During the examination period from June until December 2014, one-dimensional isometric trunk strength measurements were carried out on healthy, middle-aged probands using the biofeedback device "Pegasus". The aim was to check out the predefined standard values of the company BfMC and generate standard value ranges for muscular imbalances. Furthermore, these examinations were used to capture occurring forces on other levels to assess their impact on one-dimensional measurements. A standardized warm up was followed by the determination of the maximum trunk strength in flexion, extension, rotation and lateral inclination. Two measurements on 54 probands at the age of 22.8 ± 3.3 years and an average BMI of 28.9 ± 3.2 were conducted. In addition, the deviating forces were determined. In target-actual value comparison, the probands were about 17.1\% above the normal range of their respective age groups, with an average isometric trunk strength development in all six directions of movement of 835Nm. For the male probands it was also found out that they positioned themselves 63.6\% above (average?). The female probands in total were also within the normal range, but 200Nm less power was generated and 34\% did not reach the normal range. Looking at the deviating forces, it is noticeable that 549.1 Nm (54\%) were also applied in the other anatomic plains. Here, there were clear differences between the deviating forces in the isometric testing in the sagittal and frontal plain. In the measurement results of the sagittal plain (Ext; Flex), the deviating forces were 45.8Nm (29.9\%) on average in the frontal and transverse direction. In the measurement results of lateral inclination and rotation, the deviating forces were 95.3\% and 110\% in sagittal and transverse direction. As a result, it can be noted that, despite the absence of pain, postural imbalances and diseases of the spine, more than half of the forces cannot be generated according to the axis. At this, synergistic functions seem to be physiological in this scope, in order to produce maximum plain equitable forces which basically questions one-dimensional measurements. The test results show significant deviations from the normal range, which raises the issue of the relevance of the defined standard values based on the evaluation of isometric force development. It is suggested that approximately 40\% of chronic low back pain (CLBP) is disc related in form of internal disc disruptions. Dehydration of the disc leads to stress concentrations in the disc itself and apophyseal joints. Upright-MRI is likely to become the 'gold-standard'-measurement for determination of IVD heights, its practical usability will remain limited because of its high cost and its low availability. The aim of this study is to investigate the accuracy and reliability of the measurement of the height of the spine with the 3D-Bodyscanner. Therefore, the reliability of different measurement settings (S) had to be investigated. S1: At first it was investigated, if the length of the spine can be measured reliable with the 3D-Bodyscanner when the same person is measured twice without moving between the measurements. The reliability and the first tester influenced bias is estimated. S2: The same person was measured after the person moved from the chair in between both measurements. S3: The participants were measured twice and the markers were renewed between both measurements. Furthermore, the standard error of measurement (SEm) was estimated to compare the accuracy of this measurement with expectable changes in spinal height in daily live and different research tasks . 14 healthy men and 14 healthy women participated in this study. The following pearson correlation coefficients (r) and SEms resulted from the different measurement settings: S1: The measurement of the spinal length in humans that did not move in between the two measurements correlated with r=0.99. This resulted in a SEm of 3.5mm. S2: The measurement of the spinal length in humans that moved in between the measurements but kept the same markers on their spine correlated with r=0.98. This resulted in a SEm of 4.1mm. S3: The measurement of the spinal length in humans that moved in between the measurements and got new markers applied on their spine correlated with r=0.95. This resulted in a SEm of 7.1mm. The measurement of the spinal height with the VITRONIC 3D-Bodyscanner appears to be highly reliable, especially if the markers do not have to be renewed for the retest (S1 and S2). The accuracy of this measurement method seems to be high enough to measure the exercise- or traction induced height changes in the spine. In the case of measuring the height change that is induced by 10 minutes of different types of positioning, the change is maybe to small to be detected. Even if the markers have to be renewed, the accuracy is still discussable to measure diurnal height changes. Athletes rarely return to the peak of performance or are limited in their ability to perform their sport. Performance tests for ACLR are still immature and often unspecific. In Judo, the ability to stand on one's feet is an essential prerequisite for many situations. An unstable standing position also promotes knee injuries. In order to check the stability of the single leg stand, a controlled test-re-test design is used in the present study. Two groups are included: A (n=27): Judoka without anamnestically relevant injuries of the lower extremities or the back and B (n=17): Judoka 5.06 ± 1.06 years after ACLR. A one-legged hop test up on a piezoelectric force plate is examined for reliability with respect to the test repetition and validity with regard to the predicted identification of deficits in a side-by-side comparison. In this test, the test person jumps with one leg on the force plate at a defined distance and tries to stand as still as possible, which means that hardly any horizontal force is generated. Both tests are carried out one after the other with both legs and with sufficient break twice each. In group A, there were no significant differences (p>0.05) between the subjectively better leg and the subjectively worse leg, or the leg in the front and rear of the kumi-kata in Judo. In group B, in the first experiment with the ACLR leg, the test persons generate a significantly higher horizontal force in a 95\% confidence interval of the bilateral coefficient of CI=[0.16-0.76] compared to the side not affected (p=0.017). In the test repetition, Group B generates with the affected leg significantly less horizontal force surges than in the first attempt. No significant differences in lateral comparison could be measured, although there is a reliability coefficient of r=0.89 for the ACLR leg, but only r=0.35 for the unaffected leg and r=0.53 for group A. It is concluded that the test for asymptomatic judokas cannot be regarded as sufficiently selective due to the low requirement. In group B, in the second experiment, a feed-forward model including pre-activation based on empirical values from the first experiment could explain the improvement, which has led to no significant differences between legs.}, language = {en} } @misc{PrillSchulzMichel, author = {Prill, Robert and Schulz, Robert and Michel, Sven}, title = {Tissue flossing: a new short-duration compression therapy for reducing exercise-induced delayed-onset muscle soreness. At this time, few therapeutic interventions have proven their effectiveness. One of the most promising interventions is Compression Therapy. The object of this study is Tissue Flossing, a therapeutic short-term compression method and whether its post-exercise application can reduce the extent of perceived DOMS. DESIGN: randomized, controlled, and double-blind pilot cross-over trial. METHODS: Participants (n = 17) without any musculoskeletal injuries were recruited from university students. Participants performed an exercise protocol and obtained the intervention subsequently on one of both arms. Participants had to return for a second day to perform the whole procedure again, this time receiving the flossing intervention on the other arm. At both time points their opposite arms served as the control. The primary outcome measure was a patient reported visual analogue scale 100mm. RESULTS: The mean value of DOMS of the flossed arm was 42mm compared to the non-flossed arm with 48mm. Differences were found to be statistically significant at 24 hours- (p=0,036; α=0.05), as well as at 48 hours post-intervention (p=0,035; α=0.05). Effect sizes were dz=0,408 at 24 hours-and dz=0,411 at 48 hours post-intervention. The clean language effect size was 0.66. CONCLUSIONS: Tissue flossing appears to be an effective method for treating DOMS which is slightly less effective but much more practicable than gold standard treatment.}, language = {en} } @misc{PrillMichelSchulzetal., author = {Prill, Robert and Michel, Sven and Schulz, Robert and Appell Coriolano, Hans-Joachim}, title = {Body Composition and Strength Parameters in Elite Judo Athletes 5 Years after Anterior Cruciate Ligament Reconstruction}, series = {International Journal of Sports Medicine}, volume = {40}, journal = {International Journal of Sports Medicine}, number = {1}, issn = {1439-3964}, doi = {10.1055/a-0787-1420}, pages = {38 -- 42}, abstract = {The purpose of this study was to investigate if there are still deficits in muscle mass or strength capabilities in elite judo athletes with a history of anterior cruciate ligament reconstruction (ACLR) after their return to the sport. Therefore, bioimpedance analysis, 3D-laser thigh circumference measurement and isokinetic dynamometry in a closed kinetic chain were used. The side-to-side differences were investigated in a group of judo athletes 5 years after ACLR (n=17) and compared with a group of healthy judo athletes (n=27). Neither thigh circumferences, nor muscle masses of the lower extremities differed in formerly injured judo athletes compared to healthy judo athletes. In contrast, isokinetic strength testing showed a significantly larger side-to-side difference of peak muscle force in formerly injured judo athletes (p=0.021). They provided significantly lower peak forces with the formerly injured leg than with the non-injured leg (p<0.001). The authors conclude that strength capabilities, but not body composition, remains altered in recovered judo athletes in mean 5 years after ACLR and definitely after their return to sports. This indicates that the focus of rehabilitation protocols and return to sports assessments should focus more on maximum strength capabilities.}, language = {en} }
