Background/Objectives: The prevalence and management options of bland (non-neoplastic) portal vein thrombosis (PVT) in patients with hepatocellular carcinoma (HCC) are poorly characterized and data remain limited. Methods: We performed a retrospective analysis of a prospectively collected single-center cohort of 638 patients with HCC. Bland PVT was identified at baseline or during follow-up. Treatment exposure was modeled as time-varying to account for differences in timing of anticoagulation initiation. Results: Bland PVT was identified in 39 patients (25 at baseline and 14 during follow-up). Patients with PVT showed features of more advanced portal hypertension (lower platelet count and higher prevalence of ascites) and had a higher tumor burden (more often outside the Milan criteria; higher AFP). A total of 30 patients received anticoagulation. Anticoagulation was associated with partial or complete recanalization in 30% of cases at three months and with decrease in bilirubin, but was not associated with improved 1 year survival. Bleeding events occurred in four anticoagulated patients (13.3%). Conclusions: Bland PVT in HCC is associated with more severe liver disease. Our data indicate that anticoagulation in patients with HCC and concomitant bland PVT was generally well-tolerated and was associated with thrombus regression in one-third of cases. However, we did not observe an improved survival in patients on anticoagulation.
Scoping reviews are increasingly used for evidence synthesis, yet a comprehensive overview of benchmarks for their conduct is lacking. This study aimed to extract and analyze key metrics of preregistered scoping reviews to aid researchers in planning, resource allocation, methodological rigor, and innovation. We examined 2,038 scoping review protocol registrations on the Open Science Framework between April and August 2024, identifying 891 corresponding publications. Extracted variables included review type (e.g., rapid scoping review), information about the number of studies in the screening process, methodological practices, and funding. Using Stata, we calculated process metrics such as review duration, yield rate, and team composition. Qualitative analysis was applied to further metrics, including aims and eligibility criteria frameworks. Among the 891 publications, 91.47% were published as scoping reviews. On average, reviews involved 6.11 authors and took 80.41 weeks from registration to publication, with rapid scoping reviews completed in 55.63 weeks. Most publications (80.13%) originated from high-income countries, where funding was more common. An average of 5.19 databases were searched, identifying a mean of 6,920.66 potential inclusions, with 92.64 studies ultimately included (yield rate: 5.68%). Endnote was the most frequently employed screening tool. Language restrictions were reported in 37.60% of cases, and 75.42% did not conduct or report critical appraisal. This study provides valuable insights into common practices within scoping review processes, offering researchers a foundation to allocate resources more efficiently and enhance research quality through methodological benchmarks. It underscores the need for greater transparency and improved methodological rigor.
Die Anforderungen an werdende Pflegefachpersonen am Übergang in ihre verantwortliche Rolle zur Steuerung der Versorgung sind heute höher denn je. Mittels angepasster Ausbildungsverordnungen oder Gesetzesverfahren zum erweiterten Kompetenzeinsatz werden ihnen aktuell Türen geöffnet, durch die sie jedoch (noch) nicht treten können. Vorhandene Regelungen zur Unterstützung im Rahmen der praktischen Ausbildung können die Bedarfe in dieser Übergangsphase nicht auffangen. Deshalb untersucht das Regionale Kompetenzzentrum für innovative und gesunde Arbeit im Sozial- und Gesundheitswesen der Region Alb-Bodensee-Iller (KompIGA) Möglichkeiten der Kompensation. Im Rahmen qualitativer Interviews mit Führungskräften aus der professionellen Pflegepraxis (n=26) – und unterstützt durch Erkenntnisse aus einem narrativen Review – konnten Veränderungsbedarfe und -potenziale identifiziert werden. Unterstützung bieten können entsprechende Programme, die sich explizit an Personen in der Übergangsphase zur Pflegefachperson richten. Diese setzen neben innovativen Lehr-Lern-Methoden auf eine Verteilung von Zuständigkeiten in der Vermittlung von Wissen auf verschiedene Personen. In der Studie wurde eine erhebliche Diskrepanz zwischen internationalen Standards transitionsfokussierter Begleitung und der praktischen Realität werdender Pflegefachpersonen in Deutschland identifiziert. Strukturierte Programme zur Entwicklung von Entscheidungs- und Steuerungskompetenzen während der Übergangsphase fehlen weitestgehend, obwohl internationale Beispiele erfolgreiche Ansätze wie partizipative Übergangsprogramme und praxisnahe Szenarien demonstrieren. Als Alternative zu den ressourcenbegrenzten klassischen Betreuungsstrukturen wird der Einsatz erfahrener Pflegefachpersonen als Mentor/-innen für spezifische Kernkompetenzen vorgeschlagen, wobei eine umfassende Vorbereitung und organisationale Unterstützung zur Gewährleistung einer adäquaten Lernkultur essenziell sind.
Introduction
The digital transformation has substantially altered the competencies required of public health professionals. In Germany, digital public health (DiPH) content is inconsistently integrated into public health degree programs, reflecting the absence of a unified educational framework. To address this gap, this study aimed to establish expert consensus on core teaching content for a DiPH curriculum tailored to public health education in Germany.
CrossCare adressiert Herausforderungen grenzüberschreitender Pflegeversorgung am Beispiel der Region Bayern–Tirol. Der Beitrag zeigt, wie Community Nursing zu einer systemübergreifenden Koordinationsfunktion weiterentwickelt werden kann, um Versorgungsbrüche zu reduzieren und Kontinuität zu stärken. Dabei werden strukturelle Barrieren sichtbar und Perspektiven für eine evidenzbasierte Weiterentwicklung integrierter Versorgung aufgezeigt.
This paper explores how participatory reflection and knowledge sharing on the usage of Participatory Design (PD) and co-creation approaches can be enabled across distributed health-IT research consortia. Based on qualitative data from an Accompanying Research Project (ARP) in a national funding program on hybrid healthcare systems in Germany, we examine how thinking about using, attitudes towards, and research practices of PD can be supported beyond the project level – through infrastructuring practices that foster connection, reflection, and mutual learning across institutional boundaries. Rather than engaging in PD directly, the ARP operated as a sparring partner: a reflexive actor who facilitated meta-level dialogue, questioned assumptions, and created spaces for cross-project resonance. Through informal relational infrastructures the ARP extended the so-called "third space" of PD into a networked context, supporting agonistic reflection and shared methodological inquiry across diverse domains. Our findings contribute to ongoing discussions in PD about how to sustain participation under conditions of scale, fragmentation, and governance pressure. We argue that enabling participation in such contexts requires not only methodological guidance, but also new supportive roles and reflexive infrastructures that maintain the conditions for epistemic negotiation. By conceptualising the sparring partner as a novel role within PD ecosystems, this paper offers both theoretical and practical insights into how participatory reflection can be infrastructured across complex, multi-actor settings.
Vital-sign-based biometric authentication enables continuous and unobtrusive identity verification in healthcare using signals like ECG and PPG. To support systematic analysis of diverse application scenarios, this paper presents a taxonomy comprising 16 dimensions and 46 characteristics. Developed iteratively through literature review and use case classification, the taxonomy enables clear comparison and design guidance for biometric systems. The result is a practical framework for researchers and developers to evaluate and apply biometric authentication technologies across various healthcare contexts.
Dieses Arbeitspapier beschäftigt sich mit den wissenschaftlichen Wirkungen transdisziplinärer und partizipativer Forschung. Es wurde in einer Teilgruppe der AG Wirkung der Gesellschaft für transdisziplinäre und partizipative Forschung (GTPF) erarbeitet.
Ziel des Papiers ist es, das Verständnis wissenschaftlicher Wirkungen transdisziplinärer und partizipativer Forschung zu erweitern und einen Impuls zu setzen für ein breites Wirkungsverständnis. Herkömmliche Indikatoren der Wirkungsmessung erfassen meist nur einen kleinen der Veränderungen, die diese Forschungsmodi bewirken können. Das dem Arbeitspapier zugrunde liegende Wirkungsverständnis umfasst sowohl die Auswirkungen transdisziplinärer und partizipativer Forschung auf die Forschungspraxis als auch die Wirkungen transdisziplinärer und partizipativer Forschung auf das Wissenschaftssystem.
Begleitung von Behördengängen, Hilfe bei der Suche nach Beratungsstellen, Bewältigung von pflegerischen Aufgaben - wenn der Unterstützungsbedarf steigt, stellen sich für Pflegebedürftige und ihre Angehörigen viele Fragen: wo gibt es passende Unterstützungsangebote? Welche Leistungen können beantragt werden? Und wie geht man mit körperlichen Einschränkungen um? Digitale Informationsangebote können niedrigschwellige Antworten auf diese Fragen geben, werden bisher aber noch selten genutzt. Im Rahmen eines Forschungsprojekts in Kooperation mit dem Medizinischen Dienst Bayern (MD Bayern) wurde untersucht, ob digitale Informations- und Unterstützungsangebote bei Pflegebedürftigen und deren Angehörigen bekannt sind und wieweit sie auf entsprechende Angebote zu Beginn der Pflege aufmerksam gemacht werden können.
