pal hat das Ziel, Leitlinien zu entwickeln, die diese unnötigen Verlegungen reduzieren sollen. Die hier vorgestellte Begleitstudie untersucht aus ethischer Sicht die im Sterbe- und Verlegungsprozess stattfinden Abläufe sowie deren potenziellen Gestaltungsmöglichkeiten. Mittels einer Literaturrecherche, der Teilnahme an einem Fokusgruppengespräch und einer modifizierten MEESTAR-Befragung werden die Bedingungen für eine optimale Betreuung
Sterbender sowie für ein würdevolles Sterben identifiziert und die aktuellen Abläufe im Rahmen des Verlegungsmanagements vor und während der Implementierung der Leitlinien unter Berücksichtigung der ethischen Dimensionen Selbstverständnis, Selbstbestimmung, Fürsorge, Gerechtigkeit, Privatheit und Teilhabe analysiert. Die Ergebnisse der Untersuchung zeigen, dass Verlegungen v.a. aus Unsicherheiten, mangelndem palliativmedizinischem Wissen oder Personalmangel resultieren und dass die Etablierung der Leitlinien und eines palliativen Konsildienstes besonders die fachliche Kompetenz des Personal stärkt und dadurch unnötige Verlegungen reduziert werden konnten.
One of the aims of technology assessment is to inform policy makers about possible future developments of technology, about their likelihood, and about the most probable impacts on individuals as well as on the fabric of society. Such information could be used to plan and create adequate regulations to minimize negative and maximize positive effects of the corresponding technology. However, certain innovations, which we call bottom up-innovations in this paper, make it quite difficult if not almost impossible to provide information of the kind technology assessment shall produce. After introducing the dichotomy of top down-and bottom up-innovations and bringing these types of innovation in the context of more established notions of innovation on a theoretical level we will use the examples of the quantified self-movement and the idea of participatory sensing to describe bottom up-innovations as they take place in real life. By depicting these examples we will try to identify some of the social consequences of the technology that is used in each of the contexts mentioned above. Finally, we will try to draw some more general conclusions concerning bottom up-innovations and technology assessment and its ability to provide reliable information with regard to possible futures. We shall argue that regulation of technology resulting from bottom up-innovations must go " beyond markets and state ".

zwingt die Medien- und Contentindustrie zur stetigen Bereitstellung
neuer medialer Inhalte; die stetig wachsende Nachfrage zu decken wird
über deren unternehmerischen Erfolg entscheiden. Es wird ein Szenario
entwickelt, in dem die Konsumenten gleichzeitig Produzenten der In-
halte sind und dabei Kommunikationsinfrastrukturen wie Mobiltelefone
nutzen. Es werden die weitreichenden sozialen Konsequenzen dieser
Entwicklung skizziert.

In Deutschland werden 402 medizinische Register geführt, darunter ein Herzklappenregister und ein In-vitro-Fertilisationsregister. Doch nicht alle medizinischen Daten, für die der Bedarf eines Registers identifiziert wurde, sind tatsächlich verfügbar. So wird z.B. seit längerem ein nationales Mortalitätsregister gefordert und im Unterschied zu anderen europäischen Ländern existiert in Deutschland kein Impfregister. Dabei sind Gesundheitsdaten unerlässlich für medizinische Forschung, Versorgungsforschung oder Epidemiologie; deren Verfügbarkeit kann zur bedarfsgerechten Gesundheitsversorgung beitragen. Ein Element, um die aktuelle Situation zu verbessern, stellt die Nutzung von Daten der (einrichtungsübergreifenden) elektronischen Patientenakte (ePA) in Forschungsdatenzentren dar, ein anderes die Nutzbarkeit medizinischer Register.
Um belastbare Aussagen über die Bereitschaft der bundesdeutschen Bevölkerung treffen zu können, ob und welche Gesundheitsdaten für Forschungszwecke in Registern freigegeben würden, wurde im Zeitraum vom 01.06. bis zum 27.06.2022 eine computergestützte Telefonbefragung (Dual-Frame) bei einer Zufallsstichprobe der deutschen erwachsenen Gesamtbevölkerung durchgeführt (n=1.308). Der Fragebogen bestand aus 46 standardisierten Fragen. Die Telefonbefragung stellt einen Bevölkerungsquerschnitt dar; im Gegensatz zu Online-Befragungen ermöglicht dies, auch Personen mit geringer Internetaffinität einzubeziehen.
Der Beitrag fasst sozialwissenschaftliche Erkenntnisse zur Impfbereitschaft und Impfung mit einem COVID-19 Vakzin zusammen. Diskutiert werden Impfskepsis und Einflussfaktoren wie Sozialstruktur, Verschwörungsüberzeugungen, kollektive Verantwortung, Einstellungen zu alternativen Heilverfahren und zur Impfpflicht.
Geocaching und Raumnutzung

Geocaching ist eine Art internetbasierte Schnitzeljagd, die im öffentlichen Raum stattfindet. Der Artikel erläutert die Funktionsweise des Geocaching als Freizeitbeschäftigung und beschreibt anhand einer Datenerhebung die Verbreitung in Deutschland. Mit den stark steigenden Teilnehmerzahlen gehen Konflikte bei der Nutzung des öffentlichen Raums – insbesondere sind hiervon naturnahe Räume wie Wälder betroffen – und von Privatbesitz einher. Die Konfliktfelder werden exemplarisch aufgeführt, die Herausforderungen für die angewandte Geographie dargelegt. Durch die Anwendung neuer Technologien wie des GPS im Freizeitbereich bildet sich eine neue Form der Raumnutzung heraus. Damit verbunden sind für die angewandte Geographie noch weitgehend ungenutzte Möglichkeiten der Stadtentwicklung, des Regionalmanagements oder der Bildung. Zugleich treten mit der zunehmenden Verbreitung des Geocaching neue Konfliktfelder der Raumnutzung offen zu Tage.
Medienkonvergenz fördert die Entwicklung neuer Anbieter, die sich nicht nur als Konkurrenz, sondern als Antithese zu traditionellen Massenmedien sehen. Ihnen wird die Hoffnung entgegengebracht, zur Integration der medial vermittelten Öffentlichkeit beizutragen, andererseits existieren Befürchtungen, dass sie die bereits bestehende Fragmentierung der Öffentlichkeit verstärken. Am Beispiel der politischen Kommunikation in konvergenten Medien wird die Plausibilität entsprechender Prognosen untersucht.
