We introduce PickyBPE, a modified BPE algorithm that carries out vocabulary refinement during tokenizer training by removing merges that leave intermediate "junk" tokens. However, the negative impact of technology, particularly technostress, are garnering attention. In the context of the concerns about technostress among higher education institutions (HEI), the aim of the study is to analyze the technostress creators and inhibitors for university teachers and students in different European countries. The topical concept of digital well-being is seen as "a subjective individual experience of optimal balance between the benefits and drawbacks obtained from mobile connectivity, focusing on the personal perception of what amount of time spent using technology is optimal so that well-being is preserved". Method To explore specific aspects related to use of technology, two - parallel online surveys for academics (N = 446) and students (N = 660) from four European countries (Romania, Germany, Norway, and Finland) were conducted between November 2022 - January 2023. The surveys included the Technostress scale and the Technostress Inhibitors Scale, the Technology self-efficacy Scale, and a questionnaire focusing on socio-demographic aspects, work experience, academic field, dimensions related to the actual use of technology and participants perception on the optimal use of technology for work, learning or personal tasks, in terms of the period of day/week and amount of time spent. We also inquired about the social support given and received when using technology and the formal and informal rules, expectations, policies, punishments, and rewards regarding the use of technology. Results The findings suggest that the perceived optimal use of technology is significantly lower than the actual use for all the contexts. Overuse of technology was associated with technostress. Our results also showed that technology self-efficacy and social support from colleagues and teachers are negatively associated with technostress. Country differences regarding technostress and time spent online were also observed. The aim was to capture experiences of PPI across the five countries, explore the benefits and challenges of PPI in dementia research, and identify contributions made to the study. Methods We surveyed PPI members and academic researchers who collaborated on the HOMESIDE study. The survey was co-designed through consultation with PPI members and academics, alongside a small scoping literature review. Survey questions covered four topics: (1) expectations for PPI, (2) perceived contributions of PPI to the research study, (3) benefits and challenges of PPI, and (4) recommendations for future PPI in dementia research. Results There were 23 responses, representing 50\% of the PPI members (n = 16) and 29\% of academics (n = 7). PPI was found to be beneficial to the research and individuals involved. Contributions to the research included supporting recruitment and publicity, advising on the design of participant-facing materials, guiding the design and delivery of the interventions, and identifying cultural differences affecting research delivery. PPI members benefited from building connections, sharing experiences and receiving support, learning about dementia and research, and gaining new unexpected experiences. Academics learned about the realities of living with dementia, which they felt informed and grounded their work. Several challenges were identified, including the need for clear expectations and objectives, inconsistency of PPI members across research stages, limitations of meeting online versus in-person, scheduling difficulties, and language barriers. Conclusions This study identifies important considerations for implementing PPI within dementia studies and international healthcare research more broadly. The main focus is on the evaluation which was done using a standardized questionnaire on intrinsic motivation built upon SDT at the beginning and at the end of our course. 39 students participated before taking the course and 16 after the completion of the course. The median diagnostic delay between onset of symptoms and diagnosis was 9.0 (range: 2.5; 33.0) months. Local health care providers (HCPs) play a central role on the way to diagnosis. Diagnosis itself typically requires a clinical but not necessarily a university setting. In the treatment phase, the focus moves to the amyloidosis centre as primary contact and coordinator, with general practitioners (GPs) acting predominantly as a contact point in crisis and link to additional services. About half of patients reported impaired quality of life and one third suffering from anxiety and depressed mood, respectively. The majority of patients talk about their concerns with close caregivers and local HCPs. Advance care planning is a relevant, yet insufficiently met need. Conclusion The journey of patients with amyloidotic disease, their contact partners and needs at different stages were characterized in detail within the German health care system. An amyloidosis-specific care concept has to master the multitude of interfaces connecting the numerous treatment providers involved with the amyloidosis centre and GPs as key players. However, it is unclear whether the actual performance of a SCA corresponds to the users' experiences. This qualitative study investigates the subjective perspectives of SCA users to close the empirical gap identified in the literature and answers the following main research question: How do individuals (healthy users and patients) experience the usage of SCA, including their attitudes, expectations, motivations, and concerns regarding their SCA use? Methods A qualitative interview study was chosen to clarify the relatively unknown experience of SCA use. Semi-structured qualitative interviews with SCA users were carried out by two researchers in tandem via video call. Qualitative content analysis was selected as methodology for the data analysis. Results Fifteen interviews with SCA users were conducted and seven main categories identified: (1) Attitudes towards findings and recommendations, (2) Communication, (3) Contact with physicians, (4) Expectations (prior to use), (5) Motivations, (6) Risks, and (7) SCA-use for others. Conclusions The aspects identified in the analysis emphasise the specific perspective of SCA users and, at the same time, the immense scope of different experiences. Moreover, the study reveals ethical issues, such as relational aspects, that are often overlooked in debates on mHealth. Methods A total of 48 SCA users documented their medical symptoms, provided open-ended responses, and recorded their SCA use along with other variables over 6 weeks in a longitudinal study. Generalized linear mixed models with and those without regularization were applied to consider the hierarchical structure of the data, and the models' outcomes were evaluated for comparison. Qualitative data were analyzed through Kuckartz qualitative content analysis. Results Significant predictors of SCA use included the initial occurrence of symptoms, day of measurement (odds ratio [OR] 0.97), self-rated health (OR 0.80, P\<.001), and the following International Classification in Primary Care-2-classified symptoms, that are general and unspecified (OR 3.33, P\<.001), eye (OR 5.56, P=.001), cardiovascular (OR 8.33, P\<.001), musculoskeletal (OR 5.26, P\<.001), and skin (OR 4.76, P\<.001). The day of measurement and self-rated health showed minor importance due to their small effect sizes. Qualitative analysis highlighted four main themes: (1) reasons for using SCA, (2) diverse affective responses, (3) a broad spectrum of behavioral reactions, and (4) unmet needs including a lack of personalization. Conclusions The emergence of new and unfamiliar symptoms was a strong determinant for SCA use. Specific International Classification in Primary Care-rated symptom clusters, particularly those related to cardiovascular, eye, skin, general, and unspecified symptoms, were also highly predictive of SCA use. Dennoch ist wenig {\"u}ber die einrichtungsbezogenen Bedarfe und den Ist-Stand im Hinblick auf tats{\"a}chlich im Setting umgesetzte Angebote bekannt. Methode Es wurde eine schriftliche Befragung aller 850 station{\"a}ren Einrichtungen der Eingliederungshilfe f{\"u}r erwachsene Menschen in Bayern durchgef{\"u}hrt. Die Befragung erfolgte online, adressiert an die Einrichtungsleitungen. Insgesamt konnten Angaben von 96 Einrichtungen (11,3\% aller Einrichtungen) in die deskriptiven Analysen und kategorisierenden Freitextauswertungen eingeschlossen werden. Ergebnisse In 60\% der befragten Einrichtungen versterben Bewohnende. 73,7\% der Einrichtungen bieten Gespr{\"a}che im Sinne einer gesundheitlichen Vorausplanung f{\"u}r ihre Bewohnenden an. In 62,9\% der anbietenden Einrichtungen werden hierf{\"u}r standardisierte Verfahren genutzt. Einrichtungsseitig wird u.a. eine bessere Qualifizierung der durchf{\"u}hrenden Personen als Grund f{\"u}r das Nicht-Anbieten berichtet. Als Hindernisse werden mitunter mangelnde F{\"a}higkeiten der Bewohnenden zur Teilnahme an Vorausplanungsgespr{\"a}chen oder die Ablehnung von Angeh{\"o}rigen genannt. Diskussion Die Versorgung am Lebensende, inklusive einer gesundheitlichen Vorausplanung f{\"u}r zuk{\"u}nftige Ereignisse, ist f{\"u}r station{\"a}re Einrichtungen der Eingliederungshilfe ein relevantes Thema. Gespr{\"a}che zur gesundheitlichen Vorausplanung werden dementsprechend bereits von einem Teil der Einrichtungen gef{\"u}hrt. Besondere Bed{\"u}rfnisse der Bewohnenden durch vorhandene Beeintr{\"a}chtigungen k{\"o}nnen hierbei Herausforderungen darstellen. Schlussfolgerungen Die Studie l{\"a}sst R{\"u}ckschl{\"u}sse auf die Optimierung von gesundheitlichen Vorausplanungsprozessen aus Einrichtungsperspektive zu, die sowohl strukturelle (z.B. Adaptation bestehender Konzepte) und personale (z.B. Qualifizierung) als auch emotionale (z.B. Another health risk these professionals experience is the time spent sitting in ergonomically at-risk postures. In the current study, a checklist with critical behaviors and behavioral products (results of behaviors) was used to assess the level of safety in a dental surgery office with seven employees. After baseline observations, task clarification, verbal and graphical feedback, goal setting and tangible reinforcers were used to improve the safety related behaviors of the employees. For all items on the checklist, the percent safe during baseline was 79.57\%. During intervention, the mean percent safe was 88.88\%, and the final level was 95\%. Follow-up observations showed a decline in percent safe after the feedback ended.
