Background: This preparatory study accelerates an implementation of individualized monitoring and feedback of physical motion using conventional motion trackers in the rehabilitation process of geriatric trauma patients. Regaining mobility is accompanied with improved quality of life in persons of very advanced age recovering from fragility fractures.
Objectives: Quantitative survey of regaining physical mobility provides recommendations for action on how to use motion trackers effectively in a clinical geriatric setting.
Methods: Method mix of quantitative and qualitative interdisciplinary and mutual complementary research approaches (sociology, health research, philosophy/ethics, medical informatics, nursing science, gerontology and physical therapy). While validating motion tracker use in geriatric traumatology preliminary data are used to develop a target group oriented motion feedback. In addition measurement accuracy of a questionnaire about quality of life of multimorbid geriatric patients (FLQM) is tested.
Conclusion: Implementing a new technology in a complex clinical setting needs to be based on a strong theoretical background but will not succeed without careful field testing.
Therapieabbruch oder -unterbrechung erhöhen das Risiko von Krankheitsschüben in der MS-Behandlung (Ennis et al., 2008). Der Abbruchanteil für die immunmodulatorische Basistherapie mit COPAXONE® betrug 2008 in den ersten drei Monaten bis zu 30 % (COPAKTIV Schwestern-Service, 2008), worauf das Aktiv-mit-MS-Patientenbetreuungsprogramm, eine begleitende pflegerische Fachberatung und ein Servicetelefon, installiert wurde. Die Auswirkungen der Zusammenarbeit innerhalb des Teams und mit Ärzt/innen sowie die peer-to-peer Beratung in einem Online-Forum auf Therapietreue und Krankheitsbewältigung der Patient/innen wurde im Längsschnitt untersucht. Ein Methoden-Mix wurde erfolgreich zur Evaluation der pflegerischen Multiprofessionalität eingesetzt. Es zeigte sich, dass die Angebote im Betreuungsprogramm zu einer Verminderung der Abbruchquote führten und die Patient/innen eine gute Therapietreue einhalten.
Es handelt sich um eine prospektive Studie zu einem beantragten BMBF-Projekt in einer Kooperation zwischen dem Institut für Sozialforschung und Technikfolgenabschätzung (IST), der Professur für E-Health der Fakultät Informatik und Mathematik an der OTH Regensburg sowie der Alterstraumatologie im Caritas Krankenhaus St. Josef Regensburg. Die geplante Machbarkeitsstudie forciert die Implementierung eines individualisierten Bewegungsmonitorings und feedbacks durch Einsatz eines alltagstauglichen Motion-Trackers in der Rehabilitation geriatrischer Traumapatient/innen. Das Wiedererlangen von Mobilität nach einer Fraktur geht einher mit dem Erhalt der subjektiven Lebensqualität (LQ) hochaltriger Menschen. Durch diagnostische Überwachung mithilfe der Motion-Tracker wird das Wiedererlangen bzw. der Erhalt von Mobilität quantitativ begleitet, um daraus Handlungsempfehlungen für deren nützlichen und kostengünstigen Einsatz in der geriatrischen Rehabilitation abzuleiten. Die Vorstudie dient der Validierung des Einsatzes handelsüblicher Motion-Tracker in der Alterstraumatologie, einschließlich der Entwicklung eines für die Zielgruppe geeigneten Bewegungsfeedbacks sowie der Überprüfung des Fragebogens zur LQ multimorbider älterer Menschen (FLQM). Hierfür werden quantitative und qualitative Methoden der Disziplinen Soziologie, Gesundheitsforschung, Philosophie/Ethik, Datenschutzrecht und medizinische Informatik, Medizin, Pflege, Gerontologie und Physiotherapie, die sich gegenseitig ergänzen (Methoden-Mix), herangezogen.
Straßenzeitungen haben sich zum Ziel gesetzt, einen Zuverdienst für Menschen in Not zu bieten und zugleich Vorurteile gegenüber Obdachlosen öffentlich zu bekämpfen. Eine Leserumfrage der Regensburger Straßenzeitung DONAUSTRUDL untersuchte die Wirksamkeit der Zeitung hinsichtlich ihrer Aufklärungsorientierung. Das Ergebnis zeigte, dass die Leser_innenschaft kaum weniger Vorurteile gegen Obdachlose hatte als die Gesamtbevölkerung.
Pediatric patients suffering from ependymoma are usually treated with cranial or craniospinal three-dimensional (3D) conformal radiotherapy (3DCRT). Intensity-modulated techniques spare dose to the surrounding tissue, but the risk for second malignancies may be increased due to the increase in low-dose volume. The aim of this study is to investigate if the flattening filter free (FFF) mode allows reducing the risk for second malignancies compared to the mode with flattening filter (FF) for intensity-modulated techniques and to 3DCRT. A reduction of the risk would be advantageous for treating pediatric ependymoma. 3DCRT was compared to intensity-modulated radiation therapy (IMRT) and volumetric-modulated arc therapy (VMAT) with and without flattening filter. Dose-volume histograms (DVHs) were compared to evaluate the plan quality and used to calculate the excess absolute risk (EAR) to develop second cancer in the brain. Dose verification was performed with a two-dimensional (2D) ionization chamber array and the out-of-field dose was measured with an ionization chamber to determine the EAR in peripheral organs. Delivery times were measured. Both VMAT and IMRT achieved similar plan quality in terms of dose sparing in the OAR and higher PTV coverage as compared to 3DCRT. Peripheral dose in low-dose region, which is proportional to the EAR in organs located in this region, for example, gonads, bladder, or bowel, could be significantly reduced using FFF. The lowest peripheral EAR and lowest delivery times were hereby achieved with VMATFFF . The EAR calculated based on DVH in the brain could not be reduced using FFF mode. VMATFFF improved the target coverage and homogeneity and kept the dose in the OAR similar compared to 3DCRT. In addition, delivery times were significantly reduced using VMATFFF . Therefore, for radiotherapy of ependymoma patients, VMATFFF may be considered advantageous for the combination of Elekta Synergy linac and Oncentra External Beam planning system used in this study.
Germany received around 300,000 refugee newcomers ages 16–25 in 2015. Educational participation is a key process for the integration of these young people. This is especially true for the German vocational education and training (VET) system that provides opportunities for a fast and successful transition into the labor market. The few extant studies on migrant students in Germany have shown considerable problems in achieving educational equity. Based on the theoretical assumption that systemic discrimination creates important barriers to educational equity, we investigate how institutional and systemic factors infl uence the educational participation of refugee-background youth and how these youth cope with such barriers. Using a two-case study in the cities of Hamburg and Munich, we performed a meta-analysis of research reports, complemented by secondary analysis of primary data. Results show institutional barriers in both the education and asylum systems, and that youth cope with these barriers through high educational aspirations.
BACKGROUND:
Validation and verification of multibody musculoskeletal models sEMG is a difficult process because of the reliability of sEMG data and the complex relationship of muscle force and sEMG.
OBJECTIVE:
This work aims at comparing experimentally recorded and simulated muscle activities considering a numerical model for crosstalk. METHODS:
For providing an experimentally derived reference data set, subjects were performing elevations of the arm, where the activities of the contemplated muscle groups were measured by sEMG sensors. Computed muscle activities were further processed and transformed into an artificial electromyographical signal, which includes a numerical crosstalk model. In order to determine whether the crosstalk model provides a better agreement with the measured muscle activities, the Pearson correlation coefficient has been computed as a qualitative way of assessing the curve progression of the data sets.
RESULTS:
The results show an improvement in the correlation coefficient between the experimental data and the simulated muscle activities when taking crosstalk into account.
CONCLUSIONS:
Although the correlation coefficient increased when the crosstalk model was utilized, it is questionable if the discretization of both, the crosstalk and the musculoskeletal model, is accurate enough.

The number of spinal fusion surgeries is steadily increasing and biomechanical consequences are still in debate. The aim of this study is to provide biomechanical insights into the sagittal balance of the spine and to compare spinal load before and after spinal fusion.
METHOD:
The joint reaction forces of 52 patients were analyzed in proximo-distal and antero-posterior direction from the levels T12-L1 to L5-S1 using musculoskeletal simulations.
RESULTS:
In 104 simulations, pre-surgical forces were equal to post-surgical. The levels L4-L5 and T12-L1, however, showed increased spinal forces compression forces with higher sagittal displacement. Improved restauration of sagittal balance was accompanied by lower spinal load. AP shear stress, interestingly decreased with sagittal imbalance.
CONCLUSION:
Imbalanced spines have a risk of increased compression forces at Th12-L1. L4-L5 always has increased spinal loads. These slides can be retrieved under Electronic Supplementary Material.
Modelling blood flow an shear induced coagulation in membraene oxygenators (MO) is challenging. The relevant geometry of oxygenator fibers (OF) and chaining threads is complex and spans several length scales. In relevant scales and regimes blood shows several significant non-Newtonian effects. Existing models are only capable of accounting for some, but not all relevant effects. Additionally, coagulation processes are influencing fluid properties and geometry significantly. Due to the enormous size of the discretised geometries highly detailed viscosity and coagulation properties of blodd flow in MOs. First step is to find a gemoetry dependent viscosity representation on basis of parametric micro channel experiments with anti-coagulated blood. Next step is a statistic coagulation model, based on micro channel experiments with human (re-calcified citrated) whole blood an evaluation of clinically used osygenators. Since shear rate dependent (i.e. viscosity dependet) coagulation in return influences the viscosity, a combined model with suitable implementation in a RANS framework is necessary. Towards this end, micro channel experiments with new and used single OFs triggering coagulation are performed. Structures of multimeric von Willebrand fibers (vWF), as indicator for shear induced coagulation, are compared to computed and measured flow conditions, using immunofluorescence microscopy, RANS-computations and µPIV, respectively. Preliminary examinations in clinically used MOs show good agreement between occurring structures of vWF, cell depositions and computed flow patterns (geometry form µCT-Scans). However, computed shear rates might be to low to actually trigger activation of vWF. The complex geometry of MOs results in huge meshes, which makes RANS with statistical modelling of viscosity and coagulation a reasonable approach. Towards this end, experimental data on micro channel level with evaluation on real application level is crucial. Especially regarding clotting processes, micro fluidic experiments are powerful research tool.
Introduction
Shear induced multimerisation of von-Willebrand-factor (vWF) is supposed to play an important role in coagulation inside extracorporeal membrane oxygenators. However, there is no proof that links observed vWF structures to computed or measured flow conditions.
Methods
The structures of multimeric vWF fibers, observed in clinically used membrane oxygenators is examined using immunofluorescence microscopy (IFM) using Carstairs’ staining method (positive ethics committee vote). The flow around the membrane fibres inside the oxygenator is investigated in terms of shear rate, wall shear velocity and streamlines by using CFD (RANS, Carreau-Yasuda viscosity, geometry remodelled after high-resolution µCT-scans). By interpreting the histological and numerical results in this common context, indications for shear induced coagulation mechanisms can be identified.
Results
The fibre structures of multimeric vWF build regular but not exactly symmetric formations around the contact face (CF) between the crosswise stacked oxygenator fibres (OF), see fig.1B, vWF marked red. Annular around the CF arranged, cells are likely to be found, see fig.1B, nuclei marked blue.
The computed streamlines around the OF show attached flow around the circular fibres. However, the irregular arrangement of real OF produce considerable cross flow between the interconnected neighbouring channels, in contrast to previous 2D-simulations. Thus, the CF are washed around closely by blood, also from neighbouring channels. The wall shear velocity streamlines form regular, slightly asymmetric shapes around the contact faces. The occurring maximum shear rates are in the range of 1,000 1/s.
Discussion
The shapes of vWF structures found in clinically used oxygenators match the computational results in terms of wall shear velocity and streamlines well. The accumulation of cells close to the CF can also be explained by fluid mechanics, as there are small shear gradients and slow velocities. However, occurring shear rates between OFs are too low to trigger multimerisation of vWF. That raises the question where in the circuit the actual activation of vWF is started and how, at least partly chained, vWF multimeres are attracted towards the OF surface. A next step will be the investigation of the actual shear rate triggered (or mediated) multimerisation of vWF. Towards this end, microfluidic experiments with shear triggered coagulation will be performed. Also of big interest is the computation of the flow situation in the oxygenator in proximity to chaining threads, which have been ignored in computations so far. However, first a realistic representation of the effective viscosity in computations is needed, which is not available yet.
BACKGROUND:
Shear-induced conformational changes of von Willebrand factor (vWF) may be responsible for coagulation disorder and clot formation inside membrane oxygenators (MOs) during extracorporeal membrane oxygenation (ECMO) therapy.
OBJECTIVE:
The aim was to identify vWF structures inside clinically used MOs and employ computational fluid dynamics to verify the corresponding flow conditions.
METHODS:
Samples from gas exchange membranes (GEM) from MOs were analysed for accumulations of vWF and P-selectin-positive platelets using immunofluorescence techniques. Streamlines and shear rates of the flow around GEMs were computed using a laminar steady Reynolds-Averaged-Navier-Stokes approach.
RESULTS:
Most samples were colonized with equally distributed leukocytes, integrated in thin cobweb-like vWF-structures. Only 25 % of the samples showed extended accumulations of vWF. Computed streamlines showed considerable cross flow between interconnected neighbouring channels. Stagnation points were non-symmetric and contact faces were washed around closely. The occurring maximum shear rates ranged from 2,500 to 3,000 1/s.
CONCLUSIONS:
If pronounced vWF structures are present, shape and extent match the flow computations well. Computed shear rates bear a critical degree of uncertainty due to the improper viscosity model. If flow conditions inside the MO were sufficient to affect vWF, a more consistent distribution of vWF across the samples should be present.

Zusätzlich wird online die Lernsoftware "PC-Statistiktrainer" zum Download zur Verfügung gestellt, mit deren Hilfe der Leser ein breites Spektrum statistischer Aufgaben selbstständig üben kann. Die 13. Auflage wurde kritisch durchgesehen, alle Praxisbeispiele und Übungsaufgaben wurden aktualisiert.

Dazu dienen die zahlreichen, praxisorientierten Übungsaufgaben zur beschreibenden Statistik, Wahrscheinlichkeitsrechnung und schließenden Statistik. Der Autor zeigt die Lösungen zu den Aufgaben ausführlich Schritt für Schritt auf. Dadurch wird ein selbstständiges Erarbeiten der Lösungstechniken sowie das eigenständige Lösen statistischer Problemstellungen gefördert. Hinweise auf typische Fehlerquellen sind besonders nutzwertig bei der Klausurvorbereitung. Die vorliegende 6. Auflage wurde durchgesehen und aktualisiert.

Forschungsfragen: Wie rekrutieren Unternehmen Auszubildende? Wie offen sind sie für Innovationen? Was sind wesentliche Faktoren für einen erfolgreichen Bewerbungsprozess?
Forschungsansatz: Im Rahmen einer Studie hat die OTH Regensburg die Merkmale des Bewerbungsprozesses entlang der Candidate Journey erfasst – von der ersten Berufswahl und Stellensuche bis zur Einstellung. Die insgesamt 160 befragten Ausbildungsbetriebe repräsentieren einen breiten Mix unterschiedlicher Unternehmensgrößen und -branchen.
Zentrale Ergebnisse: Unternehmen nutzen Rekrutierungskanäle nicht in voller Breite und schöpfen auch das Potenzial technischer Innovationen noch nicht umfassend aus. Klarheit bezüglich der erwarteten Anforderungen sowie Schnelligkeit, Transparenz und Einfachheit im Bewerbungsprozess sind empirisch nachweisbare Erfolgsfaktoren. Wertschätzung ist ein Aspekt, der (noch stärker) an jedem Touchpoint im Bewerbungsprozess beachtet werden sollte.
Die Operationalisierung von »Behinderung« steht vor mehrfachen Herausforderungen: Sie muss Kriterien definieren, die Verknüpfung mit sozialrechtlichen Institutionen reflektieren und beantwortbar sein – insbesondere gilt dies für Surveys, die Menschen mit unterschiedlichen Behinderungen adressieren. Basierend auf einer Analyse bestehender Lösungsstrategien fokussiert der Beitrag die Perspektive einer lebensweltangemessenen Erhebung. Dazu wurden innerhalb von Fokusgruppen und Einzelerhebungen mit Jugendlichen mit Behinderungen die Dimensionen Schwerbehindertenausweis, Selbstbild, Funktionen und Einschränkungen, Klassifikationen in Anlehnung an medizinisch-diagnostische Begriffe, die Nutzung von Hilfsmitteln sowie das Erleben von Barrieren bearbeitet. Diskutiert werden daraufhin die Reaktionen und Wünsche der Jugendlichen zur Erfassung von »Behinderung«, die Beantwortbarkeit der jeweiligen Dimensionen sowie Zusammenhänge im Sinne eines empirisch differenzierten Modells von Behinderung. Der Beitrag schließt mit einem Hinweis auf aussichtsreiche Kombinationen von Dimensionen in der Operationalisierung sowie mit einem Plädoyer für den Erkenntnisgewinn mehrdimensionaler Erhebungen von »Behinderung«.
In der Debatte um ein Einwanderungsgesetz werden die jeweiligen Motive oftmals nicht hinreichend auseinandergehalten, die für die Neuregelung des Einwanderungsrechts sprechen. Der Beitrag nimmt eine Differenzierung der Anliegen vor und geht besonders auf die aktuellen Gesetzentwürfe der Bundestagsfraktionen von SPD und Bündnis 90/Die Grünen sowie das so genannte „Nimm 2+-Modell“ des Sachverständigenrats deutscher Stiftungen für Integration und Migration (SVR) ein.
The implant stability and biomechanical response of periprosthetic bone in acetabulum around total hip joint replacement (THR) devices depend on a host of parameters, including design of articulating materials, gait cycle and subject parameters. In this study, the impact of shell design (conventional, finned, spiked, and combined design) and liner material on the biomechanical response of periprosthetic bone has been analyzed using finite element (FE) method. Two different liner materials: high density polyethylene-20% hydroxyapatite-20% alumina (HDPE-20%HA-20%Al2O3) and highly cross-linked ultrahigh molecular weight polyethylene (HC-UHMWPE) were used. The subject parameters included bone condition and bodyweight. Physiologically relevant load cases of a gait cycle were considered. The deviation of mechanical condition of the periprosthetic bone due to implantation was least for the finned shell design. No significant deviation was observed at the bone region adjacent to the spikes and the fins. This study recommends the use of the finned design, particularly for weaker bone conditions. For stronger bones, the combined design may also be recommended for higher stability. The use of HC-UHMWPE liner was found to be better for convensional shell design. However, similar biomechanical response was captured in our FE analysis for both the liner materials in case of other shell designs. Overall, the study establishes the biomechanical response of periprosthetic bone in the acetabular with preclinically tested liner materials together with new shell design for different subject conditions.
We show that templating a Si surface with a focused beam of Si2+ or Si+ ions can create suitable nucleation sites for the subsequent growth of self-assembled Ge quantum dots by chemical vapor deposition. To determine the mechanism of patterning we use atomic force microscopy to show that, similar to Ga+ patterning, the formation of a surface pit is required to enable control over Ge quantum dot locations. We find that relatively high implantation doses are required to achieve patterning, and these doses lead to amorphization of the substrate. We assess the degree to which the substrate crystallinity can be recovered by subsequent processing. Using in situ transmission electron microscopy heating experiments we find that recrystallization is possible at the growth temperature of the Ge quantum dots, but defects remain that follow the pattern of the initial implantation. We discuss the formation mechanism of the defects and the benefits of using Si ions for patterning both defects and quantum dots on Si substrates.
The growing complexity of the digital ecosystem in combination with increasing global risks has created the following dilemma. Overemphasizing cybersecurity may violate fundamental values
like equality, fairness, freedom, or privacy but neglecting cybersecurity could undermine citizens’ trust and confidence in the digital infrastructure as well as in policy makers and state authorities. In order to increase our understanding of this dilemma, this contribution aims to provide an overview on value conflicts in cybersecurity based on a systematic review on the ethical, legal and
technological literature on cybersecurity. It shall be emphasized that a focus on the opposition of cybersecurity vs. privacy is insufficient for understanding the ethical complexity of cybersecurity.
Menschen mit Aphasie erleben häufig Einbußen in Lebensqualität und Identitäts-wie Autonomieerleben sowie in der Aufrechterhaltung sozialer Beziehungen.Aphasie-Selbsthilfegruppen (ASHG) können Kommunikation und soziale Inklusion und somi tAnpassungsprozesse und Sinnerleben fördern. Meist werden sie von Therapeut/-innen oder Angehörigen geleitet und integrieren Betroffene wie Angehörige. Dadurch verbleiben Betroffene in der Rolle passiver Hilfsempfänger/-innen. Von Betroffenen geleitete spezifische Gruppen können in besonderer Weise Selbstwert erleben und Partizipation der Gruppenleitungen und-mitglieder fördern. Mit der Ausrichtung auf biographische Inhalte können explizit die Auseinandersetzung mit der eigenen veränderten Identität und damit Selbstwert erleben stimuliert werden. Das Ziel des Projekts shalk ist, Menschen mit Aphasie durch eine passgenaue Schulung und Begleitung mit Moderationsübungen und biographischen Inhalten zu befähigen, die Leitung von ASHG zu übernehmen. Damit soll die Steigerung der Lebensqualität von Leitung und Teilnehmenden erreicht werden. Die Maßnahme wird aktuell in einem Mixed-Methods-Design mit Vor-und Nachtest sowie einer Follow-up-Untersuchung nach sechs Monaten evaluiert. Erste quantitative Daten zeigen signifikante Verbesserungen der Lebensqualität. Die qualitativen Daten weisen daraufhin, dass die Gruppenmoderation durch Betroffene trotz kommunikativer Schwierigkeiten in den ASHG zunehmendgelingt. In diesem Artikel werden Einflussfaktoren auf die ASHG wie Organisation, inhaltliche Ausrichtung und Gruppenusammensetzung näher beleuchtet. Weiterhin werden der theoretische Hintergrund sowie das Vorgehen in Schulung und Begleitung beschrieben.Erste Erkenntnisse werden diskutiert.
Hintergrund
Das Internet im Allgemeinen und digitale soziale Netzwerke im Speziellen entwickeln sich zunehmend zu wichtigen Informationsquellen für Gesundheitsinformationen; dies gilt auch für Informationen im Kontext der Behandlung von Kinderwunschpatienten. Erste Studien zeigen, dass die Betroffenen nicht nur nach sachlichen Informationen, sondern auch nach emotionalem Beistand suchen. Ob insbesondere digitale soziale Netzwerke in Ergänzung oder in Konkurrenz zu klassischen Informations- und Beratungsangeboten stehen, ist noch unklar.
Methode
Um eine vorläufige Antwort hierauf geben zu können, wurde eine Literaturstudie erstellt, mit der die wichtigsten empirischen Aussagen bzgl. der Nutzungsweisen von Internet und digitalen sozialen Netzwerken eruiert werden sollten. Dies wurde mithilfe einer Keyword-Suche in Literaturdatenbanken und anschließendem Schlagwort-Snowballing umgesetzt.
Ergebnisse
Informationen und Beratung aus dem Internet und aus digitalen sozialen Netzwerken können als Ergänzung der ärztlichen Expertise angesehen werden, stellen aber keinen Ersatz dar. Insbesondere digitale soziale Netzwerke ergänzen soziale Nahbeziehungen und das Verhältnis zum behandelnden Arzt und werden in Zukunft noch an Wichtigkeit zunehmen.
Schlussfolgerung
Für die professionell Tätigen im Bereich der Reproduktionsmedizin könnte dies ein Hinweis auf Defizite in Bezug auf Beratung und Behandlung sein, dem in Zukunft durch eigene neue Angebote begegnet werden sollte.
Background. The Internet in general and digital social networks in particular are increasingly important sources for health information; this also applies to information in the context of treatment of patients wishing for a baby. Initial studies show that patients are not only looking for factual information but also for emotional support. Whether digital social networks add to or compete with classical information and consulting is still unclear.
Methods. In order to provide a preliminary answer to this question, a literature study was conducted to determine relevant empirical findings regarding the use of the Internet and digital social networks. This was done using a keyword search in literature databases and then keyword snowballing.
Results. Information and advice from the Internet and from digital social networks can be seen as an addition to medical expertise, but are not a substitute. Digital social networks, in particular, supplement social peer groups and the relationship with physicians and will become even more important in the future.
Conclusion. For professionals active in the field of reproductive medicine, this could be an indication of deficits with regard to counseling and treatment, which in the future might be countered by providing new services.
Virtual training of bone milling requires realtime and realistic haptics of the interaction between the ”virtual mill” and a ”virtual bone”. We propose an exponential abrasion model between a virtual one and the mill bit and combine it with a coarse representation of the virtual bone and the mill shaft for collision detection using the Bullet Physics Engine. We compare our exponential abrasion model to a widely used linear abrasion model and evaluate it quantitatively and qualitatively. The evaluation results show, that we can provide virtual milling in real-time, with an abrasion behavior similar to that proposed in the literature and with a realistic feeling of five different surgeons.
Mathematical analysis of the 1D model and reconstruction schemes for magnetic particle imaging

Magnetic particle imaging (MPI) is a promising new in vivo medical imaging modality in which distributions of super-paramagnetic nanoparticles are tracked based on their response in an applied magnetic field. In this paper we provide a mathematical analysis of the modeled MPI operator in the univariate situation. We provide a Hilbert space setup, in which the MPI operator is decomposed into simple building blocks and in which these building blocks are analyzed with respect to their mathematical properties. In turn, we obtain an analysis of the MPI forward operator and, in particular, of its ill-posedness properties. We further get that the singular values of the MPI core operator decrease exponentially. We complement our analytic results by some numerical studies which, in particular, suggest a rapid decay of the singular values of the MPI operator.
Im Rahmen der Entwicklung eines haptisch-visuellen Trainingssystems für das Fräsen am Felsenbein werden ein Haptikarm und ein autostereoskopischer 3D-Monitor genutzt, um Chirurgen die virtuelle Manipulation von knöchernen Strukturen im Kontext eines sog. Serious Game zu ermöglichen. Unter anderem sollen Assistenzärzte im Rahmen ihrer Ausbildung das Fräsen am Felsenbein für das chirurgische Einsetzen eines Cochlea-Implantats üben können. Die Visualisierung des virtuellen Fräsens muss dafür in Echtzeit und möglichst realistisch modelliert, implementiert und evaluiert werden. Wir verwenden verschiedene Raycasting Methoden mit linearer und Nearest Neighbor Interpolation und vergleichen die visuelle Qualität und die Bildwiederholfrequenzen der Methoden. Alle verglichenen Verfahren sind sind echtzeitfähig, unterscheiden sich aber in ihrer visuellen Qualität.
Purpose
The purpose of this paper is to explore the impact of information technology (IT) on supply chain performance in the automotive industry. Prior studies that analyzed the impact of IT on supply chain performance report results representing the situation of the “average industry.” This research focuses on the automotive industry because of its major importance in many national economies and due to the fact that automotive supply chains do not represent the supply chain of the average industry.
Design/methodology/approach
A research model is proposed to examine the relationships between IT capabilities, supply chain capabilities, and supplier performance. The model divides IT capabilities into functional and data capabilities, and supply chain capabilities into internal process excellence and information sharing. Data have been collected from 343 automotive first-tier suppliers. Structural equation modeling with partial least squares is used to analyze the data.
Findings
The results suggest that functional capabilities have the greatest impact on internal process excellence, which in turn enhances supplier performance. However, frequent and adequate information sharing also contributes significantly to supplier performance. Data capabilities enable supply chain capabilities through their positive impact on functional capabilities.
Practical implications
The findings will help managers to understand the effect of IT implementation on company performance and to decide whether to invest in the expansion of IT capacities.
Originality/value
This research reports the impact of IT on supply chain performance in one of the most important industries in many industrialized countries, and it provides a new perspective on evaluating the contribution of IT on firm performance.

This Research Work in Progress Paper evaluates students’ motivation sources and shows the high impact that work of professors has on students’ motivation. Common goal of most professors is to help students acquiring knowledge and competencies that are relevant for their further life. From the beginning of Universities’ history, lectures are usually chosen to reach this goal but it is seen in recent years that they are in many cases not the optimal choice. In the last years, many efforts were made to increase students knowledge gain. From learning and motivation psychology research, many details are known how humans remember and transfer knowledge. These research results are used to create new lecture formats to activate students. Research based learning, project based lab courses, problem based learning, feedback systems, flipped classroom, blended learning and gamification in lectures are only some examples for new formats. Common goal of all these new types of learning formats is to increasing students’ motivation and to enhance learning success. In this paper, evaluations using a questionnaire with open questions were done among first year students, bachelor students before graduation and alumni, about their sources of motivation and demotivation. Interesting curricula and lectures with application related topics and possibilities for own work are the main sources of motivation. Enthusiastic professors with high competences and good lecture didactics also contribute to students’ motivation. On the other side, demotivated professors with boring lectures play a much higher role for demotivating students. Therefore, it is necessary to integrate aspects of student motivation into curriculum and lecture design and professors should become aware that they are important role-models for motivating or demotivating students.

Objectives
Germany has one of the highest numbers of coronary angiography worldwide. Nevertheless, we find a great variation between German regions for both, diagnostic coronary angiographies and percutaneous coronary interventions (PCI). We assume that this variation is not only due to variation in morbidity but also reflects factors such as guideline adherence, physician-patient communication and access to care.
In this mixed method project, we aim to first describe the variation of coronary angiographies and PCIs in different German regions. Secondly, we explore current practices and motives for (non-)adherences to guidelines in the diagnostic process of patients with suspected CHD. Based on these results, we consequently plan to develop a complex intervention (treatment pathway) to improve guideline adherence and thus appropriateness of coronary angiography.
Method
The projects will be organized according to the recommendation of the Medical Research Council for the development and evaluation of complex interventions. The first study will use descriptive methods based on routine data of three German Health Care Insurances and registry data to describe status quo and associated factors of coronary angiography. The second study will use qualitative methods to understand barriers and facilitators of guideline adherence and medical decision making. Furthermore, we will discuss variations in care and thereby identify implementation targets for the planned treatment pathway. Based on this information, we will develop local treatment pathways in four selected regions. Relevant peers will develop the local pathway in group discussions. Using this bottom-up approach, we directly address implementation challenges.
In recent years, musculoskeletal computation has become a widely used tool to investigate joint and muscle forces within the human body. However, the issue of muscle fatigue is not considered adequately in most models and is a challenging task. One aspect that needs to be examined is the interaction of muscles during an exhausting task. Therefore, an experimental study was designed to analyze the changes of back muscle recruitment pattern during such exercises.
In this study 38 subjects (27 male, 11 female, height = 177±8.5 cm, weight = 74.0±13.6 kg) participated. Each subject had to perform three static and three dynamic exhausting exercises where the back muscles were loaded with subject specific forces using a dynamometer adapter especially designed for the trunk muscles. To collect the muscle activity, twelve surface electromyography sensors were applied on the back, and four on the abdominal muscles. Muscle activity and fatigue were analyzed by calculating the maximum voluntary contraction normalized signal and the median frequency. At first the fatigue of m. erector spinae and m. multifidi was analyzed, since these muscles carry the main load during the exercises. Subsequently the activity of the m. trapezius, m. rectus abdominis and m. obliquus externus were investigated to determine recruitment patterns. To gain more detailed information of these patterns a numerical model was built using the AnyBody Modeling System™. Analyzing the measurements, we can observe an increasing muscle activity during isokinetic exercises while the force is constant. Since the activity in the simulation is defined as the current force output divided by the strength of the muscle, the strength parameter was scaled down based on the measured data, assuming a linear force – activity correlation, and using a numerical algorithm considering the influence of cross talk.
The results show, that changes in recruitment pattern can be divided into three major subgroups. Prior to total exhaustion, some of the subjects show additional activation of muscles in the trapezius region, while other subjects show an additional activation of abdominal muscles, increasing the intra-abdominal pressure which supports the spine. In the third group an activation in both regions can be observed.
The numerical simulations show an increasing activity of abdominal muscles as well as muscles in the upper back. Especially the m. latissimus dorsi shows a significantly higher activity.
The results lead to the conclusion that prior to total exhaustion, additional muscles are recruited to support the main muscles. It was shown that abdominal muscles are activated to support back muscles by pressurizing the trunk cavity to delay total exhaustion as long as possible. In conclusion, the results show that changes in muscle recruitment pattern need to be considered when introducing muscle fatigue to musculoskeletal models.
Die unterschiedlichen Bildungswege in der deutschen Sekundarstufe führen abhängig von der Schulform zu abweichenden Lernergebnissen der Englischkompetenzen. Doch in der Tertiärstufe werden die Bildungswege wieder zusammengeführt und die Absolvent(inn)en aller Schulformen befinden sich in einer gemeinsamen Lernumgebung. Die vorgestellte empirische Untersuchung von über 450 Studierenden an zwei bayerischen Fachhochschulen hinsichtlich des Bildungshintergrunds und der Sprachkompetenzen belegt, dass die Lernenden in den nunmehr gemeinsamen Englischkursen im Tertiärbereich über stark unterschiedliche Kompetenzniveaus verfügen. Die Daten zeigen eine ausgeprägte Kaskade im Kompetenzniveau basierend auf der früheren Sekundarschulform, wobei Studierende der vertieften allgemeinbildenden Schule (Gymnasium) ihre Qualifikationen höher einschätzen als Studierende von berufsbildenden Sekundarschulen (FOS und BOS). Obwohl in allen bayerischen Sekundarschulformen mindestens das GER-Niveau B2 als Standard für Schulabgänger festgelegt ist, geben in der Stichprobe von Studierenden an Fachhochschulen mehr als die Hälfte der Lernenden das Niveau B1 in mindestens einer der Fertigkeiten für Englisch an; und etwas mehr als 8% der Studierenden berichten ihre Selbsteinschätzung in jeder Fertigkeit als Niveau B1 oder niedriger. Die Ergebnisse lassen erkennen, dass ein breites Spektrum an Kompetenzniveaus von B1 bis C1 an den Fachohochschulen häufig endemisch ist, und dass die derzeitige Praxis, Englischkurse im Tertiärbereich für die idealisierten B2-Lernenden zu gestalten, den Bedürfnissen vieler Studierenden nicht ausreichend entgegenkommt.

Die Einstellungen von Asylsuchenden gegenüber Berufsorientierung, Familie, Geschlechterrollen, Religion und religiöser Toleranz sind häufig Gegenstand kontroverser Diskussion geworden und sie spielen eine wichtige Rolle innerhalb der Sozialen Arbeit. Im Rahmen des Forschungsprojekts "Asylsuchende in Bayern" wurden Asylsuchende und Expert_innen danach befragt, jedoch auch zu ihrer Motivation, in Deutschland ihren Platz zu finden und zu ihren Zukunftserwartungen.
This study examines attitudes toward assisted reproductive technologies (ART) among immigrant women and non-migrants in Germany. The social relevance of ART is increasing in Western countries due to overall low birth rates, a high rate of childlessness, and a gap between the desired and the actual numbers of children. Previous literature has been scarce, however, on attitudes toward ART, and immigrant minorities have rarely been included in studies on ART.
Our working hypotheses are drawn from theoretical considerations on political socialisation and cultural integration. The analysis is based on data collected in a pilot study in 2014 and 2015. The sample includes 960 women aged 18 to 50 living in Germany. About 81 percent of the sample are immigrants who originate from Turkey, Poland, the Balkan countries, or countries of the (Russian) Commonwealth of Independent States (CIS). We study the social norm to use ART to have a child, the personal attitude of whether a woman would use ART herself, and the methods that they would consider for their own use.
Our results show that ART is overall socially acceptable, and the majority of women said that they would use it if necessary. There is significant variation between the origin groups, however. Non-migrants show the lowest acceptance rates and migrants from Poland and Turkey the highest approval. There is also variation in the ART procedures considered for use with the migrants more approving of heterologous methods than non-migrants. The differences between the origin groups diminish only partly when controlling for further explanatory variables, i.e. gender-role attitudes, religiosity, and socio-demographic characteristics of the respondents.
We conclude that attitudes toward ART are shaped less by socio-demographic characteristics, but rather by cultural factors and the socialization in the migrants’ countries of origin. The diversity in attitudes toward ART by cultural background should be acknowledged in research and public discourses on ART as well as in regulating policies.

Was wissen Frauen, insbesondere solche mit Migrationshintergrund, über die Angebote der Reproduktionsmedizin? Wie hoch ist die Bereitschaft, reproduktionsmedizinische Maßnahmen tatsächlich zu nutzen? Auf der Grundlage umfangreicher empirischer Studien können die Autorinnen und Autoren erstmals Antworten auf diese Fragen geben. Ihre Ergebnisse lassen Aussagen zu über Informationswege und die Rolle des Internets beim Wissenstransfer sowie über die Akzeptanz assistierter Reproduktion und die Inanspruchnahme von Beratungs- und Behandlungsangeboten.
Die Studie verknüpft auf der Grundlage einer einzigartigen Methodenvielfalt medizinsoziologische Fragestellungen mit solchen der empirischen Familien-, Gesundheits- und Mediennutzungsforschung. In Teilstudien wurden Frauen mit und ohne Migrationshintergrund in einem Telefonsurvey befragt, aber auch ärztliches Personal sowie Expertinnen und Experten aus reproduktionsmedizinischen Zentren. Darüber hinaus wurde eine qualitative Analyse eines Internetforums durchgeführt und eine umfangreiche Literaturauswertung vorgenommen.
This paper deals with the validation and modification of the German questionnaire "F-Komp". In its original version, it was intended to measure university students' research competences. In the beginning of this study, there were only a few tools available which were reliable. For the purposes of this study, they were not suitable. At the same time, there was no validated version of the F-Komp available, which made the whole validation process for further usage necessary. This questionnaire is based on a structure, which consist of different skills and knowledge and is focused on measuring research competence in general. The validation and modification of the F-Komp is therefore the aim of our contribution as well as a revised version of the questionnaire. We proceeded an explorative factor and a reliability analysis to do a general evaluation of the tool. Some modifications were done in the questionnaire to make it more suitable to the requirements of technical oriented universities of applied sciences [5]. Our revised version is slightly longer and contains several items to gather data about the participants demographics. The modified questionnaire is based on a more appropriate factor structure. This structure is more practically oriented and pays attention to ethical issues. In future cases, this questionnaire will be used in research oriented courses to measure students' progress in acquiring the knowledge and methods which are necessary to perform as a scientist in different research areas.
Krankenhäuser sind nicht nur Spendennehmer, sie sind auch selbst sozial engagierte Akteure. Der Beitrag liefert Beispiele für dieses Engagement und erläutert sie als Aufbau von Sozialkapital, dem mit der Perspektive der Corporate Citizenship ein geeigneter Rahmen gegeben werden kann. Der Citizenship-Ansatz wird für Kliniken erläutert. Den Abschluss bilden Überlegungen zur Akzeptanz im ärztlichen Dienst und in der Pflege.
The Canadian Armed Forces (CAF) are using chemical, biological, radiological and nuclear (CBRN) general service respirators (GSRs) to defend against CBRN threats and hazards to protect personnel and ensure that they can safely and successfully carry out missions. The CBRN GSR currently in use by the CAF is the C4 respirator. The National Research Council of Canada assessed the communication performance of the C4 respirator. This report presents the results of the performance test. The method for assessing the communication performance used in this study was the NIOSH standard test procedure TEB-CBRN-APR-STP-0313 [1]. Other methods exist to evaluate the communication performance of respirators, but historically the NIOSH standard test procedure has been used. This report details the implementation and results of the tests conducted at the National Research Council of Canada, following the NIOSH standard test procedure.
BACKGROUND: Validation and verification of multibody musculoskeletal models sEMG is a difficult process because of the reliability of sEMG data and the complex relationship of muscle force and sEMG. OBJECTIVE: This work aims at comparing experimentally recorded and simulated muscle activities considering a numerical model for crosstalk. METHODS: For providing an experimentally derived reference data set, subjects were performing elevations of the arm, where the activities of the contemplated muscle groups were measured by sEMG sensors. Computed muscle activities were further processed and transformed into an artificial electromyographical signal, which includes a numerical crosstalk model. In order to determine whether the crosstalk model provides a better agreement with the measured muscle activities, the Pearson correlation coefficient has been computed as a qualitative way of assessing the curve progression of the data sets. RESULTS: The results show an improvement in the correlation coefficient between the experimental data and the simulated muscle activities when taking crosstalk into account. CONCLUSIONS: Although the correlation coefficient increased when the crosstalk model was utilized, it is questionable if the discretization of both, the crosstalk and the musculoskeletal model, is accurate enough.
As a result of increasing globalisation, universities are being confronted by the challenge of adapting their teaching standards to meet the demands of a more dynamic labour market. Entrepreneurship education provides an important contribution to improving students ́ entrepreneurial competencies and stimulating innovative work behaviour. Based on the current state of research, as well as on the results of an empirical comparative study at partner institutions of applied sciences, this article describes how the internationalisation of curricula with regards to the academic teaching of entrepreneurship and the promotion of entrepreneurial competence can be structured. An online survey of students carried out within the framework of an EU project, as well as joint class observations, revealed some important differences in study-related matters, which emphasises the need for stronger cooperation in terms of cross-border employability. With regards to the promotion of entrepreneurial thinking and behaviour, it appears appropriate to intensify the use of case-based instructional designs within teaching standards, to extend the supportive offers to students and to enable engagement with entrepreneurship in various courses of studies.
Writing, with no doubt, is besides reading a core competency which allows us to "exploit" knowledge in general. It also makes possible the exploration of software engineering's core issues. Especially within this context it is necessary to master the reading of complex texts as well as to be able to write in an appropriate academic expression. With regard to studies in software engineering this seems to be obvious, but in fact the opposite is the reality. Therefore measures to improve these skills seemed to be necessarily applied. At the Ostbayerische Technische Hochschule (OTH, University of Applied Sciences, Regensburg) a new format, the so called c*lab, was installed during the winter semester 2017. This was a course which followed the principle of "Writing Across the Curriculum" (WAC). Organized parallel to a lecture of learning how to program the language C, and addressing students of the first semester, the course was a complete voluntary offer in addition to the general standard courses and lectures of the faculty. Students who participated not only reflected on C and its principles, nor only on writing as a self purpose, but they also learned to express technical thoughts and ideas by the use of didactic methods. The idea to transfer also basic LTeX concepts to write a paper based on the IEEE bare_conf.tex-template were also planned. The course followed the idea of student's-centred learning. This paper presents the main structure, goals, and means of the c*lab, and the theory behind. It also embeds the course within the horizon of experiences of teaching writing skills at the Laboratory for Safe and Secure Systems (LaS 3 ) at the faculty of electronic and information engineering at the OTH Regensburg. First experiences have shown that participants increase writing skills and their idea of the importance of writing.
Purpose
The complexity of the treatment techniques IMRT (Intensity Modulated Radiation Therapy) and VMAT (Volumetric Modulated Arc Therapy) introduced the requirement of an individual plan verification. The latest development is 3D verification which allows evaluation in terms of 3D gamma values but also a judgment based on differences in dose distributions on individual patient CT data and in dose volume histograms. For a verification tool the error detection sensitivity is important. The purpose of this work is to evaluate the accuracy and the sensitivity of the gamma evaluation of the 3D verification software MobiusFx when a systematic error in one parameter is applied. The results are compared to an established 2D-measurement based method.
Methods
11 IMRT and 11 VMAT plans were selected as reference plans. For every reference plan a systematic offset of 1 mm, 2 mm and 3 mm in the same direction was applied to the Multi-Leaf-Collimator (MLC) positions. All plans were irradiated and verified simultaneously with MobiusFx and MatriXX Evolution 2D array measurement. For both verification systems the applied dose distributions were evaluated using the gamma method based on the reference plans. In MobiusFx additionally the MLC position errors were evaluated.
Results
Regarding the gamma evaluation results, MobiusFx and MatriXX Evolution 2D measurements have almost the same sensitivity: The gamma evaluation of both systems detected shifts of 1 mm in VMAT plans and shifts of 2 mm and larger in IMRT plans. The information of the MLC position error provided by MobiusFx allows further to detect errors down to a shift of 1 mm in IMRT plans, giving an advantage over the MatriXX measurement.
Conclusions
MobiusFx shows a high error detection sensitivity, comparable to MatriXX Evolution. Through the gamma calculation and the MLC position error it is possible to detect errors down to a shift in MLC positions of 1 mm. It is recommended to inspect all the above parameters during plan verification.
Acht Prozent der Studierenden in Deutschland weisen eine Behinderung oder chronische Erkrankung auf 1, ca. vier Prozent leiden an einer sog. Teilleistungsstörung (z.B. Legasthenie und Dyskalkulie) 2, etwa 2% aller Studierenden sind von Legasthenie betroffen3. Damit müssen ca. 40.000 Studierende 4 ihr Studium mit und um und unter Legasthenie organisieren. Während es außerhalb von Prüfungen bereits zahlreiche Unterstützungsangebote gibt, verkommt die Berücksichtigung von Legasthenie im sensiblen Bereich der Hochschulprüfungen mitunter zu einem Vabanquespiel, dessen Ausgang insbesondere davon abhängig ist, an welcher Hochschule das Studium absolviert wird, welche Lehrperson die Prüfung stellt und welche Sachbearbeiterin bzw. welcher Sachbearbeiter über einen etwaigen Ausgleich entscheidet; so existiert an Hochschulen keine einheitliche Handhabung des Nachteilsausgleichs bei Legasthenie und teilweise haben es Studierende noch immer schwer, ihren Anspruch auf einen entsprechenden Nachteilsausgleich durchzusetzen6, teilweise nur bestehen spezifische Beratungsangebote und adäquate Hilfsmittel , teilweise wird ein nur regelmäßiger i.S.e. pauschalen Ausgleichs gewährt, teilweise herrscht Unkenntnis vorund teilweise bleibt Legasthenie ein schambesetztes Tabuthema.
Instructors of English for Special Purposes often describe the challenge of determining which specific content and competencies to address in their courses. After all, it is the focus on technical subject matter which students expect to differentiate engineering ESP from standard EFL. Yet ESP instructors, who frequently have a background in fields such as applied linguistics or education, often receive little input from technical specialists as to which specialist topics are most relevant to their target domain.
Our research indicates that ESP instructors in higher education can leverage a valuable resource to enrich teaching effectiveness: students who have already gained professional experience. As part of a longitudinal study at two universities of applied sciences (UAS) in southern Germany, engineering students at the beginning of their first semester of compulsory technical English participate in an online survey. The course participants provide relevant biographical information such as work experience and evaluate the perceived importance of a selection of skills and applications for a Technical English course.
The surveys, administered in October 2016 and October 2017 and encompassing nearly 1000 respondents, provide compelling insights. Analysis reveals conclusive correlations between the perceived importance of course components and students' declared level of previous work experience. Listening comprehension, reading comprehension, process description, and test and measurement are topics whose importance correlates positively with work experience level. In addition, there are clear trends relating work experience level with the importance of the reading and writing of technical documents as well as communication and interaction.
This data provides empirically-founded criteria for selecting the topics to include in the scope of a Technical English course for engineers, informed by the judgment of students with occupational experience. Additionally, recognizing that students with work experience have legitimate practitioner credibility allows instructors to identify and select relevant subject matter in concordance with experienced students' learning objectives. ESP learners will profit greatly from cultivating skills with proven practical applicability.
Chemische Prozesse können durch nichtlineare Differentialgleichungen beschrieben werden, typischerweise mit unbekannten Modellparametern. Durch Parameterschätzung können die Modelle a\n experimentelle Daten angepasst werden. Die geschätzten Parameter sind mit statistischen Unsicherheiten behaftet, die von der Art der Experimente abhängen und bereits vor deren Durchführung berechnet und durch optimale Versuchsplanung minimiert werden können. In diesem Beitrag werden diese Optimierungsprobleme formuliert, Methoden zur Lösung diskutiert und ihre Wirksamkeit durch Anwendung auf ein chemisches Reaktionssystem demonstriert.
Nicht alles, was statistisch signifikant ist, ist auch klinisch relevant. Statistische Signifikanz vorausgesetzt, kann die Frage der Relevanz für die meisten Problemstellungen nur individuell beantwortet werden. Die entscheidende Kennzahl dafür ist die Effektgröße einer Maßnahme, wie sie sich vor allem aus randomisiert kontrollierten Studien ergibt. Kleine Therapieeffekte können im Individualfall relevant sein, auch wenn sie für die meisten Patienten entbehrlich sind – und umgekehrt. In diesem Artikel geht es um quartäre Prävention: die Verhinderung irrelevanter oder nutzloser Medizin. Die Unterscheidung zwischen „illness“ (Leiden des Patienten) und „disease“ (Diagnose des Arztes) ermöglicht ein besseres Verständnis der präventiven Aufgaben der Primärmedizin und erleichtert die Abgrenzung von sinnvoller zu nutzloser Medizin. Man kann mithilfe dieser Differenzierung die Prävention in vier Formen einteilen: Primäre Prävention – der Einsatz medizinischer Maßnahmen, obwohl der Patient kein Leiden verspürt und auch keine Diagnose vorliegt, wie zum Beispiel Impfungen. Sekundäre Prävention – Früherkennungsmaßnahmen bei Personen mit Risikofaktoren, die aber bisher nicht krank sind. Tertiäre Prävention – Verhinderung weiterer Ereignisse bei bereits erkrankten Patienten. Die quartäre Prävention meint im weiteren Sinne die Verhinderung nutzloser Medizin und bezieht sich auf die in der Primärmedizin häufige Situation, dass zwar ein Leiden vorliegt, ärztlicherseits jedoch keine behandlungsbedürftige Krankheit diagnostiziert werden kann (und man deshalb mit Diagnostik und Therapie besonders zurückhaltend sein sollte).
We propose a new permanent magnet system for Kibble balance experiments, which combines advantages of the magnet designs invented by the National Physical Laboratory (NPL) and by the Bureau International des Poids et Mesures (BIPM). The goal of the proposed magnet system is to minimize the coil-current effect and to optimize the shielding at the same time. In the proposed design, a permanent magnet system with two gaps, each housing a coil, is employed to minimize the coil current effect, by reducing the linear coil-current dependence reported for the single air gap design by at least one order of magnitude. Both air gaps of the magnet are completely surrounded by high-permeability material, and hence the coils are shielded from outside magnetic fields and no magnetic field leaks outside of the magnet system. An example of the new magnet system is given and the analysis shows that the magnetic field in the air gap can be optimized to meet the requirement to be used in Kibble balances.
In order to fix a fracture using minimally invasive surgery approaches, surgeons are drilling complex and tiny bones with a 2 dimensional X-ray as single imaging modality in the operating room. Our novel haptic force-feedback and visual assisted training system will potentially help hand surgeons to learn the drilling procedure in a realistic visual environment. Within the simulation, the collision detection as well as the interaction between virtual drill, bone voxels and surfaces are important. In this work, the chai3d collision detection and force calculation algorithms are combined with a physics engine to simulate the bone drilling process. The chosen Bullet-Physics-Engine provides a stable simulation of rigid bodies, if the collision model of the drill and the tool holder is generated as a compound shape. Three haptic points are added to the K-wire tip for removing single voxels from the bone. For the drilling process three modes are proposed to emulate the different phases of drilling in restricting the movement of a haptic device.

Hintergrund/Fragestellung:
Eine Rheumatoide Arthritis (RA) verläuft chronisch schubweise und Betroffene leiden unter zunehmenden und schwankenden Symptomen. In der Physiotherapie (PT) werden individuell abgestimmte Interventionen in einem ambulanten (aPT) oder Gruppensetting (gPT) eingesetzt. In Leitlinien wird PT unspezifisch empfohlen. Ärzte verordnen spezifische Formen der PT. Wie ist eine evidenzbasierte und am Bedarf aus Sicht Betroffener orientierte Versorgung zu gestalten?
Methoden/Material:
Drei Methoden wurden kombiniert: 1) Evidenz zu PT wurde mittels eines Systematischen Reviews ermittelt und deren Qualität in Anlehnung an GRADE beurteilt. 2) Einflussfaktoren auf eine Versorgung wurden mittels Sekundäranalyse von Versorgungsdaten aus einer Längsschnittstudie bestimmt. 3) Einflussfaktoren auf den Bedarf an sowie die Verordnung von PT aus Sicht der Betroffenen wurden mittels Fragebogen erhoben. Die Daten wurden soweit möglich mittels Metaanalysen (1), binären (2) sowie multiplen linearen Regressionen und Gruppenvergleichen für abhängige Stichproben (3) analysiert. Es wurden Häufigkeiten (3) der Nennung spezifischer Interventionen sowie von Übereinstimmungen zwischen Bedarf und wahrgenommenem Verordnungsverhalten bestimmt.
Ergebnisse:
1) Eine gPT zeigte bei jungen Betroffenen mit guter Funktion Effekte, war aber bei Vorschäden der Gelenke weder effektiv noch sicher. Zu aPT liegt keine Evidenz vor. 2) Der einzige relevante Vorhersagefaktor für eine Versorgung mit PT ist eine bereits bestehende Versorgung (Exp(B)=10,81 - 171,53, p<0,001). 3) Den stärksten bedarfssteigernden Einfluss hat ein schlechtes körperliches Befinden (B=0,25, p<0,05/MWD 1,3 (NAS 1-10), p<0,001). Teilnehmer (n=305) gaben am häufigsten Bedarf an aPT an, bei schlechtem Befinden vor allem an passiven Maßnahmen. In besseren Phasen besteht zusätzlich Bedarf an gPT. Betroffene haben individuell unterschiedliche Bedarfe und eine Berücksichtigung bedarfsbestimmender Faktoren bei der Verordnung wird kaum wahrgenommen.
Schlussfolgerungen:
Angesichts der geringen Evidenzlage und individueller und schwankender Bedarfe wird ein stark individualisiertes Vorgehen empfohlen. Effektivität, Sicherheit und Passgenauigkeit sind regelmäßig gemeinsam mit den an der Versorgung Beteiligten sowie den Betroffenen zu überprüfen. Insbesondere bei schlechtem Befinden sollte über die Möglichkeit einer aPT und die notwendige Überprüfung aufgeklärt werden. Es besteht dringender Forschungsbedarf für aPT bei RA.
Over the past decade, veno-venous extracorporeal membrane oxygenation (vvECMO) has been increasingly utilized in respiratory failure in patients. This study presents our institution´s experience focusing on the life span of ECMO systems reflecting the performance of a particular system. A retrospective review of our ECMO database identified 461 adult patients undergoing vvECMO (2010-2017). Patients that required more than one system and survived the first exchange >24 hours (n = 139) were included. Life span until the first exchange and exchange criteria were analyzed for all systems (PLS, Cardiohelp HLS-set, both Maquet Cardiopulmonary, Rastatt, Germany; Deltastream/Hilite7000LT, iLA-activve, Xenios/NovaLung, Heilbronn, Germany; ECC.O5, LivaNova, Mirandola, Italy). At our ECMO center, the frequency of a system exchange was 30%. The median (IQR) life span was 9 (6-12) days. There was no difference regarding the different systems (p = 0.145 and p = 0.108, respectively). However, the Deltastream systems were exchanged more frequently due to elective technical complications (e. g. worsened gas transfer, development of coagulation disorder, increased bleedings complications) compared to the other exchanged systems (p = 0.013). In summary, the used ECMO systems are safe and effective for acute respiratory failure. There is no evidence for the usage of a specific system. Only the increased predictability of an imminent exchange preferred the usage of a Deltastream system. However, the decision to use a particular system should not depend solely on the possible criteria for an exchange.
Anatomical differences between individuals are often neglected in musculoskeletal models, but they are necessary in case of subject-specific questions regarding the lumbar spine. A modification of models to each subject is complex and the effects on lumbar loading are difficult to assess. The objective of this thesis is to create a validated musculoskeletal model of the human body, which facilitates a subject-specific modification of the geometry of the lumbar spine. Furthermore, important parameters are identified in sensitivity studies and a case study regarding multifidus muscle atrophy after a disc herniation is conducted. Therefore, a generic model is heavily modified and a semi-automatic process is implemented. This procedure remodels the geometry of the lumbar spine to a subject-specific one on basis of segmented medical images. The resulting five models are validated with regard to the lumbar loading at the L4/L5 level. The influence of lumbar ligament stiffness is determined by changing the stiffness values of all lumbar ligaments in eleven steps during a flexion motion. Sensitivities of lumbar loading to an altered geometry of the lumbar spine are identified by varying ten lumbar parameters in simulations with each model in four postures. The case study includes an analysis of the loading of the multifidus muscle and of the lumbar discs throughout various stages of disc herniation. This time each model performs four motions with two different motion rhythms. The results indicate that lumbar motion and loading is dependent on lumbar ligament stiffness. Furthermore, subject-specific modelling of the lumbar spine should include at least the vertebral height, disc height and lumbar lordosis. The results of the case study suggest that an overloading of the multifidus muscle could follow disc herniation. Additionally, a subsequent atrophy of the muscles could expose adjacent levels to an increased loading, but these findings are highly dependent on the individual.
This paper provides a systematic literature review, analysis and discussion of methods that are proposed to practise ethics in research and innovation (R&I). Ethical considerations concerning the impacts of R&I are increasingly important, due to the quickening pace of technological innovation and the ubiquitous use of the outcomes of R&I processes in society. For this reason, several methods for practising ethics have been developed in different fields of R&I. The paper first of all presents a systematic search of academic sources that present and discuss such methods. Secondly, it provides a categorisation of these methods according to three main kinds: (1) ex ante methods, dealing with emerging technologies, (2) intra methods, dealing with technology design, and (3) ex post methods, dealing with ethical analysis of existing technologies. Thirdly, it discusses the methods by considering problems in the way they deal with the uncertainty of technological change, ethical technology design, the identification, analysis and resolving of ethical impacts of technologies and stakeholder participation. The results and discussion of our literature review are valuable for gaining an overview of the state of the art and serve as an outline of a future research agenda of methods for practising ethics in R&I.
This paper deals with the identification of learning ob-stacles using the questionnaire method. Therefore, two iterations were proceeded: The first one was part of a survey that was carried out at four lo-cations at universities of applied sciences. We asked students about obstructive facts in general providing items for five learning ob-stacle dimensions that were set up before; emotional/motivational, epistemological/cognitive, didactical, resource-related and meta-cognitive learning obstacle dimensions. After the general part, we asked them to answer the same question, but in relation to the – in their opinion – most difficult learning content. With this question, we aim to get indications regarding to epistemological obstacles. In a second step, we used the “Motivated Strategies for Learning Questionnaire”, which was developed by Pintrich [1] as a basis to develop a questionnaire that extracts learning obstacles. In its original version, the “Motivated Strategies for Learning Question-naire” was intended to measure students’ learning strategies, but, as the obstacle dimensions were partly derived from learning strategy classification, we chose this already validated question-naire [2]. Within this iteration, we could confirm a five-factor structure of the questionnaire that could be mapped to the five be-fore set learning obstacle dimensions.
Learning tasks play an important role in education and especially in higher education. However, there is a significant lack in research regarding these in higher education. A learning task shows several characteristics, whereas the didactic function of a task is mostly considered. Two characteristics of learning tasks are focused in this paper: The didactic function and the type knowledge. Existing types of learning tasks are presented as well as a proposal for learning tasks in software engineering education that considers didactic functions, like elaboration, training or application and types of knowledge, i.e. factual, conceptual, procedural and metacognitive knowledge. This paper aims to serve as a guidance for lecturers who have the purpose to create learning tasks that address both characteristics -- the didactic function and the type of knowledge.
