    <title language="eng">Current state of vocationally oriented medical rehabilitation - a German perspective</title>
    <abstract language="eng">The aim of this paper is to outline a current trend in inpatient medical rehabilitation in Germany, i.e. vocationally oriented medical rehabilitation (VOMR). Since employment-related demands and challenges pose a potential source of strain regarding employees' health and work ability, rehabilitative treatment in Germany is increasingly geared towards including work-related issues in diagnostics and therapy. To date, German rehabilitation research and practice have focussed primarily on the development of screening instruments for identifying vocational problems, interface management and inter-sectoral cooperation and the specification and standardisation of vocationally oriented interventions and programmes (e.g. work hardening, group interventions). Further developments in VOMR should examine the varying needs of patients for work-related interventions as well as the neglect of specific subgroups of patients (e.g. long-term unemployed) and topics (e.g. interference between work and other life domains).</abstract>

    <title language="deu">Entwicklung einer Homepage und eines Praxishandbuchs zur arbeits- und berufsbezogenen Orientierung in der medizinischen Rehabilitation als nutzerorientierte Medien für die rehabilitative Versorgungspraxis</title>
    <abstract language="deu">Arbeits- und berufsbezogene Orientierung in der medizinischen Rehabilitation stellt eine bedeutsame aktuelle Entwicklungslinie in Reha-Wissenschaft und Versorgungspraxis dar. Wenngleich in den letzten Jahren viele berufsbezogene Behandlungsangebote entwickelt wurden, bestand bislang kein Konsens hinsichtlich der Definition einzelner Maßnahmentypen bzw. der entsprechenden Kriterien (z. B. bezüglich Methoden, Instrumenten, Dauer und Frequenz, Zielgruppen). Ausgehend von vorläufigen Arbeitsdefinitionen wurden Kerntypen arbeits- und berufsbezogener Maßnahmen im Rahmen eines Konsensverfahrens begrifflich weiterentwickelt und ergänzt. In einer 2-stufigen schriftlichen Delphi-Befragung wurden 39 Experten kontaktiert, um die bisherigen Beschreibungen arbeits- und berufsbezogener Maßnahmen anhand strukturierender Leitfragen zu evaluieren. In Kooperation mit einer interdisziplinären Expertengruppe wurden standardisierte Beschreibungen für 5 arbeits- und berufsbezogene Kernmaßnahmen konsentiert. Diese werden in einem Praxishandbuch und einer Homepage dargestellt, welche neben der Beschreibung der wichtigsten arbeits- und berufsbezogenen Interventionen auch Umsetzungshinweise und Beispiele guter Praxis umfassen. Damit soll der Transfer von Forschungsergebnissen in die rehabilitative Praxis unterstützt werden.</abstract>
    <abstract language="eng">Vocational orientation in medical rehabilitation has become an important issue in rehabilitation science and practice in Germany. Although a variety of vocationally oriented interventions has been developed in recent years, there is still a lack of consensus with regard to the definition of specific types of interventions as well as relevant criteria (e. g., methods; instruments; duration/frequency; patient groups). Building on preliminary definitions, basic types of vocationally oriented measures/interventions were conceptually modified and refined in the context of a consensual process. 39 experts of various professions were contacted during a 2-stage Delphi survey to evaluate existing definitions of vocationally oriented interventions using key questions. Standardized descriptions of 5 vocationally oriented basic interventions were specified in cooperation with an interdisciplinary panel of experts. Descriptions were published in a workbook and a homepage for researchers and clinicians, which also contain information on the implementation of measures into rehabilitative care as well as good practice examples. The media developed in this project may contribute to the transfer of research results on vocationally oriented rehabilitation into health care practice.</abstract>

    <title language="eng">Psychosocial Patient Education Groups Focusing on Work-related Issues – Results of a Survey of German Medical Rehabilitation Centers</title>
    <abstract language="deu">Einleitung:&#13;
&#13;
Psycho-edukative Patientenschulungsprogramme, die arbeits- und berufsbezogene Probleme thematisieren, sind als Kernmaßnahme im Anforderungsprofil der Deutschen Rentenversicherung (DRV) für die medizinisch-beruflich orientierte Rehabilitation (MBOR) beschrieben.&#13;
&#13;
Fragestellung:&#13;
&#13;
Dieser Artikel analysiert die Häufigkeit, Themen und Qualität von berufsbezogenen Gruppenangeboten in deutschen medizinischen Rehabilitationseinrichtungen.&#13;
&#13;
Methodik:&#13;
&#13;
Bundesweit wurden 1 473 ambulante und stationäre Reha-Einrichtungen schriftlich zu ihrer Schulungspraxis befragt. Die Daten wurden quantitativ und qualitativ ausgewertet. Die freitextlichen Nennungen zu berufsbezogenen Gruppenangeboten wurden inhaltsanalytisch kategorisiert und ihre Qualität anhand der Kriterien des Zentrums Patientenschulung und des MBOR-Anforderungsprofils bewertet.&#13;
&#13;
Ergebnisse:&#13;
&#13;
Von 908 antwortenden Reha-Einrichtungen gaben 283 Einrichtungen Informationen zu 454 berufsbezogenen Gruppenangeboten an. Die häufigsten Nennungen entfielen auf die Kategorien Arbeitslosigkeit und Bewerbungstraining, Arbeitsplatztraining, Stress/Entspannung. Die Anforderungen der DRV hinsichtlich der Themen und die Qualitätskriterien des Zentrums Patientenschulung bezüglich kleiner Gruppengrößen und Didaktik wurden mehrheitlich erfüllt.&#13;
&#13;
Schlussfolgerung:&#13;
&#13;
Entwicklungspotenziale bestehen hinsichtlich der Manualisierung, Evalua­tion und Publikation berufsbezogener Gruppenangebote sowie in der Entwicklung indikationsspezifischer Programme mit arbeits- und berufsbezogenem Schwerpunkt.</abstract>

Psychosocial patient education programs focusing on work-related issues are a core element in the German statutory pension insurance’s profile of requirements (POR) regarding inpatient vocationally oriented medical rehabilitation (VOMR).&#13;
&#13;
Aim:&#13;
&#13;
This study aims at analyzing the prevalence of patient education programs focusing on work-related issues in German rehabilitation centers with regard to their content and quality.&#13;
&#13;
Method:&#13;
&#13;
Data were collected in a national survey on the current state of patient education within medical rehabilitation programs in Germany in 1 473 inpatient and outpatient medical rehabilitation centers. Data were analyzed both quantitatively and qualitatively, with free text res­ponses being assigned to categories, drawing upon criteria developed by the German Center of Patient Education and the pension insurance’s POR.&#13;
&#13;
Results:&#13;
&#13;
283 of the 908 institutions participating in the survey provided information on 454 psychosocial patient education programs focusing on work-related issues. “Unemployment and job training”, “work hardening”, “stress”/“relaxation” were named most frequently. The criteria derived from the POR regarding group content and from the Center of Patient Education regarding group size and education methods were largely fulfilled.&#13;
&#13;
Conclusions:&#13;
&#13;
There is a need for existing group programs in VOMR to be further manualized, evaluated and published. More patient education programs focusing on work-related issues should be developed specifically for relevant indications.</abstract>

Die Studie untersuchte, ob der Rehabilitationsbedarf in der medizinisch-beruflich orientierten Rehabilitation (MBOR) häufiger durch besondere berufliche Problemlagen gekennzeichnet ist als in der herkömmlichen medizinischen Rehabilitation (MR).&#13;
&#13;
Methoden:&#13;
&#13;
Personen mit Rückenschmerz (M50, M51, M53, M54) wurden in 6 Rehabilitationseinrichtungen zu erwerbsbezogener Aktivität und Teilhabe befragt.&#13;
&#13;
Ergebnisse:&#13;
&#13;
Die unterschiedlich starken Einschränkungen der in der MBOR bzw. in der MR behandelten Personen bestätigten einen bedarfsbezogenen Zugang. Diese Unterschiede waren allerdings ausschließlich bei Patienten mit screeninggestütztem Zugang zu beobachten. Wurde der Zugang nicht durch ein Screening unterstützt, unterschieden sich die Gruppen kaum. Verfahren mit primärer Klinik- und Trägersteuerung führten zu einer vergleichbaren Unterscheidbarkeit beider Gruppen.&#13;
&#13;
Schlussfolgerung:&#13;
&#13;
Screeninggestützte Entscheidungen ermöglichen einen stärker bedarfsbezogenen Zugang in die MBOR. Da sich keine Unterschiede zwischen Klinik- und Trägersteuerung zeigten, können Zuweisungsentscheidungen bereits beim Rehabilitationsträger getroffen werden.</abstract>

The study determined if need for rehabilitation in work-related medical rehabilitation (WMR) is more frequently characterised by specific work-related problems than in conventional medical rehabilitation (MR).&#13;
&#13;
Methods:&#13;
&#13;
In 6 rehabilitation centres, persons with back pain (M50, M51, M53, M54) were surveyed about work-related restrictions of work ability at begin of their rehabilitation.&#13;
&#13;
Results:&#13;
&#13;
Differences in work ability of WMR and MR patients confirmed need-related access. However, these differences were exclusively explained by screening-based access decisions. If access was not supported by a screening, WMR and MR patients did not differ. Decisions by rehabilitation centres compared with decisions by insurance agencies resulted in similar differences between WMR and MR patients.&#13;
&#13;
Conclusion:&#13;
&#13;
Screening-based decisions about access enable a more need-related access to WMR. As there were no differences between access decisions by rehabilitation centres and insurance agencies, access decisions can be realised already by the insurance agency.</abstract>

    <title language="eng">Is the Guideline for Work-Related Medical Rehabilitation Successfully Implemented?</title>
    <abstract language="deu">Hintergrund:&#13;
&#13;
Das Anforderungsprofil zur Durchführung der medizinisch-beruflich orientierten Rehabilitation (MBOR) im Auftrag der Deutschen Rentenversicherung beschreibt Standards für berufsorientierte Kernmaßnahmen in der medizinischen Rehabilitation. Wir überprüften, wie die Umsetzung der Inhalte und des empfohlenen Leistungsumfangs gelang und welche Veränderungseffekte mit der Umsetzung assoziiert ­waren.&#13;
&#13;
Methoden:&#13;
&#13;
Die Umsetzung des Anforderungsprofils wurde in 7 stationären orthopädischen Rehabilitationseinrichtungen mittels schriftlicher Patientenbefragungen am Rehabilitationsbeginn und Rehabilitationsende sowie 3 Monate nach Abschluss der Rehabilitation untersucht. Zusätzlich wurden aus den ärztlichen Entlassungsberichten die durchgeführten therapeutischen Leistungen extrahiert.&#13;
&#13;
Ergebnisse:&#13;
&#13;
Der empfohlene Umfang der Sozialberatung und der berufsbezogenen Gruppen wurde erreicht. Diskrepanzen gab es zum empfohlenen Umfang des Arbeitsplatztrainings. Die standardisierte Mittelwertdifferenz (SMD) der Arbeitsunfähigkeitsdauer vor und 3 Monate nach der Rehabilitation entsprach einer nahezu moderaten Verringerung (SMD=0,47; 95%-KI: 0,28–0,66). 5 zusätzliche Stunden berufsorientierter Therapien gingen mit einer Verringerung der Arbeitsunfähigkeitsdauer um 1,2 Wochen einher (95%-KI: −2,38 bis −0,03).&#13;
&#13;
Schlussfolgerungen:&#13;
&#13;
Das Anforderungsprofil wurde weitgehend erfolgreich umgesetzt und setzt wichtige Standards für den Ausbau der MBOR. Die flächendeckende Umsetzung des MBOR-Anforderungsprofils bedarf einer kontinuierlichen Qualitätssicherung, die zeitnahe Rückmeldungen über den erreichten Umsetzungsgrad ermöglicht.</abstract>
    <abstract language="eng">Background:&#13;
&#13;
The guideline for work-related medical rehabilitation (WMR) in responsibility of the German Pension Insurance describes standards of work-related measures in medical rehabilitation. We investigated if contents and recommended amount of treatment were successfully implemented and which improvements were associated with the implementation.&#13;
&#13;
Methods:&#13;
&#13;
Implementation of the WMR guideline was evaluated at 7 inpatient orthopaedic rehabilitation centres. Patients completed ques­tionnaires at beginning of rehabilitation, at dis­charge and 3 months after discharge. Details ­regarding the treatments provided were extracted from the standardised discharge report.&#13;
&#13;
Results:&#13;
&#13;
The recommended amount of social counselling and work-related psychosocial ther­apy measures were appropriate. However, there were discrepancies regarding the recommended amount of functional capacity training. The standardised mean difference (SMD) between baseline and 3-month follow-up sick leave duration indicated an almost medium-sized effect (SMD=0.47; 95% CI: 0.28–0.66). An additional 5 h of work-related therapy was associated with a 1.2-week decrease in sick leave duration (95% CI: −2.38 to −0.03).&#13;
&#13;
Conclusion:&#13;
&#13;
The guideline was for the most part successfully implemented and sets important standards for the roll-out of WMR. The na­tionwide implementation of the WMR guideline requires a continuous quality assurance that ­enables promptly feedback about the achieved implementation level.</abstract>
