Gleichwohl eine ärztliche Beratung bei der Erstellung von Patientenverfügungen nicht verpflichtend ist, wird diese empfohlen. Hausärzt:innen können geeignete Ansprechpersonen für die Erstellung und Aktualisierung von Vorsorgedokumenten sein. Ziel der Studie ist die Erfassung der Häufigkeit und Art der hausärztlichen Beratungstätigkeit zu Vorsorgedokumenten. Es soll zudem erfasst werden, welche subjektiv empfundenen Kompetenzen und welchen Wissensstand Hausärzt:innen bzgl. der Beratungstätigkeit zu Vorsorgedokumenten aufweisen.
Von Oktober bis November 2024 wurde eine schriftliche Online-Befragung durchgeführt. Adressat:innen waren alle in Niedersachsen praktizierenden Hausärzt:innen. Es kam eine adaptierte Version eines erprobten Fragebogens inkl. eines fallvignettenbasierten Wissenstests zu Vorsorgedokumenten und Einwilligungsunfähigkeit zum Einsatz. Die Daten wurden deskriptiv und inferenzstatistisch ausgewertet.
Es wurden Daten von 502 Hausärzt:innen eingeschlossen. 85,1% der Befragten geben an, ihre Patient:innen zu Vorsorgedokumenten zu beraten. Gründe, Patient:innen nicht zu beraten, sind u.a. nicht vorhandener Bedarf der Patient:innen oder mangelnde Qualifikation auf Seiten der Hausärzt:innen. Die Teilnehmenden schätzen ihre subjektive Beratungskompetenz als eher hoch ein. Unsicherheiten werden vor allem beim Umgang mit Emotionen und insbesondere mit Ängsten von Patient:innen angegeben. Der objektive Wissenstest ergab eine durchschnittliche Punktzahl von 22,5 von 30 möglichen Punkten. Ein hohes Wissen wird vor allem bei den Wirksamkeitsvoraussetzungen von Patientenverfügungen sichtbar. Wissensdefizite bestehen insbesondere im Bereich des Vertretungsrechts.
Ein Großteil der befragten Hausärzt:innen berät Patient:innen zu Vorsorgedokumenten. Es werden jedoch Wissensdefizite und Unsicherheiten deutlich, die einen Einfluss auf die Beratungsqualität haben können.
This study investigates the significance mindfulness, self-care, and mental health and psychosocial support (MHPSS) practices hold for (female) residents in Za’atari Refugee Camp. The Syrian Uprising of 2011 has caused approximately five million Syrians to seek refuge in neighboring countries (Operational Data Portal 2024b). Jordan, as one of the neighboring countries, is currently hosting approximately 618,973 Syrian refugees (Operational Data Portal 2024b). Approximately 78,000 live in Za’atari Refugee Camp (UNHCR 2024b). While research on refugees’ experiences, mindfulness, and MHPSS exist, the researcher identified a research gap of qualitative and participatory studies concerned about the significance of the practices that aim at alleviating suffering among refugees. This study captures and displays the opinions, understandings, and perceptions of those affected by displacement and encampment. The study consists of twelve semi-structured interviews. Four interviews were conducted with female residents of Za’atari Refugee Camp. Eight interviews were held with MHPSS professionals from various international and national non-governmental organizations (INGO, NGO), and governmental organizations. Additionally, a participatory asset mapping workshop was conducted with seventeen female residents.
The study demonstrates that mindfulness based-stress reduction (MBSR), self-care, and other MHPSS support programs provide refugees with the chance to regain control, increase selfconfidence, self-awareness, and build strength, consciousness, and resilience. These practices help refugees to cope with encampment and represent hope, purpose, empowerment, and have improved physical and mental well-being. While participants reflected that through MBSR practices they have become calmer, stronger, and more confident individuals, family and religion are the most important self-care techniques and represent safety, power, strength, and comfort. Through MHPSS programs refugees can regain relative stability, acquire skills, and gain a sense of purpose.
Background
The bioethical principle of respect for patients' autonomy should be implemented in every area of health care. The German rehabilitation system has some characteristics (e.g. the prevalence of inpatient rehabilitation) that make the topic particularly relevant. The aim of the study is to determine the current state of respecting patient autonomy in inpatient medical rehabilitation in Germany and to identify barriers and promoting factors from the perspective of relevant groups.
Methods
A multi-center, prospective, cross-sectional study is being conducted, including interviews with patients (
n = 24), interviews with professionals (n = 21) and a survey of medical directors (n = 900). The empirical findings are then reviewed in three validation workshops. In addition, consensus recommendations for practice are derived by means of a Delphi process with experts (n = 30).
Discussion
This study will address an important gap in the empirical literature by identifying the current state and needs of patients, professionals and medical directors of rehabilitation clinics regarding the practice of respecting patient autonomy in inpatient medical rehabilitation in Germany. It is expected that the results can contribute to coming closer to the ideal of respect for patient autonomy in rehabilitation practice.

Freiwilligkeit bezeichnet die Freiheit einer Entscheidung von unzulässiger Einflussnahme oder Druck. Sie ist ein Element der Freiverantwortlichkeit von Entscheidungen über assistierten Suizid und damit ein wichtiger Beurteilungsgegenstand im Umgang mit Anfragen nach Suizidassistenz. Der vorliegende Beitrag widmet sich der Frage, welche Faktoren die Freiwilligkeit von solchen Anfragen potenziell einschränken können. Anhand der medizinethischen Literatur sowie empirischer Studien aus verwandten Kontexten werden u. a. Mitarbeitende im Gesundheitswesen, Sterbehelfer*innen und Angehörige als zentrale Akteure aufgezeigt, die bei Entscheidungen über assistierten Suizid relevant sein können. Mögliche Formen interpersonaler Einflussnahme können Drohungen, Angebote, zwischenmenschliche Beeinflussung und Überredung sein. Zusätzlich stellen strukturelle Rahmenbedingungen – etwa sozioökonomische Notlagen – mögliche Einflussfaktoren auf die Freiwilligkeit dar. Bei der Beurteilung der Freiverantwortlichkeit eines Suizidentschlusses sollte daher systematisch geprüft werden, ob solche Einflussfaktoren vorliegen. Angesichts der bislang geringen empirischen Datenlage bedarf es dringend weiterer Forschung in diesem Bereich. Dies ist auch für die Entwicklung von Leitlinien zur Beurteilung der Freiwilligkeit bei Anfragen nach assistiertem Suizid essenziell.
(1) Background: An increasing number of people are living with late-life depression, yet non-pharmacological treatments to help manage symptoms are limited. Two interventions, positive psychology and music therapeutic songwriting, have independently led to decreased depressive symptoms and an improved wellbeing in older people over 65 years old. (2) Methods: This article describes the development of a therapeutic songwriting program for people living with late-life depression. Knowledge from positive psychology and therapeutic songwriting was combined to maximize the potential benefits. (3) Results: The intervention program has ten weekly 45 min sessions that incorporate elements from positive psychology into therapeutic songwriting. Using a three-song approach encompassing ongoing musical practices, different positive psychology interventions were incorporated to support the experiences associated with a flourishing life. The intervention protocol for older people presented here is distinct from previous deficit-orientated approaches in that it shifts the focus to positive experiences, resources, and the individual’s ability to decrease their own depressive symptoms and improve their wellbeing. (4) Discussion: This protocol presenting a therapeutic songwriting program meets the need to develop new non-pharmacological treatment options. However, further studies are needed to examine the feasibility and impact of the intervention program on late-life depression and wellbeing in older people.

Schnell bilden sich zwei Lager, die beide überzeugt sind, im Recht zu sein (Schwarz 2014). Solche Zielkonflikte erzeugen Spannung – vermutlich ein Erfolgsfaktor für hohe Einschaltquoten. Doch viele Sendungen beanspruchen für sich mehr und wollen ihrem Bildungsauftrag gerecht werden: Sie wollen individuelle und öffentliche Meinungsbildung fördern und Verständnis für andere Perspektiven anregen.

Zusammenfassung
Die Indikationsstellung basiert auf medizinischem Fachwissen und wird flankiert von professionsethischen Standards sowie rechtlichen Anforderungen. Diese Arbeit beleuchtet, inwiefern Werturteile Indikationsstellungen beeinflussen und welcher Umgang damit gerechtfertigt erscheint.
Die interdisziplinäre begriffliche und normative Analyse umfasst medizintheoretische Ansätze, Fallbeispiele, empirische Ergebnisse und juristische Überlegungen, um verschiedene Ebenen von Werturteilen im Indikationsprozess zu identifizieren und kritisch zu diskutieren.
Neben dem medizinischen Wissenskorpus beeinflussen Werturteile die Indikationsstellung. Dies trifft bereits auf die Wahl des Ziels aus allgemeinen Zielen der Medizin zu, auf das hin eine Maßnahme als indiziert geprüft wird. Zudem beeinflussen der individuelle Umgang mit Wahrscheinlichkeiten und Unsicherheit sowie persönliche Überzeugungen und gesellschaftliche Normen die Indikationsstellung. Manche Werturteile sollten nicht in die Indikationsstellung einfließen. Insbesondere wirtschaftliche Überlegungen oder Verteilungsentscheidungen sollten unabhängig von Indikationsstellungen geprüft werden. Es dient der Klarheit und Transparenz, ärztliche Wertungen und Präferenzen von Patientinnen getrennt zu analysieren, zu explizieren und im Entscheidungsprozess zusammenzuführen.
Werturteile in der Indikationsstellung sind unvermeidbar, erfordern aber einen bewussten Umgang. Sie sollten transparent gemacht werden, um ärztlicherseits indizierte oder nichtindizierte Maßnahmen fallspezifisch zu begründen und die Selbstbestimmung von Patientinnen zu stärken. Außerdem ist ein gesellschaftlicher Diskurs notwendig, um bestimmte Einflüsse, beispielsweise ökonomische, sachgerecht zu adressieren und nicht implizit mit Indikationsstellungen zu vermengen.
