    <title language="deu">Programmevaluation von GLA:D® Deutschland: vorläufige Ergebnisse einer Mixed-methods-Studie unter Verwendung des RE-AIM QuEST-Frameworks</title>
    <abstract language="deu">Hintergrund/Fragestellung: Arthrose der Hüft- und Kniegelenke ist eine weitverbreitete Erkrankung, die mit Schmerz, Funktionseinschränkungen und einem Verlust der Lebensqualität einhergeht. Um Patient:innen den Zugang zu einer evidenzbasierten Versorgung zu gewähren, wurde im Jahr 2013 das Good-Life-with-osteoArthritis-from-Denmark (GLA:D®)-Programm in Dänemark initiiert. Dieses beinhaltet ein strukturiertes Edukations- und Übungsprogramm, welches von geschulten Therapeut:innen angeleitet wird. Im Jahr 2022 startete die Implementierung von GLA:D® in Deutschland. Das Ziel der Studie ist die Evaluation der Implementierung von GLA:D® in Deutschland. Unter Verwendung des RE-AIM QuEST-Frameworks sollen Reichweite, Effektivität, Akzeptanz, Implementierung, Aufrechterhaltung sowie Hindernisse und Förderfaktoren evaluiert werden.&#13;
&#13;
Methoden: Zur Beantwortung der Fragestellungen wird auf drei Datenquellen zurückgegriffen. Neben (1) einer Online-Befragung von Physiotherapeut:innen unmittelbar vor, sowie zwei Wochen und zwölf Monate nach der GLA:D®-Schulung (09/2022–02/2024), werden die (2) GLA:D®-Patientendatenbank (03/2023–02/2024) sowie das (3) Register mit Implementierungsdaten zu GLA:D®-Deutschland (09/2022–02/2024) genutzt. Die Datenerhebung der Online-Befragung folgt der Methodik von GLA:D® Australia [1], [2]. Erhoben werden die aktuelle Versorgungspraxis, Einschätzungen und Überzeugungen in Bezug auf die Versorgung von Personen mit Hüft- und/oder Kniegelenksarthrose und die Kenntnis von Leitlinien.&#13;
&#13;
Ergebnisse: Zwischen November 2022 und Juni 2023 haben 147 Therapeut:innen aus 49 physiotherapeutischen Einrichtungen an einer GLA:D®-Schulung teilgenommen. 115 Therapeut:innen beantworteten den Online-Fragebogen zur Evaluation des Programms unmittelbar vor der Schulung, 33 Personen nahmen an der erneuten Befragung nach zwei Wochen teil. Seit dem Start der ersten Trainingsgruppe im März 2023 ist das GLA:D®-Programm in 10 verschiedenen Physiotherapiepraxen mit insgesamt 75 Patient:innen angelaufen. Die Erhebung der Daten erfolgt kontinuierlich, sodass im Rahmen der 25. Jahrestagung des EbM-Netzwerks aktualisierte Ergebnisse der Programmevaluation von GLA:D® Deutschland präsentiert werden sollen.&#13;
&#13;
Schlussfolgerung: Eine Evaluation des Implementierungsprozesses mit Blick auf Reichweite, Effektivität, Akzeptanz, Umsetzung und Aufrechterhaltung sowie möglicher Barrieren und Förderfaktoren kann zum Erfolg von GLA:D® Deutschland beitragen.</abstract>

    <title language="deu">Barrieren und Förderfaktoren in der Umsetzung leitliniengerechter Versorgung von Patient*innen mit Hüft- und/oder Kniegelenksarthrose in der physiotherapeutischen Praxis: Eine qualitative Interviewstudie</title>
    <abstract language="deu">Hintergrund und Stand der Forschung: Hüft- und Kniegelenksarthrose ist eine in der Bevölkerung weitverbreitete Erkrankung, die mit einer deutlichen Einschränkung der Lebensqualität sowie hohen Kosten für das Gesundheitssystem einhergeht. Leitlinien fassen die Evidenz verschiedener diagnostischer und therapeutischer Maßnahmen zusammen und sollen so Angehörigen der Gesundheitsberufe und Patient*innen als Entscheidungshilfe in der medizinischen Versorgung dienen. Aktuelle Untersuchungen zeigen jedoch, dass Physiotherapeut*innen in der Behandlung von Hüft- und Kniegelenksarthrose häufig von den in Leitlinien formulierten Empfehlungen abweichen. Die Gründe für das Nichteinhalten von Leitlinienempfehlungen sind komplex und umfassen persönliche Barrieren, z.B. das eigene Wissen und Einstellungen, sowie leitlinienbezogene und externe Faktoren. Landesspezifischen Barrieren wird dabei ebenfalls eine besondere Rolle zugesprochen.&#13;
&#13;
Fragestellung und Zielsetzung: Das Ziel der Studie ist die Erfassung von Barrieren und Förderfaktoren in Bezug auf die Anwendung von Leitlinien in der physiotherapeutischen Versorgung von Patient*innen mit Hüft- und/oder Kniegelenksarthrose in Deutschland.&#13;
&#13;
Folgende Fragestellungen sollen beantwortet werden:&#13;
&#13;
1.&#13;
    Welche Faktoren beeinflussen die Nutzung von Leitlinien in der physiotherapeutischen Praxis? &#13;
2.&#13;
    Was muss verändert werden, damit die Empfehlungen aus Arthrose-Leitlinien häufiger umgesetzt werden?&#13;
&#13;
Methode: Im Frühjahr 2023 werden semi-strukturierte Interviews mit in der ambulanten Versorgung tätigen Physiotherapeut*innen durchgeführt. Über die Auswahl einer gezielten Stichprobe („Purposive Sampling“) anhand von Alter, Geschlecht, Berufserfahrung und Ausbildungsniveau soll ein möglichst breites Spektrum an Informationen abgebildet werden. Der Interviewleitfaden wurde mithilfe des Theoretical Domains Framework (TDF) entwickelt und in zwei Pilotinterviews vorab getestet. Die Interviews werden aufgezeichnet, transkribiert und inhaltsanalytisch nach Braun und Clarke ausgewertet.&#13;
&#13;
Ergebnisse: Die Erhebung ist zum Sommer 2023 abgeschlossen und die Ergebnisse sollen im Rahmen des Deutschen Kongresses für Versorgungsforschung (DKVF) 2023 präsentiert werden.&#13;
&#13;
Diskussion: Ein optimaler Weg zur Implementierung von Leitlinien scheint noch nicht gefunden zu sein. Eine Veränderung im Verhalten der Behandelnden steht häufig im Fokus von Interventionen und ist essenziell für die erfolgreiche Implementierung von Leitlinien. Expert*innen empfehlen die Verwendung theoretischer Modelle, um Hindernisse in der Umsetzung von Leitlinien zu verstehen und um die Auswahl geeigneter Maßnahmen zu informieren. Eine Untersuchung (landes-)spezifischer Barrieren ist daher eine wichtige Voraussetzung für die Entwicklung einer zielgerichteten Implementierungsintervention, welche langfristig zu einer verbesserten Qualität der Gesundheitsversorgung beitragen soll.&#13;
&#13;
Implikation für die Versorgung: Unter Berücksichtigung der identifizierten Barrieren und Förderfaktoren sollen anschließend mithilfe des Behaviour Change Wheel (BCW) systematisch relevante Maßnahmen zur Verhaltensänderungen geplant werden.</abstract>

    <title language="eng">Physiotherapy for patients with hip and knee osteoarthritis in Germany: a survey of current practice</title>
    <abstract language="eng">Backround&#13;
&#13;
Osteoarthritis (OA) of the hip and the knee is common and leads to pain, stiffness and disability. Clinical practice guidelines (CPG) provide recommendations to assist healthcare professionals in clinical decision-making. Although evidence-based physiotherapy has been shown to be effective in the management OA, a gap between clinical practice and guideline recommendations exists. Little is known about OA management provided by physiotherapists in Germany and whether treatment aligns with CPGs. The objectives of this study were (1) to investigate the current physiotherapy practice in patients with hip and/or knee OA in Germany, (2) to evaluate physiotherapists’ adherence to guideline recommendations and (3) to explore barriers and facilitators to guideline use.&#13;

Methods&#13;
&#13;
A cross-sectional online survey was conducted among physiotherapists. The questionnaire collected information on demographic characteristics, physiotherapists’ management of hip and knee OA and the use of CPGs in clinical practice. Guideline adherence was evaluated by comparing the survey findings with guideline recommendations. Full adherence was assumed if all recommended treatment options were chosen.&#13;
&#13;
Results&#13;
&#13;
In total, 447 (74.9%) of 597 eligible physiotherapists completed the survey. Data from 442 participants (mean age 41.2 ± 12.8 years; 288 female (65.1%)) were included in the analysis. The most common treatment choices for both hip and knee OA were exercise therapy (hip OA: 424/442, 95.9%; knee OA: 426/442, 96.4%), self-management advice (hip OA: 413/442, 93.2%; knee OA: 395/442, 89.4%) and education (hip OA: 325/442, 73.5%; knee OA: 331/442, 74.9%), followed by manual therapy (hip OA: 311/442, 70.4%; knee OA: 311/442, 70.4%) and joint traction (hip OA: 208/442, 47.1%; knee OA: 199/442, 45.0%). Full guideline adherence was found in 17.2% (76/442) of physiotherapists for hip OA management and in 8.6% (38/442) for knee OA. Less than half of the respondents (212/430, 49.3%) were aware of an OA guideline.&#13;

Conclusions&#13;
&#13;
In accordance with current guideline recommendations, most physiotherapists provide exercise therapy and education for patients with hip and/or knee OA. Interventions with low or conflicting evidence were also frequently provided. The limited awareness of existing OA guidelines and the low guideline adherence indicate an insufficient implementation of CPGs in German physiotherapy practice.</abstract>

    <abstract language="eng">Background: Hip and knee osteoarthritis is common and leads to pain, stiffness, and disability. Clinical practice guidelines provide recommendations based on the best available evidence to assist health care professionals and patients in clinical decision-making. However, several studies have reported a gap between guideline recommendations and clinical practice in physiotherapy. Improved implementation strategies and the removal of existing barriers may facilitate the transfer of evidence into clinical practice and contribute to optimized quality of care.&#13;
Objective: This paper presents the protocol for a study that aims to describe the current physiotherapy practice in patients with hip and knee osteoarthritis and to investigate physiotherapists’ adherence to clinical practice guidelines, to identify and specify barriers to and facilitators of guideline use and implementation, and to develop and pilot test a theory-based tailored implementation intervention aiming to increase guideline use in osteoarthritis care.&#13;
&#13;
Methods: The research project is divided into 4 parts. During the first part, we will conduct a nationwide web-based survey among German physiotherapists to evaluate the current management of hip and knee osteoarthritis and to evaluate whether treatment aligns with guideline recommendations. Subsequently, semistructured interviews will be conducted to specify barriers to and facilitators of guideline use and implementation among physiotherapists (part 2). On the basis of these findings, in part 3, we will develop a theory-driven implementation intervention based on the Theoretical Domains Framework and the Behavior Change Wheel, which will be evaluated in a controlled pilot study in terms of effectiveness, feasibility, and acceptability (part 4).&#13;
Results: Data collection of the web-based survey among German physiotherapists (part 1) was completed in December 2021. The semistructured interviews (part 2) were conducted between January and September 2023. Recruitment of physiotherapy practices to participate in the development of the implementation intervention is expected to start in January 2024.&#13;

Conclusions: This research project aims to develop a theory-driven implementation intervention to facilitate the transfer of evidence from hip and knee osteoarthritis guidelines in physiotherapy practice. We hypothesize that successful implementation will lead to increased guideline adherence in physiotherapists, which in turn will improve the quality of care. The results from our project will provide valuable knowledge concerning the development process and effectiveness of tailored implementation interventions.</abstract>

    <title language="deu">Einstellungen und Adhärenz zur evidenzbasierten Praxis von in Brandenburg tätigen Physiotherapeut*innen</title>
    <abstract language="deu">Hintergrund Für Deutschland liegen noch unzureichende Daten zur Einstellung und Adhärenz von Physiotherapeut*innen gegenüber Evidenzbasierter Praxis (EBP) vor, insbesondere für im Bundesland Brandenburg tätige Physiotherapeut*innen.&#13;
&#13;
Ziel Erfassung der 1) Einstellungen und Adhärenz gegenüber EBP von in Brandenburg tätigen Physiotherapeut*innen und die 2) Ableitung von Barrieren und Förderfaktoren für die Einhaltung der EBP.&#13;
&#13;
Methodik Von März bis April 2022 wurde eine Online-Befragung unter in Brandenburg tätigen Physiotherapeut*innen durchgeführt. Die Teilnehmenden füllten die deutschsprachige Version des „Evidence-based Practice Inventory“ aus, einem Fragebogen mit 5 Dimensionen zur Erfassung von Barrieren und Förderfaktoren hinsichtlich EBP. Zusätzlich wurden soziodemografische Daten erfragt. Die Auswertung erfolgte deskriptiv.&#13;
&#13;
Ergebnisse Es nahmen 101 Physiotherapeut*innen an der Befragung teil, davon 75 (74 %) Frauen und 26 (26 %) Männer. Die Befragten waren im Durchschnitt 39 Jahre (± 13) alt und verfügten über eine Berufserfahrung von 16 Jahren (± 12). Mehr als 70 % der Teilnehmenden hatten eine positive Einstellung gegenüber EBP. Knapp 55 % der Physiotherapeut*innen fühlten sich in der Lage, Prinzipien der EBP in ihren klinischen Entscheidungen anzuwenden. Gleichzeitig gaben weniger als 20 % an, dass in ihrer Abteilung sehr darauf geachtet wird, Prinzipien der EBP in klinischen Entscheidungen anzuwenden. Im Durchschnitt wiesen 30 % der Befragten eine hohe EBP-Adhärenz in den 5 Dimensionen auf. Die wichtigsten Förderfaktoren für Adhärenz zur EBP bildeten die Autor*innenschaft oder Mitarbeit an wissenschaftlichen Veröffentlichungen und die Teilnahme an einem Kurs zur EBP.&#13;
&#13;
Schlussfolgerung Die Einstellung von in Brandenburg tätigen Physiotherapeut*innen gegenüber EBP ist grundlegend positiv. Es zeigte sich jedoch, dass die Adhärenz zur EBP nur bei einem geringeren Anteil der Physiotherapeut*innen stark ausgeprägt ist. Barrieren und Förderfaktoren sollten näher untersucht werden, um zielgerichtete Interventionen zu entwickeln und die Implementierung von EBP im klinischen Alltag zu verbessern.</abstract>

Methode&#13;
Physiotherapeut*innen nahmen unmittelbar vor und 2 Wochen nach der Schulung an einer Online-Befragung teil. Erfasst wurden u. a. das Vertrauen in die eigenen Kompetenzen zur Erbringung einer leitliniengerechten Therapie für Patient*innen mit Arthrose, das Wissen über Behandlungsempfehlungen sowie die Kenntnis von Leitlinien. Die Effektivität der Schulung wurde im Rahmen eines Vorher-Nachher-Vergleichs evaluiert.&#13;

Ergebnisse&#13;
Von 290 geschulten Physiotherapeut*innen nahmen 254 an der ersten Befragung teil (Rücklaufquote: 87,6 %), darunter 114 Frauen (44,9 %). Das Durchschnittsalter der Befragten betrug 35 Jahre (± 10,7). Insgesamt 83 Therapeut*innen (32,7 %) verfügten über einen akademischen Abschluss. Der zweite Fragebogen wurde von 85 Personen beantwortet (Rücklaufquote: 29,3 %). Die Teilnahme an der zweitägigen Schulung führte zu positiven Veränderungen beim subjektiv wahrgenommenen Wissen und bei der empfundenen Sicherheit zur Durchführung leitliniengerechter Therapie. Über 98 % der Befragten fühlten sich nach dem Kurs dahingehend geschult und in der Lage, Trainingstherapie und Edukation gemäß den aktuellen Leitlinien zu erbringen.&#13;

Schlussfolgerung&#13;
Die Teilnahme an der zweitägigen GLA:D-Schulung führte zu Verbesserungen beim subjektiv wahrgenommenen Wissen und bei der empfundenen Sicherheit von Physiotherapeut*innen zur Behandlung von Personen mit Hüft- und Kniegelenksarthrose. Clinical practice guidelines recommend a combination of exercise therapy and education for the management of hip and knee osteoarthritis. Initiated in 2013, the Good Life with osteoArthritis in Denmark (GLA:D) programme ensures standardised and evidence-based treatment. In a two-day training course, therapists are trained to provide the structured education and exercise program.&#13;

Aim&#13;
To evaluate the effect of the two-day training course on physiotherapists’ subjectively perceived knowledge and confidence in treating hip and knee osteoarthritis.&#13;
&#13;
Methods&#13;
Physiotherapists completed online surveys immediately before and two weeks after the GLA:D training course. They were asked to rate their confidence in providing guideline-adherent therapy for osteoarthritis, their beliefs about the evidence supporting different treatment options, and awareness of clinical practice guidelines. The effectiveness of the training course was evaluated using a pre- and post-training comparison.&#13;
&#13;
Results&#13;
Of a total of 290 physiotherapists invited, 254 responded to the first survey (response rate: 87.6 %), including 114 women (44.9 %). The mean age of the respondents was 35 years (± 10.7). A total of 83 therapists (32.7 %) had obtained a higher academic degree. The follow-up questionnaire was completed by 85 respondents (response rate: 29.3 %). The two-day training course resulted in improvements in both subjectively perceived knowledge and confidence in providing guideline-adherent therapy. Over 98 % of respondents felt well-trained and able to provide exercise therapy and education according to guideline recommendations.&#13;

Conclusions&#13;
Participation in the two-day GLA:D training course led to positive changes in physiotherapists’ subjectively perceived knowledge and confidence in the management of hip and knee osteoarthritis. These findings underscore the need for targeted education to ensure the comprehensive implementation of evidence and guideline recommendations in clinical practice.&#13;
