Objective A multi-stage, randomised, controlled study design is concerned with the establishment of an evidence-based perioperative physiotherapeutic treatment programme to evaluate physical functional test procedures with regard to their predictive value and the efficacy of physiotherapeutic interventions. Setting The RCT will be realised in a medical clinic in the department of orthopaedics, sports medicine and rehabilitation. Participants A first stage will include young, healthy subjects (Group A) and subjects who are due to undergo TKA and are then to be treated with a standard rehabilitation programme (Group B). Design Selected promising physical functional test procedures will be tested for their reliability. Inter-tester reliability will be determined in Group A. The test-retest reliability at different perioperative timepoints will be determined in Group B. Measurements The following test procedures will be investigated for reliability: 10-m walk test, timed up and go, three times sit to stand, isometric maximal force of knee extension, stair climbing and the cumulative leg circumference. In addition, these test procedures will be correlated among each other and with the WOMAC and KSS for selected aspects of validity. In a second stage, the reliable test procedures will be applied to investigate selected promising physiotherapeutic interventions. Conclusions The investigation could change perioperative assessments and physiotherapeutic treatment in patients who have undergone TKA. A randomized, controlled and double-blind pilot cross-over trial T2 - The Journal of Sports Medicine and Physical Fitness N2 - OBJECTIVES: The symptomatic reduction of exercise-induced delayed onset muscle soreness (DOMS) is of great interest in the fields of Sports Medicine and Physical Therapy. At this time, few therapeutic interventions have proven their effectiveness. One of the most promising interventions is Compression Therapy. The object of this study is Tissue Flossing, a therapeutic short-term compression method and whether its post-exercise application can reduce the extent of perceived DOMS. DESIGN: randomized, controlled, and double-blind pilot cross-over trial. METHODS: Participants (n = 17) without any musculoskeletal injuries were recruited from university students. Participants performed an exercise protocol and obtained the intervention subsequently on one of both arms. Participants had to return for a second day to perform the whole procedure again, this time receiving the flossing intervention on the other arm. At both time points their opposite arms served as the control. The primary outcome measure was a patient reported visual analogue scale 100mm. RESULTS: The mean value of DOMS of the flossed arm was 42mm compared to the non-flossed arm with 48mm. Differences were found to be statistically significant at 24 hours- (p=0,036; α=0.05), as well as at 48 hours post-intervention (p=0,035; α=0.05). Effect sizes were dz=0,408 at 24 hours-and dz=0,411 at 48 hours post-intervention. The clean language effect size was 0.66. CONCLUSIONS: Tissue flossing appears to be an effective method for treating DOMS which is slightly less effective but much more practicable than gold standard treatment. Therefore, bioimpedance analysis, 3D-laser thigh circumference measurement and isokinetic dynamometry in a closed kinetic chain were used. The side-to-side differences were investigated in a group of judo athletes 5 years after ACLR (n=17) and compared with a group of healthy judo athletes (n=27). Neither thigh circumferences, nor muscle masses of the lower extremities differed in formerly injured judo athletes compared to healthy judo athletes. In contrast, isokinetic strength testing showed a significantly larger side-to-side difference of peak muscle force in formerly injured judo athletes (p=0.021). They provided significantly lower peak forces with the formerly injured leg than with the non-injured leg (p<0.001). The authors conclude that strength capabilities, but not body composition, remains altered in recovered judo athletes in mean 5 years after ACLR and definitely after their return to sports. This indicates that the focus of rehabilitation protocols and return to sports assessments should focus more on maximum strength capabilities. Performance-based measures are necessary to capture different aspects of physical function. High reliability and agreement of five performance-based measures were hypothesized to differentiate between measurement error and change in test performance. Secondary outcomes are the correlation of performance-based measurements to KSS and WOMAC prior to surgery (baseline) and 10 weeks thereafter (t3). Methods The test–retest reliabilities and agreements of the 1-m walk test, the stair-climbing test, the timed-up-and-go test, the weight-balanced-chair-rising test and the isometric maximum knee extension force in patients undergoing total knee replacements were studied. The intraclass correlation coefficient was calculated and a Bland–Altman analysis performed. The weight-balanced-chair-rising test showed a symmetry at baseline = 0.77, 5 ± 1 days after surgery (t1) = 0.50, 9 ± 1 days (t2)= 0.59 and (t3)= 0.80. All performance tests showed high intraclass correlation coefficients (ICC= 0.81–0.99). The 10-m walk test, stair climbing test, and the timed-up-and-go test showed high agreement in the Bland–Altman analysis. The Bland–Altman analysis for the weight-balanced-chair-rising test and isometric knee extension force indicated high agreement at 5 and 9 days postoperatively, but the relative measurement error increased pre- and 10 weeks postoperatively. In conclusion, symmetry, as an important outcome after TKA, is a reliable and rather unique item that should unquestionably be added to established measurements like walking tests or survey-based function assessment. The implementation of standardized performance-based measures to assess physical function in rehabilitation procedures will help to improve the more objectively based assessment of different rehabilitation protocols. It is suggested that approximately 40% of chronic low back pain (CLBP) is disc related in form of internal disc disruptions. Dehydration of the disc leads to stress concentrations in the disc itself and apophyseal joints. Upright-MRI is likely to become the ‘gold-standard’-measurement for determination of IVD heights, its practical usability will remain limited because of its high cost and its low availability. The aim of this study is to investigate the accuracy and reliability of the measurement of the height of the spine with the 3D-Bodyscanner. Therefore, the reliability of different measurement settings (S) had to be investigated. S1: At first it was investigated, if the length of the spine can be measured reliable with the 3D-Bodyscanner when the same person is measured twice without moving between the measurements. The reliability and the first tester influenced bias is estimated. S2: The same person was measured after the person moved from the chair in between both measurements. S3: The participants were measured twice and the markers were renewed between both measurements. Furthermore, the standard error of measurement (SEm) was estimated to compare the accuracy of this measurement with expectable changes in spinal height in daily live and different research tasks . 14 healthy men and 14 healthy women participated in this study. The following pearson correlation coefficients (r) and SEms resulted from the different measurement settings: S1: The measurement of the spinal length in humans that did not move in between the two measurements correlated with r=0.99. This resulted in a SEm of 3.5mm. S2: The measurement of the spinal length in humans that moved in between the measurements but kept the same markers on their spine correlated with r=0.98. This resulted in a SEm of 4.1mm. S3: The measurement of the spinal length in humans that moved in between the measurements and got new markers applied on their spine correlated with r=0.95. This resulted in a SEm of 7.1mm. The measurement of the spinal height with the VITRONIC 3D-Bodyscanner appears to be highly reliable, especially if the markers do not have to be renewed for the retest (S1 and S2). The accuracy of this measurement method seems to be high enough to measure the exercise- or traction induced height changes in the spine. In the case of measuring the height change that is induced by 10 minutes of different types of positioning, the change is maybe to small to be detected. Even if the markers have to be renewed, the accuracy is still discussable to measure diurnal height changes. In conclusion, the 3D-Bodayscanner appears to be a more cost-effective alternative for the measurement of specific segments of the spine than MRI and a practicable alternative for science in the field of de- and rehydration. The study material consisted of 24 patients with end-stage hip osteoarthritis scheduled for THA. The patients were examined one day before surgery and consecutively on the 4th day, 9th day, and 10th week postoperatively using the SBMs (weight-bearing chair-rising test, measurements of the maximal isometric torque of the quadriceps muscle); the PBMs (10-m walk, timed up-and-go, and stair-climbing tests); and the functional outcome measure (Harris Hip Score). The results obtained in a given category of tests at different time points were compared, and the correlations between the tests were determined. The reliability of the outcome measures was determined. The results of tests in the studied categories statistically significantly (p < 0.05) improved at the 10th week postoperatively compared to preoperative results. No strong correlations were revealed between the three studied types of outcome measures in THA patients. Therefore, none of them can be considered redundant. It also means that the relevance of symmetry for a core measurement set to describe the domain function in THA patients must be further clarified. The added value of prosthetic treatment is largely undisputed. The comparability of clinical studies to quantify this added value, on the other hand, is hardly given. The initiative "Outcome Measures in Rheumatolgy" (OMERACT) achieved consensus between different actors in the research process regarding the core domain sets to be applied. Pain, function, patient satisfaction, revision rate, adverse effects and Death were identified as mandatory domains. However, the so‐called core measurement sets, which are used to describe a domain, are still controversial. For example, the domain function can be recorded using questionnaires or with physically functional test procedures. Symmetry, walking ability and leg strength are accepted subdomains of this domain. This study was conducted in an acute clinical setting. Included were twenty‐four volunteers who received TJR there. All patients completed the questionnaires KOOS and WOMAC. A test supported by two piezoelectric force plates to measure symmetry during five transfers from the seat to the stand serves to describe this subdomain. The common test methods Timed up and go, Ten Meter Walking Test and Stair Climbing Test were used to measure the subdomain walking ability. A manual extension torque test, using a Micro Fed 2 dynamometer, was used to determine the force of the Quadricps femoris muscle. The test retest design was applied preoperatively, 5 and 10 days postoperatively and 10 weeks postoperatively with complete breaks within a test day. The test procedures used here to determine symmetry and walking ability at all measurement times impress reliable (r>0.80). The reliability of the force measurement is insufficient. Disturbances are the constitution of the tester and the pain condition of the patient. The test procedures for determining walking ability have a high degree of redundancy over time, so that only one has to be used in a Core Measurement Set. Since the correspondence between the physically functional tests performed and the questionnaires for recording the function is insufficient, the thesis is supported that procedures must be used from both areas to describe the domain function accurate. For the treatment of ULBP, exercise therapy is the treatment of choice. The deadlift exercise is commonly used in athletes to develop basic strength of the trunk muscles but lately it is also suggested to be used in patients with ULBP. Until now, there is no data available how the different gripping techniques affect symmetry during the deadlift and therefore the safety of this exercise in a rehabilitative or preventive setting. The aim of thisstudy wasto investigate whether different gripping techniques during the deadlift exercise affect the symmetry of the ground reaction forces during the exercise. Sixteen healthy participants were asked to perform a deadlift on a ground reaction force plate three times with three different gripping techniques each time. The different gripping techniques were double pronated (DP), handed supinated (HS) and non‐handed supinated (NHS). All participants were randomly assigned to three different chronologies of the gripping techniques. The main outcome measurement was the symmetry of the ground reaction forces during the deadlift exercise. Therefore, the maximum side to side differences and the mean side to side differences during the performance of the deadlift were analysed with a repeated measures ANOVA. Post‐hoc testing was done with the Bonferroni‐test. The three different gripping techniques show significant differences between the groups for the maximal differences (F(1.2)=7.523, p=0.01) and the mean differences (F(1.419)= 6.347, p=0.013). Posthoc testing revealed significant differences between DP and HS for the maximum asymmetry (p=0.009) and for mean asymmetry (p=0.035). Between DP and NHS there were significant differencesfor maxiumum asymmetry (p=0.003) and a tendency for differences for mean asymmetry (p=0.051). No significant differences were found between HS and NHS (p>0.05). The results of this study show that the use of different gripping techniques significantly affect the side to side asymmetry during the deadlift. The DP grip results in lower asymmetry the switch grips HS and NHS. Therefore, this study indicates that the DP gripping style should be used in patients with ULBP. The postoperative outcomes as well as the different rehabilitation procedures are most commonly evaluated by patient-reported outcome measurements (PROMs). However, in acute clinical-stage stages, performance-based measures were shown to yield greater responsiveness than PROMs, which failed to capture functional declines and may overestimate long-term functional improvements. Moreover, these tests represent a variety of daily life activities which helps to estimate whether a patient is functioning adequately prior to discharge from hospital and in the following rehabilitation. The ability to rise from a sitting position is one of those activities which is important for an elderly person to maintain an independent life. Also, it was shown that chair rising test performance is highly correlated with deviations in gait patterns. However, there is no data available yet if this test can be used at perioperative time-points in patients undergoing TKA and how reliable this test is. Therefore, this study investigates the reliability and the minimal detectable change and the development of the performance of the weight-balanced chair rising test in patients before and after TKA. This study was designed as a test-retest reliability study. Participants were recruited in an orthopedic hospital after they were scheduled for total knee arthroplasty. The participants performed two chair rising tests (five-times-sit-to-stand) on a ground reaction force plate at one-day preoperatively (M0), five days- (M1), ten days (M2)- and ten weeks postoperatively (M3). The test-retest reliability of the weight balanced asymmetry measured as ground reaction force in Newton was analyzed for each of the four time-points. Therefore, the minimal detectable change within a confidence interval of 95% (MDC95) and the ICC were calculated. 16 male and 8 female subjects (N=24) with a mean age of age 63,93 (±9,59) were included. The mean side-to-side difference in N was 138(±104) at M0, 319(±125) at M1, 254(±109) at M2 and 116(±74) at M3. The test-retest reliability ranged between ICC=.84-.96 at all four time points, indicating good to excellent reliability. The MDC95 ranges from 79–113 N. No adverse events were recorded after the performance of the test and no participants dropped out because of study-related reasons. The weight-balanced chair rising test shows good- to excellent reliability. After an initial decrease of loading symmetry at M1 and M2 preoperative levels are reached as early as at M3. The weight-balanced chair rising test provides objective data about loading symmetry during an activity of daily living. It is reliable and feasible to use in as well in-patient as out- patient rehabilitation of patients with TKA.
