Background : Acoustic signals can modulate metabolic and central nervous functions and evoke physiological effects within humans. Especially, the anti-hypertensive effect of certain iterative sound patterns as possible intervention in essential hypertension, which originates in many cases from stress, is examined with COMES ® , the Cognitive Medical System based on telemedicine.
Methods : This contribution describes the structure, the mode of operation and introduces the detailed results of the prospective observational study ( N = 20) of musical feedback in hypertension therapy.
Results : It is shown that the music used for intervention can decrease blood pressure significantly ( t -test). The reduction averages −2.57 mmHg ( p = 0.013) RRsys, −1.54 mmHg ( p = 0.003) RRdia. This is comparable with pharmaceutical interventions. Discussion : COMES offers a great number of application options in diagnostics and therapy including an automatic intervention channel. It is possible to gather authentic data patterns in the patient’s personal environment and thus to obtain an immediate, realistic impression of arbitrary intervention structures. Therapists get further instruments to involve their patients actively into the therapy process at home. The measurement of various physiological parameters allows both patient and therapist a direct feedback on the efficacy of therapeutic intervention or relaxation exercises.
Demographic changes lead to an increased importance of telemedical assistance systems and telematically based therapeutic concepts during medical aftercare, for instance in the development of new training programs for neurological rehabilitation. The efficiency of long-term medical rehabilitation often depends on the commitment and compliance of the affected person. After the initial therapy phase in a rehabilitation center, the motivation to maintain the gained activity level in the domestic environment can decrease rapidly. Therefore, the technologies of medical technology and telemedicine can and should be combined for virtual coaching and medical rehabilitation that can be performed at home. In this article, four possibilities of telematic rehabilitation will be presented.
The number of people with age-related neurological impairments is rapidly increasing. In recent years, studies into brain research and music processing have shown that music can have strong neurological effects. Advances in rehabilitation technology and music technology have led to novel music-based rehabilitation interventions that are motivational, non-invasive and effective. In this paper, we aim to summarize the current literature on the subject and present results from related projects of the authors that illustrate how music-based therapies and technology can be successfully integrated and effectively used for the rehabilitation and relaxation of neurologically impaired elderly people.
Our aging society can benefit from a better medical monitoring to detect health problems earlier and thus enabling phy-sicians to intervene faster to avoid complicated, expensive and potentially risky treatment. But to really improve the liv-ing quality of patients this monitoring has to be integrated in daily routine and should not become a burden. If the effort for the user is too high, he or she wont conduct measurements regularily or any at all and the monitoring is not effective anymore.
The IBH Living Lab "Active & Assisted Living" (AAL) is an international network of universities, social services and technology providers who are collaborating to facilitate active and assisted living for people who feel excluded from social and economic life due to physical or psychological restraints associated with ageing. The group is looking to assist these people by developing individually customised ambient technologies designed to support and enhance the work of professional and informal carers.
Demographic change is advancing inexorably. The number of people in need of care is growing rapidly, while at the same time there is already a shortage of several thousand caregivers to provide adequate, needs-based nursing care. Intelligent assistance systems can make a contribution to solving this problem. In this article, diverse assistance systems from the areas of nursing care, health care, rehabilitation and training, and mobility are presented as examples. These different components, equipped with the appropriate sensors, data transmission units and interfaces, can all be integrated into a system platform and thus form a comprehensive intelligent, digital assistance system or a mobile diagnosis and therapy platform.
Demographic change is making more and more elderly people dependent on care. As a result, there are staff shortages at nursing homes, which now require technical aid. Mobility is considered an essential component for ensuring quality of life, social contact, and a selfdetermined life. With the introduction of the autonomous stair-climbing wheelchair, unlimited and independent mobility is ensured for movement-limited people. Because of dynamic stabilization, the wheelchair moves on only one axis. Thereby, it has highly agile driving behavior and is compact in size. The autonomous climbing system is based on a leg mechanism that consists of two legs with lower and upper leg support. While climbing a stairway, the legs push the wheelchair on the next higher situated step. Afterwards the legs are pulled successively on to the next step as well. While climbing down, the opposite sequence of movements occurs.
Telematic rehabilitation 2.0
(2013)
Today, the compliance of device-based therapy for extended periods lies often below 50 percent [1]. Furthermore every second household is equipped with both a powerful computer and a flat-screen TV ([3], [4]). This project is motivated by the idea to connect these facts. For an increased motivation to use the daily hours of media consumption simultaneously for rehabilitation, a data animation is combined with a browser window or media player. With the COMES® 1 interface, a therapist has additionally the possibility to interact with the user and animation. Moreover a usability test has been made, to examine the acceptance of both patients and therapists for a media supported rehabilitation. The objective of this project is to reduce the motivational threshold of a daily training and thus increase the compliance in long term rehabilitation.
This article describes a design for integrating a telemedical rehabilitation system into a smart home concept. The focus thereby lies on the interface communication, the user benefit and the challenges of combining these complex systems. The objective of this project is, to create an easy-to-use and user-orientated system, which shows the potential of combining smart-home technology with a telemedical rehabilitation system for the user's benefit. Difficulties as well as best-practice solutions are further discussed in this article.
This paper deals with the development of a system established on a telematic rehabilitation exerciser, a plug-PC and a real-time web-animation. An easy to use device, which allows a telematically controlled training at home is target of this project. It is thereby based on the diagnosis and therapy platform COMES ® of the Heinz Nixdorf-Lehrstuhl für Medizinische Elektronik of the Technische Universität München. In COMES ® mobile measured vital parameters are intelligently evaluated by an expert system before they are provided to the user along with medical information and trends. A Java application allows the real-time data, which is sent by a telematic exerciser and received over the Bluetooth interface of the plug-PC to be filtered and transmitted over a SQL-connection on, to a database. These data is retrieved by the animation and evaluated in form of a game. To visualize this web-animation, the screen of a potential terminal device is splitted in two and taking only a small horizontal part, this animation is shown on the screen. The other part could be filled by a TV-programm, Video or Skype window depending on the abilities of the terminal device which could be any device with an internet browser (e.g. a Smart TV).
This paper deals with the development of a new mobility concept for disabled and elderly people. The focus of our project is a wheelchair which can climb stairs. In order to give the user maximum freedom regarding absolute mobility, the wheelchair has to suitable for indoor and outdoor usage and therefore be extremely manoeuvrable. A further goal is the integration of the wheelchair into a car. With its help the user is able to travel longer distances independently by car.
Die Digitalisierung der Pflege in Bayerisch-Schwaben – Vorstellung des Verbundprojektes CARE REGIO
(2020)
At the Kempten University of Applied Sciences, an interdisciplinary and cross-faculty AAL Living Lab was established to validate and optimize assistance solutions amid the target group. A major goal is to increase the acceptance of (potential) users, as well as industrial partners and service providers.
