Die Zielsetzung war, {\"u}ber die Identifizierung, Analyse und Beschreibung von Einzelleistungen fachlich fundierte Anregungen zu formulieren f{\"u}r ein Berliner Altenhilfestrukturgesetz, das der Differenziertheit und Diversit{\"a}t der Lebensphase Alter Rechnung tr{\"a}gt. Ziel des Beitrags Vorgehensweise und zentrale Ergebnisse des Gutachtens werden vorgestellt und diskutiert. Material und Methoden In einem mehrschrittigen Vorgehen, verbunden mit einem multimodalen Forschungsansatz, wurden verschiedene Methoden miteinander verkn{\"u}pft, um ein umfassendes Verst{\"a}ndnis der aktuellen Situation der sog. Altenhilfe und gerontologische Begr{\"u}ndungen ihrer Leistungen zu generieren. Gearbeitet wurde mit kritischen und selektiven Literaturreviews, Dokumentenanalysen und qualitativ-leitfadengest{\"u}tzten Expert*innen-Interviews. Increased perceived safety can positively support the use of digital technologies, whereas low perceived safety can limit or even hinder its use. The needs of the different target groups should be considered throughout the entire process of digital technology development and healthcare provision to support their implementation. To investigate perceived safety in the context of the digital transformation of healthcare while considering relevant needs, influencing factors and impacts, including crisis events, to provide recommendations for action based on a participatory, multiperspective, strategic 5-year foresight viewpoint. A strategic foresight study is conducted via a participatory mixed-methods design to understand the present related factors that are likely to be relevant to future developments in the digital transformation of healthcare.ResultsWe observed that feeling safe plays a complex role in the digital transformation of healthcare. How perceived safety is considered has and will continue to impact the individual, organisational and system levels. Regarding a potential crisis event, controversial consequences have been observed. At its core, digital (health) literacy related to equity of access and human support is one of the crucial aspects in the context of perceived safety related to the successful implementation of digital technologies in healthcare. The scenarios showed that a continuation of the current situation over the next 5 years may result in partly desirable and partly undesirable outcomes. Concrete key factors should be used in practice to support both education and healthcare quality development and research. The essence of the scenarios should serve as a starting point for research agenda setting and political decision-making in the future. In this context, perceived safety plays a critical role in their acceptance and implementation. Previous research had focused more on data security or specific digital technologies. There has also been a lack of participatory approaches to consider and empower healthcare recipients (and relatives), providers, and technology experts to broaden the phenomenon. Objective The aim of this study was to present a comprehensive perspective on the needs, influencing factors and related outcomes in the context of feeling safe with digital technologies in healthcare. Method A qualitative, exploratory, and participatory methodology was used with five guide-based focus group workshops. Each workshop explored one of five digital technology demonstrations provided at the beginning: (1) electronic health records (EHR), (2) robotics, (3) artificial intelligence (AI), (4) smart home, and (5) smart hospital. All participants were invited for a guide-based discussion. The study focused on the levels of 'involvement' and 'collaboration' by also empowering participation. The workshop target groups were actively involved in the development and execution of the workshops and were empowered. The data were analysed via a content analysis approach, with a mostly inductive procedure. Results Feeling safe was found to affect thoughts, emotions, and actions. For example, a higher level of perceived safety increased the acceptance of digital technologies, whereas a lower level of perceived safety decreased it. The corresponding needs and influencing factors differed in terms of their scope and focus depending on the context. The aspect of 'control' was most frequently addressed by all the target groups and was viewed as relevant in all the workshops. In general, digital technologies were viewed as supplements rather than substitutes for healthcare providers. Conclusion This study extends beyond the current state of research on perceived safety and the use of digital technologies in healthcare by providing a comprehensive overview of the corresponding needs and influencing factors at various levels, such as the individual, community-organizational, and system-society levels.
