Der gesetzlich verbesserte Zugang zur geriatrischen Rehabilitation – Voraussetzungen und Schwächen

Geriatric rehabilitation is a cornerstone of the German healthcare system to maintain the functional capacity of older patients and prevent the need for long-term care. Until recently, access to geriatric rehabilitation was largely limited to post-acute care. With the introduction of new legislation, patients aged 70 and over can be referred to geriatric rehabilitation by practitioners. However, in order to prescribe geriatric rehabilitation, physicians must document relevant diagnoses supported by the results of a series of functional or cognitive assessments. Alongside this information, the SINGER profile has been introduced. It is mandatory, although there is currently no manual to guide assessment with this tool. Diagnoses and test results must be consistent in documenting the need for geriatric rehabilitation. However, individual assessment of resources and motivation are prerequisites that need to be assessed by prescribing practitioners prior to prescription, considering the structure of the available formats. First analyses document improvement in access to geriatric rehabilitation but point to limited availability.
Bewegen und Stärken: Gesundheitsförderung in der Alltagsbegleitung (BeStärken+) – Projektvorstellung

Hintergrund:
In Deutschland leben 5 Mio. Menschen mit Pflegebedarf, davon rund 80 % im ambulanten Bereich (Statistisches Bundesamt, 2023). Dabei weisen Pflegebedürftige das geriatrische Syndrom Frailty auf. Die Wirksamkeit von bewegungsbezogenen Interventionen bei Menschen mit Frailty konnte bereits auf verschiedene Variablen nachgewiesen werden (u. a. Casas‐Herrero et al., 2022). Mitunter kann es zu einer Zunahme der Leistungsfähigkeit kommen und das Sturzrisiko und somit die Gefahr schwerwiegender Verletzungen reduzieren. Der Umsetzung solcher Interventionen stehen oftmals der Fachkräftemangel und begrenzte Angebote im ländlichen Raum entgegen. Das Projekt „Bewegen und Stärken: Gesundheitsförderung in der Alltagsbegleitung“ hat zum Ziel, durch Einbindung von Alltagsbegleitern als Nicht-Fachkräfte Zugang zu Pflegebedürftigen zu erhalten, um gesundheitsförderliche Aktivitäten im häuslichen Setting umzusetzen.
Methode:
Die auf 3 Jahre ausgelegte Machbarkeitsstudie gliedert sich in eine Vor- und Hauptstudie. Ziel ist die Rekrutierung von 80 „Pflegetriaden“ bestehend aus a) Alltagsbegleitern, b) Pflegebedürftigen und c) Angehörigen. In der Vorstudie werden qualitative Interviews geführt, um die BeStärken+-Interventions- und Fortbildungsinhalte zu optimieren und im Rahmen der Hauptstudie in die Regelversorgung einzubetten. Geplante Befragungsinhalte der Hauptstudie sind auch Frailty-Merkmale.
Ergebnisse:
In der Vorbereitungsphase wurden evidenzbasierte, gesundheitsförderliche Aktivitäten herausgearbeitet und mit Experten u. a. aus den Bereichen der Gerontologie, Sporttherapie, Psychotherapie und mit Trägern und Alltagsbegleitern diskutiert. Im nächsten Schritt sollen Pflegetriaden für die Vorstudie rekrutiert und die ersten Alltagsbegleiter eine BeStärken+-Fortbildung erhalten.
Diskussion:
Aktuell offene Fragen stellen sich in Bezug auf Zugangsmöglichkeiten zu bestehenden Triaden, die Motivation zur Teilnahme und Umsetzbarkeit gesundheitsförderlicher Aktivitäten durch Nicht-Fachkräfte sowie letztlich wie eine nachhaltige Implementierung bei Trägern aussehen könnte.

Ein Sturz ist ein Ereignis, bei dem eine Person unbeabsichtigt zu Fall kommt. Stürze sind dabei nicht als Erkrankung, sondern als Ereignisse bzw. Unfälle aufzufassen. Das Sturzgeschehen ist ein geriatrisches „Kernsyndrom“, d. h. es ist Folge verschiedener Krankheits- und Alterungsprozesse und gehört zu den dominierenden Problemen hochaltriger Menschen. Ärztliches Handeln sollte neben der Behandlung sturzbedingter Verletzungen darauf gerichtet sein, Stürze und sturzbedingte Verletzungen zu verhindern. Hierfür ist ein Verständnis für das Zusammenspiel von intrinsischen, extrinsischen und situativen (Risiko-)Faktoren notwendig.
Background
Geriatric rehabilitation aims to maintain the functional reserves of older adults in order to optimize social participation and prevent disability. After discharge from inpatient geriatric rehabilitation, patients are at high risk for decreased physical capacity, increased vulnerability, and limitations in mobility. As a result, ageing in place becomes uncertain for a plethora of patients after discharge from geriatric rehabilitation and effective strategies to prevent physical decline are required. Collaboration between different health-care providers is essential to improve continuity of care after discharge from inpatient geriatric rehabilitation. The aim of this study is to evaluate the effectiveness of a multi-professional home-based intervention program (GeRas) to improve functional capacity and social participation in older persons after discharge from inpatient geriatric rehabilitation.
Methods
The study is a multicenter, three-arm, randomized controlled trial with a three-month intervention period. Two hundred and seventy community-dwelling older people receiving inpatient geriatric rehabilitation will be randomized with a 1:1:1 ratio to one of the parallel intervention groups (conventional IG or tablet IG) or the control group (CG). The participants of both IGs will receive a home-based physical exercise program supervised by physical therapists, a nutritional recommendation by a physician, and social counseling by social workers of the health insurance company. The collaboration between the health-care providers and management of participants will be realized within a cloud environment based on a telemedicine platform and supported by multi-professional case conferences. The CG will receive usual care, two short handouts on general health-related topics, and facultative lifestyle counseling with general recommendations for a healthy diet and active ageing. The primary outcomes will be the physical capacity measured by the Short Physical Performance Battery and social participation assessed by the modified Reintegration to Normal Living Index, three months after discharge.
Discussion
The GeRas program is designed to improve the collaboration between health-care providers in the transition from inpatient geriatric rehabilitation to outpatient settings. Compared to usual care, it is expected to improve physical capacity and participation in geriatric patients after discharge from inpatient geriatric rehabilitation.
During the early stages of the COVID-19 pandemic, stringent measures were implemented in most countries to limit social contact between residents of long-term care facilities (LTCF) and visitors. The objective of this scoping review was to identify and map evidence of direct and indirect consequences of contact restrictions, guided by three conceptual perspectives: (1) stress and learned helplessness (i.e., failure to use coping behaviors even when they are available and actionalble); (2) social contact loss; and (3) ‘total institution’ (i.e., a facility operates following a fixed plan due to spelled-out rules and norms, controlled by institutional representatives). We used the framework for conducting a scoping review by Arksey and O'Malley; included were peer-reviewed manuscripts reporting on the outcomes of contact restrictions from the beginning of the pandemic until the end of 2020. After removing duplicates, 6,656 records were screened and 62 manuscripts included. Results pertaining to the stress and learned helplessness perspective primarily focused on depressive symptoms, showing substantial increases compared to the pre-pandemic period. Studies examining cognitive and functional decline, as well as non-COVID-19 related mortality, were limited in number and presented mixed findings. The majority of study outcomes related to the social contact loss perspective focused on loneliness, but the study designs did not adequately allow for comparisons with the pre-pandemic status. The evidence concerning outcomes related to the 'total Institution' perspective was inconclusive. Although detrimental effects of social isolation in the long-term care context found support particularly in the negative affect domain, other outcome areas did not allow for definitive conclusions due to considerable variations in findings and, in some cases, insufficient statistical power.
Validation of a telephone-based administration of the simplified nutritional appetite questionnaire

Background:
Anorexia of aging is characterized by an age-associated reduction of appetite, whose aetiology in most cases is multifactorial and which often triggers malnutrition. The Simplified Nutritional Appetite Questionnaire (SNAQ) is an established screening tool. This study aimed to investigate reliability, validity, and feasibility of its telephone administration (T-SNAQ) in German community-dwelling older adults.
Methods:
This cross-sectional single-centre study recruited participants from April 2021 to September 2021. First, the SNAQ was translated into German according to an established methodology. After translation, reliability, construct validity, and feasibility of the T-SNAQ were analysed. A convenience sample of community-dwelling older adults aged ≥70 years was recruited. The following measurements were applied to all participants: T-SNAQ, Mini Nutritional Assessment – Short Form (MNA-SF), six-item Katz index of independence in activities of daily living (ADL), eight-item Lawton instrumental activities of daily living (IADL), telephone Montreal Cognitive Assessment (T-MoCA); FRAIL scale, Geriatric Depression Scale (GDS-15) and Charlson co-morbidity index as well as daily caloric and protein intake.
Results:
One hundred twenty participants (59.2% female) with a mean age of 78.0 ± 5.8 years were included in the present study. The percentage of participants identified with poor appetite based on T-SNAQ was 20.8% (n = 25). TSNAQ showed a good internal reliability with a Cronbach’s alpha coefficient of 0.64 and a good test–retest reliability [intraclass coefficient of 0.95 (P < 0.05)]. Regarding construct validity, T-SNAQ was significantly positively correlated
with MNA-SF (r = 0.213), T-MoCA (r = 0.225), daily energy (r = 0.222) and protein intake (r = 0.252) (P < 0.05). It also demonstrated a significant negative association with GDS-15 (r = 0.361), FRAIL scale (r = 0.203) and Charlson co-morbidity index (r = 0.272). Regarding applicability, the mean time for T-SNAQ was 95 s and completion rate was 100%.
Conclusions:
The T-SNAQ is a feasible screening instrument for anorexia of aging in community-dwelling older adults via telephone interviews.
Many patients in geriatric rehabilitation (GR) are physically frail at the time of admission and suffer from malnutrition and sarcopenia, which may worsen rehabilitation outcomes. This study aims to obtain insight into the current nutritional care practices in GR facilities across Europe.
Methods: In this cross-sectional study, a questionnaire focused on nutritional care practices in GR was distributed across experts in EUGMS member countries. Data were analyzed by using descriptive statistics. Results: In total, 109 respondents working in 25 European countries participated, and the results showed that not all GR patients were screened and treated for malnutrition, and not all participants used (inter)national guidelines when performing nutritional care. The results also showed variations across European geographical areas related to screening and treatment of malnutrition, sarcopenia, and frailty. Even though the participants underlined the importance of dedicating time to nutritional care, they experienced barriers in its implementation, which were mostly due to a lack of resources.
Conclusion: As malnutrition, sarcopenia, and frailty are often
present in patients admitted to GR, in addition to being interrelated, it is recommended to develop an integrated approach to screening and treatment of all three clinical problems.
Background: Frailty makes older adults vulnerable to adverse health outcomes and can modify pharmacokinetics and drug
exposure.
Objective: We aimed to explore the relationship between different frailty assessments and trough plasma concentrations of direct oral anticoagulants in older patients.
Methods: The frailty status of adults aged ≥ 70 years receiving regular direct oral anticoagulant medication was assessed
by four different instruments: Fried physical phenotype, Rockwood frailty index, Short Physical Performance Battery, and FRAIL scale. The two performance measures “slow gait speed” and “weak grip strength” were used to build a separate score depending on the number of positive criteria (none, one, two). For each participant, a single steady-state direct oral anticoagulant trough plasma concentration was collected, dose-normalized, and its relationship to the various frailty assessments analyzed.
Results: Forty-two participants completed the study, with most using apixaban (n = 22). Dose-normalized apixaban trough
concentrations were 2.48-fold higher in frail participants (Fried phenotype) than in robust participants (p = 0.009) and correlated positively with Fried physical phenotype (rs = 0.535, p = 0.010) and negatively with Short Physical Performance
Battery (rs = − 0.434, p = 0.044). Compared with participants who met none of the criteria “slow gait speed” and “weak
grip strength”, apixaban trough concentrations were approximately 1.9-fold higher in participants who were positive for one (p = 0.018) or two (p = 0.013) of these measures.
Conclusions: In this exploratory study, higher levels of frailty on performance-based frailty assessments were associated
with higher apixaban exposure in older adults.
Durch das Konzept von Frailty wird eine ältere Risikopopulation beschrieben, die eine erhöhte Wahrscheinlichkeit für das Auftreten negativer Gesundheitsereignisse aufweist. Aus gesellschaftlicher und auch aus individueller Sicht erscheint es erstrebenswert, die Prognose dieser Menschen durch gezielte Interventionen zu verbessern. Zwischenzeitlich liegt eine Reihe von systematischen Übersichtsarbeiten und Metaanalysen vor, die ganz unterschiedliche Interventionen vergleichen. Diese Arbeiten kommen zu dem Schluss, dass Trainingsinterventionen die besten Effekte erzielen. Gruppenangebote scheinen effektiver zu sein als Trainingsprogramme, die (teilweise) im häuslichen Setting absolviert werden. Das „case finding“ für solche Interventionen sollte mithilfe von Frailty-Instrumenten erfolgen, die auf Kraft, Ausdauer und/oder körperliche Aktivität fokussiert. Hier eignen sich insbesondere die Frailty-Kriterien nach Fried oder aber das Gehtempo und die Short Physical Performance Battery (SPPB). Ein flächendeckendes Angebot entsprechender Trainingsgruppen erscheint aus geriatrischer Sicht dringend erforderlich. Die Umsetzung erfordert jedoch einen deutlichen Ausbau bestehender Strukturen und neue Wege, um ältere Menschen mit Frailty zu erreichen.
Aufgrund der gegenwärtigen demographischen Entwicklung wird dem Erhalt der Funktionalität des älteren Patienten in den nächsten Jahren sowohl aus individueller als auch aus gesellschaftlicher Perspektive eine hohe Bedeutung zukommen. Das geriatrische Syndrom der Sarkopenie, welches den altersassoziierten Abbau von Muskelkraft und Muskelmasse bezeichnet, ist in diesem Kontext von zentraler Bedeutung. Ältere Patienten mit peripherer arterieller Verschlusskrankheit (PAVK) sind in besonderer Weise hinsichtlich des Auftretens einer Sarkopenie gefährdet. Zudem kann sich bei gleichzeitigem Vorliegen von PAVK und Sarkopenie im Kontext einer abnehmenden Mobilität ein Circulus vitiosus entwickeln, der die funktionelle und die allgemeine Prognose dieser Patientengruppe deutlich verschlechtert. Es gilt daher, die Sarkopenie in dieser Patientengruppe frühzeitig anhand einfacher Tests, z. B. Messung der Handkraft, in der Routinediagnostik zu erkennen und ihr durch geeignete Maßnahmen konsequent entgegenzuwirken. In diesem Zusammenhang kommt geeigneten Trainingsmaßnahmen wie einer Kombination aus einem progressiven Gang- und Krafttraining sowie einer optimierten Ernährung mit einer erhöhten Eiweißzufuhr von 1,0–1,2 g pro kg Körpergewicht pro Tag besondere Bedeutung zu. Phasen der weitestgehenden Immobilisation und einer unzureichenden kalorischen Versorgung sind unbedingt zu vermeiden.
Taking into account the current demographic trend, the preservation of functionality in older adults will be of utmost relevance, from an individual as well as from a societal perspective. The geriatric syndrome sarcopenia, which is characterized by the age-associated loss of muscle strength and muscle mass, is a key factor in this context. Older patients with peripheral arterial occlusive disease (PAOD) are particularly at risk for the development of sarcopenia. In addition, as a consequence of declining mobility a vicious circle resulting in a worsening functional and overall prognosis may occur when PAOD and sarcopenia are simultaneously present. It is therefore of utmost importance that sarcopenia is diagnosed early in this patient group as part of the clinical routine with the help of practical tests, e.g., the handgrip test, and that adequate therapeutic measures are consistently applied. For the latter, suitable exercise programs based on progressive walking and progressive resistance training as well as an optimized nutrition including an increased protein intake between 1.0–1.2 g per kg body weight per day are highly recommended. Periods of near total immobilization and an inadequate caloric intake have to be avoided as much as possible.
Background
Delirium is a common condition in elderly inpatients. Health care professionals play a crucial role in recognizing delirium, initiating preventive measures and implementing a multicomponent treatment strategy. Yet, delirium often goes unrecognized in clinical routine. Nurses take an important role in preventing and managing delirium. This study assesses clinical reasoning of nurses using case vignettes to explore their competences in recognizing, preventing and managing delirium.
Methods
The study was conducted as an online survey. The questionnaire was based on five case vignettes presenting cases of acutely ill older patients with different subtypes of delirium or diseases with overlapping symptoms. In a first step, case vignettes were developed and validated through a multidisciplinary expert panel. Scoring of response options were summed up to a Geriatric Delirium Competence Questionnaire (GDCQ) score including recognition and management tasks The questionnaire was made available online. Descriptive analyses and group comparisons explores differences between nurses from different settings. Factors explaining variance in participants’ score were evaluated using correlations and linear regression models.
Results
The questionnaire demonstrated good content validity and high reliability (kappa = 0.79). The final sample consisted of 115 nurses. Five hundred seventy-five case vignettes with an accuracy of 0.71 for the correct recognition of delirium presence or absence were solved. Nurses recognized delirium best in cases describing hyperactive delirium (79%) while hypoactive delirium was recognized least (44%). Nurses from geriatric and internal medicine departments had significantly higher GDCQ-score than the other subgroups. Management tasks were correctly identified by most participants.
Conclusions
Overall, nurses’ competence regarding hypoactive delirium should be strengthened. The online questionnaire might facilitate targeting training opportunities to nurses’ competence.
Hintergrund: Das Delir ist eine häufige Komplikation bei älteren Patient*innen, welche stationär versorgt werden. Für die Prävention, das Erkennen und das Management ist ein multiprofessioneller Ansatz erforderlich. Hier spielen Pflegekräfte eine zentrale Rolle. Ziel dieser Studie war es, ein Instrument zu entwickeln, das es Pflegekräften ermöglicht, ihre Hand- lungskompetenzen selbst einzuschätzen.
Methode: Es wurden fünf Fallvignetten entwickelt und in einem multi- professionellen Team geprüft und angepasst. Diese beschreiben verschiedene Subtypen bzw. Differentialdiagnosen. Die Fragen prüften nicht nur das Erkennen der Symptome und Benennen des Delirs sondern auch das richtige Identifizieren von Risikofaktoren und Auslösern, angemessener präventiver Strategien, adäquater Handlungsoptionen sowie geeigneter Strategien zur Kommunikation im Team. Der Fragebogen wurde als Online-Befragung mit LimeSurvey zur Verfügung gestellt und in einer Pilot-Studie erprobt.
Ergebnisse: Die Fallvignetten wiesen bei einer Testung durch Experten eine hohe Inter-Rate Reliabilität auf. Zwischen August und Oktober 2021 wurde die Webseite 248 Mal besucht und die Befragung von 126 Teilnehmenden abgeschlossen, davon waren 115 Pflegekräfte. Die Bearbeitung dauerte durchschnittlich 22 min. 88 % der Teilnehmenden gaben ein positives Feedback bezüglich der Fallvignetten. 54 % der Teilnehmenden konnten ein hyperaktives Delir erkennen und als solches benennen, während ein hypoaktives Delir nur von 43 % als hypoaktive Form erkannt und benannt wurde. Das Gesamtabschneiden korrelierte deutlich mit der Selbsteinschätzung der Delir-Handlungskompetenz der Teilnehmenden nach Beendigung der Bearbeitung.
Fazit: Die entwickelten Fallvignetten können Pflegekräfte dabei unterstützen, ihre Handlungskompetenz einzuschätzen. Damit wäre es möglich, Schulungsangebote an das Kompetenzniveau der Teilnehmenden anzupassen oder aber Pflegekräfte zu motivieren, geeignete Schulungsangebote wahrzunehmen.
Fallschilderung
Eine 87-jährige Patientin wird mit leichtem Fieber und Desorientiertheit in die Notaufnahme eingewiesen. Der Pflegedienst hat die Tochter der Patientin alarmiert, als er Letztere am Morgen im Bett vorfand und diese zu diesem Zeitpunkt auffallend schläfrig und nur teilweise orientiert war. Bereits seit drei Tagen war eine zunehmende Antriebsminderung aufgefallen. Die Patientin lebt bislang mit Unterstützung des erwähnten ambulanten Pflegediensts im betreuten Wohnen. An Vorerkrankungen sind eine chronische Niereninsuffizienz im Stadium 3a, eine arterielle Hypertonie, eine koronare Herzkrankheit, eine Herzinsuffizienz im New-York-Heart-Association(NYHA)-Stadium 2, eine Gonarthrose beidseits sowie eine senile Makuladegeneration bekannt. Es liegt eine Patientenverfügung vor.
Nach Bearbeitung dieses Kurse sind Sie mit den Tests zur Beurteilung der körperlichen Kapazität älterer Menschen vertraut. Kennen Sie intrinsische und extrinsische Risikofaktoren für Stürze bei älteren Menschen. Haben Sie einen Überblick über die Massnahmen zur Sturzprävention und Mobilitätsforschung und können ältere Patienten entsprechend beraten.
Nach Bearbeitung dieses Kurses sind Ihnen die beiden wichtigsten Frailty-Konzepte bekannt. Können Sie den Nutzen der Identifikation von Patient*innen mit Frailty einschätzen. Kennen Sie verschiedene Instrumente zur Erfassung von Frailty. Sind Sie in der Lage, für verschiedene klinische Einsatzgebiete ein geeignetes Frailty-Instrument auszuwählen.
Background: Among potentially modifiable risk factors for delirium, transfers between wards, hospitals and other facilities have been mentioned with low evidence. TRADE (TRAnsport and DElirium in older people) was set up to investigate i) the impact of transfer and/or discharge on the onset of delirium in older adults and ii) feasibility and acceptance of a developed complex intervention targeting caregiver’s participation during and after hospital discharge or transfer on cognition and the onset of delirium in older adults.
Methods: The study is designed according to the guidelines of the UK Medical Research Council (MRC) for development and evaluation of complex interventions and comprises two steps: development and feasibility/piloting. The development phase includes i) a multicenter observational prospective cohort study to assess delirium incidence and cognitive decline associated with transfer and discharge, ii) a systematic review of the literature, iii) stakeholder focus group interviews and iv) an expert workshop followed by a Delphi survey. Based on this information, a complex intervention to better and systematically involve family caregivers in discharge and transport was developed. The intervention will be tested in a pilot study using a stepped wedge design with a detailed process and health economic evaluation.
The study is conducted at four acute care hospitals in southwest Germany. Primary endpoints are the delirium incidence and cognitive function. Secondary endpoints include prevalence of caregiver companionship, functional decline, cost and cost effectiveness, quality of discharge management and quality of admission management in admitting hospitals or nursing homes. Data will be collected prior to discharge as well as after 3, 7 and 90 days.
Frailty ist ein multidimensionales geriatrisches Syndrom, das durch einen Verlust an individueller Reservekapazität und eine erhöhte Vulnerabilität gegenüber internen und externen Stressoren gekennzeichnet ist. Frailty ist mit einem erhöhten Risiko für Stürze und einen Autonomieverlust sowie mit einer erhöhten Mortalität verbunden. Die Identifikation von Personen, die eine Frailty aufweisen, kann ein umfangreiches geriatrisches Assessment triggern und gezielte Therapieangebote begründen. Darüber
hinaus können durch die Erfassung von Frailty jene Patient*innen identifiziert werden, die ein erhöhtes Risiko für ungünstige Behandlungsverläufe haben. Für die Erfassung von Frailty gibt es eine unübersichtliche Zahl an Instrumenten, wobei nur eine eingeschränkte Auswahl dieser Instrumente in einer deutschen Version vorliegt. Hintergrund: Die COVID-19 Pandemie stellt Bewohner von Altenpflegeeinrichtungen, deren Angehörige bzw. Besucher ebenso wie Mitarbeitende vor große Herausforderungen. Viruseindämmenden Maßnahmen wirken sich stark auf das Wohlbefinden der betroffenen Personengruppen aus. Material und Methode: Systematische Literatursuche nach Studien zu psychosozialen Folgen der Pandemie für Bewohner, deren Angehörige bzw. Besucher und Mitarbeitende und Zusammenführung der Ergebnisse mittels narrativer Synthese.
Ergebnisse: 756 Studien wurden gesichtet, 15 Arbeiten eingeschlossen. Die Daten wurden zwischen Februar und Juni 2020 mit Teilnehmenden aus 14 Ländern erhoben. Es wurden v.a. Einsamkeit, Trauer und Depressivität, aber auch Angst, als häufige Reaktionen der Bewohner auf die Kontakt- und Besuchsrestriktionen berichtet. Bewohner mit kognitiven Einschränkungen litten stärker unter den Auswirkungen, auch wenn es gegenteilige Hinweise gibt. Angehörige bzw. Besucher berichteten ebenfalls von einer Zunahme ihrer Einsamkeit und einer reduzierten Lebensqualität. In den Befragungen der Mitarbeitenden schildern diese Angst vor einer Infektion sowohl bei sich als auch bei den Bewohnern. Infizierte Mitarbeitende in den USA äußerten Wut darüber, nicht ausreichend geschützt geworden zu sein. Darüber hinaus berichteten Mitarbeitende von einer erheblichen Mehrbelastung.
Schlussfolgerung: Infolge der Pandemie und der ergriffenen Maßnahmen wurden negative psychosozialen Folgen bei Bewohnern, deren Angehörigen bzw. Besuchern und den Mitarbeitenden berichtet. Der abzuleitende Unterstützungsbedarfe der drei Personengruppen sind unterschiedlich und sollten bei zukünftigen Maßnahmen hinsichtlich der Pandemie stärker mitevaluiert werden.
Was tun mit den „Stürzern“?

Im Rahmen der demographischen Entwicklung in Deutschland nimmt die Zahl der Personen, welche einen Schlaganfall erleiden, stetig zu. Überlebende eines Schlaganfalls haben im Vergleich zur altersgleichen Bevölkerung ein erhöhtes Risiko, eine Fragilitätsfraktur zu erleiden. Für Personen über 65 Jahre mit Schlaganfall wurde eine Inzidenz von 33,6 Frakturen pro 1000 Personenjahre berechnet im Vergleich zu 21,4 pro 1000 Personenjahre in der altersgleichen Bevölkerung. Ursachen sind hier die erhöhte Sturzneigung sowie eine Verminderung der Knochendichte auf der paretischen Seite. Das Risiko für eine sturzbedingte Fraktur ist dabei abhängig von der wiedererlangten Selbständigkeit. Personen mit anhaltendem Pflegebedarf weisen daher ein deutlich erhöhtes Frakturrisiko auf. In Ermangelung einer ausreichenden spezifischen Evidenz für diese Risikogruppe sollten die Empfehlungen zur Sturzprävention für ambulant lebende Menschen berücksichtigt werden. Hier ist insbesondere ein Gleichgewichtstraining in Kombination mit einem Krafttraining anzuraten. Die Indikation zu einer Basisdiagnostik nach DVO Leitlinie sollte unter besonderer Berücksichtigung des Sturzrisikos überprüft werden. Bei Personen mit einer residuellen Hemiparese sollte die DXA Messung auf der paretischen Seite erfolgen.
Background
Hip fractures are regarded as a worldwide epidemic and a major public health concern. Changing risk factors, local differences and temporal trends contribute to the particular epidemiology of hip fractures. This overview gives a comprehensive insight into the epidemiology of hip fractures and reviews where German data have contributed to the literature.
Methods
The review of the epidemiology of hip fractures in Germany is based on a systematic literature search in PubMed.
Information about the global epidemiology of hip fractures was provided by a selective literature review focusing on specific aspects
of the epidemiology of hip fractures.
Results
Hip fracture rates vary more than 100-fold between different countries. In most high-income countries, a rise in agestandardized
hip fracture rates was observed until the 1980s and 1990s and a decrease thereafter. Such a decrease has not been observed for Germany so far. Many factors,diseases and drugs have been found to be
associated with hip fractures and there is some evidence that fracture risk in later life is already programmed during fetal life and
early childhood. Of the hip fracture burden 50% occur in people with disability and in need of care. In nursing homes approximately 4 fractures can be expected in 100 women per year. In people with intellectual or developmental disabilities comparable risks of hip fracture occur 10–40 years earlier than in the general population. Incidence of disability, institutionalization and death are frequent consequences of hip fractures.
Conclusion
The epidemiology of hip fractures is characterizedby a high burden of disease, local differences, temporal trends, well-defined high-risk populations and many established risk factors.
Introduction Sarcopenia has been recognised as a disease that is consistently associated with a range of geriatric syndromes and negative health consequences. The prevalence of sarcopenia is high among nursing home residents. Several systematic reviews have assessed the efficacy of a range of treatment strategies against sarcopenia. However, no systematic review discussing specifically the treatment options for sarcopenic nursing home residents has been conducted so far. The objective of this scoping review, therefore, is to identify and map existing studies that assessed the feasibility and effectiveness of interventions that were conducted with the aim to treat sarcopenic nursing home residents. Methods and analysis The protocol was developed using an established scoping review methodological framework. A systematic search of relevant literature databases will be conducted. We will also conduct a search of ClinicalTrials. gov and the WHO International Clinical Trials Registry Platform Search Portal for ongoing and recently completed trials, and will search for grey literature. Two reviewers will independently screen titles and abstracts for inclusion, followed by screening of the full text of potentially relevant articles to determine final inclusion. A data extraction sheet will be developed including key study characteristics that will be relevant for collating, summarising and reporting the results of the scoping review. Ethics and dissemination The proposed scoping review will undertake a secondary analysis of publicly available data, and therefore does not require ethical approval. The results will be disseminated to researchers in the field by submitting the review to a peer-reviewed international journal and by presenting our findings at relevant conferences. We expect that the results of the final review will help to guide future research in the field of sarcopenia treatment for nursing home residents.

Stürze sind mit zunehmendem Alter häufig auftretendes Geschehen. Stürze ereignen sich beim Gehen und Stehen, aber auch bei Transfers. Je weniger mobil ein älterer Mensch ist, desto eher ereignen sich Stürze bei alltäglichen Transfers, z.B. im Rahmen von Toilettengängen. Bei mobilen älteren Menschen sind Störungen von Gleichgewicht und Gehen von erheblicher Bedeutung für die Entstehung von Stürzen.

Obwohl die Zahl der chronischen Erkrankungen mit dem Alter deutlich ansteigt, erfreuen sich viele ältere Menschen einer guten Lebensqualität bei hoher Selbstständigkeit bis in das höchste Alter. Andere erleben jedoch deutliche Einschränkungen in der Funktionalität und Autonomie. Ältere Menschen unterscheiden sich auch hinsichtlich ihrer Einstellungen und Wünsche, Lebenserfahrungen und Lebensumstände ganz erheblich. Background
Hospital admissions resulting from traumatic intracranial haemorrhages (TIH) in older people are increasing. There are concerns regarding an increased risk of a TIH in people taking oral anticoagulants (OAC) like phenprocoumon.
Aims
The aim of this study was to estimate the incremental risk of a TIH associated with OAC in older people. Furthermore, this study explored differences in risk according to functional status.
Methods
The study took data from a large German health insurance provider and combined hospital diagnoses with data regarding drug dispensing to estimate rates of a TIH in people with and without exposure to phenprocoumon. Analyses were stratified by sex and by severe functional impairment as disclosed by the long-term care insurance provider.
Results
Overall, exposure to OAC resulted in 2.7 times higher rates of TIH. People with severe functional impairment had a higher baseline risk of TIH than people without severe functional impairment. However, the incremental risk in those exposed to OAC was similar among people with and without severe functional impairment (standardised incidence rate difference 15.73 (95% CI 7.84; 23.61) and 12.10 (95% CI 9.63; 14.57) per 10,000 person-years, respectively.
Conclusions
OAC increases the risk of TIH considerably. The incremental risk of TIH in those exposed to OAC is comparable between people with and without severe functional impairment. The presence of severe functional impairment per se should not exclude such patients from the potential benefits of OAC. For now, the prescription should be personalized based on individual fall risk factors and risk-taking behaviour.
Previously independent living older people suffering fractures of the hip have a high risk of new admission to a nursing home during the subsequent months. This study shows that older people admitted to hospital for fractures of the pelvis and spine have a similar risk of admission to a nursing home.
