T1 - Variation in market access decisions for cell and gene therapies across the United States, Canada, and Europe JF - Health Policy N2 - Transformative cell and gene therapies have now launched worldwide, and many potentially curative cell and gene therapies are in development, offering the prospect of significant health gains for patients. Access to these therapies depend on decisions made by health technology assessment (HTA) and payer organizations. We sought to describe the emerging cell and gene therapies market access landscape by analyzing 17 US commercial payer medical policies, and HTA reports from five European countries and Canada. We found that some US health plans applied coverage restrictions more often than others (four plans applied restrictions in all decisions, while four plans applied restrictions in <30% of decisions). The European and Canadian HTA bodies recommend access to fewer therapies than US health plans, reflecting a more stringent approach in the context of limited evidence and high scientific uncertainty that is commonly associated with these treatments. Our findings suggest that patient access to approved cell and gene therapies is restricted in all regions studied, though the nature of these restrictions differs between US health plans and the European/Canada HTA recommendations. Payers, HTA groups, pharmaceutical companies, and other stakeholders should collaborate to more clearly define the "uncertainties" and develop market access policies that balance benefits of early access with ongoing data collection to close evidence gaps over time. The shift from intravenous to oral anticancer medication enhances patient independence but may threaten adherence. This study aims to identify patients' concerns and needs for treatment of oral hormone receptor-positive for advanced breast cancer (CDK4/6 inhibitors). Methodology: A comprehensive literature review on factors influencing patient adherence was conducted, followed by AI-based big data analyses of user-generated content. Techniques such as natural language processing and machine learning-based word embeddings were applied to achieve a patient-centered understanding of adherence. Results: The literature review shows that adherence is often overlooked in clinical research due to the impracticality of long-term monitoring in most trial designs. This highlights the need for alternative methods such as real-world data and AI-based analyses to understand patients' long-term adherence to treatment. Key factors influencing adherence to oral breast cancer treatment include the cost-benefit ratio in terms of survival and quality of life, realistic expectations regarding side effects, social support and personalized care. AI analysis of user-generated content supports these findings and reveals more frequent discussions on non-adherence in early breast cancer stages. Semantic graphs and topic modeling also identified additional patient concerns and informational needs for better patient communication. Conclusions: Enhancing patient-centeredness is crucial for improving adherence and persistence to oral treatment for breast cancer. The increased autonomy provided by oral medications offers a significant opportunity for healthcare providers to empower patients in managing their treatment. Comprehensive support, including addressing emotional and psychological issues, is essential, especially in advanced breast cancer stages. Virtual co-creation is considered a viable strategy to develop consumer-centered products in the digital era. As an additional effect, this research highlighted that co-creation experiences may establish beneficial consumer–brand relationships. Design/methodology/approach Using survey data stemming from 727 consumers who virtually engaged in new product development projects, the authors test the hypotheses, applying structural equation modeling. Findings The results of this study show that consumers’ personal features such as novelty seeking and dissatisfaction with existing products moderate the impact of an enjoyable experience on aspects of the consumer–company relationship. Consumers’ dissatisfaction with current product solutions is also found to moderate the relationship between an enjoyable co-creation experience and evoked product interest as well as between product involvement and evoked product interest. The study further substantiates previous findings on the relationship-effects of co-creation and particularly highlight the potential of co-creation experiences for nurturing “imaginary” relationships with the product being co-created significantly prior to market launch. Originality/value Participants in virtual co-creation approaches are widely heterogeneous individuals ranging from customers and Facebook fans to brand community members and innovative users. The study contributes to a better understanding of how the diversity of the crowd can be handled in virtual co-creation and advances the theory of value co-creation as a new marketing paradigm. However, little is known about the effects of more negative personal characteristics that might result in more competitive, antisocial, and even unethical behavior. This paper considers Machiavellianism as one of the traits that constitute the “dark triad of personality” and explores the relationship between Machiavellianism and participants’ contribution behavior in online innovation contests. Specifically we investigate how Machiavellian characteristics influence individuals’ contribution intensity, communication, and interaction behavior within the contest community as well as the quality and kind of their contributions. This study relies on multisource individual-level data from a large innovation contest in the field of public transportation. We find that the three dimensions of Machiavellianism—distrust of others, amorality, and desire for status—have very distinct behavioral consequences in the context of online innovation contests. Specifically, the oppositional consequences of amoral manipulation and striving for status on the one hand and showing distrust of others on the other hand concerning contribution quantity and contribution quality are found. This study contributes to a deeper understanding of negative personality traits such as Machiavellianism as powerful predictors of behavior and of success within competitive innovation environments and leads to important managerial implications regarding the design and management of innovation contests. We address this gap through an analysis of 24 lead user projects investigating the viability, underlying processes, and main differences of these new search strategies. We argue that both methods justify the significant investments by additionally providing a user-centric basis for subsequent ideation sessions with lead users. When distribution activities are outsourced to a logistics service provider (LSP), shippers need to evaluate the LSPs they may (potentially) engage in order to ensure that supply chain sustainability goals are met. In this paper, we present a methodology for LSP evaluation and focus on the ecological dimension of sustainability. We examine how the network carbon footprint of a real-world distribution system is affected by the LSP network that is chosen to forward goods from production facilities to customers. To do this, we analyze the shipment data of an existing Fast Moving Consumer Goods (FMCG) manufacturer. A quantitative distribution network model is set up to study the network carbon footprint of 125 scenarios. Real-world shipments are forwarded via five generic, idealized types of LSP networks. In total, 625 network carbon footprints, specified by three distribution logistics variables and the structure of the LSP network, have been calculated. The results show that LSP structures practicing a geographically decentralized consolidation of shipments are most efficient in reducing of greenhouse gas (GHG) emissions. Furthermore, the effects of changing the manufacturer's and the LSP's strategies for deciding on the tonnage to be forwarded via the LSP network or moved by direct transports are quantified. Finally, we estimate the GHG effect of improving the capacity utilization of the vehicles that move the products. The use of social media has been linked to mental health consequences. Especially TikTok is under scrutiny because of its captivating UX Design as well as controversial privacy policies. This study shows how Mobile Affective Computing Research can reliably and objectively measure the implicit-emotional UX. The results suggest that TikTok triggers positive emotions during onboarding and strengthens the emotional bond with increasing usage - mainly due to the personalization algorithm. In particular, the study further reveals that there are no clear relationships when “only self-reported data” is used to understand app usage behavior. However, emotion data based on facial coding shows that TikTok users get emotionally aroused. Initially, a positive and relaxed mood triggers further usage time, while - counter-intuitively - later sessions result in an increasingly negative and aroused mood. However, people are largely unaware of these relationships when questioned. Thus, implicit measures, such as facial coding, may help identify valid social media usage reactions. These results yield implications that call for political regulation considering aspects of social media apps and user behavior.
