    <title language="eng">Rural areas and social work: Reflection on research in German-speaking countries</title>
Demographic change, securing social services, dealing with migration and agreeing to democratic participation are particularly rural challenges in Austria, Switzerland and Germany. Although social work is responsible for all these issues, there is a lack of a differentiated view. What exactly are rural areas? Who are the relevant actors in and for social work? As the social contexts are very different, research must differentiate and take into account the cultural and historical contexts of regional challenges. This article aims to strengthen the research perspective on rural areas and demonstrate their relevance in the discourse on social work.

    <title language="eng">Impact of sarcopenia on outcomes in surgical patients: A systematic review and meta-analysis</title>
    <abstract language="eng">BACKGROUND&#13;
Surgeons have historically used age as a preoperative predictor of postoperative outcomes. Sarcopenia, the loss of skeletal muscle mass due to disease or biological age, has been proposed as a more accurate risk predictor. The prognostic value of sarcopenia assessment in surgical patients remains poorly understood. Therefore, we aimed to synthesize the available literature and investigate the impact of sarcopenia on peri- and postoperative outcomes across all surgical specialties.&#13;
METHODS&#13;
We systematically assessed the prognostic value of sarcopenia on postoperative outcomes by conducting a systematic review and meta-analysis according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, searching the PubMed/MEDLINE and EMBASE databases from inception to 1st October 2022. Our primary outcomes were complication occurrence, mortality, length of operation and hospital stay, discharge to home, and postdischarge survival rate at one, three, and five years. Subgroup analysis was performed by stratifying complications according to the Clavien-Dindo classification system. Sensitivity analysis was performed by focusing on studies with an oncological, cardiovascular, emergency, or transplant surgery population, and on those of higher quality or prospective study design.&#13;
RESULTS&#13;
A total of 294 studies comprising 97,643 patients, of which 33,070 had sarcopenia, were included in our analysis. Sarcopenia was associated with significantly poorer postoperative outcomes including greater mortality, complication occurrence, length of hospital stay, and lower rates of discharge to home (all P&lt;0.00001). A significantly lower survival rate in patients with sarcopenia was noted at one, three, and five years (all P&lt;0.00001) after surgery. Subgroup analysis confirmed higher rates of complications and mortality in oncological (both P&lt;0.00001), cardiovascular (both P&lt;0.00001), and emergency (P=0.03 and P=0.04, respectively) patients with sarcopenia. In the transplant surgery cohort, mortality was significantly higher in patients with sarcopenia (P&lt;0.00001). Among all patients undergoing surgery for inflammatory bowel disease, the frequency of complications was significantly increased among sarcopenic patients (P=0.007). Sensitivity analysis based on higher-quality studies and prospective studies showed that sarcopenia remained a significant predictor of mortality and complication occurrence (all P&lt;0.00001).&#13;
CONCLUSION&#13;
Sarcopenia is a significant predictor of poorer outcomes in surgical patients. Preoperative assessment of sarcopenia can help surgeons to identify patients at risk, critically balance eligibility, and refine perioperative management. Large-scale studies are required to further validate the importance of sarcopenia as a prognostic indicator of perioperative risk, especially in surgical sub-specialties.

    <abstract language="eng">Plastic surgeons are trained to perform a wide repertoire of surgeries-ranging from standard local procedures to highly specialized operations. Therefore, plastic surgeons treat a plethora of clinical presentations and address multiple patient needs. Their daily workflow is increasingly entwined with legal topics. The concrete legal&#13;

However, by understanding the legal basics of selected surgical procedures, plastic surgeons may generate synergies in patient care and clinical practice. The legal situation is to be elucidated based on the German Basic Law (GBL) and the European Convention on Human Rights&#13;

    <title language="deu">Akzeptanz des Einsatzes von Künstlicher Intelligenz im Anamneseprozess: Ergebnisse einer Bevölkerungsbefragung in Deutschland</title>
    <abstract language="deu">Hintergrund und Zielsetzung&#13;
Zunehmend werden digitale Anamneseverfahren, wie z. B. Chatbots, entwickelt und evaluiert, sind jedoch in Arztpraxen in Deutschland noch nicht flächendeckend implementiert. Ziel dieser Arbeit ist die Erhebung der Akzeptanz der Bevölkerung in Deutschland gegenüber KI-gestützten Verfahren im Anamneseprozess, durch die es möglich wird, Rückschlüsse auf die Nutzung der Verfahren und ihre Eignung als Bestandteil von Digitalisierungsstrategien der Praxen zu ziehen.&#13;
Methode&#13;
Die Studie basiert auf einer Onlinebefragung der Bevölkerung in Deutschland (Feldphase 18.-24.11.2024, n = 1.000). Zurückgehend auf die „Unified Theory of Acceptance and Use of Technology“ (UTAUT) wurden drei zentrale Hypothesen zur Nutzung von Künstlicher Intelligenz (KI) im Anamneseprozess aufgestellt und mit linearen Regressionsmodellen getestet.&#13;
Ergebnisse&#13;
Die Akzeptanz für die Nutzung von KI-gestützten Systemen fällt in beiden untersuchten Szenarien, der Nutzung eines Chatbots zu Hause und der Nutzung eines Chatbots in einer Arztpraxis, hoch aus. Dabei bestätigen sich die erwarteten Zusammenhänge von Leistungserwartung, Aufwandserwartung und erwartetem Sozialem Einfluss mit der Verhaltensabsicht, einen Chatbot in der Arztpraxis oder vor dem Besuch einer Arztpraxis zu nutzen. Die Einflüsse von Alter, Geschlecht oder Erfahrung mit der Technologie zeigen jedoch nicht die vermuteten Zusammenhänge.&#13;
Diskussion und Schlussfolgerung&#13;
    <abstract language="eng">Background and objectives&#13;
Digital tools for medical history-taking, such as chatbots, are increasingly being developed and evaluated but have not yet been implemented across the board in medical practices in Germany. The aim of this study was to survey the acceptance of AI-supported tools for medical history-taking among the German population, thereby allowing conclusions to be drawn about the use of these tools and their suitability as part of digitalization strategies in medical practices.&#13;
Method&#13;
The study is based on an online survey among the German population (fieldwork November 18 to 24, 2024, n = 1,000). Based on the ‘Unified Theory of Acceptance and Use of Technology’ (UTAUT), three central hypotheses on the use of artificial intelligence (AI) in the medical history-taking process were generated and tested using linear regression models.&#13;
Results&#13;
In the two scenarios examined — patients using a chatbot at home and at the doctor’s office — there was a high level of acceptance for the use of AI-supported systems. The expected relationships between Performance Expectancy, Effort Expectancy and expected Social Influence with the behavioral intention to use a chatbot either in the doctor’s office or before visiting the doctor’s office have been confirmed. However, influential factors such as age, gender, or experience with technology do not show the expected correlations.&#13;
Discussion and conclusion&#13;
Practicability is crucial for a favorable attitude towards AI applications. There were no apparent gender differences. When implementing chatbots in the medical history-taking process, the aspects of practicability, data protection, and data security should play a key role. Given that a large number of the elderly in Germany still have limited experience with digital technologies, usability is highly relevant for both this population group and for people with disabilities.

<title language="eng">Comparing kinematic and kinetic demands on the knee joint during selected physiotherapy exercises and activities of daily living</title>
Background: The knee is one of the most common areas to suffer injuries or be affected by surgery. Physiotherapy rehabilitation was shown to support recovery, but evidence guiding optimal rehabilitation practices is limited. To recommend appropriate exercises, it is essential to understand the musculoskeletal requirements involved in both physiotherapy and activities of daily living (ADLs).ObjectiveThis study aimed to evaluate and compare the knee joint kinematics, joint forces and muscle activity in knee flexors and extensors during selected rehabilitation exercises and ADLs.MethodsKinematic and kinetic data from 30 healthy participants were collected during 20 different tasks. Full-body musculoskeletal simulations were performed to estimate peak knee joint angles, angular velocities, joint reaction forces, and muscle activity of the knee flexors and extensors.ResultsComparatively high requirements were observed for lunges, squats, stair walking and gait. Medium requirements were observed for sitting down and rising from a chair. Low requirements were observed for balance shifts and variations of the single leg stand.ConclusionOverall, ADLs like gait and stair walking show surprisingly high requirements compared to many exercises employed in physiotherapy. These findings are a step towards biomechanically informed exercise selection and the development of personalized rehabilitation programs.

    <title language="deu">Chancen und Herausforderungen der Digitalisierung bei der Rehabilitation – Ergebnisse einer Befragung von Rehabilitationseinrichtungen</title>
    <abstract language="deu">Ziel der Studie: &#13;
Ziel der Studie ist die Messung des Stands der Digitalisierung und die mit einer Anbindung an die Telematikinfrastruktur verbundenen Chancen und Herausforderungen für Rehabilitationseinrichtungen.&#13;
&#13;
Methodik: &#13;
Teilstandardisierte Online-Befragung bei Trägern von Rehabilitationseinrichtungen in Bayern (n=33). Der Fragebogen mit 36 Fragen beinhaltet eine leicht veränderte Skala auf Basis des „Electronic Medical Record Adoption Model (EMRAM)“.&#13;
&#13;
Ergebnisse: &#13;
Der Digitalisierungsgrad wurde in 70 Prozent der Rehabilitationseinrichtungen mit Stufe 0 angegeben (Stufenmodell bis 7). Die Übermittlung patientenbezogener Daten (Eingang und Ausgang) erfolgt häufig analog, wohingegen die Verarbeitung innerhalb der Einrichtung in vielen Fällen bereits überwiegend digital ist. Beim Anschluss an die Telematikinfrastruktur wird hoher Aufwand bei der Installation, aber auch der Schulung des Personals und der Anpassung der Arbeitsorganisation gesehen.&#13;
&#13;
Schlussfolgerung: &#13;

    <title language="eng">Group rehabilitation for adults with acquired neurological disorders: A systematic review of mono- and interdisciplinary interventions in physical and speech-language therapy</title>
    <abstract language="eng">Background: Group treatments and interdisciplinary collaboration are recommended in evidence-based guidelines for neurorehabilitation, including physical and speech-language therapy. Evidence suggests that activating overlapping neural networks for upper extremity motor control and speechlanguage processing produces synergistic effects during therapy. This systematic&#13;
review aims to overview and appraise the efficacy of group treatments in traditional rehabilitation and telerehabilitation. In addition to summarizing evidence on monodisciplinary approaches in physical and speech-language therapy, it seeks data on integrative approaches involving one or both disciplines to inform further interdisciplinary collaboration.&#13;
Methods: The review was registered with PROSPERO (CRD42021288012) and followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Systematic searches were conducted in PubMed, CINAHL, and the Cochrane Library. Two reviewers independently screened studies, extracted data, and assessed quality using AMSTAR 2, the Physiotherapy Evidence Database (PEDro) scale, or the Joanna Briggs Institute (JBI) Checklist, as appropriate. The evidence was summarized in a systematic narrative synthesis and its certainty rated based on the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach.&#13;
Results: A total of 29 studies were included: 16 on speech-language therapy (861 participants) and 13 on physical therapy (1757 participants). No studies addressed interdisciplinary group interventions, and only two evaluated group telerehabilitation. Outcome domains and measures varied across studies and the certainty of evidence was predominantly low. However, moderate-certainty evidence supports that group speech-language therapy improves quality of life,&#13;
communication, and language in stroke survivors, especially when interventions emphasize verbal production in communicative settings with multimodal materials and cueing. In physical therapy, circuit class training may be more effective than other group approaches for enhancing quality of life and mobility. &#13;
Conclusion: Group treatments in neurorehabilitation show some benefits, but further research is needed – especially regarding interdisciplinary approaches and telerehabilitation.
