In previous studies, musculoskeletal simulations helped to understand and facilitate the complex issues of musculoskeletal disorders in several different branches [2, 3]. A fundamental prerequisite for the generation of these models is to have a basic understanding of the stresses acting on the pelvic floor. The aim of this study is to investigate the biomechanical factors influencing the activity of the pelvic floor muscles by establishing a parameterised model. Methods To identify the factors that affect the pelvic floor muscles, the process is divided into several stages. The first step is to investigate the loads acting on the pelvic floor. For this, a simplified sagittal cross-sectional model of the upper body is analysed. The discretization of the abdominal cavity is presented with resulting force vectors representing the organs in the abdominal and pelvic region, the tensile force of the structures that fixate the organs as well as the intraabdominal pressure. A second step is the examination of the basic mechanics of the pelvic floor muscles. As the functional anatomy of these muscles demonstrate differences compared to other muscle groups [4], the understanding of how they differ is crucial for the construction of musculoskeletal models. After a static examination of loads on a simplified muscle model, an extended model including the physiological properties of a muscle must be involved in the considerations. Therefore, the influence of parameters such as the ideal muscle strength and fibre length, as well as elasticity or passive stiffness of the elements must be taken into account. The influencing factors of the variables can be analysed in a simplified pelvic floor muscle model by systematically changing the parameters. Results The process of analysing the factors influencing the activity of the pelvic floor muscles follows two main strategies. The review of the effects of the whole body identifies the forces acting on the pelvic floor muscles and effect their activity. With the examination of the basic mechanics of a simplified pelvic floor muscle model a better understanding is achieved of how the pelvic floor muscles absorb the loads acting in the body. Discussion This process results in the main influence factors on the activity of the pelvic floor. The consequent parameters form the basis for a biomechanically justified construction of musculoskeletal simulation models of the pelvic floor. In this work, we are interested in developing tools for the automatic identification of Parkinson’s disease using machine learning and the concept of BoVW. The proposed approach concerns a hierarchical-based learning technique to design visual dictionaries through the Deep Optimum-Path Forest classifier. The proposed method was evaluated in six datasets derived from data collected from individuals when performing handwriting exams. Experimental results showed the potential of the technique, with robust achievements. The negative impact on the patient's daily routine has moved the science in search of new techniques that can reduce its negative effects and also identify the disease in individuals. One of the main motor characteristics of PD is the hand tremor faced by patients, which turns out to be a crucial information to be used towards a computer-aided diagnosis. In this context, we make use of handwriting dynamics data acquired from individuals when submitted to some tasks that measure abilities related to writing skills. This work proposes the application of recurrence plots to map the signals onto the image domain, which are further used to feed a Convolutional Neural Network for learning proper information that can help the automatic identification of PD. The proposed approach was assessed in a public dataset under several scenarios that comprise different combinations of deep-based architectures, image resolutions, and training set sizes. Experimental results showed significant accuracy improvement compared to our previous work with an average accuracy of over 87%. Moreover, it was observed an improvement in accuracy concerning the classification of patients (i.e., mean recognition rates above to 90%). The promising results showed the potential of the proposed approach towards the automatic identification of Parkinson's disease. Measuring these internal forces is very difficult or almost impossible; therefore, advanced models are necessary to perform musculoskeletal simulations. The objective of this investigation was to see what loads are acting in the individual foot joints from the phalanges to calcaneus and talus during different brake scenarios. The Glasgow-Maastricht AnyBody Foot Model with 26 separate segments, connected by joints, ligaments and muscles was used inside the AnyBody Modeling System to compute individual mid foot joint loads. The amount, the direction of the force and additionally also the load insertion point was varied for several simulations. The simulation showed that for the different brake scenarios, different muscles will be activated in the human and therefore different loads are apply in the fore-and mid-foot, respectively. Electroencephalography (EEG) has shown to be a critical tool for identifying driver fatigue as it can record brain activity associated with sleepiness, which gives it an advantage over other physiological modalities. Although raw EEG data can provide insightful information, accurate fatigue identification requires strong feature extraction techniques because of the inherent complexity of the data. This emphasizes how urgent it is to investigate revolutionary deep-learning architectures that can successfully extract discriminative features from raw EEG data. This study provides an innovative framework for driver fatigue identification from EEG that combines continuous wavelet transform (CWT) with convolutional neural networks (CNN) and long-short term memory (LSTM). In order to gain the advantage over the drawbacks of manual feature extraction, this study generates time-frequency spectrum representations of EEG data using the CWT. Following extracting information, each channel’s time-frequency images are concatenated and fed into a CNN-LSTM architecture. This combination models the temporal and spatial characteristics of the EEG data and automatically learns discriminative features for identifying drivers’ normal and fatigued states. A publicly available EEG dataset with recordings from twelve subjects is used to evaluate the proposed CWT-CNN-LSTM architecture. The result shows a prominent classification accuracy of 98.34% for both the average of each subject and combined subjects. These findings show how well the CNN-LSTM framework captures EEG patterns associated with fatigue, which may result in more reliable driver fatigue detection systems and improved traffic safety. However, the limited background variation and simple motion rendered the dataset uninformative in learning about which techniques would be suitable for segmentation in real surgery. In 2017, at the same workshop in Quebec we introduced the robotic instrument segmentation dataset with 10 teams participating in the challenge to perform binary, articulating parts and type segmentation of da Vinci instruments. This challenge included realistic instrument motion and more complex porcine tissue as background and was widely addressed with modfications on U-Nets and other popular CNN architectures [1]. In 2018 we added to the complexity by introducing a set of anatomical objects and medical devices to the segmented classes. To avoid over-complicating the challenge, we continued with porcine data which is dramatically simpler than human tissue due to the lack of fatty tissue occluding many organs. Regaining mobility is accompanied with improved quality of life in persons of very advanced age recovering from fragility fractures. Objectives: Quantitative survey of regaining physical mobility provides recommendations for action on how to use motion trackers effectively in a clinical geriatric setting. Methods: Method mix of quantitative and qualitative interdisciplinary and mutual complementary research approaches (sociology, health research, philosophy/ethics, medical informatics, nursing science, gerontology and physical therapy). While validating motion tracker use in geriatric traumatology preliminary data are used to develop a target group oriented motion feedback. In addition measurement accuracy of a questionnaire about quality of life of multimorbid geriatric patients (FLQM) is tested. Conclusion: Implementing a new technology in a complex clinical setting needs to be based on a strong theoretical background but will not succeed without careful field testing. Fragestellung: Die Studie untersucht die Impfbereitschaft von Eltern minderjähriger Kinder und Personen ohne minderjährige Kinder, wobei insbesondere Geschlechtsunterschiede überprüft werden. Methoden: Die Studie basiert auf einer Zufallsstichprobe (Telefon-Survey, n = 2014, Erhebung zwischen 12.11.2020 und 10.12.2020). Die Auswertung stützt sich insbesondere auf die Teilstichprobe von Personen mit minderjährigen Kindern im Haushalt (n = 461). Ergebnisse: Eltern weisen durchgängig eine geringere Impfbereitschaft mit einem COVID-19-Vakzin auf als Befragte ohne minderjährige Kinder (54,1 % vs. 71,1 %). Väter weisen eine stärker ausgeprägte eigene Impfbereitschaft auf als Mütter. Darüber hinaus sind Männer eher als Frauen bereit, das eigene Kind mit einem COVID-19-Vakzin impfen zu lassen. Schlussfolgerungen: Bei Eltern und insbesondere Müttern ist eine erhebliche Fehleinschätzung von Impfrisiken und häufiger Glaube an Impfverschwörungstheorien zu beobachten. Volume 260: dHealth 2019 - From eHealth to dHealth N2 - Background: This article is based on an ongoing long-term study, in which customary motion trackers measure steps during rehabilitation of geriatric trauma patients (Med=86 years). Objectives: Exploring steps after 28 days of measurement. Finding similarities in the data by running cluster analysis and formulating linear regressions models to predict steps through time. Methods: Two types of motion trackers (FitBitAlta HR and Garmin vívofit 3) have been used to measure patients' (N=24) steps after hip fracture in two study groups. Cluster analysis detected three clusters for progress in number of steps that were tested for group differences with ANOVA. Regression analysis tested models for individual patients. Results: Three-cluster solutions showed significant differences for the average amount of steps after 5, 14, 21 and 28 days. Regression models could predict 71 % of the individual patients' progress in study group 2. Conclusion: The long-term study will provide more data in the future to examine the three-cluster solution and to find out in what stage of rehabilitation the measurement of the steps could be used to predict individual rehabilitation. Depiction of the survey situation, with regard to the subjects in particular, can serve as practical examples for designing future research projects. The group of older adults is a rather large and growing group for which research is required, especially concerning their heterogeneity, their individual autonomy and quality of life. It is assumed, that research designs of studies on the target group must be specifically adjusted, in particular when considering the attribution of vulnerability of the group members. At the same time, it is not clear yet what exact specifics of the subjects and target group must be considered in research designs, as surprisingly little is known about the target group as subjects and corresponding theories have been insufficiently tested. The exploratory long-term design of the research project presented in the second section of this chapter has a positive evaluation of an ethics committee. Still ethical challenges occurred in the field situation, that are illustrated in the third section of this chapter, by providing information on the patients, their role as research subjects, how they were recruited, how an informed consensus was reached, and in some cases how participation was rejected or abandoned. After a summary, the end of the paper is marked by recommendations on how to design future research projects. Cumulatively it must always be expected that interaction between researchers and research subjects of this target group can become very intensive, what requires to follow clearly defined procedures and at the same time to be prepared to act flexibly. Intensity-modulated techniques spare dose to the surrounding tissue, but the risk for second malignancies may be increased due to the increase in low-dose volume. The aim of this study is to investigate if the flattening filter free (FFF) mode allows reducing the risk for second malignancies compared to the mode with flattening filter (FF) for intensity-modulated techniques and to 3DCRT. A reduction of the risk would be advantageous for treating pediatric ependymoma. 3DCRT was compared to intensity-modulated radiation therapy (IMRT) and volumetric-modulated arc therapy (VMAT) with and without flattening filter. Dose-volume histograms (DVHs) were compared to evaluate the plan quality and used to calculate the excess absolute risk (EAR) to develop second cancer in the brain. Dose verification was performed with a two-dimensional (2D) ionization chamber array and the out-of-field dose was measured with an ionization chamber to determine the EAR in peripheral organs. Delivery times were measured. Both VMAT and IMRT achieved similar plan quality in terms of dose sparing in the OAR and higher PTV coverage as compared to 3DCRT. Peripheral dose in low-dose region, which is proportional to the EAR in organs located in this region, for example, gonads, bladder, or bowel, could be significantly reduced using FFF. The lowest peripheral EAR and lowest delivery times were hereby achieved with VMATFFF . The EAR calculated based on DVH in the brain could not be reduced using FFF mode. VMATFFF improved the target coverage and homogeneity and kept the dose in the OAR similar compared to 3DCRT. In addition, delivery times were significantly reduced using VMATFFF . Therefore, for radiotherapy of ependymoma patients, VMATFFF may be considered advantageous for the combination of Elekta Synergy linac and Oncentra External Beam planning system used in this study. The challenges faced by educational and social work professionals working with this target group are documented alongside the particular resources which these highly-motivated young people as a rule bring with them. Specific requirements for psychotherapy professionals are addressed as well as a range of recommendations formulated by the author to enable the longer-term success of this vocational training programme in integrating these young people into the German educational and employment system. It has become apparent that a holistic approach to the lives of the target group by professionals in education, social work and therapy is necessary to enable young asylums seekers to complete vocational training successfully. Transitional management is a key part of the process of setting asylum seekers on the path to an apprenticeship. Coordination with the Labour Office, local authority administration and local decision-makers are further parts of the equation. This must intermesh with civil society engagement to provide the maximum use of resources giving young refugees access to training, the labour market  and long term integration into German life. Based on the theoretical assumption that systemic discrimination creates important barriers to educational equity, we investigate how institutional and systemic factors infl uence the educational participation of refugee-background youth and how these youth cope with such barriers. Using a two-case study in the cities of Hamburg and Munich, we performed a meta-analysis of research reports, complemented by secondary analysis of primary data. Die am häufigsten ausgewählten Gründe für den Einsatz waren die Empfehlung der (früheren) Ausbildungseinrichtung, die eigene Berufserfahrung und Erkenntnisse der besuchten Fortbildungen. T1 - Standalone performance of artificial intelligence for upper GI neoplasia: a meta-analysis JF - Gut N2 - Objective: Artificial intelligence (AI) may reduce underdiagnosed or overlooked upper GI (UGI) neoplastic and preneoplastic conditions, due to subtle appearance and low disease prevalence. Only disease-specific AI performances have been reported, generating uncertainty on its clinical value. Design: We searched PubMed, Embase and Scopus until July 2020, for studies on the diagnostic performance of AI in detection and characterisation of UGI lesions. Primary outcomes were pooled diagnostic accuracy, sensitivity and specificity of AI. Secondary outcomes were pooled positive (PPV) and negative (NPV) predictive values. We calculated pooled proportion rates (%), designed summary receiving operating characteristic curves with respective area under the curves (AUCs) and performed metaregression and sensitivity analysis. Results: Overall, 19 studies on detection of oesophageal squamous cell neoplasia (ESCN) or Barrett's esophagus-related neoplasia (BERN) or gastric adenocarcinoma (GCA) were included with 218, 445, 453 patients and 7976, 2340, 13 562 images, respectively. AI-sensitivity/specificity/PPV/NPV/positive likelihood ratio/negative likelihood ratio for UGI neoplasia detection were 90% (CI 85% to 94%)/89% (CI 85% to 92%)/87% (CI 83% to 91%)/91% (CI 87% to 94%)/8.2 (CI 5.7 to 11.7)/0.111 (CI 0.071 to 0.175), respectively, with an overall AUC of 0.95 (CI 0.93 to 0.97). No difference in AI performance across ESCN, BERN and GCA was found, AUC being 0.94 (CI 0.52 to 0.99), 0.96 (CI 0.95 to 0.98), 0.93 (CI 0.83 to 0.99), respectively. Overall, study quality was low, with high risk of selection bias. No significant publication bias was found. Conclusion: We found a high overall AI accuracy for the diagnosis of any neoplastic lesion of the UGI tract that was independent of the underlying condition. This may be expected to substantially reduce the miss rate of precancerous lesions and early cancer when implemented in clinical practice. Die zu beurteilenden Proband*innen wurden während der Durchführung des abgewandelten STS standardisiert in der Frontal- und Sagittalebene gefilmt und mittels eines Fragebogens von Berufsanfänger*innen der Physiotherapie beurteilt. Bei dem erstellten Fragebogen handelte es sich um eine systematische Abfrage aller möglichen Bewegungen der großen Gelenke der unteren Extremität mit dichotomen Antwortmöglichkeiten. Eingeschlossen wurden Proband*innen mit oder ohne diagnostizierten Knieproblemen, die älter als 18 Jahre und deutschsprachig sind. Proband*innen mit einschränkenden Nebenerkrankungen wie z.B. neurologische Erkrankungen und Operationen in den letzten 6 Monaten wurden ausgeschlossen. One goal of this work is to use musculoskeletal models to investigate the influence of mental stress on lower extremity loading. Moreover, the models themselves are evaluated for highly dynamic movements and practical recommendations for action will be derived. For this purpose, fast movements of youth competitive and amateur athletes will be recorded using different measurement systems. Subsequently, the models calculate the target parameters using inverse dynamics. Furthermore, measured and calculated muscle activities of the lower extremities are compared and artificial balancing forces (residuals) in the models are analyzed and minimization approaches are presented. The investigation of muscle and joint loading under mental stress has shown that the response to mental stress is highly individual. Athletes may experience a significant increase in muscle and knee forces with a simultaneous decrease in performance. The comparison of measured and calculated muscle activity proved the reliability of the models also for highly dynamic movements. With the frequently used default settings in the model and optical and inertial motion capture, the muscle activities in the model could be calculated reliably. The residual forces were highest, when the model transitioned from foot-ground contact to no contact and vice versa. By adjusting the settings of the kinematic filter and the ground reaction force prediction, the residuals were reduced by up to 54%. The analysis of musculoskeletal loading under mental stress has shown that the models can make a valuable contribution to the biomechanical analysis of highly dynamic movements. Subsequently, the models have also proven to be a reliable tool for the analysis of highly dynamic movements when the calculated parameters as well as the model-specific optimization options are reviewed. With this in mind, these models can contribute to further understand highly dynamic movements and prevent muscle injuries in athletes. N2 - Die Analyse und das Verständnis hochdynamischer Bewegungen ist ein fundamentaler Bestandteil der Biomechanik. Da Sportverletzungen häufig die unteren Extremitäten und Muskeln betreffen, können muskuloskelettale Modelle dazu beitragen, sie zu vermeiden. Diese Modelle ermöglichen die Berechnung von Boden- und Gelenkreaktionskräften sowie von Muskelkräften und -aktivitäten für einzelne Muskelstränge. Ein Ziel dieser Arbeit ist es, mit Hilfe von muskuloskelettalen Modellen den Einfluss von mentalen Belastungen auf die Belastung der unteren Extremitäten zu untersuchen. Darüber hinaus werden die Modelle selbst für hochdynamische Bewegungen evaluiert und praktische Handlungsempfehlungen abgeleitet. Zu diesem Zweck werden schnelle Bewegungen von jugendlichen Leistungs- und Freizeitsportlern mit verschiedenen Messsystemen aufgezeichnet. Anschließend berechnen die Modelle mittels inverser Dynamik die Zielparameter. Weiterhin werden gemessene und berechnete Muskelaktivitäten der unteren Extremitäten verglichen sowie künstliche Ausgleichskräfte (Residuen) in den Modellen analysiert und Minimierungsansätze vorgestellt. Die Untersuchung der Muskel- und Gelenkbelastung unter mentalem Stress hat gezeigt, dass die Reaktion auf mentalen Stress sehr individuell ist. Bei einzelnen Personen kann es zu einem deutlichen Anstieg der Muskel- und Kniegelenkreaktionskräfte bei gleichzeitiger Leistungsminderung kommen. Der Vergleich von gemessener und berechneter Muskelaktivität beweist die Zuverlässigkeit der Modelle auch bei hochdynamischen Bewegungen. Mit den häufig verwendeten Standardeinstellungen im Modell und der vielseitigen Bewegungserfassung konnten die Muskelaktivitäten im Modell zuverlässig berechnet werden. Die Residualkräfte waren am höchsten, wenn das Modell von Fuß-Boden-Kontakt zu keinem Kontakt und umgekehrt überging. Durch Anpassung der Einstellungen des kinematischen Filters und der Berechnung der Bodenreaktionskraft konnten die Residualkräfte um bis zu 54% reduziert werden. Die Analyse der muskuloskelettalen Belastung unter psychischer Beanspruchung hat gezeigt, dass die Modelle einen wertvollen Beitrag zur biomechanischen Analyse von hochdynamischen Bewegungen leisten können. In der Folge haben sich die Modelle auch bei der Überprüfung der berechneten Parameter sowie der modellspezifischen Optimierungsmöglichkeiten als zuverlässiges Werkzeug für die Analyse hochdynamischer Bewegungen erwiesen. Twelve professional youth soccer players performed highly dynamic runs with and without additional cognitive stress. The runs were analysed with a musculoskeletal simulation software. The data analysis shows no difference in knee joint reaction loading under additional mental stress compared to the baseline. Yet running times are significantly lower in the baseline. While there is no increase in the joint loads, the running times indicate an altered movement behaviour when the subjects are exposed to additional mental demand. The hypothesis is that mental stress reduces performance and changes the muscular forces exerted. Materials & methods: Twelve elite youth football players were subjected to mental stress while performing sports-specific change-of-direction movements. A modified version of the d2 attention test was used as stressor. The kinetics are computed using inverse dynamics. Running times and exerted forces of injury-prone muscles were analysed. Results: The stressor runs were rated more mentally demanding by the players (p = 0.006, rs = 0.37) with unchanged physical demand (p = 0.777, rs = 0.45). This resulted in 10% longer running times under stress (p < 0.001, d = −1.62). The musculoskeletal analysis revealed higher peak muscle forces under mental stress for some players but not for others. Discussion: The study shows that motion capture combined with musculoskeletal computation is suitable to analyse the effects of stress on athletes in highly dynamic movements. For the first time in football medicine, our data quantifies an association between mental stress with reduced football players’ performance and changes in muscle force. Nevertheless, there is still little understanding on how biomechanical loading in the human body changes when exoskeletons are used. Therefore, further analyses are needed. A promising candidate for these are musculoskeletal models, which are based on an inverse dynamics approach and can calculate external parameters such as ground reaction forces or other interaction forces as well as internal parameters such as joint reaction forces or muscle activities. The various examples in the literature show that these models are increasingly used for assessing the biomechanical effects of exoskeletons on the human body. Furthermore, musculoskeletal models can calculate biomechanical loadings of humans with and without exoskeletons for all kinds of applications and allow an evaluation of their purpose. OBJECTIVE: Focusing on joint angles, the study aimed to evaluate the accuracy of VMC as a viable alternative for biomechanical research. METHODS: Eighteen healthy subjects performed isolated movements with 17 joint degrees of freedom, and their kinematic data were collected using both an MMC and a VMC setup. The kinematic data were entered into the AnyBody Modelling System, which enables the calculation of joint angles. The mean absolute error (MAE) was calculated to quantify the deviations between the two systems. RESULTS: The results showed good agreement between VMC and MMC at several joint angles. In particular, the shoulder, hip and knee joints showed small deviations in kinematics with MAE values of 4.8∘, 6.8∘ and 3.5∘, respectively. However, the study revealed problems in tracking hand and elbow movements, resulting in higher MAE values of 13.7∘ and 27.7∘. Deviations were also higher for head and thoracic movements. CONCLUSION: Overall, VMC showed promising results for lower body and shoulder kinematics. However, the tracking of the wrist and pelvis still needs to be refined. The research results provide a basis for further investigations that promote the fusion of VMC and musculoskeletal models. OBJECTIVE: This work aims at comparing experimentally recorded and simulated muscle activities considering a numerical model for crosstalk. METHODS: For providing an experimentally derived reference data set, subjects were performing elevations of the arm, where the activities of the contemplated muscle groups were measured by sEMG sensors. Computed muscle activities were further processed and transformed into an artificial electromyographical signal, which includes a numerical crosstalk model. In order to determine whether the crosstalk model provides a better agreement with the measured muscle activities, the Pearson correlation coefficient has been computed as a qualitative way of assessing the curve progression of the data sets. RESULTS: The results show an improvement in the correlation coefficient between the experimental data and the simulated muscle activities when taking crosstalk into account. CONCLUSIONS: Although the correlation coefficient increased when the crosstalk model was utilized, it is questionable if the discretization of both, the crosstalk and the musculoskeletal model, is accurate enough.
