Ungewollte Kinderlosigkeit in Zeiten digitaler Informations-, Kommunikationsund Interaktionsangebote

Der Beitrag stellt die Ergebnisse der standardisierten Akzeptanzbefragungen vor, die im Rahmen der Feldstudie des Forschungsprojektes Dein Haus 4.0 Oberpfalz durchgeführt wurden. Schlaganfallbetroffene Personen wurden zu verschiedenen Aspekten der Akzeptanz, Nutzungsbereitschaft und des Nutzens der getesteten Telepräsenzrobotik in einer Längsschnittstudie befragt.
Wie verändert sich das individuelle und gesellschaftliche Verständnis von Gesundheit und Krankheit durch die Einführung neuer digitaler Technologien? Die Beiträger*innen berücksichtigen technische Aspekte, fokussieren jedoch vor allem darauf, dass die Bestimmung und Grenze zwischen Gesundheit und Krankheit immer stärker auf Quantifizierung oder Daten beruht. Das zieht Veränderungen im Verhältnis von Patient*innen und Ärzt*innen nach sich, betrifft aber auch das Selbstverhältnis zum eigenen Körper und wirft die Frage auf, wie gesundheitsbezogene Ressourcen zukünftig zugeteilt werden. Der interdisziplinäre Blick auf diese Themen bietet sowohl für Gesundheitstheorie als auch -praxis wertvolle Anschlussmöglichkeiten und legt gemeinsame Bezugspunkte offen.

GLOBE Tabletop - Capability Development through Pre-Exercise Learning for Humanitarian Missions

Simulation exercises play a significant role in training for humanitarian missions and provide suitable space for appropriate preparation of humanitarian personnel. It was observed that participants in the GLOBE simulation, a humanitarian crisis simulation exercise, lacked fundamental knowledge concerning structure and process organization, situational awareness, and complex planning tasks. Thus, this paper explores the concept of capability development through pre-exercise learning, suggests improvements of the efficacy and effectiveness of learning for individuals engaged in humanitarian mission simulations, and provides implications for improving the overall effectiveness and outcomes of simulation exercises. The conceptualization of pre-exercise learning with gamification and tabletop exercise elements prior to participation in the simulation exercise is expected to lead to enhanced capability development during the simulation of humanitarian missions.
Assessing Heuristic Evaluation in Immersive Virtual Reality: A Case Study on Future Guidance Systems

A variety of evaluation methods for user interfaces (UI) exist such as usability testing, cognitive walkthrough, and heuristic evaluation. However, UIs such as guidance systems at transit hubs must be evaluated in their intended application field to allow the effective and valid identification of usability flaws. However, what if evaluations are not feasible in real environments, or laboratorial conditions cannot be ensured? Based on adapted heuristics, in the present study, the method of heuristic evaluation is combined with immersive Virtual Reality (VR) for the identification of usability flaws of dynamic guidance systems (DGS) at transit hubs. The study involved usability evaluations of nine DGS concepts using the newly proposed method. The results show that compared to computer-based heuristic evaluations, the use of immersive VR led to the identification of an increased amount of “severe” usability flaws as well as overall usability flaws. Within a qualitative assessment, immersive VR is validated as a suitable tool for conducting heuristic evaluations involving significant advantages such as the creation of realistic experiences in laboratorial conditions. Future work seeks to further prove the suitability of using immersive VR for heuristic evaluations and compare the proposed method to other evaluative methods.
Erworbene Sprach- oder Sprechstörungen haben nicht nur Konsequenzen für die Patient*innen wegen ihrer beeinträchtigten Sprach- oder Sprechleistungen, sondern wirken sich auch auf ihr gesamtes soziales Umfeld aus. Denn Kommunikationsschwierigkeiten führen oft zu Veränderungen der Selbstidentität und der Beziehungen der Betroffenen. Es kann zu Resignation, Frustration, Vermeidungsverhalten gegenüber kommunikativen Situationen und sozialem Rückzug kommen. Aphasien und Dysarthrien betreffen damit in der sozialen Teilhabe Unterhaltungen im Familien- und Freundeskreis, am Arbeitsplatz oder in der Öffentlichkeit. Um die Veränderungen erfassen zu können, werden ein personzentriertes Verständnis der Sprech- und Sprachstörungen sowie eine genaue Erfassung der Symptomatik und der daraus resultierenden Einschränkungen benötigt. Dazu steht eine Reihe von Testverfahren zur Verfügung, die verschiedene Parameter erfassen. Die personzentrierte Therapie bezieht sich gleichermaßen auf Körperfunktion, Aktivitäten und Teilhabe sowie auf Umweltfaktoren. Neben der Aufgabe, die Erwartungen, Hoffnungen und Wünsche der Patient*innen wahrzunehmen und zu verstehen, gilt es, Therapieansätze der jeweils bestehenden Symptomatik und den Zielen anzupassen und – auch im Zuge der zunehmenden Forderung nach evidenzbasierten Behandlungsmethoden – kritisch zu hinterfragen.
BACKGROUND
IL-6 is a pleiotropic cytokine modulating inflammation and metabolic pathways. Its proinflammatory effect plays a significant role in organ failure pathogenesis, commonly elevated in systemic inflammatory conditions. Extracorporeal blood purification devices, such as the Advanced Organ Support (ADVOS) multi hemodialysis system, might offer potential in mitigating IL-6's detrimental effects, yet its efficacy remains unreported.
METHODS
We conducted a proof-of-concept in vitro study to assess the ADVOS multi system's efficacy in eliminating IL-6. Varying concentrations of IL-6 were introduced into a swine blood model and treated with ADVOS multi for up to 12 h, employing different blood and concentrate flow rates. IL-6 reduction rate, clearance, and dynamics in blood and dialysate were analyzed.
RESULTS
IL-6 clearance rates of 0.70 L/h and 0.42 L/h were observed in 4 and 12-h experiments, respectively. No significant differences were noted across different initial concentrations. Reduction rates ranged between 40 and 46% within the first 4 h, increasing up to 72% over 12 h, with minimal impact from flow rate variations. Our findings suggest that an IL-6-albumin interaction and convective filtration are implicated in in vitro IL-6 elimination with ADVOS multi.
CONCLUSIONS
This study demonstrates for the first time an efficient and continuous in vitro removal of IL-6 by ADVOS multi at low blood flow rates. Initial concentration-dependent removal transitions to more consistent elimination over time. Further clinical investigations are imperative for comprehensive data acquisition.
Electromyostimulation (EMS) is an up-and-coming training method that demands further fundamental research regarding its safety and efficacy. To investigate the influence of different stimulation parameters, electrode positions and electrode sizes on the resulting voltage in the tissue, a tissue mimicking phantom is needed. Therefore, this study describes the fabrication of a hydrogel arm phantom for EMS applications with the tissue layers of skin, fat, blood and muscle. The phantom was dielectrically validated in the frequency range of 20 Hz to 100 Hz. We also conducted electromyography (EMG) recordings during EMS on the phantom and compared them with the same measurements on a human arm. The phantom reproduces the dielectric properties of the tissues with deviations ranging from 0.8% to more than 100%. Although we found it difficult to find a compromise between mimicking the permittivity and electrical conductivity at the same time, the EMS-EMG measurements showed similar waveforms (1.9-9.5% deviation) in the phantom and human. Our research contributes to the field of dielectric tissue phantoms, as it proposes a multilayer arm phantom for EMS applications. Consequently, the phantom can be used for initial EMS investigations, but future research should focus on further improving the dielectric properties.

Inhalte/Trends
Wie man Personal abbaut, muss zum Unternehmen passen. So bevorzugen kleinere Betriebe teilweise andere Maßnahmen als größere Unternehmen. Bei konkretem Abbaubedarf präferieren die Befragten generell eher Austritte und Abfindungen, die individuell und freiwillig vereinbart werden.
Alle Befragten können sich bei einem Personalabbau in die Ängste von betroffenen und verbleibenden Mitarbeiter*innen hineindenken. Doch die Sorgen werden nicht „behandelt“. Nur ein Teil der Unternehmen finanziert die psychologische Betreuung Betroffener oder ein Outplacement bzw. stellt ein Budget bereit, um zukünftige Leistungsträger zu halten.
Personalabbauprogramme dienen aus Sicht der Geschäftsführungen der Zukunftssicherung des Unternehmens. Die wirtschaftliche Motivation für einen Personalabbau im Sinne von Kosteneinsparungen ist auch den Betriebsräten und Personalabteilungen bekannt. Personalabteilungen richten ihren strategischen Blick allerdings vor allem auf eine zukunftsgerichtete Gestaltung der Personalstruktur.
Geltungsbereich
Für die Umfrage wurden 192 Personen online befragt.
War and armed conflict have not traditionally been central topics in social work. The globalisation of armed conflicts and the internationalisation of social work have made issues of international conflicts and their human consequences a salient topic confronting social work with new theoretical, political and practical problems. Three of those will be highlighted:
1.
The issue of fundamental changes in social welfare, and—in the wake of it—in social work globally over past decades and their impact on post-conflict social policy and social work.
2.
The problems social work and social work education faced in their grassroots work in neoliberal post-conflict reconstruction or liberal peace.
3.
The need for social work and social work education to confront issues of international politics and the inside of violent conflicts when engaging in post-conflict social work.
Dieser Artikel untersucht die Machbarkeit und Wirksamkeit einer physiotherapeutischen Tele-Intervention bei Schlaganfallpatient*innen. Die prospektive, nicht kontrollierte Interventionsstudie basiert auf einem Mixed-Methods-Design und analysiert funktionelle Veränderungen sowie die Lebensqualität der Teilnehmenden vor und nach der Intervention. Eingeschlossen wurden postakute und chronische Schlaganfallpatient*innen mit stabilen Krankheitsbildern, die in ihrem häuslichen Umfeld lebten. Die Intervention bestand aus videobasierten Einzelsitzungen und einer optionalen Gruppenintervention, die Physiotherapie und Logopädie kombinierte. Die Ergebnisse zeigten signifikante Verbesserungen in der Arm-Hand-Funktion und der Teilhabefähigkeit. Die Teletherapie erwies sich als eine praktikable Ergänzung zur traditionellen Rehabilitation, insbesondere in ländlichen Regionen, und förderte die Autonomie und das aktive Engagement der Patient*innen in ihrem Genesungsprozess. Die Studie belegt das Potenzial teletherapeutischer Maßnahmen zur Überbrückung von Versorgungslücken und zur Sicherstellung einer kontinuierlichen, hochfrequenten Rehabilitation.
Introduction
About 40 % of the women worldwide suffer from Pelvic Floor Dysfunctions [1]. In previous studies, musculoskeletal simulations helped to understand and facilitate the complex issues of musculoskeletal disorders in several different branches [2, 3]. A fundamental prerequisite for the generation of these models is to have a basic understanding of the stresses acting on the pelvic floor. The aim of this study is to investigate the biomechanical factors influencing the activity of the pelvic floor muscles by establishing a parameterised model.
Methods
To identify the factors that affect the pelvic floor muscles, the process is divided into several stages. The first step is to investigate the loads acting on the pelvic floor. For this, a simplified sagittal cross-sectional model of the upper body is analysed. The discretization of the abdominal cavity is presented with resulting force vectors representing the organs in the abdominal and pelvic region, the tensile force of the structures that fixate the organs as well as the intraabdominal pressure.
A second step is the examination of the basic mechanics of the pelvic floor muscles. As the functional anatomy of these muscles demonstrate differences compared to other muscle groups [4], the understanding of how they differ is crucial for the construction of musculoskeletal models.
After a static examination of loads on a simplified muscle model, an extended model including the physiological properties of a muscle must be involved in the considerations. Therefore, the influence of parameters such as the ideal muscle strength and fibre length, as well as elasticity or passive stiffness of the elements must be taken into account. The influencing factors of the variables can be analysed in a simplified pelvic floor muscle model by systematically changing the parameters.
Results
The process of analysing the factors influencing the activity of the pelvic floor muscles follows two main strategies. The review of the effects of the whole body identifies the forces acting on the pelvic floor muscles and effect their activity.
With the examination of the basic mechanics of a simplified pelvic floor muscle model a better understanding is achieved of how the pelvic floor muscles absorb the loads acting in the body.
Discussion
This process results in the main influence factors on the activity of the pelvic floor. The consequent parameters form the basis for a biomechanically justified construction of musculoskeletal simulation models of the pelvic floor. Nevertheless, it must be considered, that these mechanical studies represent simplifications of the reality.

Workflow for the development of a non-invasive feedback device to assess pelvic floor contractions

Introduction
Pelvic floor disorders affect about 40% of women worldwide [1]. Pelvic floor muscle (PFM) training is both a preventive and a therapeutic intervention. Current PFM training devices are invasive and have little scientific evidence. The idea is to develop a noninvasive feedback device to assess adequate PFM contraction. Therefore, evidence-based female musculoskeletal models, non-invasive data acquisition, sensor technology and artificial intelligence (AI) will be combined. This work presents the workflow to achieve such a feedback device and describes the interaction of the technologies used.
Methods
Exercises that induce PFM contractions have been evaluated and defined. Motion capture of these exercises will provide input for female musculoskeletal models. A combination of biomechanical rigid body and FEM simulations will be used to estimate PFM contractions. In addition, a non-invasive sensor will measure pelvic floor activity. The simulated and measured data will be used to develop an AI model that provides feedback on PFM contractions based on non-invasive data collection.
Results
The AMMR (AnyBody Managed Model Repository) of the AnyBody modelling system (AMS, Aalborg, Denmark) serves as the initial model for performing inverse dynamic simulations of the exercises. To calculate the PFM forces, the full-body model must be supplemented with the relevant pelvic floor structures and a mass model of the internal organs. A modified abdominal pressure model must also be incorporated. The AMS calculates the PFM activities caused by the internal organ loads and the generated abdominal pressure during the exercises. The muscle activities are transferred to a FEM model of the female pelvic floor (SfePy, simple finite elements in Python). The identical pelvic floor structures were integrated into the FEM model as in the AMS. Active PFM contractions can be simulated using the FEM model. Movement of the coccyx due to PFM contractions has been reported in the literature [2,3]. Therefore, a noninvasive coccyx motion sensor will be developed to provide additional information on PFM contractions. The measured data (coccyx motion sensor, motion capture) and the simulation results of the models will be combined to create an AI feedback model using Python. The final feedback device will consist of the AI model and the developed coccyx motion sensor, which can reproduce the resulting PFM contractions based on the sensor data and simplified motion tracking.
Discussion
The creation of the AMS and the FEM model is a prerequisite for the development of the feedback device. The relevant structures in the models are located inside the body. This limits the ability to observe the structures during the exercises, which can lead to difficulties in model validation. The development of a user-friendly sensor with sufficient measuring accuracy of the coccyx motion is another challenge. Nevertheless, the workflow represents a promising approach to develop a noninvasive feedback system to assess PFM contraction.

Introduction
Dislocation of the shoulder joint is one of the more common complications after reverse total shoulder arthroplasty [1], which is often associated with malposition of the prosthetic components [2]. Therefore, achieving sufficient shoulder stability should not be neglected when positioning the implant components. One parameter for assessing shoulder stability can be shoulder stiffness. The aim of this work is to develop a reverse shoulder implant prototype that allows intraoperative measurement of shoulder stiffness while varying the position of the implant components. The measured stiffness could provide a quantitative statement regarding the optimal positioning of the implant components, which can be adjusted accordingly in the final reverse shoulder prosthesis.
Methods
To measure the stiffness of the shoulder joint, it is necessary to record the joint angles and the torques generated during movement. The changes in the rotation angles were measured using 3D hall sensors and magnets. The magnets were placed under the humerosocket, and the hall sensors were integrated into the glenosphere. The strength of the magnetic field was used to determine the position of the humerosocket in relation to the glenosphere. The accuracies of the angle measurements were tested using a test bench.
Three thin film pressure sensors were used to record forces at different points under the humerosocket. To obtain a force value from the sensor signal, the sensors were calibrated using a load cell. The variation of the implant components positions was integrated into the prototype implant through different constructive mechanisms to adjust the stiffness of the shoulder joint.
Results
In the range of ±45° flexion/extension combined with ±15° adduction/abduction, the joint position could be determined with sufficient accuracy (error e ≤ 5°). The areas near the combined maximum deflections of ±45° flexion/extension and ±45° adduction/abduction indicate the greatest deviation from the target angle. The force values of the thin film sensors enable the calculation of moments around two axes. As variable component position parameters, the tray offset, the neck-shaft angle and the humerus version were integrated into the implant prototype.
Discussion
Ideally, the accuracy of the angle measurements should only depend on the amount of deflection and not on the direction of deflection. The asymmetric behavior indicates a deviation from the correct positioning of the hall sensors. The application of a calibration matrix could compensate for the measurement errors and could demonstrate the potential of the new method for joint angle measurements. The accuracy of the torque measurements and the functionality of the mechanical arresting mechanisms must be investigated in further studies. Overall, the developed measurement method can help to avoid malpositioning of the implant components in reverse total shoulder arthroplasty.

Dieser Artikel untersucht die Machbarkeit und Wirksamkeit einer physiotherapeutischen Tele-Intervention bei Schlaganfallpatient*innen. Die prospektive, nicht kontrollierte Interventionsstudie basiert auf einem Mixed-Methods-Design und analysiert funktionelle Veränderungen sowie die Lebensqualität der Teilnehmenden vor und nach der Intervention. Eingeschlossen wurden postakute und chronische Schlaganfallpatient*innen mit stabilen Krankheitsbildern, die in ihrem häuslichen Umfeld lebten. Die Intervention bestand aus videobasierten Einzelsitzungen und einer optionalen Gruppenintervention, die Physiotherapie und Logopädie kombinierte. Die Ergebnisse zeigten signifikante Verbesserungen in der Arm-Hand-Funktion und der Teilhabefähigkeit. Die Teletherapie erwies sich als eine praktikable Ergänzung zur traditionellen Rehabilitation, insbesondere in ländlichen Regionen, und förderte die Autonomie und das aktive Engagement der Patient*innen in ihrem Genesungsprozess. Die Studie belegt das Potenzial teletherapeutischer Maßnahmen zur Überbrückung von Versorgungslücken und zur Sicherstellung einer kontinuierlichen, hochfrequenten Rehabilitation.
Polyetheretherketone is a promising material for implants due to its good mechanical prop-
erties and excellent biocompatibility. Its accessibility to a wide range of applications is facilitated by the ability to process it with an easy-to-use manufacturing process such as fused filament fabrication. The elimination of disadvantages associated with the manufacturing process, such as a poor surface quality, is a main challenge to deal with. As part of the mass finishing process, centrifugal disc finishing has demonstrated good results in surface optimization, making it a promising candidate for the post-processing of additively manufactured parts. The objective of this study is to identify the key parameters of the centrifugal disc finishing process on the waviness of additively manufactured PEEK specimens, which has not been investigated previously. The waviness of the specimen was
investigated by means of confocal laser scanning microscopy (CLSM), while weight loss was additionally tracked. Six parameters were investigated: type, amount and speed of media, use of compound, amount of water and time. Type of media, time and speed were found to significantly influence waviness reduction and weight loss. Surface electron microscopy images demonstrated the additional effects of deburring and corner rounding. Results on previous studies with specimens made of metal showed similar results. Further investigation is required to optimize waviness reduction and polish parts in a second post-processing step.
Neutrophil extracellular traps (NETs) have recently emerged as a potential link between inflammation, immunity, and thrombosis, as well as other coagulation disorders which present a major challenge in the context of extracorporeal membrane oxygenation (ECMO). By examining blood from ECMO patients for NETs and their precursors and correlating them with clinical and laboratory biomarkers of coagulation and inflammation, this study aims to evaluate the association between the presence of NETs in the bloodstream of ECMO patients and the development of potentially severe coagulation disorders during ECMO therapy.
Therefore, blood samples were collected from healthy volunteers (n=13) and patients receiving veno-venous (VV) ECMO therapy (n=10). To identify NETs and their precursors, DNA and myeloperoxidase as well as granulocyte marker CD66b were visualized simultaneously by immunofluorescence staining in serial blood smears. Differentiation of DNA-containing objects and identification of NETs and their precursors was performed semiautomatically by a specific algorithm using the shape and size of DNA staining and the intensity of MPO and CD66b signal.
Neutrophil extracellular traps and their precursors could be detected in blood smears from patients requiring VV ECMO. Compared to volunteers, ECMO patients presented significantly higher rates of NETs and NET precursors as well as an increased proportion of neutrophil granulocytes in all detected nucleated cells. A high NET rate prior to the initiation of ECMO therapy was associated with both increased iL-6 and TNF-α levels as an expression of a high cytokine burden. These patients with increased NET release also presented an earlier and significantly more pronounced decrease in platelet counts and ATIII activity following initiation of therapy compared with patients with less elevated NETs. These findings provide further indications for the development of immune-mediated acquired thrombocytopenia in ECMO patients.

Die ePA ist bei Haushalten mit Kindern mehrheitlich unbekannt. Ihre Nutzungsbereitschaft der ePA und die Zustimmung zur Speicherung von Gesundheitsdaten ist geringer, aber sie sind häufiger als Haushalte ohne Kinder bereit zur Freigabe ihrer Gesundheitsdaten für die Forschung.
Schlussfolgerung
Es besteht Aufklärungsbedarf über die Speicherung von Gesundheitsdaten. Gewünscht wird zunehmend eine digitale Arzt-Patienten-Kommunikation.
The success of surgical treatment for fractures hinges on various factors, notably accurate surgical indication. The process of developing and certifying a new osteosynthesis device is a lengthy and costly process that requires multiple cycles of review and validation. Current methods, however, often rely on predecessor standards rather than physiological loads in specific anatomical locations. This study aimed to determine actual loads experienced by an osteosynthesis plate, exemplified by a standard locking plate for the femoral shaft, utilizing finite elements analysis (FEA) and to obtain the bending moments for implant development standard tests. A protocol was developed, involving the creation and validation of a fractured femur model fixed with a locking plate, mechanical testing, and FEA. The model’s validation demonstrated exceptional accuracy in predicting deformations, and the FEA revealed peak stresses in the fracture bridging zone. Results of a parametric analysis indicate that larger fracture gaps significantly impact implant mechanical behavior, potentially compromising stability. This study underscores the critical need for realistic physiological conditions in implant evaluations, providing an innovative translational approach to identify internal loads and optimize implant designs. In conclusion, this research contributes to enhancing the understanding of implant performance under physiological conditions, promoting improved designs and evaluations in fracture treatments.

In der Physiotherapie existieren zahlreiche Assessments zur Befunderhebung der unteren Extremität bei Kniepatient*innen. Damit können praktizierende Physiotherapeut*innen das Problem der Patient*innen genauer erfassen und dementsprechend die Behandlung aufbauen. Das Ergebnis der Beurteilung einer Patient#ast#in hängt oftmals stark von den Bewertenden ab. Fragestellung: Deshalb überprüft die Studie die Hypothese, ob innerhalb einer Tester*innengruppe mehr gleiche Ausgleichsbewegung bei der Befundung des Sit-to-Stand (STS) anhand von Videomaterial erkannt werden, als es der Standard erwarten würde.
Methode:
In dieser beobachtenden klinischen Querschnittsstudie wurde die Vergleichbarkeit der Befundergebnisse von 4 Therapeut*innen zum abgewandelten STS untersucht. Die zu beurteilenden Proband*innen wurden während der Durchführung des abgewandelten STS standardisiert in der Frontal- und Sagittalebene gefilmt und mittels eines Fragebogens von Berufsanfänger*innen der Physiotherapie beurteilt. Bei dem erstellten Fragebogen handelte es sich um eine systematische Abfrage aller möglichen Bewegungen der großen Gelenke der unteren Extremität mit dichotomen Antwortmöglichkeiten. Eingeschlossen wurden Proband*innen mit oder ohne diagnostizierten Knieproblemen, die älter als 18 Jahre und deutschsprachig sind. Proband*innen mit einschränkenden Nebenerkrankungen wie z.B. neurologische Erkrankungen und Operationen in den letzten 6 Monaten wurden ausgeschlossen. Anschließend wurden die Antworten der Therapeut*innen auf die Übereinstimmung der Ergebnisse überprüft.
Ergebnisse:
Die vollständigen Ergebnisse werden auf dem Poster nachgereicht. Die Interrater-Reliabilität wird mit Hilfe des Fleiss’ Kappa bestimmt. Voraussichtlich werden 25 Proband*innen in die Studie eingeschlossen.

Mittels der Ergebnisse soll auf die Interrater-Reliabilität von Bewegungsbefunden des abgewandelten STS bei Kniepatient*innen geschlossen werden
Using markerless motion capture and musculoskeletal models: An evaluation of joint kinematics

BACKGROUND: This study presents a comprehensive comparison between a marker-based motion capture system (MMC) and a video-based motion capture system (VMC) in the context of kinematic analysis using musculoskeletal models. OBJECTIVE: Focusing on joint angles, the study aimed to evaluate the accuracy of VMC as a viable alternative for biomechanical research.
METHODS: Eighteen healthy subjects performed isolated movements with 17 joint degrees of freedom, and their kinematic data were collected using both an MMC and a VMC setup. The kinematic data were entered into the AnyBody Modelling System, which enables the calculation of joint angles. The mean absolute error (MAE) was calculated to quantify the deviations between the two systems.
RESULTS: The results showed good agreement between VMC and MMC at several joint angles. In particular, the shoulder, hip and knee joints showed small deviations in kinematics with MAE values of 4.8∘, 6.8∘ and 3.5∘, respectively. However, the study revealed problems in tracking hand and elbow movements, resulting in higher MAE values of 13.7∘ and 27.7∘. Deviations were also higher for head and thoracic movements.
CONCLUSION: Overall, VMC showed promising results for lower body and shoulder kinematics. However, the tracking of the wrist and pelvis still needs to be refined. The research results provide a basis for further investigations that promote the fusion of VMC and musculoskeletal models.

The aim of this review was to provide a general overview of the current state oftechnology used to assess pelvic floor functionality. It also provides literature research of the phys-iological and anatomical factors that correlate with pelvic floor health. The systematic review wasconducted according to the PRISMA guidelines. PubMed, ScienceDirect, Cochrane Library andIEEE databases were searched for publications on sensor technology for the assessment of pelvicfloor functionality. Anatomical and physiological parameters were identified through a manualsearch. In the systematic review 115 publications were included. 12 different sensor technologieswere identified. Information on the obtained parameters, sensor position, test activities and subjectcharacteristics were prepared in tabular form from each publication. 16 anatomical and physiologi- cal parameters influencing pelvic floor health were identified in 17 published studies and rankedfor their statistical significance. Taken together, this review could serve as a basis for the develop-ment of novel sensors which could allow for quantifiable prevention and diagnosis, as well as par-ticularized documentation of rehabilitation processes related to pelvic floor dysfunctions.

Extracorporeal membrane oxygenation (ECMO) was established as a treatment for severe cardiac or respiratory disease. Intra-device clot formation is a common risk. This is based on complex coagulation phenomena which are not yet sufficiently understood. The objective was the development and validation of a methodology to capture the key properties of clots deposed in membrane lungs (MLs), such as clot size, distribution, burden, and composition. One end-oftherapy PLS ML was examined. Clot detection was performed using multidetector computed tomography (MDCT), microcomputed tomography (μCT), and photography of fiber mats (fiber mat imaging, FMI). Histological staining was conducted for von Willebrand factor (vWF), platelets (CD42b, CD62P), fibrin, and nucleated cells (4′, 6-diamidino-2-phenylindole, DAPI). The three imaging
methods showed similar clot distribution inside the ML. Independent of the imaging method, clot loading was detected predominantly in the inlet chamber of the ML. The μCT had the highest accuracy. However, it was more expensive and time consuming than MDCT or FMI. The MDCT detected the clots with low scanning time. Due to its lower resolution, it only showed clotted areas but not
the exact shape of clot structures. FMI represented the simplest variant, requiring little effort and resources. FMI allowed clot localization and calculation of clot volume. Histological evaluation indicated omnipresent immunological deposits throughout the ML. Visually clot-free areas were covered with leukocytes and platelets forming platelet-leukocyte aggregates (PLAs). Cells were embedded in vWF cobwebs, while vWF fibers were negligible. In conclusion, the presented
methodology allowed adequate clot identification and histological classification
of possible thrombosis markers such as PLAs.

Background:
Stroke as a cause of disability in adulthood causes an increasing demand for therapy and care services, including telecare and teletherapy.
Objectives: Aim of the study is to analyse the acceptance of telepresence robotics and digital therapy applications. Methods: Longitudinal study with a before and after survey of patients, relatives and care and therapy staff.
Results: Acceptance of the technology analysed is high in all three groups. Although acceptance among patients declined in parts of the cases in the second survey after having used telerobotics, all in all approval ratings remained high. With regard to patients no significant correlation was found between the general technology acceptance and the acceptance of use of telerobotics.
Conclusion:
Accepted new telecare and teletherapies can be offered with the help of telepresence robotics. This requires knowledge of and experience with the technology.
Determinants of household electricity consumption measured by smart meters found by the authors in a scoping review were analyzed for the example of Germany utilizing the 2018 Survey of Income and Expenditure. All variables identified in the scoping review were covered in the survey (number and type of appliances, sociodemographic, and dwelling-related aspects). One can therefore use this large representative data set to test these relationships for German households. Expenditure on electricity is considered an indicator of household electricity consumption. The determinants show weak to moderate correlations with energy expenditure in bivariate analyses. The multivariate analysis shows effects of household-specific, dwelling-related, and appliance-specific factors. Models considering only one aspect overestimate this effect. Thus, all three aspects should be considered simultaneously when explaining residential electricity consumption. The largest effects are found for electricity as the main energy source for heating, the number of household members, as well as their presence at home. While household structure plays an important part in explaining residential energy consumption, dwelling and appliance-related aspects influence it as well. The latter aspects may be influenced by appropriate policy measures.
Even though artificial intelligence and machine learning have demonstrated remarkable performances in medical image computing, their accountability and transparency level must be improved to transfer this success into clinical practice. The reliability of machine learning decisions must be explained and interpreted, especially for supporting the medical diagnosis.For this task, the deep learning techniques’ black-box nature must somehow be lightened up to clarify its promising results. Hence, we aim to investigate the impact of the ResNet-50 deep convolutional design for Barrett’s esophagus and adenocarcinoma classification. For such a task, and aiming at proposing a two-step learning technique, the output of each convolutional layer that composes the ResNet-50 architecture was trained and classified for further definition of layers that would provide more impact in the architecture. We showed that local information and high-dimensional features are essential to improve the classification for our task. Besides, we observed a significant improvement when the most discriminative layers expressed more impact in the training and classification of ResNet-50 for Barrett’s esophagus and adenocarcinoma classification, demonstrating that both human knowledge and computational processing may influence the correct learning of such a problem.
ntervention with motivational emails can have a positive effect on course retention in e-learning. It is, however, not yet clear whether different forms of emails affect course retention and how students make progress during the sending of emails. We therefore used a voluntary asynchronous online course with 206 students. Students were randomly divided into four groups: text–picture personalised email vs. text personalised email vs. generalised email vs. no email. Emails were sent weekly for 3 months. Results yield that more students made progress in the text–picture personalised email group than in the control group. Students in all email groups progressed by more units than students in the control group. Only students in email groups completed the course and only students in personalised email groups reacted to the emails. Emails were accepted by most students enrolled. The findings suggest that cost-effective and easily implemented emails can encourage students to progress from unit to unit.
Background:
With the prevalence of burnout among surgeons posing a significant threat to healthcare outcomes, the mental toughness of medical professionals has come to the fore. Mental toughness is pivotal for surgical performance and patient safety, yet research into its dynamics within a global and multi-specialty context remains scarce. This study aims to elucidate the factors contributing to mental toughness among surgeons and to understand how it correlates with surgical outcomes and personal well-being.
Methods:
Utilizing a cross-sectional design, this study surveyed 104 surgeons from English and German-speaking countries using the Mental Toughness Questionnaire (MTQ-18) along with additional queries about their surgical practice and general life satisfaction. Descriptive and inferential statistical analyses were applied to investigate the variations in mental toughness across different surgical domains and its correlation with professional and personal factors.
Results:
The study found a statistically significant higher level of mental toughness in micro-surgeons compared to macro-surgeons and a positive correlation between mental toughness and surgeons' intent to continue their careers. A strong association was also observed between general life satisfaction and mental toughness. No significant correlations were found between the application of psychological skills and mental toughness.
Conclusion:
Mental toughness varies significantly among surgeons from different specialties and is influenced by professional dedication and personal life satisfaction. These findings suggest the need for targeted interventions to foster mental toughness in the surgical community, potentially enhancing surgical performance and reducing burnout. Future research should continue to explore these correlations, with an emphasis on longitudinal data and the development of resilience-building programs.

Mittlerweile werden zahlreiche technologische Anwendungen genutzt, die auf maschinellem Lernen und den damit verbundenen Möglichkeiten der Datensamm¬lung, -nutzung und -verwertung aufbauen. Indem KI große Datenmengen beherrschbar und verborgene Muster und Zusammenhänge sichtbar macht, wird vieles schneller, einfacher und effizienter – sei es im Alltag, in der Arbeit oder in Organisationen. Background
This study evaluated the effect of an artificial intelligence (AI)-based clinical decision support system on the performance and diagnostic confidence of endoscopists in their assessment of Barrett’s esophagus (BE).
Methods
96 standardized endoscopy videos were assessed by 22 endoscopists with varying degrees of BE experience from 12 centers. Assessment was randomized into two video sets: group A (review first without AI and second with AI) and group B (review first with AI and second without AI). Endoscopists were required to evaluate each video for the presence of Barrett’s esophagus-related neoplasia (BERN) and then decide on a spot for a targeted biopsy. After the second assessment, they were allowed to change their clinical decision and confidence level.
Results
AI had a stand-alone sensitivity, specificity, and accuracy of 92.2%, 68.9%, and 81.3%, respectively. Without AI, BE experts had an overall sensitivity, specificity, and accuracy of 83.3%, 58.1%, and 71.5%, respectively. With AI, BE nonexperts showed a significant improvement in sensitivity and specificity when videos were assessed a second time with AI (sensitivity 69.8% [95%CI 65.2%–74.2%] to 78.0% [95%CI 74.0%–82.0%]; specificity 67.3% [95%CI 62.5%–72.2%] to 72.7% [95%CI 68.2%–77.3%]). In addition, the diagnostic confidence of BE nonexperts improved significantly with AI.
Conclusion
BE nonexperts benefitted significantly from additional AI. BE experts and nonexperts remained significantly below the stand-alone performance of AI, suggesting that there may be other factors influencing endoscopists’ decisions to follow or discard AI advice.
Aims
Recent evidence suggests the possibility of intraprocedural phase recognition in surgical operations as well as endoscopic interventions such as peroral endoscopic myotomy and endoscopic submucosal dissection (ESD) by AI-algorithms. The intricate measurement of intraprocedural phase distribution may deepen the understanding of the procedure. Furthermore, real-time quality assessment as well as automation of reporting may become possible. Therefore, we aimed to develop an AI-algorithm for intraprocedural phase recognition during ESD.
Methods
A training dataset of 364385 single images from 9 full-length ESD videos was compiled. Each frame was classified into one procedural phase. Phases included scope manipulation, marking, injection, application of electrical current and bleeding. Allocation of each frame was only possible to one category. This training dataset was used to train a Video Swin transformer to recognize the phases. Temporal information was included via logarithmic frame sampling. Validation was performed using two separate ESD videos with 29801 single frames.
Results
The validation yielded sensitivities of 97.81%, 97.83%, 95.53%, 85.01% and 87.55% for scope manipulation, marking, injection, electric application and bleeding, respectively. Specificities of 77.78%, 90.91%, 95.91%, 93.65% and 84.76% were measured for the same parameters.
Conclusions
The developed algorithm was able to classify full-length ESD videos on a frame-by-frame basis into the predefined classes with high sensitivities and specificities. Future research will aim at the development of quality metrics based on single-operator phase distribution.
Aims
While AI has been successfully implemented in detecting and characterizing colonic polyps, its role in therapeutic endoscopy remains to be elucidated. Especially third space endoscopy procedures like ESD and peroral endoscopic myotomy (POEM) pose a technical challenge and the risk of operator-dependent complications like intraprocedural bleeding and perforation. Therefore, we aimed at developing an AI-algorithm for intraprocedural real time vessel detection during ESD and POEM.
Methods
A training dataset consisting of 5470 annotated still images from 59 full-length videos (47 ESD, 12 POEM) and 179681 unlabeled images was used to train a DeepLabV3+neural network with the ECMT semi-supervised learning method. Evaluation for vessel detection rate (VDR) and time (VDT) of 19 endoscopists with and without AI-support was performed using a testing dataset of 101 standardized video clips with 200 predefined blood vessels. Endoscopists were stratified into trainees and experts in third space endoscopy.
Results
The AI algorithm had a mean VDR of 93.5% and a median VDT of 0.32 seconds. AI support was associated with a statistically significant increase in VDR from 54.9% to 73.0% and from 59.0% to 74.1% for trainees and experts, respectively. VDT significantly decreased from 7.21 sec to 5.09 sec for trainees and from 6.10 sec to 5.38 sec for experts in the AI-support group. False positive (FP) readings occurred in 4.5% of frames. FP structures were detected significantly shorter than true positives (0.71 sec vs. 5.99 sec).
Conclusions
AI improved VDR and VDT of trainees and experts in third space endoscopy and may reduce performance variability during training. Further research is needed to evaluate the clinical impact of this new technology.
Aims
Artificial Intelligence (AI) systems in gastrointestinal endoscopy are narrow because they are trained to solve only one specific task. Unlike Narrow-AI, general AI systems may be able to solve multiple and unrelated tasks. We aimed to understand whether an AI system trained to detect, characterize, and segment early Barrett’s neoplasia (Barrett’s AI) is only capable of detecting this pathology or can also detect and segment other diseases like early squamous cell cancer (SCC).
Methods
120 white light (WL) and narrow-band endoscopic images (NBI) from 60 patients (1 WL and 1 NBI image per patient) were extracted from the endoscopic database of the University Hospital Augsburg. Images were annotated by three expert endoscopists with extensive experience in the diagnosis and endoscopic resection of early esophageal neoplasias. An AI system based on DeepLabV3+architecture dedicated to early Barrett’s neoplasia was tested on these images. The AI system was neither trained with SCC images nor had it seen the test images prior to evaluation. The overlap between the three expert annotations („expert-agreement“) was the ground truth for evaluating AI performance.
Results
Barrett’s AI detected early SCC with a mean intersection over reference (IoR) of 92% when at least 1 pixel of the AI prediction overlapped with the expert-agreement. When the threshold was increased to 5%, 10%, and 20% overlap with the expert-agreement, the IoR was 88%, 85% and 82%, respectively. The mean Intersection Over Union (IoU) – a metric according to segmentation quality between the AI prediction and the expert-agreement – was 0.45. The mean expert IoU as a measure of agreement between the three experts was 0.60.
Conclusions
In the context of this pilot study, the predictions of SCC by a Barrett’s dedicated AI showed some overlap to the expert-agreement. Therefore, features learned from Barrett’s cancer-related training might be helpful also for SCC prediction. Our results allow different possible explanations. On the one hand, some Barrett’s cancer features generalize toward the related task of assessing early SCC. On the other hand, the Barrett’s AI is less specific to Barrett’s cancer than a general predictor of pathological tissue. However, we expect to enhance the detection quality significantly by extending the training to SCC-specific data. The insight of this study opens the way towards a transfer learning approach for more efficient training of AI to solve tasks in other domains.
Aims
Endoscopic retrograde cholangiopancreaticography (ERCP) is the gold standard in the diagnosis as well as treatment of diseases of the pancreatobiliary tract. However, it is technically complex and has a relatively high complication rate. In particular, cannulation of the papillary ostium remains challenging. The aim of this study is to examine whether a deep-learning algorithm can be used to detect the major duodenal papilla and in particular the papillary ostium reliably and could therefore be a valuable tool for inexperienced endoscopists, particularly in training situation.
Methods
We analyzed a total of 654 retrospectively collected images of 85 patients. Both the major duodenal papilla and the ostium were then segmented. Afterwards, a neural network was trained using a deep-learning algorithm. A 5-fold cross-validation was performed. Subsequently, we ran the algorithm on 5 prospectively collected videos of ERCPs.
Results
5-fold cross-validation on the 654 labeled data resulted in an F1 value of 0.8007, a sensitivity of 0.8409 and a specificity of 0.9757 for the class papilla, and an F1 value of 0.5724, a sensitivity of 0.5456 and a specificity of 0.9966 for the class ostium. Regardless of the class, the average F1 value (class papilla and class ostium) was 0.6866, the sensitivity 0.6933 and the specificity 0.9861. In 100% of cases the AI-detected localization of the papillary ostium in the prospectively collected videos corresponded to the localization of the cannulation performed by the endoscopist.
Conclusions
In the present study, the neural network was able to identify the major duodenal papilla with a high sensitivity and high specificity. In detecting the papillary ostium, the sensitivity was notably lower. However, when used on videos, the AI was able to identify the location of the subsequent cannulation with 100% accuracy. In the future, the neural network will be trained with more data. Thus, a suitable tool for ERCP could be established, especially in the training situation.

Die hohe Rate an Geburtsverletzungen unter Spontangeburt mit teilweise gravierenden Kurz- und Langzeitfolgen stellt eine physische und psychosoziale Belastung für Gebärende dar, sodass der Wunsch nach vollständig erhaltenem Damm und erhaltener Beckenbodenmuskulatur stark vorhanden ist. Als Erhaltung der Gesundheit von Mutter und Kind stellt der Schutz des Dammes und Beckenbodens eine Kernkompetenz der Hebammen dar. Um dieser nachzukommen, wird die Pathophysiologie des Dammrisses erörtert. Nachdem der weibliche Beckenboden anatomisch und histologisch aufgearbeitet wurde sowie die Pathophysiologie der Wundentstehung, konnte unter Einbeziehung des Verhaltens des Beckenbodens unter Spontangeburt die Pathophysiologie des Dammrisses aufgezeigt werden. Diese ist ein Zusammenspiel aus Druck auf das Gewebe durch das tiefertretende Kind und zum Teil aus den wirkenden Scherkräften der stattfindenden Dehnung. Der entscheidende Mechanismus liegt jedoch beim Druck, da dieser eine Minderversorgung der Zellen verursacht, sodass es zur hypoxischen Ischämie und schließlich zum nekrotischen Zelltod kommt. Es konnte aufgezeigt werden, dass zusätzliche Anspannung einen stark negativen Faktor für Geburtsverletzungen darstellt, weshalb die genetische Disposition sowie das maternales Alter valide Risikofaktoren sind und aus welchen Gründen eine trainierte Beckenbodenmuskulatur von Vorteil sein dürfte.
Aims
Human-computer interactions (HCI) may have a relevant impact on the performance of Artificial Intelligence (AI). Studies show that although endoscopists assessing Barrett’s esophagus (BE) with AI improve their performance significantly, they do not achieve the level of the stand-alone performance of AI. One aspect of HCI is the impact of AI on the degree of certainty and confidence displayed by the endoscopist. Indirectly, diagnostic confidence when using AI may be linked to trust and acceptance of AI. In a BE video study, we aimed to understand the impact of AI on the diagnostic confidence of endoscopists and the possible correlation with diagnostic performance.
Methods
22 endoscopists from 12 centers with varying levels of BE experience reviewed ninety-six standardized endoscopy videos. Endoscopists were categorized into experts and non-experts and randomly assigned to assess the videos with and without AI. Participants were randomized in two arms: Arm A assessed videos first without AI and then with AI, while Arm B assessed videos in the opposite order. Evaluators were tasked with identifying BE-related neoplasia and rating their confidence with and without AI on a scale from 0 to 9.
Results
The utilization of AI in Arm A (without AI first, with AI second) significantly elevated confidence levels for experts and non-experts (7.1 to 8.0 and 6.1 to 6.6, respectively). Only non-experts benefitted from AI with a significant increase in accuracy (68.6% to 75.5%). Interestingly, while the confidence levels of experts without AI were higher than those of non-experts with AI, there was no significant difference in accuracy between these two groups (71.3% vs. 75.5%). In Arm B (with AI first, without AI second), experts and non-experts experienced a significant reduction in confidence (7.6 to 7.1 and 6.4 to 6.2, respectively), while maintaining consistent accuracy levels (71.8% to 71.8% and 67.5% to 67.1%, respectively).
Conclusions
AI significantly enhanced confidence levels for both expert and non-expert endoscopists. Endoscopists felt significantly more uncertain in their assessments without AI. Furthermore, experts with or without AI consistently displayed higher confidence levels than non-experts with AI, irrespective of comparable outcomes. These findings underscore the possible role of AI in improving diagnostic confidence during endoscopic assessment.
Academic self-regulation is a key factor for motivation and learning achievement. Yet with the large range of individual factors, this is not a one-size-fits-all proposition. This study of L2 Technical English students at two German universities explored learners’ expectations and motivations, in particular regarding self-regulation and self-efficacy via the individual’s time investment in self-led study. In an initial survey, learners (N=1646) reported on their English skill levels and anticipated learning habits. Complementarily, the retrospective survey investigated learners’ (N=796) actual behavior during the course, their perceptions of language skill improvement, and their satisfaction. The initial survey indicates a clear understanding that time investment in self-regulated study will lead to greater improvement, an outcome confirmed in the retrospective survey. Additionally, students who invested more time in their coursework were more satisfied with their achievement, although most learners acknowledge they should have studied more. The results verify that learners recognize the nexus between self-regulation and language skill improvement, yet university students are not satisfied with their capacity to self-regulate their language learning strategies. While differences in students’ skill levels and academic self-efficacy result in divergent degrees of progress, students of all types report benefits to their language skills when motivated to self-regulated study.

Small and medium-sized enterprises (SMEs) increasingly need to manage nformation technology (IT) effectively in order to remain competitive. However, compared to larger organizations, SMEs often face challenges in terms of resources and employer attractiveness, and regularly do not have the need to employ a Chief Information Officer (CIO) on a full-time basis. To address this issue, a growing number of global experts have begun to provide CIO services on a part-time basis for multiple clients simultaneously. This approach allows SMEs to tap into the expertise of experienced IT leaders at a fraction of the cost and without committing to long-term arrangements. While these professionals, known as “Fractional CIOs”, have proven their value in the field, there has been a lack of academic research on this emerging trend. Therefore, we carried out a comprehensive research project between 2020 and 2023, involving 62 Fractional CIOs from 10 countries. The research produced a definition, different types of engagements, and success factors for Fractional CIOs and their engagements. This paper summarizes these findings for a wider audience of academics and practitioners.
Real-time computational speed and a high degree of precision are requirements for computer-assisted interventions. Applying a segmentation network to a medical video processing task can introduce significant inter-frame prediction noise. Existing approaches can reduce inconsistencies by including temporal information but often impose requirements on the architecture or dataset. This paper proposes a method to include temporal information in any segmentation model and, thus, a technique to improve video segmentation performance without alterations during training or additional labeling. With Motion-Corrected Moving Average, we refine the exponential moving average between the current and previous predictions. Using optical flow to estimate the movement between consecutive frames, we can shift the prior term in the moving-average calculation to align with the geometry of the current frame. The optical flow calculation does not require the output of the model and can therefore be performed in parallel, leading to no significant runtime penalty for our approach. We evaluate our approach on two publicly available segmentation datasets and two proprietary endoscopic datasets and show improvements over a baseline approach.
Limitations in computer-assisted diagnosis include lack of labeled data and inability to model the relation between what experts see and what computers learn. Even though artificial intelligence and machine learning have demonstrated remarkable performances in medical image computing, their accountability and transparency level must be improved to transfer this success into clinical practice. The reliability of machine learning decisions must be explained and interpreted, especially for supporting the medical diagnosis. While deep learning techniques are broad so that unseen information might help learn patterns of interest, human insights to describe objects of interest help in decision-making. This paper proposes a novel approach, DeepCraftFuse, to address the challenge of combining information provided by deep networks with visual-based features to significantly enhance the correct identification of cancerous tissues in patients affected with Barrett’s esophagus (BE). We demonstrate that DeepCraftFuse outperforms state-of-the-art techniques on private and public datasets, reaching results of around 95% when distinguishing patients affected by BE that is either positive or negative to esophageal cancer.
Heavy smoke development represents an important challenge for operating physicians during laparoscopic procedures and can potentially affect the success of an intervention due to reduced visibility and orientation. Reliable and accurate recognition of smoke is therefore a prerequisite for the use of downstream systems such as automated smoke evacuation systems. Current approaches distinguish between non-smoked and smoked frames but often ignore the temporal context inherent in endoscopic video data. In this work, we therefore present a method that utilizes the pixel-wise displacement from randomly sampled images to the preceding frames determined using the optical flow algorithm by providing the transformed magnitude of the displacement as an additional input to the network. Further, we incorporate the temporal context at evaluation time by applying an exponential moving average on the estimated class probabilities of the model output to obtain more stable and robust results over time. We evaluate our method on two convolutional-based and one state-of-the-art transformer architecture and show improvements in the classification results over a baseline approach, regardless of the network used.
Training data for Neural Networks is often scarce in the medical domain, which often results in models that struggle to generalize and consequently showpoor performance on unseen datasets. Generally, adding augmentation methods to the training pipeline considerably enhances a model’s performance. Using the dataset of the Foot Ulcer Segmentation Challenge, we analyze two additional augmentation methods in the domain of chronic foot wounds - local warping of wound edges along with projection and blurring of shapes inside wounds. Our experiments show that improvements in the Dice similarity coefficient and Normalized Surface Distance metrics depend on a sensible selection of those augmentation methods.

The human nose serves as the primary gateway for air entering the respiratory system and plays a vital role in breathing. Nasal breathing difficulties are a significant health concern, leading to substantial healthcare costs for patients. Understanding nasal airflow dynamics is crucial for comprehending respiratory mechanisms. This article presents a detailed study using tomo-Particle Image Velocimetry (PIV) to investigate nasal airflow dynamics while addressing its accuracy. Embedded in the OpenNose project, the work described aims to provide a validation basis for different numerical approaches to upper airway flow. The study includes the manufacturing of a transparent silicone model based on a clinical CT scan, refractive index matching to minimize optical distortions, and precise flow rate adjustments based on physiological breathing cycles. This method allows for spatial high-resolution investigations in different regions of interest within the nasopharynx during various phases of the breathing cycle. The results demonstrate the accuracy of the investigations, enabling detailed analysis of flow structures and gradients. This spatial high-resolution tomo-PIV approach provides valuable insights into the complex flow phenomena occurring during the physiological breathing cycle in the nasopharynx. The study's findings contribute to advancements in non-free-of-sight experimental flow investigation of complex cavities under nearly realistic conditions. Furthermore, reliable and accurate experimental data is crucial for properly validating numerical approaches that compute this patient-specific flow for clinical purposes.
Utility of Smartphone-based Three-dimensional Surface Imaging for Digital Facial Anthropometry

Background
The utilization of three-dimensional (3D) surface imaging for facial anthropometry is a significant asset for patients undergoing maxillofacial surgery. Notably, there have been recent advancements in smartphone technology that enable 3D surface imaging.
In this study, anthropometric assessments of the face were performed using a smartphone and a sophisticated 3D surface imaging system.
Methods
30 healthy volunteers (15 females and 15 males) were included in the study. An iPhone 14 Pro (Apple Inc., USA) using the application 3D Scanner App (Laan Consulting Corp., USA) and the Vectra M5 (Canfield Scientific, USA) were employed to create 3D surface models. For each participant, 19 anthropometric measurements were conducted on the 3D surface models. Subsequently, the anthropometric measurements generated by the two approaches were compared. The statistical techniques employed included the paired t-test, paired Wilcoxon signed-rank test, Bland–Altman analysis, and calculation of the intraclass correlation coefficient (ICC).
Results
All measurements showed excellent agreement between smartphone-based and Vectra M5-based measurements (ICC between 0.85 and 0.97). Statistical analysis revealed no statistically significant differences in the central tendencies for 17 of the 19 linear measurements. Despite the excellent agreement found, Bland–Altman analysis revealed that the 95% limits of agreement between the two methods exceeded ±3 mm for the majority of measurements.
Conclusion
Digital facial anthropometry using smartphones can serve as a valuable supplementary tool for surgeons, enhancing their communication with patients. However, the proposed data suggest that digital facial anthropometry using smartphones may not yet be suitable for certain diagnostic purposes that require high accuracy.
BACKGROUND
Differentiation of high-flow from low-flow vascular malformations (VMs) is crucial for therapeutic management of this orphan disease.
OBJECTIVE
A convolutional neural network (CNN) was evaluated for differentiation of peripheral vascular malformations (VMs) on T2-weighted short tau inversion recovery (STIR) MRI.
METHODS
527 MRIs (386 low-flow and 141 high-flow VMs) were randomly divided into training, validation and test set for this single-center study. 1) Results of the CNN's diagnostic performance were compared with that of two expert and four junior radiologists. 2) The influence of CNN's prediction on the radiologists' performance and diagnostic certainty was evaluated. 3) Junior radiologists' performance after self-training was compared with that of the CNN.
RESULTS
Compared with the expert radiologists the CNN achieved similar accuracy (92% vs. 97%, p = 0.11), sensitivity (80% vs. 93%, p = 0.16) and specificity (97% vs. 100%, p = 0.50). In comparison to the junior radiologists, the CNN had a higher specificity and accuracy (97% vs. 80%, p < 0.001; 92% vs. 77%, p < 0.001). CNN assistance had no significant influence on their diagnostic performance and certainty. After self-training, the junior radiologists' specificity and accuracy improved and were comparable to that of the CNN.
CONCLUSIONS
Diagnostic performance of the CNN for differentiating high-flow from low-flow VM was comparable to that of expert radiologists. CNN did not significantly improve the simulated daily practice of junior radiologists, self-training was more effective.
In diesem Beitrag werden am Beispiel des berufsbegleitenden Bachelorstudiengangs Pflegemanagement der OTH Regensburg die Charakteristika dieses Studiums sowie die Merkmale der Studierenden und die daraus resultierenden methodischen, didaktischen und organisatorischen Spezifika dargestellt. Um Hintergrund und Relevanz aufzuzeigen, soll zunächst ein Blick auf die Akademisierung der Pflege geworfen und eine Standortbestimmung von Pflegemanagementstudiengängen innerhalb der verschiedenen inzwischen etablierten Pflegestudiengänge vorgenommen werden. Im zweiten Kapitel wird der Studiengang der OTH Regensburg vorgestellt und im dritten Kapitel werden die Erfahrungen, Herausforderungen und Lösungsstrategien beschrieben, die aus dem berufsbegleitenden Format mit „nicht-traditionell“ Studierenden resultieren. Abschließend finden sich eine kurze Zusammenfassung und ein Ausblick.
Wer bekommt eine Verordnung zu welcher physiotherapeutischen Leistung und wie kommt die Entscheidung darüber zustande? Ist die Therapie leitliniengerecht? Dem Zusammenhang zwischen Heilmittel-Richtlinie, Heilmittelkatalog, ärztlicher Verordnung und durchgeführter Therapie wird in diesem Artikel am Beispiel chronischer Rückenschmerz auf der Basis von Routinedatenanalysen der BARMER nachgegangen.
In the field of computer- and robot-assisted minimally invasive surgery, enormous progress has been made in recent years based on the recognition of surgical instruments in endoscopic images and videos. In particular, the determination of the position and type of instruments is of great interest. Current work involves both spatial and temporal information, with the idea that predicting the movement of surgical tools over time may improve the quality of the final segmentations. The provision of publicly available datasets has recently encouraged the development of new methods, mainly based on deep learning. In this review, we identify and characterize datasets used for method development and evaluation and quantify their frequency of use in the literature. We further present an overview of the current state of research regarding the segmentation and tracking of minimally invasive surgical instruments in endoscopic images and videos. The paper focuses on methods that work purely visually, without markers of any kind attached to the instruments, considering both single-frame semantic and instance segmentation approaches, as well as those that incorporate temporal information. The publications analyzed were identified through the platforms Google Scholar, Web of Science, and PubMed. The search terms used were “instrument segmentation”, “instrument tracking”, “surgical tool segmentation”, and “surgical tool tracking”, resulting in a total of 741 articles published between 01/2015 and 07/2023, of which 123 were included using systematic selection criteria. A discussion of the reviewed literature is provided, highlighting existing shortcomings and emphasizing the available potential for future developments.
Im folgenden Text möchte ich die verschiedenen Sterbehilfeformen etwas sortieren und einordnen. In der medialen Diskussion im Vorfeld der jüngst (ohne Ergebnis) erfolgten Bundestagsabstimmung zum § 217 StGB wurde die Schwierigkeit bei der Verwendung des Sterbehilfebegriffes besonders deutlich. Es war dabei oft nicht klar, ob es sich um die Beendigung von lebensverlängernden Maßnahmen, die Unterstützung beim Suizid oder die Tötung auf Verlangen handelt.
Preliminary considerations on the form-finding of a tensegrity joint to be used in dynamic orthoses

Early motion therapy plays an important role for effective long-term healing of joint injuries. In many cases, conventional dynamic orthoses fail to address the intricate movement possibilities of the underlying joints, limited by their simplistic joint representations, often represented by revolute joints, enabling rotations by only one axis. In this paper, a two-dimensional compliant tensegrity joint for use in biomedical applications is investigated. It consists of two compressed members and five compliant tensioned members. Relative movement possibilities are realized by the intrinsic compliance of the structure. In the development of these systems, the first step is the determination of the static stable equilibrium. This analysis is conducted in this paper by considering the potential energy approach or by using the geometric nonlinear finite element method. The mechanical behavior of the structure is assessed with a specific emphasis on its mechanical compliance. The primary objective of this study is the investigation of the influence of structural parameters on the overall stiffness and movability of the structure. The results underscore the significant effect of member parameters on the stiffness and movability of the compliant tensegrity joint, particularly under varying load magnitudes. These findings provide insights for optimizing the joint’s performance, contributing to its potential application in advanced orthotic and exoskeleton devices.
The utilization of virtual reality (VR) technology has shown promise in various therapeutic applications, particularly in exposure therapy
for reducing fear of certain situations objects or activities, e.g. fear of height, or negative evaluation of others in social situations. VR has been shown to yield positive outcomes in follow-up studies, and provides a safe and ecological therapeutic environment for therapists and their patients. This paper presents a collaborative
effort to develop a VR speech therapy system which simulates a virtual audience for users to practice their public speaking skills. We describe a novel web-based graphica user interface that enables
therapists to manage the therapy session using a simple timeline. Lastly, we present the results from a qualitative study with therapists and teachers with functional dysphonia, which highlight the potential of such an application to support and augment the therapists’ work and the remaining challenges regarding the design of natural interactions, agent behaviours and scenario customisation for patients.
AIM OF THE STUDY
This study was conducted in a pre-post design with a survey of patients who had undergone deep brain stimulation (DBS) as treatment for a neurological movement disorder. The aim of the study was to compare patients' expectations and beliefs before a DBS intervention with patients' subjective experience of this intervention.
METHODOLOGY
The longitudinal study of patients (n=132) with an indication for DBS therapy was based on a written survey at the time points of preoperative screening (pre-op) and one-year follow-up (post-op).
RESULTS
Preoperatively, a clear majority of respondents believed DSB to be similar to a pacemaker intervention, but one year after the intervention less than one third did so, as they compared DBS to using a walking stick or glasses.
CONCLUSION
The experience of DBS in the patient's own body seems to be comparable by means of individually different associations, whereby the comparison with non-invasive aids predominates postoperatively. The discussion of these descriptions in the educational interview can contribute to a realistic horizon of patients' expectations before DBS.
ZIEL DER STUDIE
Es wird eine Studie im Prä-Post-Design mit einer Befragung von Patient*innen vorgestellt, bei denen zur Therapie einer neurologischen Bewegungsstörung die Indikation für eine Tiefe Hirnstimulation (THS) gegeben war. Die Patient*innen wurden vor und nach der THS-Operation zu ihren präoperativen Assoziationen bzw. postoperativen Wahrnehmungen im Umgang mit einem Hirnschrittmacher befragt. Ziel der Untersuchung ist, die bisherige Praxis der ärztlichen Aufklärung vor einer THS-Intervention um Angaben zur subjektiven Erlebnisqualität dieser Intervention ergänzen zu können.
METHODIK
Es wurde eine Längsschnittstudie mit Patient*innen (n=132) durchgeführt, in der die Teilnehmenden zweizeitig schriftlich zu Imaginationen zu ihrer subjektiven Erlebnisqualität von THS befragt wurden. Die Untersuchungen erfolgten beim präoperativen Screening (Prä-OP) sowie beim Ein-Jahres-Follow-Up (Post-OP).
ERGEBNISSE
Präoperativ gab die Mehrheit der Befragten die Assoziation Herzschrittmacher an. Postoperativ wählten nur weniger als ein Drittel diese Option, gleich häufig wie die Assoziationen Gehstock oder Brille.
SCHLUSSFOLGERUNG
Die Erlebnisqualität von THS im eigenen Körper scheint sich mittels individuell verschiedener Assoziationen vergleichen zu lassen, wobei postoperativ der Vergleich mit nicht invasiven Hilfsmitteln überwiegt. Im Aufklärungsgespräch kann die Thematisierung dieser Beschreibungen bei den Patient*innen zu einem realitätsnahen Erwartungshorizont vor THS beitragen.
Media watchdog weblogs can play an important role in media accountability, provided that the bloggers are regularly active and have an appropriate awareness of their role in the media landscape. The following article examines the presence and influence of media watchblogs in selected nations, as well as the operators’ standards and perceptions of their roles. In semi-structured qualitative interviews, operators of watchblogs in Germany, Austria, Switzerland and the United Kingdom discussed their perceptions of their role, their ethical guidelines, their financing and any conflicts of interest, and their projections for the future. The interviews reveal that most media watchbloggers do not interpret their role to be a Fifth Estate with a mandate to monitor the mass media. The watchbloggers are reluctant to accept and uphold formal ethical and quality criteria, and limitations on time and money make them unable to guarantee ongoing critical media analysis. For these reasons, media watchblogs are determined to be a weak instrument of media accountability.
This paper is based on the talk of the same title delivered at the “Dock of Learning” Symposium. I reflect on design research with an eye on the critical issues of our time. Design research has the potential to take on these issues; however, there are challenges. A threatening one, I see, is the divergence of design research and art research. I attempt to tackle this by examining Jonas’ Research through Design and Borgdorff’s Artistic Research with help from John Dewey’s philosophy.
Customer project selection is a challenge for many industrial companies. An inappropriate project selection approach can lead to constraint violations, high fixed costs, and suboptimal portfolios. To overcome these problems a cash-flow-based linear optimization model was developed in partnership with a tier-1 automotive supplier. Implementation barriers had been verified through a case study conducted at two organizational hierarchies. Results suggest that an application at the operating levels is possible. At higher levels, though, product and firm complexity require major implementation efforts. This article serves theorists as well as practitioners in multiple regards. First, an overview of existing project selection methods and their application in practice is provided. Additionally, the supplier's current appraisal process is depicted. Second, operations research implementation barriers are identified and validated for the adoption of the proposed mathematical project selection approach. Third, a guideline including procedures to overcome experienced difficulties is presented.
Seit den Anfängen der bürgerlichen Gesellschaft gibt es »künstlich produzierte Armut« (Marx) – künstlich, weil sie, wie Hegel analysiert hat, nicht auf natürlichem Mangel beruht, sondern ihre Notwendigkeit im Recht auf Eigentum liegt. Auch wusste der Philosoph: Ohne materielle Mittel gibt es keine Selbstverwirklichung. Die von Hegel theoretisch vorweggenommenen Konsequenzen wie der Zusammenhang mit geringerer Bildung, kürzerer Lebenserwartung und politischer Apathie sind heutzutage empirisch belegt.
Ina Schildbach liest Hegel neu und stellt fest: Erkenntnisbringend sind nicht nur seine Einsichten zu Ursachen und Konsequenzen der Armut – auch die staatsfunktionale Perspektive auf Armut als Problem der Obrigkeit offenbart frappierende Ähnlichkeit zum heutigen Diskurs.
Politische Bildung und Zeit

Michael Görtler richtet in dieser Untersuchung den Fokus auf die Bedeutung von Zeit für die politische Bildung, da die Zeitlichkeit politischer Bildungsprozesse und demokratischer Politik in der Didaktik der politischen Bildung bisher kaum beachtet wurde. Der Autor verkleinert diese Lücke, erweitert den Fachdiskurs und gibt praktische Impulse für die politische Bildungsarbeit. Im Mittelpunkt steht die Frage nach dem expliziten wie impliziten Sichtbarwerden der Zeit in Ansätzen der Didaktik der politischen Bildung und der Bildungs- und Sozialwissenschaften. Dabei wird auch geprüft, welche didaktischen Herausforderungen in Bezug auf die Ziele, Inhalte und die Gestaltung von Lehr- und Lernprozessen auf der Subjekt- und Objektseite sowie in der Vermittlung zwischen beiden Seiten beachtet werden müssen.
In der politischen Bildung wird die Debatte um eine kritische politische Bildung erneut geführt. Im Fokus der Debattenbeiträge stehen unterschiedliche Vorstellungen hinsichtlich Notwendigkeit, Begründung, Ziele, Inhalte und Methoden einer kritischen politischen Bildung im schulischen wie außerschulischen Bereich. Dieser Band zeigt unterschiedliche Standpunkte und Forschungsperspektiven auf, wie sie diesbezüglich auf der GPJE-Nachwuchstagung im Frühjahr 2016 an der Universität Bremen diskutiert worden sind. Der Band gliedert sich in drei Teile. Der erste umfasst theoretische wie konzeptionelle Beiträge. Im zweiten Teil steht die empirische Forschung im Mittelpunkt. Im dritten Teil werden schließlich die politisch-bildnerische Praxis und deren Reflexion thematisiert. Insgesamt werfen die Beiträge einen differenzierten Blick auf die Debatte um den Gehalt einer kritischen politischen Bildung. Dabei setzen sich die Autorinnen und Autoren mit Argumenten aus der aktuellen Diskussion auseinander und beleuchten blinde Flecken.
Forschungsfragen: Wie rekrutieren Unternehmen Auszubildende? Wie offen sind sie für Innovationen? Was sind wesentliche Faktoren für einen erfolgreichen Bewerbungsprozess?
Forschungsansatz: Im Rahmen einer Studie hat die OTH Regensburg die Merkmale des Bewerbungsprozesses entlang der Candidate Journey erfasst – von der ersten Berufswahl und Stellensuche bis zur Einstellung. Die insgesamt 160 befragten Ausbildungsbetriebe repräsentieren einen breiten Mix unterschiedlicher Unternehmensgrößen und -branchen.
Zentrale Ergebnisse: Unternehmen nutzen Rekrutierungskanäle nicht in voller Breite und schöpfen auch das Potenzial technischer Innovationen noch nicht umfassend aus. Klarheit bezüglich der erwarteten Anforderungen sowie Schnelligkeit, Transparenz und Einfachheit im Bewerbungsprozess sind empirisch nachweisbare Erfolgsfaktoren. Wertschätzung ist ein Aspekt, der (noch stärker) an jedem Touchpoint im Bewerbungsprozess beachtet werden sollte.
