Jugendliche als produktive Realitätsverarbeiter: zur Neuausgabe des Buches "Lebensphase Jugend"
(2012)
Youth protest, sometimes accompanied by riots, in a number of
European countries over the last few years repeatedly trigger the question whether similar forms of protest can be expected in the near future from young people in Germany as well. While any, often spontaneous eruption of (violent) protest, can never be predicted with certainty, we argue, based on an analysis of the last Shell Youth Studies (cf. Shell Deutschland 2002, 2006, 2010), that it is rather unlikely to happen in Germany, indeed that if anything this likelihood has decreased over recent years. We identify three explanations for the – compared to a range of other European countries – striking acquiescence of the current young generation in Germany: 1. A range of institutions that keep young Germans in the education system until they finally find employment, 2. a strong culture of pragmatic individualism, and 3. a strong distrust of political parties.
In highly developed countries, the adolescent phase of life is expanding more and more. The consequences are status insecurity and inconsistency. A side effect is that adolescence is no longer clearly separable as demarcated biographical stage from adulthood. In this paper, we argue that the developmental tasks of a typical adolescent and a typical adult can only be partially differentiated, and the exact point in time when adolescents become adults can no longer be clearly identified. We find evidence that adolescence is becoming more ‘adult’ and adulthood is correspondingly becoming more ‘juvenile’. In almost all important social areas of life, the behavioral patterns of adults and of adolescents are coalescing. Living with insecurity in the working life and in social relations, experiencing limitations of personal autonomy, and the continuous need to work on social status inconsistencies today are characteristic features of the life of adolescents and adults. As many adolescents meet these demands skillfully and flexibly, they have in many respects become ‘social models’ for the adult life phase.
Aim
Comparative research on health and health inequalities has recently begun implementing a welfare regime perspective. The aim of the study was to review the existing evidence for identifying the determinants of health and health inequalities in highly developed welfare states and to develop a theoretical model for future research approaches.
Subject
A welfare state regime typology is applied to comparatively analyse (1) the relationship between the level of economic prosperity in a society and its respective level of overall population health and (2) the nature of the corresponding relationship between economic inequalities and health inequalities in different groups of countries.
Results
Although the Social Democratic welfare states have a relatively equal distribution of material wealth as well as the highest levels of population health, they are not characterised by the smallest levels of health inequality. Rather, with respect to health equality, conservative countries seem to perform better than social democracies. We propose a comprehensive theoretical model that takes into account different factors on the structural (macro), organisational (meso) and individual (micro) level in order to contribute to a better understanding of this important challenge for public health policy and practice.
Conclusion
Future research will require an appropriate theoretical model with the potential to explain health and health inequalities in different types of welfare states. On the basis of this model, future research should test the hypothesis that in highly developed countries not only economic, but also social, cultural and lifestyle factors are important in determining health outcomes in different segments of the population.
Hintergrund
In 2007, we published the first survey on children in Germany. In that “1st World Vision Children Study,” we decided to perform more than just a representative and standardized questionnaire survey of 1,600 children aged 8–11 years. We also chose to carry out qualitative interviews linked together with a game, and to extend these interviews to include children from the age of 6 years onward. One important finding of the 2007 survey was the devastating impact of social inequality on children’s lives and the growing numbers of children who experience poverty. In 2010, we published a new survey of children, the “2nd World Vision Study.” This covered a sample of 2,500 children aged 6 to 11 years. We added some questions on the feelings and everyday life experiences children have when they are poor. We also included questions on respect, and self-efficacy. Both studies are based on a concept of child well-being. We consider that the World Vision Study of 2010 delivers one major finding: What contributes to the well-being of children in Germany is the granting of freedoms and the experiences of autonomy embedded among experiences of parental care within committed relationships. Hence, it is precisely a combination of freedom and parental care, of co-determination and protection that leads to a high life satisfaction in 6- to 11-year-olds.(Publishers Information)
Bezugswissenschaften der Physiotherapie: Soziologie und Gesundheitswissenschaften / Public Health
(2009)
Background: Since in three preceeding articles already five possible reference sciences of physiotherapy (philosophy, medicine, sports sciences, pedagogics and psychology) were portrayed this last one outlines sociology and health sciences/public health. While philosophy made the start due to its fundamental importance for all sciences the second article illustrated medicine and sports sciences which both are characterised by a historical and content closeness to physiotherapy. The third article investigated the relevance of pedagogics and psychology for physiotherapy, two disciplines which tend to focus on individual processes and structures. The two disciplines to be outlined in this article sociology and health sciences/public health, however, are characterised by their social perspective.
Objective/method: Like in the preceeding articles the disciplines are first outlined by an expert in view of their historical development as a discipline and their over the years emerging subsets, their central purpose and last but not least their current challenges. Following on from this two further experts substantiate exemplary the possible transfer potential for physiotherapy.
Results: Not only with the background of limited resources, growing competition and rising health products plurality in the health sector, it is becoming increasingly important for physiotherapy to professionalise and develop not only in its occupational group. It is rather called upon to position itself in society and its health-care system as well as to actively involve its physiotherapeutic competence in view of new challenges. Both sociology and health sciences offer physiotherapy useful perspectives, theories, methods and techniques.
Conclusions: A prospective physiotherapy science can use valuable impulses out of all seven disciplines portrayed in this article series for its further development and constitution. Apart from dealing with highlighted disciplines it is necessary to develop supplemental reference sciences and to critically investigate their potential for physiotherapy. In this process it is important for all loans from other disciplines to perform an appropriate objective and content transfer into physiotherapeutic questions and not attach great importance to the physiotherapeutic context per se without evaluation.
This paper reviews life course approaches applied in epidemiology, sociology and psychology and proposes a theoretical framework that integrates the concept of socialisation. We argue that the paradigmatic principles of interdisciplinary life course research (human agency, timing in lives, linked lives and historical context) provide a strong basis for a more holistic and theory-driven view of the life course and its interdependent pathways. The ´agency within structure´ paradigm on which these principles are based is an essential part of socialisation theory. We emphasize that socialisation theory has the potential to act as a theoretical link between social science and epidemiology, because it offers a multi-causal framework to illuminate the association between social structure and health across the life course. In order to better understand the development of health and health inequalities in a life course perspective, an interdisciplinary approach is required. The increased attention to socialisation principles might open new dialogue and empirical research perspectives for health sociology and epidemiology. Keywords: ageing, health, life course, social inequalities, socialisation theory
Objectives
Little is known about the impact of recessions on young people’s socioeconomic inequalities in health. This study investigates the impact of the economic recession in terms of youth unemployment on socioeconomic inequalities in psychological health complaints among adolescents across Europe and North America.
Methods
Data from the WHO collaborative ‘Health Behaviour in School-aged Children’ (HBSC) study were collected in 2005/06 (N = 160,830) and 2009/10 (N = 166,590) in 31 European and North American countries. Logistic multilevel models were used to assess the contribution of youth unemployment in 2009/10 (enduring recession) and the change in youth unemployment (2005–2010) to adolescent psychological health complaints and socioeconomic inequalities in complaints in 2009/10.
Results
Youth unemployment during the recession is positively related to psychological health complaints, but not to inequalities in complaints. Changes in youth unemployment (2005–2010) were not associated with adolescents’ psychological health complaints, whereas greater inequalities in complaints were found in countries with greater increases in youth unemployment.
Conclusions
This study highlights the need to tackle the impact of increasing unemployment on adolescent health and health inequalities during economic recessions.
This study aims to determine whether educational differentiation (i.e. early and long tracking to different school types) relate to socioeconomic inequalities in adolescent smoking. Data were collected from the WHO-Collaborative 'Health Behaviour in School-aged Children (HBSC)' study 2005/2006, which included 48,025 15-year-old students (Nboys = 23,008, Ngirls = 25,017) from 27 European and North American countries. Socioeconomic position was measured using the HBSC family affluence scale. Educational differentiation was determined by the number of different school types, age of selection, and length of differentiated curriculum at the country-level. We used multilevel logistic regression to assess the association of daily smoking and early smoking initiation predicted by family affluence, educational differentiation, and their interactions. Socioeconomic inequalities in both smoking outcomes were larger in countries that are characterised by a lower degree of educational differentiation (e.g. Canada, Scandinavia and the United Kingdom) than in countries with higher levels of educational differentiation (e.g. Austria, Belgium, Hungary and The Netherlands). This study found that high educational differentiation does not relate to greater relative inequalities in smoking. Features of educational systems are important to consider as they are related to overall prevalence in smoking and smoking inequalities in adolescence.
The cutaneous carotenoid concentration correlates with the overall antioxidant status of a person and can be seen as biomarker for nutrition and lifestyle. 50 high school students were spectroscopically measured for their cutaneous carotenoid concentrations initially in a static phase, followed by an intervention phase with biofeedback of their measured values, living a healthy lifestyle and on healthy food this time. The volunteers showed higher carotenoid concentrations than found in previous studies. A significant correlation of healthy lifestyle habits and a high antioxidant status could be determined. Subjects improved their nutritional habits and significantly increased their carotenoid concentration during intervention. Follow-up five months later showed a consolidation of the increase. The investigations show that a healthy diet and a well-balanced lifestyle correlate with a high cutaneous antioxidant concentration and that spectroscopic biofeedback measurement of cutaneous carotenoids as part of an integrated prevention program is a feasible and effective means to raise the health awareness in adolescents.
Aim
Health literacy has gained increasing importance worldwide. As a result of several studies indicating low health literacy levels in large parts of the population, strategies and action plans promoting health literacy were developed in many countries. The article analyses the origin, development and structure of the German Health Literacy Action Plan.
Subject and methods
In 2016, a civil society initiative was formed, aiming at developing a national action plan in an expert-based and collaborative manner. A group of 15 experts elaborated the core elements of the action plan, based on a literature review on existing health literacy approaches, action plans and empirical evidence. These core elements were further developed collaboratively in workshops with key stakeholders, self-help and patient organizations and individual consultation meetings. Afterwards, the National Action Plan was reviewed and previously formulated recommendations were refined before being published.
Results and conclusion
The National Action Plan Health Literacy in Germany presents 15 recommendations in four areas of action for promoting health literacy in Germany. This bottom-up strategy and the expert-based yet at the same time collaborative approach have proved to be successful when developing a national action plan.
Background
The COVID-19 pandemic has disrupted the lives of children and adolescents worldwide. The German COPSY study is among the first population-based longitudinal studies to examine the mental health impact of the pandemic. The objective of the study was to assess changes in health-related quality of life (HRQoL) and mental health in children and adolescents and to identify the associated risk and resource factors during the pandemic.
Methods
A nationwide longitudinal survey was conducted with two waves during the pandemic (May/June 2020 and December 2020/January 2021). In total, n = 1923 children and adolescents aged 7 to 17 years and their parents participated (retention rate from wave 1 to wave 2: 85%). The self-report and parent-proxy surveys assessed HRQoL (KIDSCREEN-10), mental health problems (SDQ with the subscales emotional problems, conduct problems, hyperactivity, and peer problems), anxiety (SCARED), depressive symptoms (CES-DC, PHQ-2) and psychosomatic complaints (HBSC-SCL). Mixed model panel regression analyses were conducted to examine longitudinal changes in mental health and to identify risk and resource factors.
Results
The HRQoL of children and adolescents decreased during the pandemic, and emotional problems, peer-related mental health problems, anxiety, depressive and psychosomatic symptoms increased over time, however the change in global mental health problems from wave 1 to wave 2 was not significant, and some changes were negligible. Socially disadvantaged children and children of mentally burdened parents were at particular risk of impaired mental health, while female gender and older age were associated with fewer mental health problems. A positive family climate and social support supported the mental health of children and adolescents during the pandemic.
Discussion
Health promotion, prevention and intervention strategies could support children and adolescents in coping with the pandemic and protect and maintain their mental health
There is an “infodemic” associated with the COVID-19 pandemic—an overabundance of valid and invalid information. Health literacy is the ability to access, understand, appraise, and apply health information, making it crucial for navigating coronavirus and COVID-19 information environments. A cross-sectional representative study of participants ≥ 16 years in Germany was conducted using an online survey. A coronavirus-related health literacy measure was developed (HLS-COVID-Q22). Internal consistency was very high (α = 0.940; ρ = 0.891) and construct validity suggests a sufficient model fit, making HLS-COVID-Q22 a feasible tool for assessing coronavirus-related health literacy in population surveys. While 49.9% of our sample had sufficient levels of coronavirus-related health literacy, 50.1% had “problematic” (15.2%) or “inadequate” (34.9%) levels. Although the overall level of health literacy is high, a vast number of participants report difficulties dealing with coronavirus and COVID-19 information. The participants felt well informed about coronavirus, but 47.8% reported having difficulties judging whether they could trust media information on COVID-19. Confusion about coronavirus information was significantly higher among those who had lower health literacy. This calls for targeted public information campaigns and promotion of population-based health literacy for better navigation of information environments during the infodemic, identification of disinformation, and decision-making based on reliable and trustworthy information.
Purpose
The German population-based longitudinal COVID-19 and Psychological Health study monitors changes in health-related quality of life (HRQoL) and mental health of children and adolescents during the COVID-19 pandemic and identifies vulnerable groups.
Methods
A nationwide, population-based survey was conducted in May 2020 to June 2020 (Wave 1), December 2020 to January 2021 (Wave 2), and September 2021 to October 2021 (Wave 3). In total, n = 2,097 children and adolescents aged 7–17 years were investigated using measures to assess HRQoL (KIDSCREEN-10), mental health problems (SDQ), anxiety (SCARED), depressive symptoms(PHQ-2), and psychosomatic complaints(HBSC-SCL).
Results
The prevalence of low HRQoL increased from 15% prepandemic to 40% and 48% in Waves 1 and 2 and improved slightly to 35% in Wave 3 (all differences significant). Similarly, overall mental health problems increased from 18% prepandemic to 29% in Wave 1 and 31% in Wave 2 to 28% in Wave 3 (all differences significant, except Wave 3 vs. 2), anxiety increased from 15% prepandemic to 24% and 30% in Waves 1 and 2 and was still 27% in Wave 3. Depressive symptoms increased from 10% prepandemic to 11% and 15% in Waves 1 and 2 and were 11% in Wave 3. A group with low parental education, restricted living conditions, migration background, and parental mental health problems was at significantly increased risk of HRQoL and mental health impairments.
Discussion
The prevalence of low HRQoL, mental health problems, and anxiety has been elevated throughout the pandemic. Thus, mental health promotion, prevention, and intervention strategies need to be implemented to support adolescents–particularly those at risk.
Ziel
The aim of this study is to examine the impact of class-level class climate on school-aged children’s life satisfaction. Data was derived from the German National Educational Panel Study (NEPS) using sixth grade school-aged children (n = 4,764, 483 classes). Class climate includes indicators of teachers' care and monitoring, demands, interaction, autonomy, as well as school-aged children's attitudes towards schoolwork at the class- and individual-level. Results showed that individual perceived class climate in terms of teachers' care and monitoring and autonomy was positively related to life satisfaction, whereas school-related demands were related to lower life satisfaction. Besides teachers' care and monitoring at class-level, indicators of class climate were not associated with school-aged children’s life satisfaction, while the individual perceived class climate is more important for life satisfaction.
BACKGROUND:
Studies among students with special educational needs (SEN) in separate special schools (SSS) and mainstream schools (MS) are particularly applicable to educational attainment and social participation. However, indicators of health and wellbeing have rarely been considered.
AIMS:
This study investigates two related topics: first, health and wellbeing differences between students with SEN in special schools (SSS) and students without SEN in regular schools, and second, the rarely considered question whether health and wellbeing among students with SEN differ between school settings (i.e. MS vs. SSS).
METHODS AND PROCEDURES:
Bivariate and multilevel analyses are applied with data from the German National Educational Panel Study (NEPS) with 5267 students (grade 7).
OUTCOMES AND RESULTS:
After having controlled for background characteristics, students in SSS report higher likelihoods of poor self-rated health compared to students in higher track schools. Self-rated health of students with SEN does not significantly differ between MS vs. SSS. For life satisfaction, students with SEN show higher likelihoods of low life satisfaction when attending MS.
CONCLUSIONS AND IMPLICATIONS:
Teachers in inclusive settings are encouraged to establish class work and teaching that support a real change from class placement to inclusive culture in order to suitably support students with SEN.
Background
Features of schools and classes are closely related to students´ health and wellbeing. However, class composition (e.g. in terms of school performance) has rarely been examined in relation to students´ health and wellbeing. This study focuses on the so called Big-Fish-Little-Pond-Effect (BFLPE), by investigating whether the level of high-performing students in classroom is negatively associated with psychosomatic complaints of students who perceive themselves as poor performers.
Methods
Data were derived from the German sample of the WHO-Collaborative “Health Behaviour in School-aged Children (HBSC)” study 2013/2014. The sample included 5226 11-, 13- and 15-year-old students. Individual perceived school performance (PSP) was included (very good/good vs. average/below average PSP) at the individual student-level. At the class-level, school performance in class was generated by aggregating the share (in percentage) of students who report a very good/good PSP to the class-level, indicating the percentage of students with good/very good PSP in classroom. Using multilevel regression models, the association between class-level school performance (in percentage of students with very good/good PSP) and individual psychosomatic complaints were analyzed, stratified by students´ individual PSP.
Results
Students who report average/below average PSP showed higher likelihoods of psychosomatic complaints (Odds Ratio: 1.75; 95% Confidence Interval: 1.52–2.03) compared to counterparts with very good/good PSP. The aggregated class-level PSP was not significantly associated with psychosomatic complaints. However, in line with the BFLPE, results further revealed that students with average/below average PSP, who attend classes with a higher percentage of students who report very good/good PSP, had higher likelihoods of psychosomatic complaints (Odds Ratio: 1.91; 95% Confidence Interval: 1.01–4.01) compared to classmates with very good/good PSP.
Conclusions
This study revealed that class composition in terms of PSP was differentially associated with students´ psychosomatic complaints, depending on their individual PSP. Findings highlight the vulnerability of students with poor PSP placed in classes with a higher percentage of students with good PSP. Results of this study therefore indicate a need for initiatives for low performing students from teachers and school staff in class.
Young women, and not young men, acquire the privileged diplomas offered by the educational system, and it is they who obtain the more promising career prospects. In this paper, we seek to identify factors that account for the declining school performance of boys and young men. After reviewing and analysing the international literature on gender and education, we integrate various explanatory approaches into a comprehensive socialisation model. The model focuses on the developmental tasks young men face during adolescence. Our central thesis is that the declining school performance of young men cannot be fully explained by their failure to cope with the developmental task ‘qualification’. Rather, the three other central clusters of developmental tasks – ‘social attachment’, ‘regeneration’, and ‘participation’ – have to be incorporated. The crucial implication is that supporting young men at school will show only limited results. Rather, to improve the school performance of young men, it is also necessary to address their deficits in coping with the other developmental tasks.
PURPOSE:
Adherence to dental preventive programmes in young adults is low. The aim of the present longitudinal study was to evaluate whether tutoring peers can be a compliance-enhancing tool or not.
METHODS:
In Part 1, two randomly selected classes (49 female students, mean age 19.8 + or - 2.3 years) were taught adult toothbrushing technique (the modified Bass technique) in a project-like manner. After the course, knowledge was tested using a class test, and compliance was evaluated using anonymous quantitative questionnaires. Compliance was defined as a reported degree of change from the easy-to-learn childhood toothbrushing techniques to the more efficient and challenging Bass technique. In Part 2 of the present longitudinal study, the compliance of these students was re-evaluated after having developed and applied themselves a programme of how to tutor peers in oral health. Re-evaluation of compliance was performed after 3 and 9 months.
RESULTS:
In Part 1, 28.5% of the students were compliant after 1 week. Compared with Part 1, the compliance in Part 2 was significantly higher (P u 0.001), both after 3 months (90%) and after 9 months (82%).
CONCLUSIONS:
Tutoring peers can significantly enhance the compliance over a period of 9 months. Tutoring can function as a form of empowerment and can establish a strong sustained health engagement. Tutoring peers in health-related subjects can readily be implemented in schools and might be an additional means of oral health promotion with fewer additional costs.
Comparative research on health and health inequalities has recently started to establish a welfare regime perspective. The objective of this study was to determine whether different welfare regimes are associated with health and health inequalities among adolescents. Data were collected from the ‘Health Behaviour in School-aged Children’ study in 2006, including 11- to 15-year-old students from 32 countries (N = 141,091). Prevalence rates and multilevel logistic regression models were calculated for self-rated health (SRH) and health complaints. The results show that between 4 per cent and 7 per cent of the variation in both health outcomes is attributable to differences between countries. Compared to the Scandinavian regime, the Southern regime had lower odds ratios for SRH, while for health complaints the Southern and Eastern regime showed high odds ratios. The association between subjective health and welfare regime was largely unaffected by adjusting for individual socioeconomic position. After adjustment for the welfare regime typology, the country-level variations were reduced to 4.6 per cent for SRH and to 2.9 per cent for health complaints. Regarding cross-level interaction effects between welfare regimes and socioeconomic position, no clear regime-specific pattern was found. Consistent with research on adults this study shows that welfare regimes are important in explaining variations in adolescent health across countries.
Objectives
Cross-national studies have rarely focused on young people. The aim of this study is to investigate whether macro-level determinants are associated with health and socioeconomic inequalities in young people's health.
Study design
Data were collected from the Health Behaviour in School-aged Children (HBSC) study in 2006, which included 11- to 15-year old adolescents from 27 European and North American countries (n = 134,632). This study includes national income, health expenditure, income inequality, and welfare regime dummy-variables as macro-level determinants, using hierarchical regression modelling.
Main outcome measure
Psychosomatic health complaints and socioeconomic inequalities in psychosomatic health complaints.
Results
Adolescents in countries with higher income inequality and with liberal welfare tradition were associated with more health complaints and a stronger relationship between socioeconomic status and macro-level determinants compared to adolescents from countries with lower income inequality or the Social Democratic regime. National income and health expenditure were not related to health complaints. Countries with higher national income, public health expenditure and income inequality showed stronger associations between socioeconomic status and psychosomatic health complaints.
Conclusion
Results showed that macro-level characteristics are relevant determinants of health and health inequalities in adolescence.
BACKGROUND:
Health literacy is of increasing importance in public health research. It is a necessary pre-condition for the involvement in decisions about health and health care and related to health outcomes. Knowledge about limited health literacy in different age groups is crucial to better target public health interventions for subgroups of the population. However, little is known about health literacy in Germany. The study therefore assesses the prevalence of limited health literacy and associated factors among different age groups.
METHODS:
The Health Literacy Survey Germany is a cross-sectional study with 2,000 participants aged 15 years or older in private households. Perceived health literacy was assessed via computer-assisted personal interviews using the HLS-EU-Q-47 questionnaire. Descriptive analyses, chi-square tests and odds ratios were performed stratified for different age groups.
RESULTS:
The population affected by limited perceived health literacy increases by age. Of the respondents aged 15-29 years, 47.3 % had limited perceived health literacy and 47.2 % of those aged 30-45 years, whereas 55.2 % of the respondents aged 46-64 years and 66.4 % aged 65 years and older showed limited perceived health literacy. In all age groups, limited perceived health literacy was associated with limited functional health literacy, low social status, and a high frequency of doctor visits.
CONCLUSIONS:
The results suggest a need to further investigate perceived health literacy in all phases of the life-course. Particular attention should be devoted to persons with lower social status, limited functional health literacy and/or a high number of doctor visits in all age groups.
