This study aimed to compare the results of two nationwide surveys of internet users in Germany that investigated the use pattern, the attitudes towards, and the factors associated with digital technology use in health context. Methods: This study adheres to the ethical guidelines according to the Declaration of Helsinki. Two cross-sectional surveys of internet users living in Germany were performed in 2020 (survey 1) and 2022 (survey 2). Participants were recruited from two different consumer panel samples owned by a market research institute (Cerner Enviza GmbH). Participants provided verbal informed consent for participation in the study according to the Cerner Enviza data privacy policy. Computer-assisted telephone interviews were conducted by Cerner Enviza using questionnaires with 28-30 items developed by study authors. Anonymous data were purchased and analysed by study authors using descriptive statistics. Associations between digital technology use and participant characteristics were tested using binary logistic regression analyses. Results: The participants were adult internet users (survey 1: n=991; survey 2: n=1020). Participant characteristics were similar in both surveys (47-52\% female, 50-55\% aged 40-65 years, 62-66\% with less than tertiary education, 42-45\% with less than country-average household income, 81-82\% residents in former West Germany, and 67-71\% residents in large cities). A similar number of participants in both surveys reported digital technology use in any health context (57-61\%) and rated digital technologies for physical activity promotion as easy to use (87-92\%). Digital technology use increased in 2022 relative to 2020. Specifically, more participants reported health app (63\% vs. 36\%) and digital technology use for physical activity promotion (any use: 44\% vs. 31\%; weekly-daily use: 91\% vs. 64\%) in 2022 vs. 2020. In contrast, digital health literacy decreased in 2022 relative to 2020. Specifically, less participants had high digital health literacy (57\% vs. 69\%) and were confident in using online information for health decisions (34\% vs. 43\%) in 2022 vs. 2020. Digital technology use was associated with higher digital health literacy and younger age (in both surveys), higher income (in survey 1) and residence in larger cities (in survey 2). Conclusion: Digital technologies targeting some health domains, such as physical activity promotion are frequently used and accepted by the users. However, digital health literacy and especially the confidence in using online information for health decisions appear to have decreased in 2022 relative to 2020. Factors that promote digital technology use for health promotion and disease prevention need to be investigated in future studies. Methoden: Verwendet wurden Daten der Schuleingangsuntersuchung im Land Brandenburg aus drei aufeinanderfolgenden Kohorten 5-7-j{\"a}hriger Kinder (2018|19: n=19.299; 2019|20: n=19.916; 2020|21: n=19.698). Der {\"o}ffentliche Gesundheitsdienst erfasst routinem{\"a}ßig die Sprachentwicklung mit validierten Instrumenten des 'Sozialp{\"a}diatrischen Entwicklungsscreenings f{\"u}r Schuleingangsuntersuchungen' [3]. Deskriptive bivariate und multivariate Querschnittsanalysen [OR, 95\%CI] untersuchten die Sprachentwicklungsverz{\"o}gerung [SE ja|nein] mit SEP [niedrig|mittel|hoch], deutscher Muttersprache [ja|nein] und den Jahren des Kindergartenbesuchs [<4|≥4]. Die Analysen wurden stratifiziert nach SEP wiederholt. Ergebnisse: Die SE Pr{\"a}valenz sank {\"u}ber die Jahre insgesamt (21,1\%2018|19; 19.2\%2019|20; 18,8\%2020|21) sowie bei Kindern mit hoher SEP (14,3\%2018|19; 13,0\%2020|21) und mit deutscher Muttersprache (20,2\%2018|19; 17,8\%2020|21). Bei Kindern mit niedrigem SEP stieg die SE Pr{\"a}valenz {\"u}ber die Jahre (42,4\%2018|19; 43,4\%2020|21) und bei Kindern mit nicht-deutscher Muttersprache blieb sie konstant (36,0\%2018|19, 35,5\%2020|21). Schlussfolgerung: Soziale Ungleichheiten bei der SE Pr{\"a}valenz aufgrund der SEP und der Muttersprache nahmen w{\"a}hrend der Pandemie leicht zu. Im Privatleben zeigte sich eine zunehmende soziale Isolation und die Ver{\"a}nderungen in den interpersonellen Beziehungen waren belastend. In addition, we show how such a causal DAG can be drawn based on prior knowledge. We will discuss further approaches to determine sufficient adjustment sets. If prior knowledge is rare, unclear, or questionable, causal discovery algorithms can be applied to estimate a causal DAG based on data. These algorithms improved greatly over the last years. Nowadays, they can be easily applied to different data structures to estimate at least a partially directed DAG. The analyses are based on data from the "German Health Interview and Examination Survey for Children and Adolescents" (KiGGS) (1,384 females and 1,332 males aged 11 to 17 years). Logistic regression models explored how familial determinants (family stress, family cohesion, parental smoking, parental sporting activity, and parental overweight) mediated the association between family SEP (parental education, occupational status, and household income) and adolescent obesity. Results. Significant total effects for the associations between family SEP in childhood and adolescent obesity were found. Splitting the total effect of the family SEP on obesity into direct and indirect effects, all direct effects turned out to be significant. However, all associations involved also indirect effects of familial determinants, except for household income for female adolescents. Parental smoking and overweight were the most relevant mediators for males and females. For male adolescents, parental sporting activity additionally mediated the association between SEP and obesity. Conclusion. A low SEP in childhood was associated with adolescent obesity. Parental health and health behaviors partly explained the association. Language development was assessed as delayed compared to peers [LD yes/no], measured by public health services using validated instruments. Descriptive bivariate and multivariate analyses [OR, 95\%CI] examined language delay [\%] with social inequalities defined by (1) an index composed of parents' reported education and occupation [SEP low|high], (2) 'German native language' [yes|no] and (3) 'years of kindergarten attendance' [<4|≥4]. Results Overall, the proportion of LD decreased over the years (21.1\%2018|19; 19.2\%2019|20; 18.8\%2020|21), both among children from families with high SEP (14.3\%2018|19; 13.0\%2020|21) and from German-speaking families (20.2\%2018|19; 17.8\%2020|21). In dieser Analyse wird untersucht, welche famili{\"a}re Determinanten den Zusammenhang zwischen SEP der Familie und Adipositas bei Jugendlichen erkl{\"a}ren. Methoden Die vorliegende Analyse wurde mit Daten der "Studie zur Gesundheit von Kindern und Jugendlichen in Deutschland" (KiGGS) (Basiserhebung, Welle 1 und 2) durchgef{\"u}hrt (1301 weibliche und 1232 m{\"a}nnliche 11- bis 17-J{\"a}hrige). Mittels logistischer Regressionsmodelle (KHB-Methode) wurden f{\"u}r den Zusammenhang zwischen der SEP der Familie (Einkommen, berufliche Stellung, Bildung, SES-Index) und Adipositas (Ja/Nein) bei Jugendlichen die mediierenden Effekte von famili{\"a}ren Determinanten (elterliches Rauchen, elterliche sportliche Aktivit{\"a}ten, elterliches {\"U}bergewicht, famili{\"a}rer Stress und famili{\"a}rer Zusammenhalt) analysiert. Ergebnisse Die Pr{\"a}valenz von Adipositas betrug 7,9 \% bei den weiblichen und 8,4 \% bei den m{\"a}nnlichen Jugendlichen. Es zeigen sich signifikante Zusammenh{\"a}nge zwischen Adipositas und SEP zu Ungunsten von jenen mit niedriger sozio{\"o}konomischer Position. Famili{\"a}re Determinanten erkl{\"a}ren teilweise oder vollst{\"a}ndig den Zusammenhang zwischen SEP und Adipositas bei Jugendlichen. Es wurden geschlechtsspezifische Unterschiede im Ausmaß der Mediation der Familie gefunden. Bei Jungen erkl{\"a}ren famili{\"a}re Determinanten vollst{\"a}ndig den Zusammenhang zwischen Adipositas und Einkommen sowie zwischen Adipositas und der beruflichen Stellung der Eltern. Bei m{\"a}nnlichen Jugendlichen l{\"a}sst sich der indirekte Effekt abh{\"a}ngig von der jeweiligen SEP-Variable zu 20,8 - 27,9 \% auf das elterliche Rauchen, zu 14,8 - 26,3 \% auf die elterliche sportliche Aktivit{\"a}t und zu 42,6 - 63,6 \% auf das elterliche {\"U}bergewicht zur{\"u}ckf{\"u}hren. Bei M{\"a}dchen wurde der Zusammenhang zwischen der SEP und Adipositas teilweise durch famili{\"a}re Determinanten erkl{\"a}rt. Bei weiblichen Jugendlichen l{\"a}sst sich der indirekte Effekt abh{\"a}ngig von der jeweiligen SEP-Variable zu 26,0 - 52,2 \% auf das elterliche Rauchen und zu 41,6 - 67,6 \% auf das elterliche {\"U}bergewicht zur{\"u}ckf{\"u}hren. Schlussfolgerung Die Adipositas-Pr{\"a}valenz von Jugendlichen variiert nach dem SEP. Konkret wurden die Assoziationen der Bildung, des Einkommens und des beruflichen Status der Eltern auf Individualebene und aggregiert auf Schulebene sowie die Interaktionen zwischen diesen Ebenen auf das subjektive Wohlbefinden von Sch{\"u}lern der Sekundarstufe I untersucht. Methoden Es wurden Daten der Startkohorte „Klasse 5" des Nationalen Bildungspanels (NEPS) herangezogen. Die Kohorte startete 2010 mit einer repr{\"a}sentativen Stichprobe von F{\"u}nftkl{\"a}sslern in Deutschland, welche j{\"a}hrlich nachverfolgt wurden. Es wurden Erhebungswellen von der f{\"u}nften bis zur neunten Klasse zusammengefasst. Damit konnten 14.265 Beobachtungen mit vollst{\"a}ndigen Angaben von 3.977 Sch{\"u}lerinnen und Sch{\"u}lern in 218 Schulen in den Analysen ber{\"u}cksichtigt werden. Mit Mixed-Models wurde getestet, ob die sozio{\"o}konomische Position auf individueller und/oder schulischer Ebene und die ebenen{\"u}bergreifende Interaktion dieser Indikatoren mit dem Wohlbefinden (adaptierter Personal Well-Being - School Children-Index) der Jugendlichen assoziiert ist. Es wurde f{\"u}r Alter, Geschlecht, Migrationshintergrund, Familienform, Schultyp und Notendurchschnitt kontrolliert. Ergebnisse Auf individueller Ebene war das elterliche Einkommen mit h{\"o}herem Wohlbefinden verbunden. Der berufliche Status der Eltern zeigte keine signifikanten Assoziationen. Auf Schulebene fanden wir einen geringen, positiven Zusammenhang zwischen dem durchschnittlichen elterlichen Einkommen und dem individuellen Wohlbefinden, aber auch einen m{\"a}ßig negativen Einfluss eines hohen Anteils hoch gebildeter Eltern auf das Wohlbefinden. In Sensitivit{\"a}tsanalysen blieben diese Zusammenh{\"a}nge auch bei separater Betrachtung dieser Indikatoren bestehen. Die Zusammenh{\"a}nge auf Individualebene wurden nicht durch die sozio{\"o}konomische Komposition von Schulen moderiert, es konnten keine signifikanten ebenen{\"u}bergreifende Interaktionen festgestellt werden. Schlussfolgerung Es wurde ein Zusammenhang der sozio{\"o}konomischen Zusammensetzung der Schule mit dem subjektiven Wohlbefinden der Jugendlichen gefunden. Internationale Befunde einer Moderation der Zusammenh{\"a}nge auf individueller Ebene durch sozio{\"o}konomische Charakteristika auf Schulebene konnten jedoch nicht repliziert werden. Die Zusammenh{\"a}nge zwischen individueller SEP und Wohlbefinden variieren somit nicht in Abh{\"a}ngigkeit von der SEP auf Schulebene. Methoden Um die Wirkung der Intervention zu untersuchen, wurden bundesweite anonymisierte Routinedaten der Techniker Krankenkasse aus den Jahren 2016-2018 sowie ein Difference-in-Differences-Design auf individueller Ebene verwendet. Unter der Annahme eines parallelen Trends auf Bundesl{\"a}nderebene wurde die J1-Teilnahme von 13- und 14-J{\"a}hrigen in Bayern mit anderen Bundesl{\"a}ndern vor und nach der Intervention verglichen. Zus{\"a}tzlich wurde die Analyse f{\"u}r die verschiedenen SES-Gruppen durchgef{\"u}hrt. We present here findings on the role of Turkish migration background for psychological stress experiences and stress biology during pregnancy. 140 pregnant women (81 non migrant German, 33 of Turkish origin, 26 of other origin) participated in a prospective cohort study that was carried out in Bielefeld and Berlin and that encompassed two study visits during pregnancy at first and second trimester pregnancy (Spallek et al., 2020). At both study visits, we derived concentrations of maternal inflammatory markers (CRP, IL-6) and diurnal cortisol profiles, and participants completed the Perceived Stress Scale (PSS) and Center for Epidemiologic Studies Depression Scale (CESD). Multilevel models showed that Turkish-origin women had increased inflammatory levels (Spallek et al., 2021), a blunted cortisol awakening response and flatter diurnal cortisol slope (Entringer et al., 2022), and they exhibited higher levels of perceived stress and depressive symptoms during pregnancy compared to non-migrant women after adjustment for socioeconomic factors. Women of Turkish origin show increased stress levels on the psychological and biological level during pregnancy. However, confidence in using the internet for health decisions was low. Digital technology use in health context was associated with higher eHealth literacy. Ziel des Samplings war eine gr{\"o}ßtm{\"o}gliche Varianz in Bezug auf repr{\"a}sentierte Settings (Kliniken, Geburtsh{\"a}user, Hausgeburten), geografische Regionen und individuelle Merkmale (u.a. Erfahrung, Alter) der Teilnehmenden. Die Interviews wurden aufgezeichnet, transkribiert, anonymisiert und mittels der Grounded Theory Methodologie nach Strauss/Corbin ausgewertet. Ein positives Ethik-Votum liegt vor. Ergebnisse: Das Modell umfasst 7 Handlungsschritte: 1. Situationsdeutung; 2. Abw{\"a}gen und Priorisieren relevanter Parameter; 3. Methods We analyze data from the BIBB/BAuA Youth Employment Survey 2012. The cross-sectional survey includes a representative sample of 3214 German employees, apprentices, and trainees aged 15-24 years. Individuals were grouped by their years of schooling into low (< 12 years) and high levels of education (≥ 12 years). Regression analysis estimated the link between education and four health outcomes: self-rated health, number of health events, musculoskeletal symptoms, and mental health problems over the last 12 months. Counterfactual mediation analysis tested for indirect effects of education via working conditions (i.e., physical and psychosocial job demands) and company characteristics (i.e., company size, health prevention measures, financial situation, downsizing). All analyses were adjusted for age, sex, nationality, region, working hours, job tenure, employment relationship, and economic sector. Results Highly educated workers reported better self-rated health (b = 0.24, 95\% CI 0.18-0.31) and lower numbers of health events (Rate Ratio (RR) = 0.74, 95\% CI 0.67-0.82), musculoskeletal symptoms (RR = 0.73, 95\% CI 0.66-0.80) and mental health problems (RR = 0.84, 95\% CI 0.76-0.93). Total job demands explained between 21.6\% and 87.2\% of the educational differences (depending on health outcome). Unfavourable company characteristics were associated with worse health, but showed no or only small mediation effects. Conclusions Health inequalities are already present at the early working career due to socio-economically stratified working hazards. This study examined the trajectory of depressive symptoms and their association with a) stress perception, b) own COVID-19 infection and c) COVID-19 exposure at work. Methods This cohort study with 91 health workers (90\% female, x⎯⎯=45 years) from a German general hospital included (1) medical, nursing and therapeutic staff and (2) administrative staff. Data were collected longitudinally every month from July to December 2020. Linear multilevel models were used to evaluate associations between trajectories of depression scores (German 'General Depression Scale') with perceived stress ('Perceived Stress Scale'). Laboratory-confirmed COVID-19 infection was tested as a moderator of depressive symptom development. Trajectoires of depressive symptom modalities (e.g., somatic, emotional) were analysed stratified by COVID-19 exposure at work. Results Depression scores increased across the study period (b=.03, .02-.05). Perceived stress was associated with depression scores (b=.12, .10-.14). The presence of health workers' own COVID-19 infection was associated with a higher increase (b=.06, .01-.10). Somatic symptoms of depression increased over time only among health workers directly exposed to COVID-19 patients at work (b=.25, .13-.38). Conclusions The increase in depressive symptomatology among health workers exposed to COVID-19 in the workplace points to the need for further research. Since the infection with COVID-19 is associated with an increase of depressive symptoms across time, awareness should be raised about measures to protect against virus transmission, especially during a pandemic. The mapping showed a tendency of higher index values towards the central areas of Brandenburg surrounding Berlin. Regressions of the number of practitioners against the index values revealed no significant differences between communities with high and low index values. Findings. Among all participants, 626/1020 reported internet use in health context and 278/626 (44\%) reported digital technology use for PA promotion. Most of the 278 users for PA promotion reported that such technologies are easy to use (92\%) and use them at least once a week (91\%). The reasons for using digital technologies were to measure own PA (74\%) or to find exercise ideas (57\%). Most participants reported using mobile devices (e.g., smartphones, 77\%, or activity trackers, 58\%) and rated device feedback as helpful (69\%). Among sociodemographic factors (age, gender, education, household income), health status and eHealth literacy, younger age was the only significant predictor (OR=.98, 95\% CI: .97-.99) of digital technology use for PA promotion. Discussion. Despite their availability, the prevalence of digital technology use for PA promotion is still low among adult internet users in Germany. The minority of participants reported that they trust health websites when recommended by friends and family (39\%), that their social network relies on the internet for health information (35\%), that they consider the internet as the immediate source of health information (39\%) and that they are interested in digital health technologies (40\%). Most participants (71\%) reported that they prefer to receive health information in person or on paper. The preference for such sources of health information was associated with lower eHealth literacy (β= -.12, p<.001), worse health status (β= -.08, p=.026) and female gender (β= -.08, p=.033). Discussion The trust in the internet as the source of health information and the interest in digital health technologies were low among internet users in Germany. Methoden: Anhand von Anwendungsbeispielen werden Analysepotentiale des GISD f{\"u}r die Sozialepidemiologie in Deutschland aufgezeigt. Daf{\"u}r wird der GISD mit Daten 1) der amtlichen Statistik (z.B. Todesursachenstatistik), 2) der Schuleingangsuntersuchung sowie 3) der Studie Corona-Monitoring bundesweit (RKISOEP- 2) auf kleinr{\"a}umiger Ebene verkn{\"u}pft. Zur Anwendung kommen epidemiologische Methoden wie die Berechnung altersstandardisierter Raten und relativer Risiken, u.a. mittels (Mehrebenen-)Regressionsmodellen. Thus, the aims of the current study were to examine whether depressive symptoms increased during the pandemic and were associated with perceived stress and own COVID-19 infection and workplace exposure to virus-infected patients. Methods: The cohort study used longitudinal data from HCW collected monthly (July 2020 till December 2020) during the first year of the pandemic before vaccination became available. The sample of n = 166 was drawn from a German hospital and included medical (e.g. nurses, therapists, and physicians) and administrative staff. Using multilevel models, we analyzed the change in depressive symptoms [assessed with General Depression Scale (GDS), a validated German version of the Center for Epidemiological Studies Depression Scale (CES-D)] and its association with perceived stress across the study period. Laboratory-confirmed own infection was tested as a potential moderator in this context. Subscales of the GDS were used to examine change over time of depressive symptom modalities (e.g. emotional, somatic, and social interactions (β, 95\% confidence interval). Results: Depression scores increased significantly during the study period (β = .03, 95\% CI [0.02, 0.05]). Perceived stress was associated with depressive symptoms (β = .12, 95\% CI [0.10, 0.14]) but did not change over time. Exposure to COVID-19 infection was associated with a higher increase of depressive symptoms (β = .12, 95\% CI [0.10, .14]). Somatic symptoms of depression increased among medical HCW with workplace exposure to COVID-19 (β = .25, 95\% CI [0.13, 0.38]), but not in administrators (β = .03, 95\% CI [-0.04, 0.11]). Results A model consisting of 7 steps was developed: 1) Interpret the situation; 2) Prioritize relevant parameters; 3) Determine a frame of possibilities; 4) Match the preference(s) to the frame of possibilities; 5) Negotiate preference(s); 6) Continuously monitor contextual/situational changes; 7) Adjust the frame of possibilities. The actions are related to contextual conditions (structural; interpersonal; personal; health situation; course of birth; understanding of birth). The model shows how midwives can promote SD even if prior preferences are discarded or modified at birth. Conclusions The model describes how midwives in Germany deal with women's preferences during birth, which factors they consider decisive in doing so, and how SD may be promoted despite of discarded or modified preferences. Further research should examine the role of relationship building, test the model with regard to its generalizability, and use it to analyze and support person-centered midwifery care. Results: Using hierarchical linear models, we found that women of Turkish origin (both first and second gen- eration) reported more perceived stress (b = 0.57, CI: 0.29-0.84, p < 0.001) and depressive symptoms (b = 0.32, CI = 0.14-0.49, p < 0.001) compared to women without migrant background across gestation. To facilitate acceptance of the digital transformation, guiding principles emphasize the need for digital health structures to be person-centered and promote high-quality care. This paper examines the implementation challenges within the German health care system, with a particular focus on how change initiatives engage with existing infrastructures and organizational modes of health care delivery. This approach provides a framework for analyzing how established infrastructure determines new developments while also highlighting the procedural dynamics of change and the integration of innovations within existing information infrastructures. These established infrastructures are referred to as the installed base. Objective: The aim of the study is to examine the installed base encountered by the digital transformation within the German health care system by investigating information exchange practices among general practitioners (GPs) and their communication with other health care actors. Methods: A mixed methods study including a quantitative survey and semistructured qualitative interviews was conducted. The study sample consisted of all publicly accessible GP practices (N=1348) situated in the state of Brandenburg, Germany. The survey captured demographic data, communication practices, and perceived barriers to digitalization. The interviews explored experiences with digital applications. Quantitative data were analyzed using R (R Foundation for Statistical Computing), and qualitative data were managed and analyzed in MAXQDA (VERBI Software GmbH) through content analysis. Results: A total of 250 questionnaires (response rate 18.5\%) and 10 interviews with GPs were included in the analysis. GPs primarily use the telephone (n=138, 55.2\%, SD 24.64), fax (n=109, 43.9\%, SD 25.40), or post (n=50, 20.2\%, SD 9.46) to exchange information. Newer digital communication channels such as messenger applications (n=2, 0.8\%, SD 0.72) and Communication in the Medical Sector (n=1, 0.5\%, SD 0.97) play a minor role. We identified three intertwined clusters displaying diverse barriers to the digitalization of GPs' communication practices: (1) incompatibility issues and technical immaturity, (2) lack of knowledge and technical requirements, and (3) additional technical, financial, and time-related burdens. These barriers were perceived as significant deterrents to the adoption of digital tools, with older GPs more reliant on analog systems and more likely to view digitalization as a source of frustration. Conclusions: Newly established communication channels in the German health care system compete with the existing information infrastructure, which is deeply integrated into GPs' practice routines and care processes. However, this installed base has been largely overlooked in digital transformation initiatives. While newer channels hold potential, they often malfunction and are incompatible with long-established, individualized GP workflows. 'Your Ticket to J1' was an invitation system implemented across the state to inform adolescents about the screening. Our study investigated whether this intervention increased J1 participation and if its effects varied by family socioeconomic position (SEP). We used pseudonymized data from a large statutory health insurer from 2016 to 2018 and containing 267 650 observations. To investigate the effect of the intervention, we employed a difference-in-differences analysis at the individual level. Assuming parallel trends at the state level, we compared J1 participation rates between Bavaria and other German states before and after the intervention. We additionally stratified analyses by SEP. The intervention led to an increase in J1 participation by about 1\%. In the stratified regressions, the effect size was larger for children from families with a lower SEP. J1 participation increased by about 4\% among adolescents whose primary insured parent had the lowest occupational status. A state-wide invitation system had a small but statistically significant positive impact on J1 participation and might reduce socioeconomic inequities in healthcare utilization. This study used population-based, cross-sectional data from a School Entry Survey in Germany (2021; female: n = 9751, male: n = 10 623; age in years: ≤6 n = 11 423; 6-≤7 n = 8 746; >7 n = 205; native language: German n = 18 752; not German n = 943; bilingual n = 679), which was linked to the 'German Index of Socioeconomic Deprivation' (GISD). Binary multilevel models [odds ratio (OR), 95\% CI] were used to analyse the interaction of GISD (measured continuously) with individual-level SEP (categorized as high/medium/low) according to delayed language development (DLD in \% yes/no), which was assessed using validated instruments of 'social-pediatric screening of developmental status for school entry'. Individual SEP was tested as a moderator by stratifying the analyses. The interaction of GISD with SEP was associated with DLD (ORmediumSEP*GISD = 1.13; 1.04-1.24; ORlowSEP*GISD = 1.27; 1.13-1.43), with the main effect of GISD (OR = 0.85; 0.77-0.93). In SEP stratification, GISD was only partially associated with DLD (ORhighSEP = 0.81; 0.73-0.91; ORmediumSEP = 0.97; 0.89-1.07; ORlowSEP = 1.14; 0.99-1.32). The aim of the study was to describe health inequalities during the pandemic based on language delay (LD) in children in order to inform public health interventions for a population at risk of long-term health and education inequalities. Methods Data of 5-7 year old children from three consecutive school entry surveys in the German federal state of Brandenburg were used, including data compulsorily collected before the pandemic (2018/2019: n = 19,299), at the beginning of the pandemic (2019/2020: n = 19,916) and during the pandemic (2020/2021: n = 19,698). Bivariate and multivariate binary regression analyses [OR, 95\% CI] cross-sectionally examined the relationship between the prevalence of LD [yes/no] and social inequalities, operationalized by family socioeconomic position [SEP low/middle/high], migration background [native-German language/non-native German language] and length of kindergarten attendance [< 4 years/ ≥ 4 years]. Factors contributing to inequality in LD were examined by socioeconomic stratification. Results Cross-sectionally, LD prevalence has decreased overall (2018/2019: 21.1\%, 2019/2020: 19.2\%, 2020/2021: 18.8\%), and among children from both high SEP and native German-speaking families. As LD prevalence increased among children from families with low SEP and remained stable among non-native German speakers, social inequalities in LD prevalence increased slightly during the pandemic i) by low SEP (2018/2019: OR = 4.41, 3.93-4.94; 2020/2021: OR = 5.12, 4.54-5.77) and ii) by non-German native language (2018/2019: OR = 2.22, 1.86-2.66; 2020/2021: OR = 2.54, 2.19-2.95). During the pandemic, both migration background and kindergarten attendance determined LD prevalence in the high and middle SEP strata. However, the measured factors did not contribute to LD prevalence in children from families with low SEP. Conclusion Social inequalities in LD increased due to opposing trends in prevalence comparing low and high SEP families. To promote health equity across the life course, early childhood should be of interest for tailored public health actions (e.g. through targeted interventions for kindergarten groups). Materials and Methods: This study uses a stepwise mixed methods approach for implementing a local guideline. A self-reported online questionnaire will be used to survey the current physical therapy practice in stroke rehabilitation. The same survey and systematic-mixed methods review will be used to evaluate the barriers and facilitators of guideline usage quantitatively. Semi-structured interviews will add a qualitative perspective on factors that influence ReMoS guideline implementation. The Behavior Change Wheel and Theoretical Domains Framework will be used to support the development of a tailored implementation intervention which will be pilot tested in a controlled study. Patient and physical therapy-related outcomes, as well as the appropriateness, such as acceptance and feasibility of the tailored implementation intervention, will be analyzed. The survey consisted of three sections with open and closed questions: 1) self-reported use of ReMoS recommendations, 2) barriers of guideline use and 3) socio-demographic characteristics. The benchmark level for guideline adherent physical therapy was set at > 80\%. Results Data from 170 questionnaires were eligible for analysis. Participants' mean age was 41.6 years, 69.4\% were female, while 60.1\% had no academic degree. The ReMoS guideline was unknown to 52.9\% of the responders. Out of all the 46 ReMoS guideline recommendations, only 'intensive walking training without a treadmill' was reported to be performed in a guideline adherent manner. Respondents usually denied any personal limitations, such as limited knowledge, or that the ReMoS guideline did not fit their routine practice. Conclusions Among German physical therapists, the ReMoS guideline is not well-known and many interventions are not performed as recommended, illustrating the discrepancies between the ReMoS guideline recommendations and current physical therapy practice. However, it remains unclear how the role of social determinants of health (SDoH) and digital determinants of health (DDoH) plays in this context. Objective This study investigates differences in sociodemographic factors and digital health literacy between wearable users and non-users, whether the association with wearable use varies across age groups and its potential mediator. Methods A cross-sectional nationwide telephone survey was conducted in November 2022 in a panel of adult internet users in Germany. Assessments included self-reported wearable use, sociodemographic factors (sex, age, education, household size and income, and residence region), and digital health literacy (measured with the eHealth Literacy Scale, eHEALS). Associations between wearable use, sociodemographic factors and digital health literacy were analyzed using binomial logistic regression models in the total sample and with age group stratification, with a supplementary mediation analysis examining digital health literacy as a mediator in the relationship between age and wearable use. Results Overall, 24\% (223/932) of participants (52\% male, mean age 55.6 years) reported using wearables for health. Wearable use was lower among participants aged 65 and above, with lower educational attainment, living in 1-2 person households, with below-average household income, and residing in smaller cities or former East Germany. Wearable use prevalence is substantially lower in older age groups (18-40: 36\%; 41-64: 26\%; 65+:14\%). Wearable users reported higher levels of digital health literacy (mean: 30.7, SD = 5) than non-users (mean: 28.3, SD = 6). Stratified analyses indicate that the association between digital health literacy and wearable use varies by age group, with significant positive association observed in older age groups (OR = 1.00, 95\% CI: 0.94 to 1.07 in age group 18-40; OR = 1.07, 95\% CI: 1.03 to 1.12 in age group 41-64; OR = 1.11, 95\% CI: 1.04 to 1.19 in age group 65+). Mediation analysis indicated that digital health literacy partially mediates the relationship between age and wearable use (indirect effect: coefficient = -0.0156, 95\% CI: -0.0244 to -0.00791, p <.001). Conclusions This study indicates sociodemographic disparities in wearable use among the German population and differences in digital health literacy between wearable users and non-users. A generational divide in wearable use was identified, with older adults being less likely to embrace this technology. This was especially true for older adults with lower digital health literacy. Although midwives play a crucial role in promoting or hindering birth autonomy, little is known about how they navigate birth autonomy in Germany. This study aimed to identify midwives' experiences and strategies of engaging with birthing persons' autonomy in different German settings. Material and methodology: Semi-structured interviews were conducted with 12 midwives from hospitals, birth centres, and home birth environments in Germany. The interviews were recorded, transcribed verbatim, and analysed according to Grounded Theory Methodology (GTM) developed by Strauss \& Corbin. In total, 2944 codes were grouped into five categories ("Phenomenon", "Causal conditions", "Context", "Action strategies", and "Consequences") and integrated into one model. Elements of Reflexive GTM facilitated the reflection on researcher positionality. Results: Midwives continuously balance multiple, often competing, demands, among them the need to define a framework that gives them confidence to stand behind every aspect of the care they provide. The midwives achieve this by interpreting various contextual conditions, prioritising them, defining their professional boundaries and options, and matching maternal preferences with these. Throughout the birth process, they monitor changes and dynamically adjust established boundaries as needed. Discussion: The study results enhance our understanding of the evolving and context-sensitive process of decision-making during childbirth. Discourses around birth autonomy would benefit from adequate terminology that reflects this complexity. In sum, this study contributes to current discourses on midwifery practice, education, and policy.}, language = {en} }
