Abstract
Die erfolgreiche Behandlung eines Diabetes mellitus vom Typ 2 hängt nicht nur von medizinischen Gesichtspunkten des Stoffwechselgeschehens ab, sondern auch von der Compliance eines Patienten und seiner sozialen Situation. Der Verlauf der Krankheit bringt es mit sich, dass nach einigen Jahren eine zufrieden stellende Stoffwechsellage durch Tabletten und Diät meist nicht zu erreichen ist und viele Typ-2-Diabetiker dann auf eine Insulintherapie angewiesen sind. Mehr als die Hälfte der Patienten lehnt jedoch zunächst die Therapieumstellung aus den verschiedensten Gründen bzw. Vorurteilen ab. In der vorliegenden Studie wurde ein Fragebogen entwickelt, der reliabel und valide die Ängste und Vorurteile eines Betroffenen misst. 328 Patienten wurden mit diesem Messinstrument befragt. Die inhaltliche Auswertung der Daten erbrachte, dass die größten Bedenken und Einwände gegen eine Insulintherapie bei Typ-2-Diabetikern die Angst vor einer Gewichtszunahme und vor einem veränderten Krankheitserleben waren. Deutlich ausgeprägt war auch die Abneigung vor der täglichen Insulinspritze und die Angst vor Schmerzen. Es konnte zudem gezeigt werden, dass diese Ängste unmittelbar vor einer Therapieumstellung auf Insulin nochmals ansteigen und sich erst zurückbilden, wenn ein Patient langfristig die Insulintherapie konsequent durchführt. Für Therapeuten ist es äußerst wichtig, die Ängste und Vorurteile ihrer Patienten vor der Einführung einer Insulintherapie zu kennen, um diese erfolgreich abbauen zu können.
It is a fact that a successful treatment of a type-2-diabetes is not only based on aspects of metabolism from a medical point of view but also on the patient´s compliance and his social situation. Diet and tablet-treated therapy in most cases fails to ensure a satisfying blood glucose level and the patient becomes insulin dependent after a number of years. Most of the patients who are advised by their therapist to inject insulin are reluctant to switch treatment and reject the idea because they are afraid of an insulin therapy due to misinformation, biases and prejudices. This thesis lists and examines all finds of fears, objections and biases of type-2-diabetics towards insulin. A survey among 328 diabetic patients has shown that the worst fear among patients is to gain weight under an insulin therapy. Further fears and objections were the feeling of having then reached a more serious stage of their disease and the discomfort of having to take injections every day and suffering from injection pain. The study has proved that those fears even increase among diabetics before they actually start an insulin therapy and decrease among patients who have been practising an insulin therapy for some time. A qualified questionnaire was developed for the survey that enables a therapist to find out quickly and effectively what kind of misinformation, fears and biases a patient holds.
